Possible Use of the Animal By-product Mixture as n Dietary Fish meal Relpacer in Growing Common Carp (Cyprinus carpio) (잉어 사료에 있어 축산 가공 부산 혼합물의 어분 대체 가능성)

  • This study evaluated the possible utilization and the replacing range of animal by-product mixture (ABPM) as a dietary fish meal replacer in growing common carp (Cyprinus carpio). ABPM is a mixture of leather meal, meat and bone meal, blood meal and squid liver powder at a specific weight based ratio. Five different diets were formulated on isonitrogenous and isocaloric basis of $40\%$ crude protein and 15.3 KJ/g diet in which white fish meal (WFM) protein was substituted with the ABPM Protein as follows: diet 1, $100\%$ WFM ($0\%$ ABPM, control); diet 2, $75\%$ WFM+$25\%$ ABPM ($25\%$ ABPM); diet $3,\;50\%$ WFM+$50\%$ ABPM ($50\%$ ABPM); diet 4 $25\%$ WFM+$75\%$ ABPM ($75\%$ ABPM); diet $5.0\%$ WFM+$100\%$ ABPM ($100\%$ ABPM). As the dietary protein sources, each diet contained $34.7\%$ of animal protein supplied by white fish meal and/or ABPM and $65.3\%$ of plant protein. After one week of conditioning period, fish averaging 10 g were randomly assigned to each diet treatment as triplicate groups and fed one of the experimental diets for 12 weeks. Weight gain of fish fed diet 1 (control) and 3 were significantly higher than those of fish fed diet 2, 4 and 5 during the first 4 weeks (P< 0.05), while there were no significant differences among all diet groups during the third 4 weeks (P>0.05). Feed conversion ratio of fish fed diet 1 was significantly higher than those fed diet 2, 3, 4 and 5 during the second 4 weeks (P<0.05), while there were no significant differences among all diet groups during the first and the third 4 weeks (P<0.05). There were no significant differences in proximate analysis among fish fed the experimental diets either for the second 4 weeks or the third 4 weeks (P>0.05). These results indicated that ABPM could be used as a fish meal replacer up to $100\%$ in growing common carp.

Feasibility and Effectiveness of a Ring-Type Blood Pressure Measurement Device Compared With 24-Hour Ambulatory Blood Pressure Monitoring Device

  • Backgrounds and Objectives: This study aimed to evaluate the applicability and precision of a ring-type cuffless blood pressure (BP) measurement device, CART-I Plus, compared to conventional 24-hour ambulatory BP monitoring (ABPM). Methods: Forty patients were recruited, and 33 participants were included in the final analysis. Each participant wore both CART-I Plus and ABPM devices on the same arm for approximately 24 hours. BP estimation from CART-I Plus, derived from photoplethysmography (PPG) signals, were compared with the corresponding ABPM measurements. Results: The CART-I Plus recorded systolic blood pressure (SBP)/diastolic blood pressure (DBP) values of 131.4±14.1/81.1±12.0, 132.7±13.9/81.9±11.9, and 128.7±14.6/79.3±12.2 mmHg for 24-hour, daytime, and nighttime periods respectively, compared to ABPM values of 129.7±11.7/84.4±11.2, 131.9±11.6/86.3±11.1, and 124.5±13.6/80.0±12.2 mmHg. Mean differences in SBP/DBP between the two devices were 1.74±6.69/-3.24±6.51 mmHg, 0.75±7.44/-4.41±7.42 mmHg, and 4.15±6.15/-0.67±5.23 mmHg for 24-hour, daytime, and nighttime periods respectively. Strong correlations were also observed between the devices, with r=0.725 and r=0.750 for transitions in SBP and DBP from daytime to nighttime, respectively (both p<0.001). Conclusions: The CART-I Plus device, with its unique ring-type design, shows promising accuracy in BP estimation and offers a potential avenue for continuous BP monitoring in clinical practice.

