Literature Review on Herbal Medicine Treatment of Psoriasis Based on Chronic Low-grade Inflammation Theory (만성 저등급 염증이론을 바탕으로 한 건선의 한약치료에 대한 문헌고찰)

  • Objectives : The aim of this study is to investigate the relationship between chronic low grade inflammation theory, psoriasis, and herbal medicine. Methods : We reviewed recent studies on the relationship between chronic low-grade inflammation, psoriasis, and herbal medicine through Pubmed. Results : The pathological basis for psoriasis is the action of inflammatory mediators by the activation of the immune response, which can be a cause of various cardiovascular, metabolic and psychological symptoms of psoriasis patients, in addition to skin lesions. The herbal medicines improve these inflammatory conditions and improve local lesions through herbal medicine such as Qingdai, which have a strong inhibitory effect on IL-17,22 production. Conclusions : Herbal medicines used in psoriasis are thought to be effective not only for the improvement of local psoriasis lesions through anti-inflammatory effect but also for the improvement of systemic inflammation associated with chronic low grade inflammation.

Postoperative radiotherapy for mucoepidermoid carcinoma of the major salivary glands: long-term results of a single-institution experience

  • Purpose: This study aimed to evaluate the long-term survival outcomes and prognostic factors that affect the clinical outcomes of patients who underwent surgery and postoperative radiotherapy for major salivary gland mucoepidermoid carcinoma (MEC). Materials and Methods: We retrospectively reviewed the clinical data of 44 patients who underwent surgery followed by radiotherapy for primary MEC of the major salivary glands between 1991 and 2014. The median follow-up period was 9.8 years (range, 0.8 to 23.8 years). Results: The overall outcomes at 5 and 10 years were 81.5% and 78.0% for overall survival (OS), 86.2% and 83.4% for disease-free survival, 90.6% and 87.6% for locoregional recurrence-free survival, and both 90.5% for distant metastasis-free survival (DMFS). Histologic grade was the only independent predictor of OS (low vs. intermediate vs. high; hazard ratio = 3.699; p = 0.041) in multivariate analysis. A poorer survival was observed among patients with high-grade tumors compared with those with non-high-grade tumors (5-year OS, 37.5% vs. 91.7%, p < 0.001; 5-year DMFS, 46.9% vs. 100%, p < 0.001). Conclusion: Surgery and postoperative radiotherapy resulted in excellent survival outcomes for patients with major salivary gland MEC. However, high-grade tumors contributed to poor DMFS and OS. Additional aggressive strategies for improving survival outcomes should be developed for high-grade MEC.

Relationship between Characteristics and Difference by the Grade of Stress in the Nursing Students - Effect of National Examination on Stress - (일 간호대학생의 학년별 스트레스 정도와 관련요인)

  • The purpose of this study was to discuss the difference of stress by the grade, examine the relationship between characteristics and main stress, and to investigate the effect of the National Examination of Registered Nurses on stress by using the stress self-rating scale targeting university nursing students (n=314), The stress self-rating scale consists of three subscales: stress responses, stressors and coping parts, such as perceived social support and a sense of humor. The data were analyzed by descriptive statistics, t-test, Pearson's correlation coefficient and regression analysis. The results showed that passing the National Examination of Registered Nurses was the first problem among the stressors. Stressors of the living factors and nursing factors, and subjective stressors of the surveyed nursing students were the highest among the senior students, but stressors of nursing factors increased according to the grade of the nursing students. Perceived stress responses were high in psychological responses of other aspects, and perceived stress responses increased according to the grade (p<.001). Senior students had a high weight of perceived stress responses (problem-focused, emotion-focused). and active and negative coping modes. Significant factors that explained the stressors were coping mode(r=.62-82. p<.01) and cognition(r=.19. p<.01). including the social support. These findings suggest that social support was very important for seniors. and that we have to develop approaches to decrease stress according to the student grade for nursing education process.

