5 Cases of Acute Pyelonephritis (급성(急性) 신우신염(腎盂腎炎)의 임상적(臨床的) 고찰(考察))

  • Purpose: In general, Acute Pyelonephritis is effectively treated with the use of antibiotics. However, some antibiotics are reported to cause side effects, and the abuse of antibiotics results in the increase of the disease's tolerance to antibiotics. Recently, I have effectively treated five cases of Acute Pyelonephritis by using only Constitution - Acupuncture and Herb, and therefore I would like to report about these cases. Methods: I diagnosed Acute Pyelonephritis of these five patients by confirming symptoms and employing a urine analysis with reagent strip(Multi $stix{\circledR}$). I used Kuon's method of constitutional diagnosis for the purpose of the diagnosis of the constitutional 8 morbidities. I relied on Sungjeong(性情) and Chehyungkisang(體刑氣像) in diagnosing Sasang Constitutions(四象人). I performed acupuncture on the left and right sides, depending on 8 constitutions, by employing Chang - temperament Inflammation Formula(臟系炎症方) that is used for the treatment of all kinds of chang-temperament inflammation diseases, as well as Bactericidal Formula(殺菌方) that is used for the treatment of all kinds of bacterially caused diseases. I prescribed by consulting the appearance of disease and general symptoms of each case with Dongyi Soose Bowon(東醫壽世保元)'s prescription symptoms. Result: Two of them showing severe symptoms were hospitalized, while three others took OPD treatment. The patient who was PANCREOTONIA and Soyangin improved through hospitalization for three days, another patient who was PULMOTONIA and Taeyangin with severe symptoms, improved through hospitalization for seven days, and completely recovered through OPD treatment later. The three others took only OPD treatment, and improved within 5-7 days. Conclusion: I confirmed that each of 8-constituions and Sasang Constitutions were all treated effectively without antibiotics.

A Study on the Nutrient Intake According to Sasang Constitutions for the Elderly in a Rural Community (농촌지역 노인들의 사상체질에 따른 영양상태)

  • The purpose of this study was to investigate the food habits and nutrient intake, among the elder according to their Sasang constitutions. The investigation group was composed of 79 elder men and 117 elder women in Sungjugun Kyungsangbukdo. Data were collected from 1 July, 2002 to 31 July, 2002. In conclusion, diet habits and nutritional intakes were poor in the elderly older than 65 years of age so that they need regular food intake habit and balanced nutrition to obtain enough nutrients to maintain healthy lives. And the following suggestions are made. Firstly, multi-directional studies are needed to improve nutritional intakes, prevent diseases, and improve quality of life in the elderly. Secondly, in-depth studies are needed with various and more people related with Sasang constitutions. Thirdly, we need to screen for risk factors of poor nutrition in each constitution for systemic maintenance of nutritional status and health management in the ever increasing elderly population. For this purpose, an objective investigative tool befitting the situation in Korea should be developed. Fourthly, comparative studies on diet habit and nutritional status are needed by investigating the relationship between Sasang constitutions and diseases. Fifthly, food menu should be proposed according to Sasang constitutions to be utilized as basic data of longevity study.

Food Habits, Nutrients Intake, and Disease Distribution according to Sasang Constitutions in the Elderly Aged over 65 Years(II) (65세 이상 노인에서 사상체질에 따른 식습관, 영양소 섭취상태 및 질병 양상에 관한 연구(II))