  • Purpose : Obesity in children and adolescence is highly correlated with adult obesity, which can provoke hypertension. Therefore, it is important to evaluate the blood pressure of obese children regularly. In this study, the results of ambulatory blood pressure monitoring (ABPM) in obese children and adolescents were evaluated. Methods : ABPM was conducted for selected patients who visited Handong University Sunlin Hospital from Feb. 1, 2006 to Dec. 1, 2007. The patients were classified into 3 groups: group 1 was normal, group 2 had normal casual blood pressure and a body mass index over the 95th percentile, and group 3 had high casual blood pressure over 120/80 mm Hg and a body mass index over the 95th percentile. Systolic and diastolic 24-hour blood pressure was measured, including both day and night. Results : There were 49 patients in the study. The results showed a significant difference for average systolic blood pressure between the three groups ($105.1{\pm}4.7$, $111.0{\pm}7.1$, $117.8{\pm}6.6mmHg$, P<0.001), but for average diastolic blood pressure only between groups 1 and 3 ($69.1{\pm}5.3$, $77.9{\pm}6.3mmHg$, P=0.001). In the daytime, only groups 1 and 3 showed a statistically significant difference for systolic and diastolic blood pressure. During the night the systolic pressure of group 3 was significantly higher than the other groups, but the diastolic pressure of group 3 was only higher than that group 1. No statistical difference was found in night dips among the groups. Conclusion : Ambulatory blood pressure monitoring in children and adolescents showed statistically higher blood pressure in obese patients with high casual blood pressure.

Effects of dietary Chromic Oxide and Possible Use of the Animal By-product Mixture as a Dietary Fish meal Replacer (무지개송어 사료에 있어 산화크롬의 첨가효과와 축산 가공 부산혼합물의 어분대체 가능성)

  • This study was conducted to evaluate the utilization of animal by-product mixture (ABPM) as a dietary animal protein source of fish meal replacer, and to determine the effect of dietary chromic oxide in growing rainbow trout, Oncorhynchus mykiss. ABPM is a mixture of five anmial by-products such as meat and bone meal (MBM) feather meal (FM), squid live, powder(SLP), poultry by-product (PBP) and blood- meal (BM) at a specific weight based ratio. Diet 1 and 2 were formulated on a isonitrogenous and a isocaloric basis of $46.5\%$ crude protein and 16.7 KJ/g diet; diet 1 (WFM 100), $100\%$ of the animal protein source came from white fish meal; diet 2 (ABPM 40), $60\%$ WFM+$40\%$ ABPM as the animal protein source; diet 3 (-Cr) commercial diet without chromic oxide; diet 4 (+Cr), commercial diet with chromic oxide. After eight weeks of feeding trials, fish fed diet 2 had a significantly lower body weight gain (WG) and feed efficiency (FE) than that of fish fed the other diets (P<0.05). When comparing diet 3 with diet 4, no significant differences were found in WG and FE (P>0.05). There were no significant differences on condition factor, hematocrit level, serum phosphorus, bone phosphorus, whole body phosphorus, and bone ash among fish from all four diet groups. Fish fed diet 4 had a significantly higher whole body lipid than that of fish fed the other diets (P<0.05), These results indicated that ABPM could be used less than $40\%$ in growing rainbow trout with a sufficient period of acclimation, In addition, the $0.5\%$ of chromic oxide can be used to determine the apparent digestibility of the nutrients in the feed without any adverse effects on growth and body composition.

Clinical Usefulness of Ambulatory Blood Pressure Monitoring in Children and Adolescents (소아 및 청소년에서 24시간 활동 혈압 측정의 임상적 유용성)

  • Purpose: With increasing prevalence of hypertension (HTN) in children and adolescent, pediatricians have become more interested in blood pressure (BP) measurements. The ambulatory blood pressure monitoring (ABPM) is known to be useful to differentiate true HTN and white coat HTN. The object of this study is to assess the clinical usefulness of ABPM in Korean children and adolescents. Methods: A retrospective review of 51 patients in Kyungpook National University Hospital from January 2002 to February 2010 was done. All patients were 6-18 years old and underwent ABPM. We calculated the mean value of ABP, BP load, nocturnal dip and compared the results with the patients' diagnosis and characteristics. Results: The mean age of the 51 patients was $17.8{\pm}1.8$ years and 19 children were obese. 37 patients (72.5%) were truly hypertensive and 1 patient was diagnosed as masked HTN and 7 children (14%) as white coat HTN. The rest of the patients were normotensive. Among patients with white coat HTN, 5 were in a prehypertensive state. Mean systolic and diastolic BP load of patients with true HTN were significantly higher than non-hypertensive children (P<0.001). Although the nocturnal dip of all patients were below 10%, there was no statistical significance. The obese patients showed higher systolic and diastolic BP. Their systolic and diastolic BP load were significantly higher than non-obese patients (P<0.001). Conclusion: ABPM in children and adolescents seems to be a valuable tool in the assessment of white coat HTN and in the confirmation of true HTN. A considerable number of white coat HTN patients are revealed to be in a prehypertensive state and need close follow-up.