Scoliosis Progression according to the Growth of Middle School Students (성장에 따른 중학생 척추측만증 이행양상)

  • Purpose: This study was conducted to investigate the progression of Cobb's angle of middle school students according to the Risser grade changes during a one-year follow up period, and to identify the prevalence rate and related factors regarding scoliosis. Method: A total of 2,124 middle school students were preliminarily screened by the Adam's forward bending test. Among them, 21 adolescents diagnosed with scoliosis (above 5Cobb's angle) by X-Raying were the subjects and followed up for checking Cobb's angle and the Risser grade for one year. Results: The overall prevalence of scoliosis in middle school students was 2.13% and the prevalence in girls was higher than in boys. The Cobb's angle was significantly high in seniors and girls. The cases in low angle and low Risser grade demonstrated significant changes in progressive Cobb's angle compared to the cases in high angle and high Risser grade after one-year. But gender and pattern of curvature were not significantly different from the changes scoliosis angle after one-year. Conclusion: Based on these study results, juniors in low Risser grade were inclined to aggravate scoliosis despite the low degree of curvature at the first measurement.

Perception and Satisfaction with Health Education of Elementary Students (초등학생의 보건교과 수업에 대한 인식과 만족도)

  • Purpose: The aim of this study was to evaluate perception and satisfaction with Health Education for elementary school students in grade 5 and 6, using data on national health education gathered since March, 2009. Methods: Data were collected by way of a questionnaire, from 475 elementary school students in grades 5 and 6 from February 1~15, 2010. Statistical analysis of collected data was performed using frequency, t-test, Pearson's correlation coefficient, and hierarchical multiple regression analysis, using SPSS 18.0 software. Result: Levels of health promoting behavior, interest, importance, and satisfaction were relatively high, respectively, and those of students in grade 5 were higher than those of students in grade 6. Levels of Satisfaction with frequency and educational environment were relatively low. Among the factors found to influence satisfaction with health education, interest in health education was found to be the most influential(grade 5:${\beta}$=.279, p<.001, grade 6:${\beta}$=.306, p<.001). Conclusion: Students expressed a high level of satisfaction with regular health education. Expansion of health education frequency and health education-only-classrooms is needed. To improve the effect of health education, constant acquisition of knowledge of subjects' perception and satisfaction is needed. In addition, contents and teaching methods that showed low level of satisfaction should be reformed.

Chest Wall Contouring of Poland's Syndrome (폴란드 증후군 환자의 가슴윤곽 재건)

  • Purpose: Poland's syndrome encompasses a constellation of congenital chest wall, breast, and upper extremity deformities. We would like to present several techniques, which may be combined if necessary, used to treat the forms involving both the breast and chest wall according to the degree of deformity. Methods: In a retrospective series of 9 patients (3 men and 6 women), we report our experience with reconstructing breast and chest contour deformities associated with Poland syndrome. We recorded their age, gender, the surgical techniques, and the grade in Poland's syndrome according to the classification of Foucras. Results: The breast and chest wall deformities associated with Poland syndrome can be treated in individualized fashion according to the classification of Foucras. In case of 3 male patients with gradeI, II, the latissimus dorsi muscle pedicled flap improved the chest contour deformity. 3 female patients with grade II underwent the latissimus dorsi muscle pedicled flap with breast implant. 2 female patients with gradeIunderwent breast reconstruction with breast implant and fat injection each other. One female patient with severe chest wall deformity (grade III) underwent breast reconstruction using the free TRAM flap. All patients were satisfied with the results without specific complications. Conclusion: The Individualized correction for this syndrome according to the degree of patient's deformity and preference made the overall satisfaction of the patients high.

