디지털 방사선 시스템은 영상의학에서 큰 비중을 차지하고 있다. 잘못된 영상이 제공 된다면 이는 환자의 건강에 나쁜 영향을 줄 수 있기에, 올바른 디지털 방사선 영상이 제공되어야만 한다. 또한, artifact는 오진으로 이어질 수 있다. 디지털 방사선 시스템에서 발생하는 artifact를 종류별로 분석하여 그 결과 분석하였다. 본 연구에서 사용된 artifact는 서울의 종합병원 급 의료기관에서 2007년부터 2014년까지 수집된 자료들이다. 수집된 자료는 발생 원인별로 구별하여 그 원인을 분석하였다. artifact는 하드웨어적 artifact, 소프트웨어적 artifact, 사용자 오류, 시스템 artifact 및 기타로 구분하였다. 하드웨어적 artifact는 Ghost가 가장 빈번하게 관찰되었으며, 이는 신호의 잔류에 의한 것이다. 다음은 RF 잡음에 의한 오류, 장비 내 이물질에 의한 오류 순이다. 소프트웨어 artifact는 많은 원인이 있다. 부정확한 영상 교정에 의한 artifact가 가장 많았으며, EDR 인식오류, 접합면 처리 오류 등이 있으며, 소프트웨어 artifact는 매우 다양한 경향을 나타낸다. 사용자 오류는 디지털 의료 영상 시스템을 바르게 이해하지 못해 발생시킨 것들이 많았다. 아울러, 시스템 artifact는 DICOM 헤더 정보 오류, 압축 오류가 있다. 분명하게 나타나는 artifact는 재촬영의 원인이 되어 환자의 피폭을 증가시키고, 불분명하게 나타나는 artifact는 오진을 유발 시킬 수 있다. 따라서 방사선사에게서 이러한 artifact를 바로 구별 할 수 있는 능력이 요구 된다. 그러므로 artifact가 발생되는 원인과 그 특성을 분명하게 이해함으로 지속적인 교육과 안정적인 시스템 운영에 힘써야 할 것으로 사료된다.
본 연구는 가상현실 프로그램을 사용한 재활치료의 효과성에 대해 체계적으로 분석하여 국내 임상현장에서 재활치료에 종사하는 의료인, 의료기사 및 연구자들에게 다양한 가상현실 기반의 재활치료와 효과성에 대한 정보를 제공하여 가상현실 기반의 재활치료가 임상에서 적절하게 적용될 수 있도록 하고자 한다. 연구 방법으로 학술연구정보서비스(Research Information Sharing Service; RISS), Google Scholar, Pubmed 데이터베이스를 사용하여 2007년 1월부터 2017년 12월까지의 논문을 검색하였다. 주요 검색어는 'Virtual Reality AND Rehabilitation', 'Virtual reality AND Effect'를 사용하였다. 최종적으로 10편의 해외 학술지에 게재된 논문을 본 연구의 분석 대상으로 선정하였다. 선정된 논문은 PICO (Patient, Intervention, Comparison, Outcome) 형식을 사용하여 정리하였다. 연구 결과 10개의 논문에서 참여한 대상자를 유형별로 분석한 결과, 대상자는 총 4가지로 분류되었으며 그 종류는 뇌졸중, 뇌종양, 파킨슨병, 신장질환이었다. 중재에 사용한 가상현실 기반 도구들은 IREX 3건(30%), X-box Kinect 2건(20%), Nintendo Wii 2건(20%), [Existing tool + VR]이 1건(10%), [Mobile application + VR]이 1건(10%)이었다. 중재 결과 10개의 논문 모두 기능향상이 나타났으며 신체기능이 24회(66%), 정신기능 6회(17%), 인지기능 5회(14%), 일상생활활동이 1회(3%)였다. 따라서 본 연구는 가상현실 기반 재활치료와 그 효과를 정리하여 재활치료서비스의 전문성과 질을 향상시키고, 영역을 확대하는 기초자료로 활용될 수 있을 것이다.
