Purpose: The purpose of the study was to examine the predictive relationships between predictors [(difficulty on daily activities due to arthritis, falls, and pain, grip strength, and activities of daily living (ADLs)], and perceived health in community-living older adults in Korea. Method: A descriptive, cross-sectional survey design was used. Data were obtained from Korean Longitudinal Study of Ageing conducted in 2006. A stratified sample was selected of 5,545 older adults who were over 60 years old for analysis of this study. Descriptive statistics, t-test, Pearson's correlations, and multiple regressions were used. Result: Mean age was 70.27 years (SD=7.27) ranged from 60 and 105 years old. Perceived health was significantly associated with grip strength, and ADL. With multiple regression, all main variables explained 22.1% for perceived health. Difficulty on daily activities due to pain was the strongest predictor on perceived health. Grip strength, ADLs, gender, and difficulty on daily activities due to arthritis and falls were also significant predictors on perceived health. Conclusion: When developing nursing intervention to improve perceived health in older adults, their difficulties on daily activities, grip strength, and ADLs should be assessed.
The purpose of this study was to identify the factors that affected to physical functions, activities of daily living (ADLs) and instrumental activities of daily living (IADLs). The subjects for this study, 236 persons, 65 years and over, who were living at rural area in Chun-Cheon city. This survey was carried out from march 3rd to April 25th, 1998. The data was analysed by using SPSS PC+ program. But the variables for medical utilization were not related to the level of physical function. 2. The variables that were associated with the condition of ADLs were as follows; age among the general characteristics; monthly using money for life, working amount among the socioㆍeconoㆍphysical conditions; subjective conditions of health among the subjective thinking. But The variables for medical utilization, lifestyle for promoting health were irrelevant to the conditions of ADLs. 3. The variables that were related to the conditions of IADLs were as follows; age, educational level among the general characteristics; raising ways of money, monthly using money for life, resident type in the household, procurement method of pocket money, level of weight, problems in mind, wether or not chronic illness among socioㆍeconoㆍphysical condition; regularity of health checking among lifestyles for promoting health; kinds of utilizing medical agent among the variables about utilization patterns of medical agencies; subjective condition of health among the subjective thinking In this study, in the aged there were related factors of physical and life functions, were age, working amount, subjective condition of health.
Purpose: The purpose of this study was to investigate the effects of a modified constraint induced movement therapy (mCIMT) using proprioceptive neuromuscular facilitation (PNF) on the upper extremity function and activities of daily living (ADLs) in patients with subacute stroke. Methods: Fourteen participants with subacute stroke were randomly assigned to a group using both mCIMT and PNF or a group using mCIMT alone. Each group underwent twenty sessions (1 h/d, 5 d/wk) for 4 weeks. Patients were assessed with the action research arm test (ARAT), the Fugl-Meyer assessment for the upper extremities (FMA-UE), the Modified Barthel Index (MBI), and motor activity logs (MALs; amount of use [AOU] and quality of movement [QOM]). Results: Both the experimental group and the control group showed significant intragroup improvement in the ARAT, FMA-UE, MBI, and MAL-AOU (p<0.05). The group using both mCIMT and PNF exhibited greater improvement in the ARAT, FMA-UE, MBI, and MAL-AOU than did the group using mCIMT alone. Statistical analyses showed significant differences in the ARAT (p=0.01), FMA-UE (p=0.01), MBI (p=0.00), and MAL-AOU (p=0.01) between the groups. Conclusion: This study applied mCIMT combined with PNF for subacute stroke patients, and the results showed significant improvements in the patients' upper extremity function and ADLs. Therefore, mCIMT using PNF may be more effective than mCIMT alone in improving upper limb function and ADLs in patients with subacute stroke.
Objective: This study aimed to investigate the effect of adding hip abductor strengthening to conventional rehabilitation on muscle strength and physical function following total knee replacement (TKR) for knee osteoarthritis. Design: Randomized controlled trial Methods: Thirty-five participants were randomly allocated to exercise groups I (n=18) and II (n=17). Group I underwent hip abductor training and conventional rehabilitation for 30 min per day, 5 days per week for 4 weeks. Group II underwent conventional rehabilitation for 30 min per day, 5 days per week for 4 weeks. The participants in both groups also received continuous passive motion therapy for 15 min per day, 5 days per week for 4 weeks. To investigate the effect of the intervention, the Biodex dynamometer was used to measure the peak torque of both knee extensors and hip abductors. This study used the Knee Outcome Survey-Activities of Daily Living Scale (KOS-ADLS) to assess physical function, as well as the figure-of-8 walk test (F8W) and the stair climb test (SCT). Results: According to the interventions, exercise groups I and II showed significantly improved muscle strength and KOS-ADLS, F8W, and SCT scores (p<0.001). Compared with that of exercise group II, exercise group I showed significantly improved hip abductor strength (p<0.001) and KOS-ADLS, F8W, and SCT scores (p<0.05). Conclusions: The results of this study indicate that the combination of hip abductor strengthening and conventional rehabilitation is an effective exercise method to increase hip abductor muscle strength and physical function after TKR.

