직장암 환자에서 Oxaliplatin 투여로 인한 혈관부종 발생 증례 보고 (Oxaliplatin Induced Angioedema in a Rectal Cancer Patient: A Clinical Case Report)

  • We report an unusual case of angioedema induced by intravenous oxaliplatin in a patient with rectal cancer. Intravenous oxaliplatin was administered to the patient according to FOLFOX regimen (oxaliplatin/leucovorin/5-fluorouracil) for the management of his colorectal carcinoma. The patient had been on FOLFOX regimen for several months without reporting any adverse effects, but he experienced angioedema suddenly during his $9^{th}$ cycle of chemotherapy. Angioedema was observed about 2 hours after the initiation of oxaliplatin infusion. Oxaliplatin was stopped then the patient became stable. This case report demonstrates that intravenous oxaliplatin may induce angioedema several hours after the initiation of oxaliplatin infusion in later cycles of chemotherapy.

Successful endovascular embolization for traumatic subcutaneous abdominal wall hematoma via the superficial inferior epigastric artery: a case report

  • Abdominal wall hematoma (AWH) after blunt trauma is common, and most cases can be treated conservatively. More invasive treatment is required in patients with traumatic AWH if active bleeding is identified or there is no response to medical treatment. Herein, we report a case of endovascular embolization for traumatic subcutaneous AWH. Almost endovascular treatment for AWH is done through the deep inferior epigastric artery. However, in this case, the superficial inferior epigastric artery was the bleeding focus and embolization target. After understanding the vascular system of the abdominal wall, an endovascular approach and embolization is a safe and effective treatment option for AWH.

소양인 족번열에 대한 숙지황고삼탕 치험 1례 (A Case Report of Soyangin Burning Feet Syndrome Treated with Sukjiwhanggosam-tang)

  • Introduction This is the first case report of a soyangin suffering from burning feet syndrome who reported a significant improvement after taking Sukjiwhanggosam-tang. Case presentation The patient complained of hot feet, pain and numbness that occurred during the night. The patient was diagnosed with 'Yin deficiency diurnal fever disease' and treated with Sukjiwhanggosam-tang as well as acupuncture and cupping therapy. The degree of improvement in heat sensation, pain and numbness was expressed in numeric rating scale compared to the first visit. Conclusion After treatment, the heat sensation, pain and numbness of feet was decreased significantly, and the condition improved much at night.

특발성 양측성 안면신경 마비 환자에 대한 한의치료 증례보고 1례 (A Case Report of Traditional Korean Medicine Treatment for a Patient with Idiopathic Bilateral Facial Nerve Palsy)

  • Objectives: This case report presents the effects of traditional Korean medicine on a patient with idiopathic bilateral facial nerve palsy. Methods: Two evaluation scales, the House-Brackmann (H-B) scale and the Yanagihara scale, were used to evaluate the symptoms of bilateral facial palsy. Herbal medicine and acupuncture treatment were administered to improve the patient's symptoms in parallel with Western oral medication (Valvirus Tab 500 mg, Solondo 5 mg, Lipitor 20 mg, Almagel 15 ml, Lanston 15 mg). Results: Before treatment, the patient had severe facial paralysis with an H-B scale of 5/5 and a Yanagihara scale of 3/2 on both sides. However, after treatment, the symptoms improved significantly with an H-B scale of 2/3 and a Yanagihara scale of 31/26. Conclusions: This case report shows that traditional Korean medicine in parallel with Western oral medication can be effective in the treatment of idiopathic bilateral facial nerve palsy.

  • Objectives: This case is to report the effect of TMJ balancing therapy (TBT) and combined Korean Medicine (CKM) on a 15-year-old male patient with suspected cervical dizziness of unknown causes. Methods: He was treated with TBT and CKM included Yin-yang Balancing Therapy of TMJ, pelvic balancing therapy, cervical chuna therapy once a week for 3 months. He continued to wear accurate balancing appliance (ABA) in the mouth, did 12-way cervical gravity straightening exercise and took banhabakchulchunma-tang-gamibang for 3 months. The effects of treatment was evaluated using the numerical rating scale (NRS). Results: After the treatments, dizziness was relieved from NRS 7 to NRS 1. Conclusions: This case report suggests that TBT and CKM treatments can relieve suspected cervical dizziness of unknown causes.

