Causal Association Between Alcohol Consumption and Atrial Fibrillation: A Mendelian Randomization Study

  • Background and Objectives: Previous observational studies presented a positive association between alcohol and atrial fibrillation (AF). However, previous studies using genetic polymorphisms on the causal relationship between alcohol consumption and AF have reported conflicting results. This study aimed to evaluate the causality between alcohol consumption and AF using the aldehyde dehydrogenase 2 (ALDH2) rs671 polymorphism, which is the genetic variant with the most potent effect on drinking behavior. Methods: A total of 8,964 participants from the Dong-gu Study were included in the present study. The causal association between alcohol consumption and AF was evaluated through a Mendelian randomization (MR) analysis using the ALDH2 rs671 polymorphism as an instrumental variable. Results: No significant relationship between alcohol consumption and AF was found in the observational analysis. However, the genetic analysis using the ALDH2 polymorphism showed a significant association in men. In the MR analysis, genetically predicted daily alcohol consumption was positively related to AF. Conclusions: MR analysis revealed a significant association between the amount of alcohol consumption and AF, which suggests that the association may be causal.

발효한약추출물 HP-1이 알코올을 투여한 쥐의 알코올 대사에 미치는 영향 (Effect of Fermented Herbal Extracts, HP-1 on Enzyme Activities and Gene Expressions Related to Alcohol Metabolism in Ethanol-loaded Rats)

  • Recently, much attention has been paid to developing various kinds of fermented herbal extracts, a new type of traditional herbal medicine, in the field of Korean traditional medicine. The fermentation of medicinal herbs is intended to exert a favorable influence on digestability, bioavailavility and pharmacological activity of herbal extract in the gastrointestinal tract. It also produces a number of fermentation products that intensify the nutritional and pharmacological aspects of the medicinal herbs. In order to develop a functional beverage of alleviating the aftereffects of the previous drinks, the extracts (HP-1) of fermented herbal mixture, including Artemisia capillaris Thunb., Lonicera japonica Thunberg, and Hovenia dulcis Thunb., were prepared and the medicinal effect as a hangover cure was evaluated in ethanol-loaded rats. The enzyme activities of alcohol dehydrogenase (ADH) and acetaldehyde dehydrogenase-2 (ALDH2) were analyzed by measuring the concentration of blood ethanol and acetaldehyde. The mRNA expression of ADH and ALDH2 was also investigated through RT-PCR analysis. In the HP-1-treated group, the concentration of blood ethanol was significantly reduced at one hour after loading of ethanol, as compared to that in the saline-treated group. The reduced ethanol was converted to acetaldehyde, which resulted in rapid increase in acetaldehyde concentration in an hour. Acetaldehyde was started to decrease at 5 hours after ethanol loading. It implies that HP-1 is highly effective to stimulate the activities of ADH and ALDH2. The HP-1 treatment also activated the mRNA expression of ADH and ALDH. This study suggests that fermented herbal extract, HP-1 can be used as a functional beverage of alleviating the alcohol-induced hangover symptoms by stimulating the activities and gene expression of hepatic alcohol metabolizing enzymes.

전통적인 발효 방법으로 제조한 황칠 식초의 항산화, 알코올 대사 효소 및 간보호 활성 (The Antioxidant, Alcohol Metabolizing Enzyme, and Hepatoprotective Activities of Dendropanax morbifera Vinegar with Traditional Fermentation Methods)

  • 본 연구에서는 전통적인 방법으로 제조한 황칠 식초의 유기산 함량을 측정하고 항산화 효과와 항당뇨 효과, 알코올 대사 효소 활성 및 HepG2 세포에 대한 간보호 효과를 알아보았다. 황칠 식초의 유기산 분석결과 acetic acid가 91.72 mg/ml로 가장 높았고, 그 다음으로는 lactic acid (7.31 mg/ml), malic acid (1.36 mg/ml), succinic acid (1.20 mg/ml) 순으로 나타났다. 황칠 식초의 총 폴리페놀 함량은 13.73 ㎍ TAE/ml으로 나타났다. DPPH radical 소거능은 농도 의존적으로 증가하였으며 60% 농도에서 76.04%로 나타났고, SOD 활성은 60% 농도에서 95.14%로 나타났다. 황칠 식초 10% 농도에서의 α-glucosidase 저해 효과는 98.94%로 높게 나타났다. 알코올 분해 활성을 알아보기 위해 ADH 및 ALDH 활성을 측정한 결과, 두 가지 효소 모두 황칠 식초의 농도 의존적으로 증가하였으며 60% 농도에서의 ADH 및 ALDH 활성은 각각 43.62% 및 60.39%로 나타났다. 황칠 식초 0.6% 농도에서는 tacrine으로 유도된 HepG2 세포주에 대하여 간보호 활성이 높게 나타났다. 이상의 결과에서 전통적인 방법으로 제조한 황칠 식초는 기능성 건강 음료로서 활용 가능성이 높은 것으로 생각된다.