Background: This study aimed to analyze the efficacy of single-dose corticosteroid injection (CSI) administered at 6 weeks postoperative to treat stiffness following arthroscopic rotator cuff repair (ARCR). Methods: In this prospective, multicentric, case-control study, post-ARCR stiffness at 6 weeks was treated with either a single dose of intra-articular CSI (CSI group) or physical therapy with oral analgesics (non-CSI group). Pain intensity according to visual analog scale (VAS), functional outcome using the Constant Murley Shoulder Score, time to return to activities of daily living (ADLs), and retear rate were recorded at 6 weeks, 9 weeks, 12 weeks, 6 months, 12 months, and 18 months postoperatively in both groups. Results: A total of 149 patients (54.5%) in the CSI group and 124 patients (45.5%) in the non-CSI group were included in this study. Pain and function were significantly better in the CSI group at 9-week, 12-week, and 6-month (P<0.001) follow-up, whereas they were not significantly different when the groups were compared at 12- and 18-month follow-up. The mean duration to return to ADLs was significantly shorter (P<0.001) in the CSI group. The incidence of retears was not significantly different (P=0.36) between groups at the end of 18 months of follow-up. Conclusions: Single-dose intra-articular CSI administered at 6 weeks postoperative to treat post-ARCR stiffness significantly improved pain, function, and duration of return to ADLs without increasing the risk of retears compared to patients who did not receive intra-articular CSI. Level of evidence: III.
Purpose: This study was to determine the mediator or moderator role of social support in the relationship between stress and depression among family caregivers of older adults with dementia. Method: Sixty nine family caregivers were randomly selected from health care centers in P city and a face-to-face interview was conducted using questionnaires from January to May of 2002. Data were analyzed with descriptive statistics, Pearson correlation, and hierarchical multiple regression using SPSS program. Result: Family caregivers of older adults with higher dependency in ADLs and higher problematic behaviors, provided care to the older adults for a longer period of time, and perceived less social support reported higher depression. Social support showed mediating effects between stress and depression, while did not show moderating effects. Elderly dependency on ADLs and caregiving duration decreased perceived social support and decreased social support increased depression. Conclusion: To increase family and social support to the caregivers of more functionally impaired elderly, family education to increase emotional support and physical assistance to the caregivers and broader and flexible application of social support such as increasing accessibility to the elderly daycare service with lower price may prove beneficial.
The purpose of this study was to determine the effects of self-help management program which strengthens self-efficacy for post stroke elders who attend day care center in Chongju. One group pretest-posttest design was used, and the subjects were 7 post stroke elders with hemiparesis. The self-help management program consisted of 5 sessions and each session included group education on stroke, ROM exercise, recreation and self-care strategies. Those strategies for encouraging and reinforcing self-efficacy provided verbal persuasion, performance accomplishment and vicarious experience. All subjects were questioned before and after the program about ADLs, IADLs, self-efficacy, self-care behavior and depression. The analysis of data revealed that the program was effective to increase the IADLs, self-efficacy and self-care behavior. However, there was no significant difference between pre and post ADLs and depression. The results indicated that this program may be adopted to improve the physical and psychological function of post stroke elderly patients in community settings.
Purpose: This follow up study was performed to survey the problem in performing ADLs at home after surgery. Method: Twenty elderly patients were assessed 3 times; from the time of hospitalization for surgery until 3 weeks after discharge. Measuring tools are numeric rating scales (NRS) for pain and 3 questionnaires developed by the authors to measure difficulties in ambulation, performing indoor ADLs, and emotional status. Pearson correlation, variance analysis and descriptive statistics were used to analyze the data. Result: The level of pain significantly decreased after surgery, but the difference between pain at the time of discharge and that of 3 weeks after discharge were not significant. Pain showed a positive relationship with emotional difficulties. Elderly with a higher education, family caregiver, and regular exercise showed a lower level of emotional difficulties. Conclusion: Comprehensive approaches for chronic pain including physical, psychological, and social aspects should be considered when caring for the elderly with spinal surgery. In addition, home care nursing interventions should include an exercise program to promote adaptation and rehabilitation after discharge.
Muscular strength in the elderly is important factors deciding on functional state in daily lives, and functional state shows a health condition. Resistance training to prevent loss of muscle mass and muscle fiber atrophy due to aging can increase muscle mass and strength. Recently, resistance training to improve muscular strength and endurance has been actively researched. Resistance training increases activities of daily living and ability of the elderly, which makes healthy senescent. ADL/IADL is suitable for measuring health condition of the elderly. In this study, target muscles of resistance training were chosen through a correlation analysis between ADLs and elementary motion, and we presented a procedure to select target muscles and exercise equipments. This procedure consist of a survey method that were described detailed motion of ADL/IADL. So we should be to extract resistance training machine to improve functions of regarding fundamental motion. Result of study may improve daily life activities of the elderly through objectives and scientific approaches. Furthermore, it will be helpful to studies about various fields of the elderly.
