일부 치매 및 치매의심환자들의 인지기능에 따른 구강보건인식 조사 (Perceived oral health awareness in dementia and dementia-suspected depending on KMME)

  • Objectives: The purpose of this study is to investigate cognitive function, performance of activities of daily living, and recognition on oral health with the cognitive function testto dementia or dementia-suspected patients in the outpatients. Methods: The subjects were 94 dementia or dementia-suspected patients visiting C University hospital for the dementia test. Study instruments included Korea Mini-Mental State Examination KMMS, The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL, Global Deterioration Scale; GDS, Korean Dementia Screening Questionnaire; KDSQ, and underlying diseases. Results: Dementia or dementia-suspected patients were 42 by KMMSE test, 25 patients had impaired functioning of daily living by B-ADL test, 27 patients showed the presence of depression by GDS test, and 45 patients showed impaired functioning of daily living. There was a statistically significant difference in the subjective recognition on oral health conditions. There was a statistically significant difference in the subjective recognition on oral health conditions by ADL. There was a positive correlation between the cognitive function and ADL performance. Higher cognitive function is proportional to ADL performance. Conclusions: The cognitive function was closely associated with ADL and subjective oral health conditions.

방문간호를 통한 일상생활동작 수행능력 개선에 대한 사례보고: 오마하시스템을 활용하여 (Improvement of Activities of Daily Living through Visiting Nursing Care under Long-Term Care Insurance: A Case Report using the OMAHA System)

  • Purpose: This study was done to report nursing case for ADL improvement of elders who have CVA(Cerebrovascular Accident) sequelae. Methods: The client had registered in the C visiting nursing center after being decided a long-term care Grade 2. Data were collected through consultation logs for recipients, Activities of Daily Living (ADL) records, fall risk assessment (Huhn) sheets, decubitus ulcer risk assessment (Braden Scale) sheets, cognition assessment (K-MMSE) sheets, long-term care benefit provision records, and interviews with visiting nurse. Data were collected and analyzed according to the Omaha System problem classification. The intervention scheme and the problem rating scale for performance were applied to present the case for home-visit nursing. Results: The client registered in August, 2018, was provided home-visit nursing care once a week as of September 2020. ADL, cognitive levels and decubitus ulcer risks were found to have improved. Conclusion: This case report presents the value of classifying nursing problems and checking nursing intervention provided to patients with problems of ADL. The presentation of home-visit nursing cases applying a standardized nursing problem classification scheme for clients with various problems showed that a high quality level of care is guaranteed and evidence-based nursing can be provided by visiting nurses.

우리나라 노인들을 대상으로 한 일상생활에서의 인간공학적 불편성 조사 연구 (A Study on Ergonomic Uncomfortableness on ADL for Korean Elderly People)

  • This paper reports the results on age and gender of Korean elderly people for the level of difficulty in performing household tasks (meal preparation, grocery shopping, house cleaning, laundry), personal tasks (dressing, bathing, grooming), transfer tasks (getting in and out of chairs, getting in and out of bath-tub, using stairs) and management tasks (using telephone, accessing mail, operating door locks). A questionnaire based on the Activities of Daily Living (ADL) scale was constructed and administered to 40 Korean elderly subjects aged from 65 to 84(mean age: 74.5, SD: 5.8) in Busan. Additionally, a logistic regression was performed with age (continuous variable) and gender as predictor variables, and reponses to individual questions as the categorical ordinal response variables. To determine appropriate age separation at which difficulty levels in performing activities of daily living change, a discriminant analysis was performed on the responses. All predictor variables were used in the analysis. Accommodating age related changes in functional abilities, and increasing functional independence of elderly people will entail significant design modifications to products, systems and environments for daily use and living.

류마티스성 관절염 환자의 일상생활활동 조사연구 (A Study of Activities of Daily Living(ADL) in Patients with Rheumatoid Arthritis)