  • 본 연구에서는 황칠나무 잎과 줄기 물 추출물의 항산화 활성과 알코올 대사 효소 활성 및 간 보호 효과를 알아보았다. 황칠나무 잎과 줄기 물 추출물의 총 폴리페놀 함량은 49.56 mg TAE/g으로 나타났다. DPPH radical 소거능은 황칠나무 잎과 줄기 물 추출물의 농도 의존적으로 소거능이 증가하였으며 1,000 ㎍/ml 농도에서 84.09%로 높게 나타났다. 알코올 분해 활성을 알아보기 위해 ADH 및 ALDH 활성을 측정한 결과, 두 가지 효소 모두 황칠나무 추출물의 농도 의존적으로 증가하였으며 1,000 ㎍/ml 농도에서의 ADH 및 ALDH 활성은 각각 37.68% 및 41.67%로 나타났다. 황칠나무 잎과 줄기 물 추출물은 tacrine으로 유도된 HepG2 세포주의 세포독성에 대한 보호 활성이 나타났다. 이상의 결과를 통해 황칠나무 잎과 줄기 물 추출물은 항산화 효과와 알코올 숙취해소 효과 및 간세포 보호 효과가 높은 것으로 나타나 기능성 식품 소재로서의 활용 가능이 높은 것으로 사료된다.

천도복숭아 식초의 이화학적 특성과 항산화 및 알코올 대사 효소 활성 (Physiochemical Properties, and Antioxidative and Alcohol-metabolizing Enzyme Activities of Nectarine Vinegar)

  • 본 연구에서는 전통적인 방법으로 제조한 천도복숭아식초의 이화학적 특성과 항산화 작용, 아질산염 소거능 및 알코올 대사 효소 활성을 알아보았다. 천도복숭아식초의 pH는 3.70이였고, 당도는 $8.87^{\circ}Brix$였으며, 총산도는 6.29%로 나타났다. 천도복숭아식초의 유기산 분석 결과 acetic acid가 32.42 mg/ml로 가장 높았고, 그 다음으로 lactic acid, succinic acid, malic acid의 순으로 나타났다. 천도복숭아식초의 총 페놀함량은 $121.84{\mu}g\;TAE/100ml$로 나타났다. 천도복숭아식초의 DPPH radical 소거능은 농도 의존적으로 증가하였으며 40% 농도에서 84.47%의 소거능을 보였다. SOD 활성 또한 농도 의존적으로 증가하였으며 60% 농도에서 89.06%로 나타났다. 아질산염 소거능 분석에서는 천도복숭아식초 100% 농도, pH 1.2, 3.0, 6.0에서 각각 94.17, 76.91, 20.21%로 pH가 낮을수록 아질산염 소거능이 증가하는 것으로 나타났다. 천도복숭아 식초의 알코올 분해효과를 알아보기 위하여 ADH 활성 및 ALDH 활성을 분석한 결과, ADH 활성 및 ALDH 활성 모두 농도 의존적으로 증가하였으며, 60% 농도에서 각각 153.61%와 178.20%로 나타났다. 이상의 결과에서와 같이 전통적인 방법으로 제조한 천도복숭아식초는 기능성 건강음료로서의 가능성이 있음을 확인할 수 있었다.

홍합과 여러 식물의 열수 추출물의 알코올분해효소에 미치는 영향과 항산화 및 ACE 저해 활성 (Effect of Hot Water Extracts of Blue Mussel and Several Plants on Alcohol Metabolizing Enzymatic, Antioxidant, and Angiotensin Converting Enzyme Inhibitory Activities)

  • 홍합을 함유한 숙취 해소용 식품 개발의 근거를 확보하기 위하여 12종류의 열수 추출물을 제조하였다 - A(겨우살이); B(냉이); C(칡); D(콩나물); E(헛개나무); F(홍합); G(홍합과 겨우살이); H(홍합과 냉이); I(홍합과 칡); J(홍합과 콩나물); K(홍합과 헛개나무). C 추출물이 alcohol dehydrogenase(ADH)와 acetaldehyde dehydrogenase(ALDH)의 활성을 가장 크게 증가시켰으나, 홍합과 혼합한 경우에는 상승 효과를 보이지 않았다. 칡을 함유한 경우를 제외하고는 H 추출물이 높은 ADH($237.4{\pm}1.7%$)와 ALDH($136.5{\pm}2.1%$) 활성을 보였다. 또한, H 추출물은 가장 높은 DPPH 라디칼 소거능($93.9{\pm}0.1%$)과 안지오텐신 전환효소(ACE) 저해능($42.7{\pm}1.6%$)을 나타내었다. 홍합과 냉이를 혼합한 경우에는 alcohol dehydrogenase 활성과 ACE 저해능이 상승되는 시너지 효과를 확인하였다.

