Effect of Nokyongdaebo-tang on chronic anemia in Taeumin wiwansuhan-pyohan disease (胃脘受寒表寒病) patient: a case report

  • Objectives: The present study reports a case of chronic anemia patient who was significantly improved after taking Nokyongdaebo-tang (NYDBT) Methods: A 48-year-old male patient diagnosed with iron deficiency anemia (IDA) visited the Daejeon Korean medicine Hospital of Daejeon University on June 2018. The patient was treated with Korean medicine for 23 months including acupuncture, herbal medicine and Sasang constitutional medicine prescription. Improvement of anemia was assessed by changes of Hemoglobin value. Laboratory analysis was used to evaluate the safety of treatment. Results: Chronic anemia has significantly improved since taking NYDBT on 9 days (2019.12.09-2019.12.17). The Hb value has been maintained in the normal range since it first entered the normal range on January 18, 2020. The patient had become transfusion-free and this condition persisted for 6 months after stopping the transfusion. Conclusion: This case demonstrates the therapeutic potential of Sasang constitutional medicine for chronic anemia as a personalized medicine.

  • Chronic maxillary sinusitis is a common disease, with symptoms of dull, aching pain or pressure below the eyes and signs such as tenderness over the involved sinus, whereas trigeminal neuralgia is described as severe, paroxysmal, and lancinating brief pain limited in distributions of one or more divisions of trigeminal nerve. In cases where these two non-odontogenic toothache symptoms overlaps, the diagnostic process can be confusing. Here, we report a case of a 54-year-old male patient with chief complaints of intermittent, severe, and electric-like pain in the upper left premolar and first molar area, initially diagnosed with trigeminal neuralgia but finally with maxillary sinusitis after pain recurrence 2 years after that. Therefore, thorough history taking and precise imaging interpretation should be considered to make correct diagnose especially in case of a patient with newly developed or altered or atypical symptoms.

Empowerment, Performance and Job Satisfaction Perceived by Hospital Nurses (일 병원 간호사의 임파워먼트, 업무성과 및 직무만족의 관계)

  • Purpose: The purpose of this study was to investigate the extent of empowerment, task performance and job satisfaction perceived by hospital nurses and the relationship among those variables. Methods: The sample consisted of 261 nurses from a university affiliated hospital. Data were collected with self-administrated questionnaires including empowerment, performance and job satisfaction and analyzed using mean, standard deviation, ANOVA and $Scheff{\acute{e}}$ test. Results: The mean scores of variables were as follows. Empowerment was 4.6472, task performance was 3.7814, and job satisfaction was 3.1240. The older, more educated and more experienced nurses showed more empowered and higher job performed than the younger, less educated and less experienced nurses. Empowerment had a positive correlation with task performance and job satisfaction. Conclusions: The results imply that hospital should develop the empowerment program for nurses to be more job performed and satisfied.

  • Residual aneurysm is a challenging problem after clipping procedure for the aneurysms. The anthors describe one patient in whom endovascular treatment was sucessfully done to treat residual aneurysm after surgical clipping. We discussed the role of endovascular coil occlusion in case of incomplete surgical obliteration of aneurysms.

  • Bronchioloalveolar carcinoma is originated from the periphery of the lung and can be mistaken for lobar pneumonia or atypical pneumonia clinically and at gross examination. Recently the authors experienced a 67-year-old woman who had slowly progressed pulmonary lesions for four years. At first, she visited this hospital for intermittent chest pain four years before. And she visited other hospitals for the same problem and had a series of evaluation including two times of biopsy but did not have any conclusive diagnosis. With aggravation of chest pain, she was referred to this hospital again and the lesion was reexamined and confirmed as bronchioloalveolar carcinoma by ultrasonography-guided needle biopsy. Being performed left lower lobectomy, she kept good condition without any complication.

  • This study is intended to summarize the contents and process of an architectural basic planning for the design competition of K University Hospital. Especially a design of low-rise and compact type hospital in sloped suburb is suggested. The major contents consists of proposed premise of planning, goal of planning, site analysis, site plan, and architectural basic plan To realize the concept of low-rise and compact type hospital which reconciles to the natural environment of the site, 4 nursing units per typical floor and hospital street system are introduced. And to design a "hospital unlike hospital", streamline form of hospital building is proposed.

  • A 47-year old man visited our hospital because of purulent sputum for 3 months. Chest X-ray showed destruction of both the upper lungs, and bronchoscopy revealed inflammatory change with whitish plaque on the left main bronchus through upper division of the left upper lobe. Tracheobronchial aspergillosis (TBA) was finally diagnosed as a result of histologic and microbiologic examination. However, he went abroad without medication before the diagnosis was made and visited again 10 months later. Follow-up bronchoscopy showed complete regression of the previously noted endobronchial lesion. We describe this case to consider the role of antifungal treatment in immunocompetent hosts, as well as to discuss a rare condition; TBA resolved spontaneously.

Association of XRCC3 Thr241Met Polymorphisms and Gliomas Risk: Evidence from a Meta-analysis

  • The relationship between the X-ray repair cross-complementing group 3 (XRCC3) Thr241Met polymorphism and gliomas remains inclusive or controversial. For better understanding of the effect of XRCC3 Thr241Met polymorphism on glioma risk, a meta-analysis was performed. All eligible studies were identified through a search of PubMed, Elsevier Science Direct, Excerpta Medica Database (Embase) and Chinese Biomedical Literature Database (CBM) before May 2013. The association between the XRCC3 Thr241Met polymorphism and gliomas risk was conducted by odds ratios (ORs) and 95% confidence intervals (95% CIs). A total of nine case-control studies including 3,533 cases and 4,696 controls were eventually collected. Overall, we found that XRCC3 Thr241Met polymorphism was significantly associated with the risk of gliomas (T vs. C: OR=1.10, 95%CI=1.01-1.20, P=0.034; TT vs. CC: OR=1.30, 95%CI=1.03-1.65, P=0.027; TT vs. TC/CC: OR=1.29, 95%CI=1.01-1.64, P=0.039). In the subgroup analysis based on ethnicity, the significant association was found in Asian under four models (T vs. C: OR=1.17, 95%CI=1.07-1.28, P=0.00; TT vs. CC: OR=1.79, 95%CI=1.36-2.36, P=0.00; TT vs. TC/CC: OR=1.75, 95%CI=1.32-2.32, P=0.00; TT/TC vs. CC: OR=1.11,95% CI=1.02-1.20). This meta-analysis suggested that the XRCC3 Thr241Met polymorphism is a risk factor for gliomas, especially for Asians. Considering the limited sample size and ethnicities included in the meta-analysis, further large scale and well-designed studies are needed to confirm our results.