  • Since the 1970s, the average life expectancy of Koreans has been increasing. Our concerns are not simply about longevity, but also about disability-free life. lately, Sasang constitutional medicine has attracted public attention, especially with respect to the elderly. The purpose of this study was to investigate the food habits, nutrients intake, and disease distribution among the elderly, according to their Sasang constitutions. The results are as follow : The distribution of constitutions of the subjects were Tae-eum 48.8%, So-yang 27.4%, So-eum 22.6%, and So-yang + So-eum 1.2%. In both elderly men and women, body weight, percent ideal body weight, and diastolic pressure of the Tae-eum were significantly higher than those of other constitutions. In both men and women, the level of hemoglobin in the Tae-eum was significantly higher than those of other constitutions. The level of LDL-cholesterol was higher, and the level of HDL-cholesterol was lower in the So-eum elderly women. Food habits of the So-eum were the most irregular, and nutrients intake of the So-eum was lower than that of other constitutions in both elderly men and women. In elderly men, the incidence of hypertension was the highest in the Tae-eum, and the incidence of gastritis was the highest in both So-yang and So-eum. In elderly women, the incidence of fatty liver was the highest in the Tae-eum and So-yang, and gastritis and hypertension were the highest in the So-eum. In the elderly, obese and anemic persons were significantly more in the Tae-eum and the So-eum, respectively. The results of this study showed that there were significant differences of physical, characeristics, food, habits, and distributions of disease for the subjects according to each Sasang constitutions. Intake of nutrient was poor, regardless of Sasang constitutions, in both men and women.

Study on the Risk Factors of Cerebrovascular Disease between Sasang Constitutions (사상체질에 따른 뇌혈관질환 위험인자의 분포에 대한 비교연구)

  • Objectives: Many studies have been reported about Sasang constitutional distribution of cerebrovascular disease(CVD) patients. But there has been little study about the risk factors of normal persons between Sasang constitutions. So we studied to evaluate the risk of CVD between Sasang constitutions. Methods: We retrospectively studied subjects without previous stroke($age{\geq}40$) who visited east-west medical examination center in Kang Nam Korean Hosipital, Kyung Hee University for last 2 years(1998.8.1.${\sim}$2000.7.31.). Sasang constitutional differentiation was performed by Questionnaire for the Constitution Classification(QSCC) II and subjects who couldn't be classifiea by QSCC II were excluded. Results : Total 363 subjects participated in this study(Taeyangin 0, Taeumin 103, Soyangin 113, Soumin 92, obscure 55). Diabetes mellitus(p<0.05), hyperlipidemia(p<0.01), obesity(p=0.000) were significantly prevalent in Taeumin. Hypertension and heart disease had no significant difference between Sasang constitutions. Conclusions: This study showed that the risk factors of CVD were more prevalent in Taeumin. Therefore we guess that Taeumin has high risk of CVD.

A Study on the Analasys of the Eight-Constitution Questionnaire (8체질 분류 설문지의 문항 분석-II - 성향 및 성격을 중심으로 -)

  • Background : Eight-Constitution Medicine (ECM) devides the human body into eight constitutions. Up until now, checking the pulse types is only way to classify the eight constitution. Recently we try to make a questionnaire that is based on the Eight-Constitution characters and analyze the answer from patients who were classified into one of the eight constitutions according to their pulse types and their responses to constitution-acupuncture therapy. Objective : This study intends to analyze each answer of the Eight-Constitution Questionnaire with independent sample t-test. Methods : Participants of this study were outpatients in ECM clinics located in Korea. The resources were collected from 925 patients who were classified into one of the eight constitutions according to their pulse types and their responses to constitution-acupuncture therapy. SPSS 13.0 for Windows was used for independent sample t-test. Results and Conclusions : 1. The proportion of participants' constitutions is in the order of Pancreotonia (28.0%), Colonotonia (17.3%), Pulmotonia (18.1%), Hepatonia (18.1%), Vesicotonia (8.5%), Cholecystotonia (7.0%), Renotonia (2.6%) and Gastrotonia (0.4%). 2. Eight-Constitution Questionnaire consists of 5 parts; Appearance, Character, Body reaction, Physiology & Pathology and Food reaction. 3. 18 items were chosen among 81 question about Character. 4. Due to the deficiency of Gastrotonia examples, useful items have to be given up. If we can gather Gastrotonia examples, we can use meaningful items more than 18.