Food allergies play an important role in Atopic Dermatitis (AD). Dietary manipulation is essential in the management of AD. However, there has been a paucity of data reporting the prevalence of food allergies in AD patients in Korea. In this study, the Food Open Challenge Tests (FOCT) were conducted to investigate food allergies in AD patients. The skin-prick test and the detection of specific IgE, as well as allergy history of patient were used to detect food allergies in all AD patients. Elimination diet was conducted for two weeks prior to FOCTs. The prevalence of food allergies by FOCT is as follows: milk (67.3%); chicken (64.2%); pork (62.8%); eggs (61.0%); beef(55.4%) ; wheat (52.0%) and soybean (45.2%). Allergenic food items in Korean AD patients were different from people in other foreign countries. The seven major foods those tested positively by FOCTs were completely eliminated from the replaced diets for two weeks, and were subsequently reintroduced one at a time. Results from FOCTs were not comparable with allergy history or skin-prick tests or specific IgE detection. The sensitivity and specificity of skin-prick tests and specific IgE detection were lower than FOCTs. Allergy history, skin-prick tests, and specific IgE detection are useful for the identification of food allergen but its clinical significance differed according to food items. Therefore, we conclude that even though a 10-day delay was necessary after food challenge, FOCT is a useful and valid method to confirm food allergies and may be essential for the effective control of food allergies for treatment of AD.
Globus sensation is a continuous, intermittent, non-painful sensation of lumps or foreign substances in the throat. It was reported that 13.7% of patients developed globus sensations after COVID-19 infection. A 73-year-old male patient with a globus sensation after a COVID-19 infection was admitted to the ◯◯ Oriental Medicine Hospital and treated with herbal medicine (Banhahubak-tang, Sosiho-tang). Subjective symptoms of globus sensation were measured with the Numerical Rating Scale (NRS) daily and found to reduce. The effect persisted during his admission. In conclusion, Korean medicine, including Banhahubak-tang, can be effective for patients with globus sensations after COVID-19 infections.
Eagle's syndrome is the manifestation of elongated styloid process that is not a common entity in dental clinics. Without the knowledge of this syndrome misdiagnosis can be made and therefore differential diag-nosis with other diseases of oral and maxillofacial area is required. The symptoms of elongated styloid process are cervical pain, foreign body sensation, dysphagia and pharyngeal pain. A 27 years old male vis-ited our hospital. He had cervical pain with unilateral facial nerve palsy. Along with meticulous clinical examinations, 3D-CT, Plain radiographic studies were used to make the diagnosis. In computed tomogra-phy, Lt. styloid process was elongated and fractured. Patient have undergone surgical resection of elongat-ed styloid process and have been carefully observed. Characteristically, preoperative facial nerve palsy showed rapid postoperative recovery. The other clinical symptoms that were present in the initial visit were diminished. The purpose of this article is to report a case of Eagle's syndrome presenting unilateral facial nerve palsy that was treated with transoral surgical approach with literature review.
충남대학교 병원 흉부외과에서 1985년 6월부터 1995년 10월까지 식도 천공 15례를 경험하였다. 남녀 비는 9 대 6 이 었으며, 연령 분포는 19세 에서 71세 (평균 49세)까지 였다. 식도 천공 원인은 다양하여, 특 발성 및 식도이물 섭취로, 천공된 례가 각각 4례, 기구 조작에 의한 경우 3례, 흉부 둔상 1례, 약물(클로 르칸키) 섭 취 1례, 기관 절개술시 식도 손상 1례, 원인 불명의 천공 1례가 있었다. 천공 부위는 흉부 식도 9례, 경부 식도 6례 였다. 증상은 동통 11례, 발열 9례, 연하 곤란 Bfl, 호흡 곤란 5례 등이었다. 수술은 경 부 식도 천공의 경우 절개 및 배액술, 단순 봉합, 위루술등, 흉부 식도 천공의 경우 단순 봉합, 단순 봉합 및 흥막 보강술, 위루술 등을 시행하였다 1례가 사망하였으며, 사인은 패혈증이 었다.
