The Sub-Peres Functions for Random Number Generation (무작위수생성을 위한 부 페레즈 함수)

  • We study sub-Peres functions that are defined recursively as Peres function for random number generation. Instead of using two parameter functions as in Peres function, the sub-Peres functions uses only one parameter function. Naturally, these functions produce less random bits, hence are not asymptotically optimal. However, the sub-Peres functions runs in linear time, i.e., in O(n) time rather than O(n logn) as in Peres's case. Moreover, the implementation is even simpler than Peres function not only because they use only one parameter function but because they are tail recursive, hence run in a simple iterative manner rather than by a recursion, eliminating the usage of stack and thus further reducing the memory requirement of Peres's method. And yet, the output rate of the sub-Peres function is more than twice as much as that of von Neumann's method which is widely known linear-time method. So, these methods can be used, instead of von Neumann's method, in an environment with limited computational resources like mobile devices. We report the analyses of the sub-Peres functions regarding their running time and the exact output rates in comparison with Peres function and other known methods for random number generation. Also, we discuss how these sub-Peres function can be implemented.

Comparision of the Improvement of Back pain and Sciatica between Common Acupuncture treatment Group and Common Acupuncture with Shin Jong Gyuk of Ohaeng Acupuncture treatment Group (요통(腰痛) 및 요각통(腰脚痛) 환자(患者)의 체침요법(體鍼療法)과 오행침법(五行鍼法)의 현정격병용(賢正格倂用)에 의한 증상호전 비교)

  • Objective : The purpose of this report is to use more Shin Jong Gyuk of Ohaeng Acupuncture acupunture treatment clinically by comparing the Improvement of Back pain and Sciatica between Common Acupuncture treatment Group and Common Acupuncture with Shin Jong Gyuk of Ohaeng Acupuncture treatment Group. Methods : This study was carried out on 40 patients with Back pain and Sciatica who were hospitalized in Dongshin Univ. Oriental Hospital from 1st, April, 2001 to Tenth, November, 2001. Group A of 20 patients were taken both common acupunture treatment and Shin Jong Gyuk of Ohaeng Acupuncture treatment. And Group B of 20 patients were only taken common acupunture treatment. And after 10 days of admission, we checked and compared VAS(Visual Analogue Scale) and improvement index out of these two groups. SPSS(Statistical Program for Sdcial Science) for Window was used for a statistical analysis and the independent T-test was performed to gauge the improvement of VAS(Visual Analogue Scale) and improvement index out of these two groups, in which case, value of P below 0.05 is considered as useful. Results : The result of the VAS(Visual Analogue Scale) and improvement index of comparision analysis between two groups after 10 days, shows that the improvement of Back pain and Sciatica in Common Acupuncture treatment with Shin Tong Gyuk of Ohaeng Acupuncture treatment Group is more effective than that in Common Acupuncture treatment Group. (independent T-test improvement index P=0.21, VAS P=0.00)

  • When open reduction of maxilla fractures is postponed due to concurrent life-threatening injuries, delayed union may result with malunion or nonunion. If delayed malunion is occurred, significant facial deformity may result, including a dished-out face, irregular retromaxillism with Angle's class III malocclusion, open anterior bite, nasal collapse, telecanthus and malar flattening. The treatment planning for this problem includes cephalometric evaluation anterior and lateral tomograms, dental casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue attachment at the fracture site. In this paper, one case presented a 58-year-old female patient with maxilla retrusion after comminuted fracture, who was treated with orthodontic methods of maxillary protraction headgear and Plaster headcap, whereas the other two cases were about male patients who were treated principally with surgically open reduction or Le Fort I-controlled transverse osteotomy with iliac bone graft.

CORRECTION OF SECONDARY CLEFT-LIP NASAL DEFORMITY BY USING ABBE FLAP: REPORT OF 4 CASES (Abbe 피판을 이용한 이차성 구순열비변형의 교정 4예)

  • Radical paring of the cleft edge during primary cleft lip operation or repeated secondary surgeries can result in tightness of the upper lip. In case, the degree of the resulting side-to-side tension is very severe, the possibility of a lip switch flap must be considered. When the lip tightness accompanies a loss of more than two-thirds of the Cupid's bow, an Abbe flap is an alternative. The disadvantages of Abbe flap are scar formation on the lower lip, design of incision line on the upper lip, disharmony of colors, and the dysfunction of the orbicularis muscle. These problems have been recognized in the literature and extreme discretion has been advised in its application. We experienced four cases of Abbe flap operation which were designed differently to correct the secondary unilateral or bilateral cleft-lip nasal deformities. The Abbe flap operations resulted in removal of the scars and tightness of the upper lip, reconstruction of the Cupid's bow, lengthening of the columella, and therefore secondary cleft-lip nasal deformity could be corrected. It is thought that carefully applied Abbe flap is an appropriate method to relieve horizontal tightness or flattening of the upper lip which occured after primary operation of cleft lip.

  • We report a case of pulmonary adenocarcinoma complicated by the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) following adjuvant chemotherapy. A 51-year-old woman with stage IIIA adenocarcinoma received left lower lobe lobectomy in July, 2006. And then combination chemotherapy with paclitaxel and cisplatin was given to the patient. In five days after completion of second cycle of the chemotherapy, she visited emergency room because of general weakness and seizure. Her brain MRI was shown to be no evidence of brain metastasis. Serum sodium, urine and plasma osmolarities were 117mEq/L, 589 and 244mOsm/kg, respectively. She was improved with fluid restriction. Although occurrence of SIADH following chemotherapy is rare, physician should give an attention the potential for development of SIADH in the course of chemotherapyin non-small cell lung cancer patient.

