Evaluation of Creep Crack Growth Failure Probability at Weld Interface Using Monte Carlo Simulation (몬테카를로 모사에 의한 용접 계면에서의 크리프 균열성장 파손 확률 평가)

  • A probabilistic approach for evaluating failure risk is suggested in this paper. Probabilistic fracture analyses were performed for a pressurized pipe of a Cr-Mo steel reflecting variation of material properties at high temperature. A crack was assumed to be located along the weld fusion line. Probability density functions of major variables were determined by statistical analyses of material creep and creep crack growth data measured by the previous experimental studies by authors. Distributions of these variables were implemented in Monte Carlo simulation of this study. As a fracture parameter for characterizing growth of a fusion line crack between two materials with different creep properties, $C_t$ normalized with $C^*$ was employed. And the elapsed time was also normalized with tT, Resultingly, failure probability as a function of operating time was evaluated fur various cases. Conventional deterministic life assessment result was turned out to be conservative compared with that of probabilistic result. Sensitivity analysis for each input variable was conducted to understand the most influencing variable to the analysis results. Internal pressure, creep crack growth coefficient and creep coefficient were more sensitive to failure probability than other variables.

A Study on Opaque Porcelain for P.F.M Crown - Focused on Paste Opaque - (도재전장관용 Opaque 도재에 관한 연구 - paste opaque을 중심으로 -)

  • The purpose of this study was to analyze the commercial paste opaque products currently available in domestic of foreign as well as domestic, such as Duceram Plus(DU; Ducera Dental GmbH, Germany), VMK 95(VM; Vita Co., Germany), Noritake EX-3(EX; Noritake Co., Japan) and Ceramax(CE; Alphadent Co., Korea). They were characterized in thermal expansion coefficient, particle size distribution, viscosity and solvent using thermomechanical analyzer, particle size analyzer, rheometer and infrared spectrophotometer. Experimental results are as follows; Firstly, thermal expansion coefficients were determined $13.9{\times}10-6/^{\circ}C$ for DU, $14.3{\times}10-6/^{\circ}C$ for VM, $13.3{\times}10-6/^{\circ}C$ for EX, and $14.0{\times}10-6/^{\circ}C$ for CE. Secondly, percent of partice size below $1{\mu}m$ were 12% for DU, VM and CE, and 13% for EX, percent between $1{\mu}m$ and $5{\mu}m$ were 42% for DU, 42% for VM, 38% for EX, and 61 % for CE, percent between $5{\mu}m$ and $10{\mu}m$ were 21 % for DU, 24% for VM, 20% for EX, and 18% for CE, and over $10{\mu}m$ were 25% for DU, 22% for VM, 29% for EX, 9% for CE. Thirdly, the basic composition of the solvent in all of the commercial paste opaques were determined as ethylene glycol from FT-IR investigation. Lastly, measured viscosities were 1798 cp for DU, 536 cp for VM, 1110 cp for EX, and 721 cp for CE.

Impact of Device Evolution in Transcatheter Closure of Patent Ductus Arteriosus Using Duct-Occlud Coils : Comparison of Mid-term Results (경피적 동맥관 폐쇄술에 사용된 Duct-Occlud Coil의 종류에 따른 중-단기 결과의 비교)

  • Purpose : We reviewed the therapeutic results of various Duct-Occlud coils(pfm AG, $K{\ddot{o}}ln$, Germany) to evaluate the efficacy of the most-recently modified Duct-Occlud coil(Nit-Occlud) in the transcatheter closure of patent ductus arteriosus(PDA), including large defects more than 4 mm in diameter. Methods : Two hundred and five patients who underwent percutaneous PDA occlusion using Duct-Occlud devices from March 1996 to December 2003 were enrolled and four types of Duct-Occlud [Standard(S), Reinforced(R), Reinforced reverse cone(RR) and Nit-Occlud(N)] were used in this study. The patients were followed up by echocardiogram and physical examination before discharge, one month, six months and 12 months after the procedure. Results : The rate of residual shunt according to the type of Duct-Occlud were as follows : S-54%, R-72%, RR-50%, N-14%(P<0.05 compared with other devices) at one month, S-25%, R-44%, RR-37%, N-0%(P<0.05 compared with other devices) at six months, S-8%, R-8%, RR-4%, N-0%(P<0.05 compared with S and R) at 12 months and later. Nit-Occlud coil showed the complete occlusion of PDA after six months of follow-up, even in 12 patients with relatively large PDA(>4 mm). Conclusion : The transcatheter closure of PDA using Duct-Occlud was an effective treatment and our study revealed that a Nit-Occlud coil which showed higher rate of occlusion even in PDA with large diameters over than 4 mm, was a more effective modality compared to previous devices.