귀뚜라미의 물 및 메탄올 추출물이 알코올 대사에 미치는 효과 (Effects of Water and Methanol Extracts of Cricket (Gryllus bimaculatus) on Alcohol Metabolism)

  • The cricket has been used in East Asia as crude drugs for treating fever and hypertension, and is presently reared as a pharmaceutical insect in China and a food for animals. For the purpose of evaluating protective extracts against alcohol-induced toxicity, the extracts of the cricket (Gryllus bimaculatus) were examined in animal models acutely administered alcohol by the cricket in ICR-mice. Water and methanol extracts from the cricket, were found to cause a significant decrease (37%) in the blood ethanol concentration as well as enhancement of liver mitochondrial alcohol dehydogenase (ADH) and acetaldehyde dehydogenase (ALDH) activitieson on a single intraperitoneal administration in mice. Futhermore methanol extract was demonstrated to exhibit more potent enhancing activity on ethanol metabolism than water extract. These results suggest that water/alcohol extract of G. bimaculatus may be used as a food for reducing the toxicity of alcohol.

Two novel mutations in ALDH18A1 and SPG11 genes found by whole-exome sequencing in spastic paraplegia disease patients in Iran

  • Hereditary spastic paraplegia is a not common inherited neurological disorder with heterogeneous clinical expressions. ALDH18A1 (located on 10q24.1) gene-related spastic paraplegias (SPG9A and SPG9B) are rare metabolic disorders caused by dominant and recessive mutations that have been found recently. Autosomal recessive hereditary spastic paraplegia is a common and clinical type of familial spastic paraplegia linked to the SPG11 locus (locates on 15q21.1). There are different symptoms of spastic paraplegia, such as muscle atrophy, moderate mental retardation, short stature, balance problem, and lower limb weakness. Our first proband involves a 45 years old man and our second proband involves a 20 years old woman both are affected by spastic paraplegia disease. Genomic DNA was extracted from the peripheral blood of the patients, their parents, and their siblings using a filter-based methodology and quantified and used for molecular analysis and sequencing. Sequencing libraries were generated using Agilent SureSelect Human All ExonV7 kit, and the qualified libraries are fed into NovaSeq 6000 Illumina sequencers. Sanger sequencing was performed by an ABI prism 3730 sequencer. Here, for the first time, we report two cases, the first one which contains likely pathogenic NM_002860: c.475C>T: p.R159X mutation of the ALDH18A1 and the second one has likely pathogenic NM_001160227.2: c.5454dupA: p.Glu1819Argfs Ter11 mutation of the SPG11 gene and also was identified by the whole-exome sequencing and confirmed by Sanger sequencing. Our aim with this study was to confirm that these two novel variants are direct causes of spastic paraplegia.

Effect of cordycepin-enriched Cordyceps militaris L. extract powder on alcohol degradation and hepatoprotection in mice alcohol model

  • Cordyceps militaris L. (C. militaris) has been traditionally used as tonic medicine for metabolic syndrome. Cordycepin, has been reported with immunomodulatory, antitumor, and hepatoprotective effect, is the main extract from C. militaris. This study was conducted to evaluate the alcohol degradation and hepatoprotective effect of cordycepin-enriched C. militaris extract (CM) powder in chronic and binge ethanol (ethanol Lieber-DeCarli diet)-fed male C57BL/6 Mice. Cordycepin-enriched C. militaris extract powder was orally administered once daily at dose levels of 0, 125, 250, and 500 mg·kg-1·day-1 for 16 days. For evaluating alcohol degradation, ethanol concentration and alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH) activity were measured in serum. Serum ethanol (EtOH) concentration was decreased at CM treated groups, and the activities of ADH and ALDH were increased dose-dependently at CM treated groups compare to EtOH model group. In clinical chemistry, the values of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were lower in CM treated groups than those in EtOH model group. Additionally, absolute and relative (to body weight) liver weights were statistically decreased in the CM treated groups compared to the EtOH model group. In conclusion, our study showed that cordycepin-enriched C. militaris extract powder exhibits hepatoprotective effect by upregulating the ADH and ALDH enzyme in an alcoholic liver disease model.

A rare case of Sjogren-Larsson syndrome with recurrent pneumonia and asthma

  • Sjogren-Larsson syndrome (SLS) is a rare autosomal recessive neurocutaneous disorder with worldwide incidence of 0.4 per 100,000 people. It is characterized by the triad of congenital ichthyosis, spastic diplegia or quadriplegia, and mental retardation. Herein we report a 2-year-old male child with SLS, asthma, and recurrent pneumonia. SLS was confirmed by a molecular genetics study that revealed a deletion mutation in the ALDH3A2 gene. An ALDH3A2 gene mutation results in dysfunction of the microsomal enzyme fatty aldehyde dehydrogenase and impaired metabolism and accumulation of leukotriene B4, which is a key molecule and a pro-inflammatory mediator in developing allergic diseases, especially asthma. An increased level of leukotriene B4 has been reported in SLS patients. As far as we are aware, this is the first report of SLS associated with asthma and recurrent pneumonia. In conclusion, pediatricians should be aware of and evaluate patients with SLS for possible associated asthma and allergic disorders.