  • Excessive occlusal wear can result in unacceptable damage to the residual teeth structure, anterior guidance and masticatory function. An 86-year-old man presented with worn dentition and anterior deep bite due to loss of the posterior support. Two implants covered by Korean national health insurance, fixed partial denture and direct composite resin were used to restore the dentition, instead of removable prosthesis. The occlusal vertical dimension was increased by 2 mm in the premolar area and shortened dental arch concept was applied. This treatment was a less expensive alternative to full-mouth rehabilitation with positive esthetic and functional outcomes after 7 months of follow-up.

  • The goal of suction denture is to enhance retention of dentures by the formation of negative pressure between the denture base and the underlying tissue and sealing around the denture by the mucosa. This patient has been converted to a completely edentulous state after the extraction of residual teeth. Fabrication of suction denture was planned because it was estimated that the conventional compete would be hard to achieve retention due to the absorption of residual ridge and lack of sublingual fold. Through appropriate clinical and laboratory technique such as preliminary impression on the mandibular rest position, provisional vertical dimension determination with Centric Tray® (Ivoclar Vivadent AG, Schaan, Liechtenstein), closed mouth definitive impression and jaw relation record using individual tray with Gnathometer M® (Ivoclar Vivadent AG, Schaan, Liechtenstein), artificial teeth arrangement considering stability of the denture, and proper polymerization technique that minimizes polymerization shrinkage, restoring the patient with suction denture resulted in satisfaction throughout the function and asesthetics.

A Case Report of ROSC for Out-of Hospital Cardiopulmonary Resuscitation: Based on one Area Heart Saver (병원 전 심폐소생술에 의한 자발순환 회복 8례: 일 지역 하트세이버 수여자를 기준으로)

  • This study was to investigate the cardiopulmonary resuscitation (CPR) outcomes and the clinical characteristics of outof- hospital cardiac arrest (OHCA) by analyzing date in one region. A total 8 patients, survived over 72 hours after ROSC. The cardiac arrest were caused by unknown (5 cases), cardiac disease (3 cases). The most common place of cardiac arrest was in the home (6 cases), witnessed cardiac arrest (8 cases), cardiopulmonary resuscitation by bystander was performed (4 cases), agonal respiration (3 cases). The initial electrocardiogram (ECG) showed ventricular fibrillation (VF) in 7 cases, pulseless electrical activity (PEA) other rhythms in 1 cases. Average time from call to scene arrival 6.1 minutes, call to hospital arrival 23.0 minutes, duration of total CPR was 8.7 minutes. The certificate of them were level 1 emergency medical technician (EMT) 6 cases, level 2 EMT 3 cases, total 3 members 7 cases. To improve the rate of survival for out-ofhospital cardiac arrest victims, development of continuous CPR educational program for lay rescue.

Coexistence of Hodgkin's Lymphoma and Tuberculosis in the Same Axillary Lymph Nodes (결핵과 호지킨 림프종이 액와부 림프절에 공존하였던 1예)

  • Herein we report the case of a 71-year-old woman who complained of fatigue and enlarged right axillary lymph nodes for 18 months. At her first visit, her chest X-ray showed diffuse nodular opacities in both lung fields. Initial excisional biopsy of the axillary lymph nodes showed granulomatous lesions and acid fast bacilli were seen on Ziehl-Neelsen staining. However, even after 15 months of anti-tuberculosis (TB) medication, her right axillary lymph nodes were enlarged. We re-performed an excisional biopsy of the nodes, which showed Hodgkin's lymphoma (HL). A retrograde review of the biopsy before anti-tuberculous medication, revealed HL coexisting with TB. HL and TB cause difficulties in differential diagnosis due to similarities in clinical course, imaging procedures and histopathological analysis of the involved tissue. Therefore, it is important to consider the possibility of concurrent HL and TB when patients who undergo treatment for TB or chemotherapy for lymphoma complain of persistent systemic symptoms or enlarged lymph nodes.

  • Soft tissue sarcomas of popliteal fossa are rare, accounting for less than 5% of all soft tissue sarcomas of the extremities. In an extracompartmental space such as the popliteal, cubital fossa and inguinal space, where major vessels and nerves traverse, performing resections with wide margin is difficult and sometimes marginal margin is inevitable for limb salvage. For popliteal tumor resection, posterior approach would be a classic method. For tumors with small size and not adherent to surrounding structures, tumor is easily resected by this approach and dissection of nerve sheath or adventitia of vessel. On the contrary, tumors of large size and infiltrating the posterior structure of knee joint may show difficulty in en-bloc resection itself. These cases were candidates for amputation. Furthermore, tumors involving both popliteal fossa and anterior compartment usually had no choice but to have an amputation to prevent local recurrence. We regarded soft tissue sarcoma showing this kind of presentation as bone tumor having extraosseous mass. We performed wide en-bloc resection of proximal tibia and fibula along with sarcoma involving both compartment on liposarcoma of 47-year old man.