  • This study attempts to provide the rudimentary data that might help patients with Rheumatoid Arthritis manage an independent life by the analysis of their activities of daily living(ADL). The subjects were forty patients who had been diagnosed Rheumatoid Arthritis and under drug therapy from RA clinics of the two university hospitals in Pusan. Data were collected by performing an individual interview for each of them during June 15th through July 15th 1998. Questionaires were imployed to investigate the general characteristics of patients. the traits of Rheumatoid Arthritis, and actual conditions of their activities of daily living. The questionaires utilized especially for the study of ADL is the version of Lee Eun Ok's tool which modified and complemented Katz scale (Katz, Down, Cogh and Grots 1970) and Bathel index(Bathel. 1973). It's consisted of 31 questions that each question is measured by 0-3points. The high score in the questionaire signifies the high independency in the ADL. The data were analyzed by SPSS/Win in terms of frequency, percentage, mean, and standard deviation, close connection with the contents of the personal interview. Total mean of the ADL shows $1.55{\pm}0.68$, indicating the overall activities of daily living is generally dependent. It also indicates that patients, in the condition of dependent activity. tends either to have recourse to drugs or to take negative strategy, getting help from inside or outside family member. Hence, it is necessary that patients are exposed to the introduction and instruction of self-help devies that might help them manage a comparatively independent ADL, easing their paychological burden.

대퇴골절 수술을 받은 노인의 통증, 우울, 건강행위와 일상생활수행능력의 관계 (Relationships among Pain, Depression, Health behavior, and Activities of Daily Living in Older Adults after Femur Fracture Surgery)

  • Purpose: The purpose of this study was to explore the relationships among pain, depression, health behaviors, and activities of daily living (ADL) in older adults after femur fracture surgery. Methods: This was a descriptive correlational study. Ninety seven outpatients aged 65 or older were selected, who agreed to participate and visited the four hospitals located in G providence. Numeric Rating Scale (NRS) for pain, a Short form Geriatric Depression Scale (SGDS) for depression, health behavior scale and Korean version of Bathel Index for ADL were used. Data were analyzed using t-test and ANOVA, Pearson's correlation and the hierarchical regression analysis in SPSS 18. Results: Pain and depression were positively correlated (r=.35, p<.001). Pain and health behaviors (r=-.30, p=.010), pain and ADL (r=-.21, p=.044), depression and health behaviors (r=-.51, p<.001), depression and ADL (r=-.29, p=.004) were negatively correlated. The variables affecting the ADL was intake of a painkiller or not (${\beta}$=-.32), age (${\beta}$=-.25), transitional period after discharge (${\beta}$=.23) and depression (${\beta}$=-.23). ADL was accounted for 33.4% in total by these four variables. Conclusion: Interventions for alleviating pain, and managing depression would be effective in enhancing ADL in older adults after femur fracture surgery.

노인요양시설 입소 노인의 통증, 우울 및 일상생활활동과의 관련성 (Relationship among Pain, Depression and Activities of Daily Living in Nursing Home Residents)

  • 본 연구는 노인요양시설 입소 노인의 통증, 우울 및 일상생활활동의 관계를 확인하고 통증의 영향요인을 파악하기 위하여 수행되었다. S시의 노인요양시설 4개소에서 2013년 1월부터 2월까지 구조화된 설문지를 이용하여 자료를 수집하였다. 자가보고 통증평가 도구인 NRS(numeric rating scale)와 행동관찰 통증평가 도구인 DS-DAT(discomfort scale-dementia of the Alzheimer's type)를 사용하여 통증을 측정하였고, GDSSF-K(Geriatric Depression Scale Short Form-Korea Version), BADL(Barthel activities of daily living)로 우울과 일상생활활동 수준을 측정하였다. 수집된 자료는 SPSS PC 20.0 program을 이용하여 Pearson's correlation coefficient, 위계적 다중회귀분석을 실시하였다. 연구결과는 NRS와 DS-DAT 모두 일상생활활동과는 음의 상관관계를 보였으며, 우울과는 양의 상관관계를 보였다. 회귀분석을 통하여 성별, 관절염, 우울, 일상생활활동이 NRS를 28.1% 설명하는 영향요인이었고, DS-DAT에서는 우울과 일상생활활동이 21.5%를 설명하는 영향요인으로 확인되었다. 이상의 결과를 바탕으로 노인요양시설 입소노인의 통증관리에 있어서 우울을 완화하고 일상생활활동 능력을 향상시킬 수 있는 프로그램의 개발과 적용이 요구된다.