Objectives : There have been several studies about the quality of acupuncture needle tip recently. We have investigated the condition of the tip of the acupuncture needles in the last studies. In the former studies, we discovered the metallic scuff, lumps and irregularities of the acupuncture needle tips under the microscope. But, no information was available on those foreign materials' identity. Methods : We have selected 200 needles of 1000 pieces from several companies by randomized methods. And we observed the tip of the 6 needles selected finally at ${\times}1000\;or\;{\times}3000$ magnification and analyzed the components of the metallic scuff, lumps and irregularities of the needle tips with a SEM-EDX analyser. Results : We found that the identity of the metallic scuff, lumps and irregularities of the needle tips were metallic materials and silicon. For example, A point was composed of Fe(69.78%), Cr(17.71%), Ni(8.11%), Zn(2.04%), Si(1.23%), Mn(1.12%), and B point was composed of Si(66.40%), Fe(26.76%), Cr(6.84%). Conclusion : The results of this study confirm that there is a real possibility of the remaining of metallic materials and silicon in body of patient, after acupuncture treatment. Therefore, it is necessary to intensify our efforts to make needles of good quality and to concentrate on manufacturing process of acupuncture needles completely to be free from danger in acupuncture treatment.
Bezoars are collections or concretions of indigestible foreign material that accumulate and coalesce in the gastrointestinal tract; they usually occur in patients who have undergone gastric surgery and have delayed gastric emptying. Treatment options include dissolution with enzymes, endoscopic fragmentation with removal or aspiration, and surgery. Recently, the efficacy of nasogastric lavage or endoscopic infusion of Coca-Cola for the dissolution of phytobezoar have been reported. We report a case of phytobezoar successfully treated by oral administration and endoscopic injection of Coca-Cola. A 62-year-old woman was referred to Yeungnam University Hospital for epigastric pain. Upper gastrointestinal endoscopy revealed one very large, dark-greenish, solid bezoar in the stomach with gastric ulcer and duodenal bulb deformity. We performed endoscopic injection of Coca-Cola into the bezoar. The patient was instructed to drink four liters of Coca-Cola per day. At endoscopy two days later, the phytobezoar was easily broken into pieces. At endoscopy on the $11^{th}$ day of admission, the phytobezoar was decreased in size and removed by endoscopic fragmentation with a polypectomy snare. At follow up endoscopy after 13 days, the bezoar was completely dissolved.
Objective : World Health Organization and International Labour Organization, in June 8, 1999, requested that working conditions should be reformed ethically right and economically solid for 2.7 billion world labour force. The purpose of this study is to request compensable surgical therapeutic options and to suggest the renewal in cognizance criteria for worker's compensation, especially in spondylolisthesis. Methods : Regarding spondylolisthesis, we obtained data from Korea Labor Welfare Corporation(KLWC). Spinal disease occurrence incidences and medical fees of National Federation of Medical Insurance(NFMI) were analysed. The compensated 122 spinal instrumented cases included 117 male and 5 female patients, aged from 23 to 72 years old(mean : $45{\pm}9.85$). We compared Korean and Foreign Workers Compensation Law. Results : Numbers of herniated nucleus pulpus(78 cases), spinal fractures(34 cases) and dislocations(4 cases) were claimed after spinal interbody fusion operation and were compensated. These compensated degenerative diseases, work related illness, occurred in the course of work. A case of 52-year old spondylolisthesis patient with Disability Grade 8 was compensated by KLWC, according to its occurrence at work by accident. With exception of trauma at work by accident, current cognizance criteria were too narrow to be compensable, especially in surgical therapeutic option, for worker's spondylolisthesis. Conclusion : Considering both worker's compensation law and clinical pathologic progress, we believe that spondylolisthesis should be regarded as a compensable occupation related disease if and when aggravates rapidly in the course of work. We suggest a new cognizance standard to KLWC for labour welfare and proper worker's compensation.

Obstructive sialadenitis, one of the diseases that most frequently causes swelling and pain in the salivary glands, is mainly caused by structural obstructions. Sialolithiasis is the most frequent cause of the disease, and other causes include calculus formation, duct strictures, foreign bodies, and anatomical variations. Although there is a possibility that facial fillers directly block the salivary ducts, no cases of obstructive sialadenitis associated with them have been reported yet. We report the case of a 34-year-old female patient who complained of recurrent swelling and pain in the left buccal mucosa. She had undergone facial filler injection procedures on her facial area for cosmetic purposes several years before. Based on the findings of magnetic resonance imaging (MRI) and MR sialography, she was diagnosed with obstructive sialadenitis due to facial fillers. Through this case, we should remember to obtain a thorough history including filler treatments in the case of parotid gland swelling. We also suggest proper utilization of advanced imaging such as MRI in evaluating the location of facial fillers.