Control of Grade Change Operations in Paper Plants Using Model Predictive Control Method (모델예측제어 기법을 이용한 제지공정에서의 지종교체 제어)

  • In this work an integrated model for paper plants combining wet-end and dry section is developed and a model predictive control scheme based on the plant model is proposed. Closed-loop process identification method is employed to produce a state-space model. Thick stock, filler flow, machine speed and steam pressure are selected as input variables and basis weight, ash content and moisture content are considered as output variables. The desired output trajectory is constructed in the form of 1st-order dynamics. Results of simulations for control of grade change operations are compared with plant operation data collected during the grade change operations under the same conditions as in simulations. From the comparison, we can see that the proposed model predictive control scheme reduces the grade change time and achieves stable steady-state.

Case Study of Atrophic Acne Scar Treatment with Re-cell Therapy (리셀테라피를 이용한 위축성 여드름 흉터 치료 11례)

  • Objective: Acne scarring is the most common sequela after dermal inflammation caused by acne. Many methods are currently used to treat acne scarring; doctors using traditional Korean medicine have also attempted to treat acne scarring in a new way that was developed from traditional methods. This study examines the effectiveness of re-cell therapy.Method: Eleven patients with various types of acne scars were treated by re-cell therapy. We used the global acne scarring classification (GASC) and ECCA (from the French “echelle d’evaluation clinique des cicatrices d’acne”), both of which are generally used to measure and grade the degree of acne scarring.Results: As a result of the treatment, the conditions of all patients were conspicuously improved in both scales. The GASC scores of the patients changed from grade 19.6±10.4 to grade 4.9±3.3 (p=0.001), and the ECCA scores changed from grade 120.9±28.7 to grade 41.8±18.3.Conclusion: Re-cell therapy showed effectiveness in treating acne scarring and could be applied in clinical cases.

A Case of Combined Korean Medicine Treatment for Recurrent Limb Weakness after Guillain-Barré Syndrome Improvement: Case Report (길랑바레 증후군 호전 이후 재발한 사지무력 증상에 대한 한방 복합치료 1예: 증례보고)

  • The objective of this study is to propose Korean Medicine treatment for recurrent limb weakness after Guillain-Barre syndrome (GBS) improvement by intraveinous immunoglobulin, and to report its effectiveness. Manual muscle test (MMT), Korean modified Bathel index (K-MBI), and tendon reflex were used to evaluate the patient. The patient was improved hip joint, knee joint, ankle joint MMT from grade 3-/3- to grade 5/5 and in the upper limb the patient can do big joint exercise but cannot do micromovement like writing or using cell phone. When discharge date the patient's wrist joint MMT grade is improved grade 5-/5- to grade 5/5. The K-MBI score is improved from 71 to 86 and there was a big change in walking and chair/bed transfer, there was no change in tendon reflex. This study suggests that Korean Medicine can be effective for patients who have recurrent limb weakness after GBS improvement.

Identification of Pb-Zn ore under the condition of low count rate detection of slim hole based on PGNAA technology

  • The grade analysis of lead-zinc ore is the basis for the optimal development and utilization of deposits. In this study, a method combining Prompt Gamma Neutron Activation Analysis (PGNAA) technology and machine learning is proposed for lead-zinc mine borehole logging, which can identify lead-zinc ores of different grades and gangue in the formation, providing real-time grade information qualitatively and semi-quantitatively. Firstly, Monte Carlo simulation is used to obtain a gamma-ray spectrum data set for training and testing machine learning classification algorithms. These spectra are broadened, normalized and separated into inelastic scattering and capture spectra, and then used to fit different classifier models. When the comprehensive grade boundary of high- and low-grade ores is set to 5%, the evaluation metrics calculated by the 5-fold cross-validation show that the SVM (Support Vector Machine), KNN (K-Nearest Neighbor), GNB (Gaussian Naive Bayes) and RF (Random Forest) models can effectively distinguish lead-zinc ore from gangue. At the same time, the GNB model has achieved the optimal accuracy of 91.45% when identifying high- and low-grade ores, and the F1 score for both types of ores is greater than 0.9.