  • Purpose: The purpose of this study was to examine the effect of use of a horticultural program on the cognitive function, activities of daily living, and hand grip strength of institutionalized dementia patients. Method: Twenty three study participants completed a five week protocol comprised of one week of baseline and four weeks of treatment. The study design was a one group repeated measurement study. For the first and fifth week of the study period, data was collected with 3 instruments: Korean Version of Hasegawa Dementia Scale (HDS-K), Katz's ADL index, hand grip strength. Data analysis was conducted by SPSS version 10.0. Result: Total HDS-K score(p=.000), Activities of daily living(p=.013) and hand grip strength(p=.001) was significantly improved after treatment compared to the baseline. Conclusion: The horticultural program was effective for improving cognitive function, ADL and hand grip strength of the dementia patients. Repeated studies utilizing a control group are needed, using various durations, plants and processes, to confirm these results and to standardize the horticultural program protocol.

만성질환으로 입원한 노인 환자의 우울, 자아존중감 및 일상생활수행능력(ADL)간 관계연구 (The Relationship among Depression, Self-esteem and ADL in the Case of the Hospitalized Elderly Patients with Chronic Disease)

  • Purpose: This study has aimed to examine the relationship among self-esteem, Activities of Daily Living (ADL) and depression reported by hospitalized elderly patients with chronic diseases. Methods: A descriptive correlation study by means of a self-report questionnaire or face-to-face interview was used to collect data from 119 elderly patients who were hospitalized in a General Hospital from January 5 to February 25, 2010. Their levels of depression was measured using Short-Form Geriatric Depression Scale, self esteem using Jeon's, and activities of daily living using K-ADL. Results: 80.7% of the subjects experienced depression. Depression correlates with self-esteem (r=-.67) and ADL (r=.45). The influencing factors on depression were self-esteem, ADL, subjective health status, and family support satisfaction ($R^2=.57$), while self esteem in itself explained 45% of variance in depression. Conclusion: These findings indicate the importance of early detection of depression, which starts from the admission of patients and the continuing evaluation/management in daily life after discharge to ensure their well-being and quality of life. The development of program empowering self esteem, ADL and subjective health status with adequate family support during hospitalization and in daily life is indispensible.

뇌졸중 노인의 기능장애 정도, 일상생활 수행능력, 우울과 삶의 질 간의 관계 (The Relationship of Dysfunctions Degree, Daily Living Activity, Depressiveness and Quality of Life among the Elderly Suffering from Stroke)

  • The purpose of this study was to know dysfunctions degree, daily living activity, depression and quality of life among stroke elderly person; and to know what kind of factors affecting to quality of life of the stroke elderly person; after that provide a fundamental data to nursing arbitration plan about increase height quality of life of the stroke elderly person. The study subjects were collected 119 people, over than 60 age, who were diagnosed with stoke in D hospital, living in B city. The data was collected by using personal interviews and questionnaire, from Nov 2008 to Jan 2009. The questionnaire were Pre-Stroke MRDS(Modified Rankin Disability Scale), Barthel-ADL(Activities of Daily Living), K-IADL(Korean-Instrumental Activities of Daily Living), GDS-15(Geriatric Depression Scale-15-question form) methods. The pre-stroke MRDS was appeared a functional obstacle. The mean score of activity of daily living was 18.24 which showed the subjects were mildly disabled, the score of instrumental activities of daily living was 7.94 which showed the mildly abled, and score of depressiveness was 7.41 which showed the subjects were mildly depression. There was a significantly correlation among MRDS, ADL, IADL, depressiveness and quality of life. Through these study result, important to check the old subjects' depressiveness, cognitive functions, daily living abilities, dysfunctions degree to main tain their quality of life.

요양시설 노인의 간호요구도 및 일상생활 수행능력 (Activities of Daily Living and Nursing Needs of the Elderly in Nursing Home)

  • Purpose: The study was a survey study to identify the Activities of Daily Living (ADL) and nursing needs of the elderly in the nursing home and derive the fundamental data for offering the better quality of nursing service to them. Methods: The subjects were the 111 elderly aged over sixty five living in the nursing home located in Seoul. Measures were the nursing needs scale and ADL scale. The data were analyzed by SAS 11.0. Results: First, with regard to the nursing needs, the general need was scored average 3.0, and the emotional' social need was 3.7, and the physical need was 3.1, and the informational need was 2.7. Second, the ADL was scored at the average of 2.7, bathing 1.9,. eating 3.1. Third, In correlation between the nursing needs by area, the informational nursing needs showed the sheer correlation with the physical nursing needs, emotional' social nursing needs. The physical nursing needs showed the sheer correlation with the informational. social nursing needs, and the inverse correlation with the ADL. Conclusion: It is necessary that it should improve the service to meet the emotional and social nursing needs and develop the extensive nursing programs satisfying their desires based on the general traits of the elderly.