The Effect of korean, Chinese and American Ginseng on Blood Pressure of Hypertensive Patients (한국삼, 중국삼, 서양삼이 고혈압 환자의 혈압에 미치는 영향)

  • Objectives : We conducted a randomized, double-blinded clinical trial to assess the anti-hypertensive effect of Ginseng and to know the difference of the effect according to it's growing districts md species by 24-hour ambulatory blood pressure measurement (ABPM). Methods : We allocated 96 hypertensive patients enrolled in this trial to Korean ginseng(KG), American ginseng (AG), Chinese ginseng (CG), and Korean red ginseng (KRG) groups by randomization. Each subject was administered 4.5mg/day of encapsulated ginseng for 4 weeks. We assessed anti-hypertensive effect, blood pressure variability using ABPM and toxicity by blood chemistry before and after treatment. We also evaluated changes of symptoms due to hypertension and adverse effect in all groups at the first visit, 2 weeks later and 4 weeks later. Results : Blood pressure after treatments showed significant decrease of systolic blood pressure (sBP) in the CG-group (p<0,05) and diastolic blood pressure (dBP) in the KRG-group (p<0.05). However, there were no significant changes of sBP (or dBP) after treatment in the other groups and no significant difference in changes of BP between before and after treatment among the 4 groups. Blood pressure variability in the CG-group showed significant decrease after treatment but not in the KRG-group. Symptoms such as headache or neck stiffness and heating sensation due to hypertension improved significantly in all groups, especially in the KRG-group. A3l patients had no adverse effect after treatment and there was no liver or kidney toxicity. Conclusions : CG and KRG seem to have anti-hypertensive effects, but there was no significantly different effect depending on growing district and species of Ginseng.

  • Backgrounds and Purpose: Hypertension is a common origin for stroke, heart disease, etc. Clinical management is needed to prevent these diseases. It has been said that auricular acupuncture treatment can be used for the control of hypertension. In this study, we studied the effects of auricular acupuncture on mild hypertension. Materials and Methods: Clinical study was performed on 22 stroke patients who didn't take the antihypertensive drugs. They were admitted in Kyunghee University, Hospital of Oriental Medicine. We checked patients' 24-hour blood pressure by Ambulatory Blood Pressure Monitor and did auricular acupuncture treatment for one day and rechecked blood pressure for 24-hours. We compared the blood pressure between, before, and after auricular acupuncture treatment by Wilcoxon test. The acupuncture points were KOHYOLABJUM, NAEBUNBI, SHINMUN, SHIM, KANGABJUM. They were all on the auricular. Results: After auricular acupuncture treatment, systolic and diastolic hypertension decreased significantly(p<0.05), but heart rates didn't change significantly. Conclusions: These results suggest that Auricular acupuncture treatment may be used for antihypertensive purpose. About the efficacy and safety, further study is needed.

Correlation between the morning hypertension on ambulatory blood pressure monitoring and the left ventricular mass in children

  • Purpose: Although high morning blood pressure (BP) is known to be associated with the onset of cardiovascular events in adults, data on its effects in children with hypertension are limited. Our retrospective study aimed to define the clinical characteristics of children with morning hypertension (MH) and to determine its associated factors. Methods: We reviewed 31 consecutive patients with hypertension, confirmed by the ambulatory blood pressure monitoring (ABPM). We divided these patients into 2 groups: the MH group (n=21, 67.7%), morning BP above the 95th percentile for age and height (2 hours on average after waking up) and the normal morning BP group (n=10, 32.3%). We compared the clinical manifestations, laboratory results, and echocardiographic findings including left ventricular hypertrophy (LVH) between the groups. Results: The early/atrial (E/A) mitral flow velocity ratio in the MH group was significantly lower than that in the normal morning BP group. In addition, LV mass was higher in the MH group than in the normal morning BP group, although the difference was not statistically significant. The age at the time of hypertension diagnosis was significantly higher in the MH group than in the normal morning BP group (P =0.003). The incidence of hyperuricemia was significantly higher in the MH group than in the normal morning BP group. Conclusion: Older patients and those with hyperuricemia are at higher risk for MH. The rise in BP in the morning is an important factor influencing the development of abnormal relaxation, as assessed by echocardiography. Clinical trials with longer follow-up periods and larger sample sizes are needed to clarify the clinical significance of MH.