Clinical outcome of transcatheter closure of patent ductus arteriosus in small children weighing 10 kg or less

  • Purpose: Transcatheter closure has become an effective therapy in most patients with patent ductus arteriosus (PDA). However, there are difficulties in transcatheter closure of PDA in small children. We reviewed clinical outcomes of transcatheter closure of PDA in children weighing less than 10 kg in a single center. Methods: Between January 2003 and December 2009, 314 patients with PDA underwent transcatheter closure in our institute. Among them, 115 weighed less than 10 kg. All of these patients underwent transcatheter closure of PDA using either COOK Detachable $Coil^{(R)}$, PFM Nit-$Occlud^{(R)}$, or Amplatzer duct $occluder^{(R)}$. A retrospective review of the treatment results and complications was performed. Results: The mean age of patients was $9.1{\pm}5.9$ months (median, 8 months), and mean weight was $7.6{\pm}1.8kg$ (median, 7.8 kg). The mean diameter of PDA was $3.2{\pm}1.4mm$ (median, 3 mm). Complete occlusion occurred in 113 patients (98%). One patient was sent to surgery because of a failed attempt at device closure, and another patient had a small residual shunt after device placement. The average mean length of hospital stay was $3.0{\pm}3.3$ days, and mean follow-up duration was $21.0{\pm}19.6$ months. There were no major complications in any of the patients. Conclusion: Transcatheter closure of PDA is considered safe and efficacious in infants weighing less than 10 kg. With sufficient experience and further effort, transcatheter closure of PDA can be accepted as the gold standard of treatment for this group of patients.

STRESS ANALYSIS OF ENDODONTICALLY TREATED ANTERIOR TEETH BY ALVEOLAR BONE HEIGHT AND RESTORATION METHOD (근관치료를 받은 전치부에서 수복방법과 치조골높이에 따른 응력분석에 관한 연구)

  • To study the mechanical behavior depended on the restoration method and alveolar bone height at endodontically treated teeth. a finite element model was made which was applied by four types of restoration methods and alveolar bone height on upper central incisor and then 1 Kg force was applied on each model as follows; 1) $45^{\circ}$ diagonal load on incisal edge. 2) $26^{\circ}$ diagonal load on lingual surface. and 3) horizontal load on labial surface. The author analyzed the displacement and stress of teeth and their supporting tissue by finite element method according to three type of loading conditions. The results were as follows : 1. The displacement by restoration method and the stress in dentin was found greater in restoration without a post than in that with a post. 2. The displacement and stress was found about the same when compared : A) in Resin model and PFM model applied by restoration method without a post and B) in PRC model and CPC model applied by restoration method with a post. 3. The lower alveolar bone height was. the greater was the displacement and stress. 4. The lower alveolar bone height was. the greater slightly was the stress of restoration without a post than in that with a post. 5. The stress in loading condition was the greatest in P1 in dentin and post. and was greatest in P3 in alveolar hone. 6. In the restoration method without a post. stress concentration in labial dentin was distributed to a figure of long belt in adjacent part to periodontal ligament. while in restoration method with a post. it was distributed in adjacent part to post side. And in all types of restoration method stress concentration in alveolar bone was distributed along the compact bone of labial and lingual surface.