Relationship Between Nutrient Intakes and Blood Biochemical Parameters of Korean Female Subjects Classified by Eight Constitution Medicine (8체질로 분류한 한국 여성의 영양소 섭취 수준과 혈중 지질 농도와의 관계)

  • Objectives: To investigate the relationship between nutrient intakes and blood biochemical parameters of Korean women classified by the Eight Constitutions. Methods: The constitutions of female subjects were determined by the methods of eight constitutional pulse formation. Anthropometric characteristics and blood biochemical parameters including fasting blood glucose (FSB), serum lipid profiles, homocysteine, and insulin were determined. Food frequency questionnaires were used to evaluate the nutrient intake of the subjects. Results: Subjects were classified as Hepatonia 16.8%, Cholecystonia 2.8%, Pancreotonia 58.9%, Pulmotonia 5.1%, Colonotonia 16.4%, respectively. Gastrotonia, Renotonia, and Vesicotonia individuals were not present in this study. BMI, total calorie and fat intakes of the Mok constitutions (Hepatonia and Cholecystonia) were higher than those of the Gum constitutions (Pulmotonia and Colonotonia) (p<0.05). Triglyceride, total cholesterol, and LDL-C concentrations for the Cholecystonia were the highest while those for the Pulmotonia (p<0.05) were lowest, which is in line with the results from nutrient intakes. Conclusions: Total calorie and fat intake among subjects with different constitutions were different. BMI and serum lipid profiles positively associated with calorie and nutrients intakes were significantly higher in Mok constitutions than Gum constitutions. Personalized diet plans seem to be needed for subjects on a special diet due to the individual constitutional differences.

A Statistical Analysis of the Distribution of Sasang Constitutions in Iksan Wonkwang Oriental Medicine (익산원광한의원 내원환자의 체질분포에 관한 통계적 분석)

  • Objective : To learn the distributional characteristics of Sasang constitutions, Methods : We statistically analyzed those 1338 patients who had been treated at Iksan Wonkwang Oriental Medicine during the period of three years from 2000 to 2002. The data were obtained through the electronic chart developed by Kim Jong- Yeol, and analyzed using the statistical Package SPSS. Results : The distributional ratio of Soeumin : Soyangin : Taeumin was 22.8 : 29.2 : 47.8. Thus the hypothesis : 'the distributional ratio of Soeumin : Soyangin : Taeumin is 2 : 3 : 5' was barely rejected by $x^2$ test for goodness-of-fit at the significance level of 5 %. When $x^2$ test for homogeneity was applied, the distributional characteristics between women and men were different and the distributional characteristics among several age groups were different under significance level of 5%. Conclusion : Though the hypothesis: 'the distributional ratio of Soeumin : Soyangin : Taeumin is 2 : 3 : 5' was rejected by $x^2$ test at the significance level of 5%, the observed distributional ratio was not so far away from the hypothesis.

Study for correlation between MMPI results and Sasang constitutions, in out-patients of Oriental Neuropsychiatry (내원 환자의 MMPI 군집분석과 사상체질과의 상관성 연구)