A Case Report of Madelung's Disease

  • Madelung's disease (MD) is a rare disease characterized by diffuse, nonencapsulated, multiple fat masses in different areas of the body. In this case report, we present a case of MD in Asia and its management. A 66-year-old man with a history of hypertension presented with massive growth of soft tissue around the neck, breasts, upper back, and lower abdomen. Preoperative magnetic resonance imaging revealed remarkably hypertrophic fat tissue around the neck and anterior chest was wall, which consistent with the diagnosis of MD. Multiple linear incisions were made on the neck and 763, 186, 635 g of posterior, right, and left fat tissues were excised, respectively. A single wide, transverse incision was done to excise 1,072 g of fat from the upper back. Masses of both breasts were excised, preserving the inferior pedicle, weighing 1,086 (right) and 1,164 g (left). The recovery was optimal and the patient was discharged without complications. In this case, we excised the adipose masses as much as possible and improved contour and symmetry. However, the fat infiltrations in the patient were diffusely distributed, making total fat excision difficult. This rare case report may help in managing patients with MD.

  • Thoracic outlet syndrome presents with symptoms resulting from pressure on either the subclavian vessels or the lower trunk of the brachial plexus. It may be caused by a number of abnormalities, including degenerative or bony disorders, trauma to the cervical spine, fibromuscular bands, vascular abnormalities, and spasm of the anterior scalene muscle. We experienced a case of thoracic outlet syndrome [ caused by cervical rib .We report a case with review of literatures.

장기간 호전되지 않던 수술 후 유착성 소장폐색이 침구 치료를 통해 치료된 1례 : 증례보고 (Postoperative Adhesive Small Bowel Obstruction Treated using Acupuncture and Moxibustion: A Case Report)

  • Objectives: This report presents a case using acupuncture and moxibustion to treat postoperative adhesive small bowel obstruction. Case Presentation: A 62-year-old male with two previous intra-abdominal surgeries, complaining of abdominal pain, abdominal distention, and general weakness, visited Korean medicine hospital. He was suffering from small bowel obstruction that had not improve for three weeks. Methods: The patient was diagnosed with postoperative adhesive small bowel obstruction. The patient received acupuncture and moxibustion in addition to intravenous fluid treatment. The patient underwent radiologic testing on a daily basis. Results: The symptoms improved quickly with the treatment, including acupuncture and moxibustion. He was able to start eating three days after he started receiving treatment. The abdomen X-ray also showed rapid improvement. No adverse effect was observed during the nine days of hospitalization. Conclusions: This report demonstrates that acupuncture and moxibustion may be effective in treating adhesive small bowel obstruction. However, further research is needed to confirm these findings.

한약으로 관해가 유도된 극심한 설사의 궤양성대장염 환자 증례보고 (Remission of ulcerative colitis with severe diarrhea by herbal medicine treatment: A case report)

  • Ulcerative colitis (UC) is one of inflammatory bowel diseases (IBD) with unknown etiopathogenesis. This case report details remission of UC induced by Korean herbal medicine treatment. A 31-year-old male patient diagnosed as UC after a series of examinations including endoscopy and fecal calprotectin (FC) test. He had severe bloody diarrhea over 10-20 times a day despite taking 5-aminosalicylic acid and steroid. He was given individualized Korean herbal medicine Dowha-tang according to pattern identification. After taking Dowha-tang for 7 months, clinical remission was achieved. The symptoms disappeared and FC level went down to normal level. No adverse events were reported. This case report shows that Korean herbal medicine treatment may have the potential for clinical remission of UC. Further investigation is warranted.

한의학적 치료로 호전된 전이성 대장암 환자의 복통에 대한 증례보고 (A Case Report of Abdominal Pain Relief in a Patient with Metastatic Colorectal Cancer Treated with Korean Medicine)

  • Objectives: The aim of this case report is to present abdominal pain relief in a patient diagnosed with colorectal cancer with several organ metastases. Methods: The patient was treated with herbal medicine, acupuncture, and moxibustion. The effect of the treatment was measured by a numeric rating scale (NRS). Results: The NRS of the patient's abdominal pain was reduced after Korean medicine treatment. Conclusions: This case report suggests that Korean medicine may be effective for treating abdominal pain in cancer patients. However, additional study is needed to confirm interactions between Korean medicine and the control of abdominal pain.