Effects of Uwhangchungsimwon(牛黃淸心元) on Cerebral Blood Flow and Systemic Blood Pressure in Humans (우황청심원이 정상인의 뇌혈류 및 혈압에 미치는 영향)

  • Uwhangchungsimwon(UC) has been used in the treatment of a wide variety of conditions including stroke, hypertension, arteriosclerosis, autonomic imbalance, mental instablity, etc in Korean traditional hospitals, In particular it is often initialy chosen for emergency care of acute stroke. The aim of this study was to evaluate the effect of UC on cerebral hemodynamics. Using transcranial Doppler ultrasound, we studied changes of mean flow velocity and pulsatility index(PI) of middle cerebral arteries (MCAs) from 11 health young volunteers who were administrated with 1 pill UC and 11 health controls who were not. We obtained hypercapnia with breath-holding and evaluated cerebrovascular reactivity with breath-holding index(BHI). Systolic blood pressure, diastolic blood pressure, and heart rate were measured using ambulatory blood pressure monitoring(ABPM). In UC administration group, the evaluation was performed during basal condition. and repeated at 20, 40, and 60 min after administration. In controls, the evaluation was performed at corresponding time intervals. Mean flow velocity in middle cerebral artery, systolic blood pressure, diastolic blood pressure, and heart rate did not change during the observation period and were not different between these two groups. However, administration of UC was associated with decreases in PI by $3.6{\sim}12.4%$ in BHI by $17.9{\sim}24.8%$ compared with pre-administration period. Decreases in PI and BHI with UC were significantly different compared with control group (p<0.05). These results indicate that UC decreases PI and BHI in cerebral artery, which is due to a dilation of cerebral resistance vessels.

Effects of Venesection at the Sybsun-points on Blood Pressure and Body Temperature in Patients with Stroke (중풍환자에 있어 십선용사혈이 혈압 및 체온에 미치는 영향)

  • Background and Purpose: Sybsun-points are located at the tips of all fingers, 0.1chon from the finger nails, totaling 10 points on both hands. These points have been used for emergency care, fainting, epilepsy, cerebrovascular accidents, hypertension, unconsciousness, high fever etc. The most common technique is bleeding with a needle at these points. Hypertension and fever are the main factors for stroke patients’progress. We investigated whether venesection at Sybsun-points has effects on blood pressure and body temperature in stroke patients. Materials and Methods: 79 stroke patients were enrolled in this study from 1 Jan. 1999 to 30 Sep. 1999. All of them were admitted in Kyunghee University, Hospital of Oriental Medicine. Among them, 62 patients were hypertensive people, 17 were normal. Among the hypertensive patients, 27 were stage 1,20 were stage 2, 15 were stage 3 by classification of JNC 1997. All of the hypertensive patients had been taking drugs, while the normal group did not. From 2pm to 3pm, every 30 minutes we checked patients' blood pressure and body temperature by 24ABPM and tympanic thermometer. After 30 minutes passed, we phlebotomized patients Sybsun-points with Samneung needle 2-3cc of blood. Right after the bleeding, we checked blood pressure and body temperature. After the bleeding, for the next hour and a half, we checked each patient’s blood pressure and body temperature every thirty minutes, or a total of three times. We compared the blood pressure and the body temperature before and after treatment(p<0.05). Results: 1. Venesection at Sybsun-points significantly decreased systolic blood pressure on stage 3 hypertensive patients(p<0.0l). 2. Venesection at Sybsun-points significantly decreased diastolic blood pressure on stage 3 hypertensive patients(p<0.05) 3. Venesection at Sybsun-points had no effect on the change of body temperature. Conclusions: Though further study is needed, our findings suggest that Venesection at Sybsun-points may alleviate hypertension in stroke patients.