  • Objective : For investigating correlation among diagnostic types, MMPI results and Sasang constitutions, in out-patients of Oriental Neuropsychiatry. Method: We got the following result by comparing the out-patients' MMPI and QSCCII from 2000/12/1 to 2001/12/30 at Sanbon Hospital of Oriental Medicine, Wonkwang University. Result and Conclusion: 1. A specific character of population for study's objects was that female (128, 68.4%) was twice bigger that the male(59, 31.6%). And their education levels were 52 of college graduate (27.8%), 40 of high school graduate (21.4%), and 14 of middle school graduate (14 %). A distribution of the occupation, there are 62 of labors (33.2%), 62 of white collars (33.2%), 30 of sales (16%), 16 of students (8.6%). Their marrige statuses were 144 of married (77%), 31 of single (16.6%), 4 of divorced (2.1%), and 1 of bereavement (0.5%). 2. In the physical constitutions there were 70 of Soeumin(37.4%), 56 of Soyangin(29.9%) 55 of Taeumin (29.4%), 6 of others (3.2%). A differentiation with their diagnosis, there were 55 of palpitations (29.4%), 45 of depression (24.1%), 18 of women's diseases (9.6%), 14 of insomnia (7.5%), 13 of headache (7%), 8 of pain (4.3%), 8 of hwabyung (4.3%), 7 of psychosis (3.7%), 4 of dizziness (2.1%), and 15 of others(8%). 3. A result of groupling MMPI, we could classify into 4 different groups. Group 1 is a profile of trasition in a neurosis. Group 2 is a proflie of a mental disease. Group 3 is a proflie of $'{\wedge}'$ type neurosis. And group 4 has normal profile. 4. Regarding each group with physical constitution and a distribution of diagnosis, Group 1 is found out 36.4 % of Soeumin, 34.10 % of palpitations. And group 2 showed 43.6% of Soyangin, 25.6% of depression. Group 3 showed 46.4% of Soyangin, 31.90% of palpitations. Group 4 showed the most in 34.3% of Soyangin, and 31.40% of palpitations 5. A result of the study using Chi-square test is that there is no correlation between the 4 different groups and the Sasang physical constitution.

Genetic Analysis study of Sasang Constitution Classification by RFLP (RFLP법을 이용한 사상체질의 유전적 분석 연구)

  • In Sasang medicine, humen are classified into four constitutions which are Taeyang, Soyang. Taeum and Soum. Depending on each different constitution, the clinical and pharmacological application for the same disease might be different. In this study, genomic DNA of different constitutions(Taeum, Soyang and Soum) were analyzed by Restriction Fragment Length Polymorphism(RFLP) to provide scientific and objective references for Sasang classification. The DNA polymorphism for each constitution detected as differences in the length of DNA fragments, after digestion with restriction enzyme Hae III, was investigated using YNH24 as DNA probe. The allele size of Taeum, Soyang and Soum group detected by YNH24 ranged from 1.3 to 3.8 kb, 1.5 to 3.9 kb and 1.3 to 4.6 kb, respectively. However, the allele size distribution of YNH24 loci of different constitutions was shown to be too variable to be classified as 3 different constitution groups investigated. For further study, it is suggested that the number of each constitution samples for RFLP analysis should be increased and statistical analysis of the allele size distribution should be carried out.

Herbal Extracts Classification and Application in Terms of Sasang Constitution (한방보험제제의 사상의학적 분류 및 운용)

  • 1. Objectives Herbal Extracts are used in Korean Oriental Medicine clinics. The sixty eight kinds of single herbal extracts and fifty six kinds of mixed herbal extracts has been using since the first year of execution of Korean medicine national insurance policy in 1987. This paper was performed to make a guideline of using herbal extracts. Single herbal extracts and mixed herbal extracts were discussed and classified according to Sasang constition. 2. Methods The sixty eight single herbal extracts were classified into four categories according to Sasang Constitution and fifty six mixed herbal extracts were classified by six Korean oriental medical doctors. Mixed herbal extracts were considered in terms of drug roles[sovereign, minister, assistant and courier(君臣佐使)] or most included ingredient herb. 3. Results Sasang constitutions which were suitable for fifty six mixed herbal extracts were Soeumin(46.4%), mixed constitution(39.2%) and Soyangin(8.9%), Taeeumin(5.3%). Sasang constitutions which were suitable for sixty eight single herbal extracts were Soeumin(47.1%), Soyangin(32.4%) and Taeeumin(20.6%). 4. Conclusions The rate of Soeumin herbal extracts was the most highest among all constitutions. It is regarded that warming and enforcing herb medicine were most used in all prescriptions.