An Evaluation of the Accuracy of Mini-Wright Peak Flow Meter (mini-Wright Peak Flow Meter에 의한 PEFR 측정의 정확도)

  • Background : Portable devices for measuring peak expiratory flow(PEF) are now of proved value in the diagnosis and management of asthma and many lightweight PEF meters have become available. However, it is necessary to determine whether peak expiratory flow rate(PEFR) measurements measured with peak flowmeters is accurate and reproducible for clinical application. The aim of the present study is to define accuracy, agreement, and precision of mini-Wright peak flow meter(MPFM) against standard pneumotachygraph. Methods : The lung function tests by standard pneumotachygraph and PEFR measurement by MPFM were performed in a random order for 2 hours in 22 normal and 17 asthmatic subjects and also were performed for 3 successive days in 22 normals. Results : The PEFR measured with MPFM was significantly related to the PEFR and $FEV_1$ measured with standard pneumotachygraph in normal and asthmatics(for PEFR, r = 0.92 ; p < 0.001 ; for $FEV_1$, r = 0.78 ; p < 0.001). The accuracy of MPFM was within 100(limits of accuracy recommeded by NAEP) in all the subjects or 22 normal, mean difference from standard pneumotachygraph being 16.5L/min(percentage of difference being 2.90%) or 10.6L/min(percentage of difference being 1.75%), respectively. According to the method proposed by Bland and Altman, the 95% limits of the distribution of differences between MPFM and standard pneumotachygraph after correction of PEFR using our regression equation were +38.2 and -71.5L/min in all the subjects or 20.49~+9.49L/min in 22 normal and was similar to the intraindividual agreements for 3 successive days in normal. There was no statistically significant difference of PEFR measured with MPFM and standard pneumotachygraph among three days(p > 0.05) and the coefficient of variation($2.4{\pm}1.2%$) of PEFR measured with MPFM was significantly lower than that($5.2{\pm}3.5%$) with standard pneumotachygraph in normal (p < 0.05). Conclusion : This results suggest that the MPFM was as accurate and reproducible as standard pneumotachygraph for monitoring of PEFR in the asthmatic subjects.

A new strategy for transcatheter closure of patent ductus arteriosus with recent-generation devices (경피적 동맥관 폐쇄술에서 최근의 기구들의 전략적 이용과 결과)

  • Purpose : The aim of this study was to assess the efficacy and safety of recent-generation patent ductus arteriosus (PDA) closure devices applied by a new selection strategy according to the characteristics of each PDA. Methods : From February 2003 to January 2006, 138 patients underwent transcatheter closure of PDA (study group). According to the size and morphology of each ductus, a COOK Detachable Coil or 'flex' PFM Nit-Occlud was used for a small ductus (group 1, n=43); 'medium' PFM Nit-Occlud (group 2, n=49) for a moderate ductus; and an Amplatzer Duct Occluder (group 3, n=46) for a large ductus. The 83 patients who underwent transcatheter closure of PDA from February 2000 to January 2003 were defined as the comparison group. The Qp/Qs ratio, pulmonary/aorta pressure ratio, and MD of the ductus were compared. Immediate and follow-up results including residual shunts and complications were also evaluated and compared among groups. Results : In all 138 patients, complete occlusions were confirmed without major complications, while procedure failure (n=2, 2.2%), device embolization (n=1, 1.1%), and persistent residual shunt (n=4, 4.5%) were documented in the comparison group. Total complication rates were lower in the study group than in the comparison group (study group, 1.4%; comparison group, 9.0%; P<0.05). Conclusion : A novel strategy adopting the merits of various recent-generation devices for transcatheter closure of PDA provides excellent clinical results with minimal risk.

A STUDY ON STRESS DISTRIBUTION OF ENDODONTICALLY TREATED TOOTH ACCORDING TO THE VARIOUS POST LENGTH USINGTHREE-DIMENSIONAL FINITE ELEMENT METHOD (포스트 길이가 치근내 응력분산에 미치는 영향에 관한 삼차원 유한요소법적 연구)

  • The endodontically treated tooth is generally restored with post & core, owing to the brittleness and the loss of large amount of tooth structure. Although there have been lots of studies about the endodontically treated teeth, the three-dimensional quantitative studies about the strees distribution of them are in rare cases. In this study, it was assumed that the coronal portion of the upper incisou had severely damaged. After the root canal therapy it was post cored, and restored with PFM crown, for this experiment nine types of model were constructed : 1); long, 2); medium, 3); short gold post for the roots supported with a narmal alveolar bone, 4); long, 5); medium, 6); short gold post for the roots supported with an alveolar bone resorbed to its 1/3 of root length, 7); long, 8); medium, 9); short base metal post for the roots supported with an alveolar bone resorbed to its 1/3 of root length. Force was applied from two directions. One was functional maximum bite force(300N) applied to the spot just lingual to the incisal edge with the angle of 45 degrees to the long axis of the tooth, and the other one was horizontal force(300N) applied to the labial surface. The results analyzed with three-dimensional finite element method were as follows : 1. Stress was concentrated on the middle portion of the labial side dentin of the root and the lingual portion of the apical dentin of the root. Stress in the post showed maximum value at 2 mm above the post apex. 2. In case of the long post and base metal post, strees was concentrated on the apex of the root and the post. 3. In case of the longer post, the displacement on the post-cement interface was lessened. The gold post was more displaceable than the base metal post. 4. In case of the alveolar bone resorption, stress concentrated on the root and the post and displacement on the post-cement interface were increased.

THREE DIMENSIONAL FINITE ELEMENT ANALYSIS OF THE EFFECTS OF ALVEOLAR BONE LOSS ON STRESS DISTRIBUTION IN POST-RECONSTRUCTED TEETH (치조골 흡수가 포오스트로 인한 치근내 응력에 미치는 영향에 관한 삼차원 유한요소법 분석)

  • There're many cases that should be reconstructed with post and core when clinical crown is destructed. But this post and core restoration may cause damaging stress on the teeth. Previous finite element study was restricted to normal bone model relatively close to cemen-toenamel junction. Moreover, the test of a model with diminished bone support was rare. The purpose of this study is to test the effects of alveolar bone loss on the magnitude, stress distribution and displacement of post reconstructed teeth. In this study, it was assumed that the coronal portion of upper incisor was severely destructed. After conventional endodontic treatment, it was restored with post and core. The PFM restoration was made on it. This crown was cemented with ZPC. Alveolar bone was classified by 4 types of bone, such as normal, 2 mm, 4 mm, 6 mm bone, according to the bone loss. Meanwhile, the material of post are divided into 2 types of materials, such as gold, co-cr. Force was applied to two directions. One was fuctional maximum bite force (300 N) applied to the spot just lingual to the incisal edge with the angle of 45 degree to the long axis of the tooth, and the other one was horizontal force (300 N) applied to the labial surface. The results analyzed with three dimensional finite element method were as follows : 1. Stress was concentrated on the adjacent dentin of the post apex, one third portion of the post apex and the labial & lingual mid-portion of the root in all case. The stress of middle third of the root was apparently concentrated on the labial aspect. 2. The stress on adjacent dentin of the post apex and one third of the post apex increased as alveolar bone height moved apically. This increase was dramatic beyond 4 mm bone loss model. 3. The stress of the post apex was spreaded to the middle third of the post and greater than gold post in the case of metal post. 4. The displacement of the neck of post was the greatest in one of the post-cement interface and this increased as alveolar bone height moved apically. Besides the displacement of the metal post is slightly lower than one of the gold post.

Full mouth rehabilitation using orthodontic treatment and implants in patient with collapsed occlusion: A case report (붕괴된 교합을 가진 환자에서 교정치료와 임플란트를 이용한 전악 수복: 증례보고)

  • The distance between the natural teeth and the implants is an important factor in preserving the periodontal tissues and esthetics. And abnormal positional displacement and tilting of the teeth during restorative procedure may require intentional root canal treatment and may affect masticatory function. This report is to present a successful full mouth rehabilitation of a patient with uneven dentition and collapsed occlusion using orthodontic and implant treatment. The patient had no symptoms or discomfort of temporomandibular joint disorder such as pain or sound. The orthodontic treatment was continued until implant provisional prosthesis delivery. And the vertical height of occlusion was elevated 2mm on anterior basis for anterior teeth protection and esthetics. After the orthodontic treatment, the implant abutments and natural teeth were finally restored with porcelain-fused-to-metal crowns and bridges. Satisfactory function and esthetic outcomes are observed after 6months of follow up.
