Development of cardiopulmonary resuscitation nursing education program of web-based instruction (웹 기반의 심폐소생술 간호교육 프로그램 개발)

  • The purpose of this study is to develop and evaluate a web-based instruction Program(WBI) to help nurses improving their knowledge and skill of cardiopulmonary resuscitation. Using the model of web-based instruction(WBI) program designed by Rhu(1999), this study was carried out during February-April 2002 in five different steps; analysis, design, data collection and reconstruction, programming and publishing, and evaluation. The results of the study were as follows; 1) The goal of this program was focused on improving accuracy of knowledge and skills of cardiopulmonary resuscitation. The program texts consists of the concepts and importances of cardiopulmonary resuscitation(CPR), basic life support(BLS), advanced cardiac life support(ACLS), treatment of CPR, nursing care after CPR treatment. And in the file making step, photographs, drawings and image files were collected and edited by web-editor(Namo), scanner and Adobe photoshop program. Then, the files were modified and posted on the web by file transfer protocol(FTP). Finally, the program was demonstrated and once again revised by the result, and then completed. 2) For the evaluation of the program, 36 nurses who in K university hospital located in D city, and related questionnaire were distributed to them as well. Higher scores were given by the nurses in its learning contents with $4.2{\pm}.67$, and in its structuring and interaction of the program with $4.0{\pm}.79$, and also in its satisfactory of the program with $4.2{\pm}.58$ respectively. In conclusion, if the contents of this WBI educational program upgrade further based upon analysis and applying of the results the program evaluation, it is considered as an effective tool to implement for continuing education as life-long educational system for nurse.

Evaluation of Image Quality Change by Truncated Region in Brain PET/CT (Brain PET에서 Truncated Region에 의한 영상의 질 평가)

  • Purpose The purpose of this study was to evaluate image quality change by truncated region in field of view (FOV) of attenuation correction computed tomography (AC-CT) in brain PET/CT. Materials and Methods Biograph Truepoint 40 with TrueV (Siemens) was used as a scanner. $^{68}Ge$ phantom scan was performed with and without applying brain holder using brain PET/CT protocol. PET attenuation correction factor (ACF) was evaluated according to existence of pallet in FOV of AC-CT. FBP, OSEM-3D and PSF methods were applied for PET reconstruction. Parameters of iteration 4, subsets 21 and gaussian 2 mm filter were applied for iterative reconstruction methods. Window level 2900, width 6000 and level 4, 200, width 1000 were set for visual evaluation of PET AC images. Vertical profiles of 5 slices and 20 slices summation images applied gaussian 5 mm filter were produced for evaluating integral uniformity. Results Patient pallet was not covered in FOV of AC-CT when without applying brain holder because of small size of FOV. It resulted in defect of ACF sinogram by truncated region in ACF evaluation. When without applying brain holder, defect was appeared in lower part of transverse image on condition of window level 4200, width 1000 in PET AC image evaluation. With and without applying brain holder, integral uniformities of 5 slices and 20 slices summation images were 7.2%, 6.7% and 11.7%, 6.7%. Conclusion Truncated region by small FOV results in count defect in occipital lobe of brain in clinical or research studies. It is necessary to understand effect of truncated region and apply appropriate accessory for brain PET/CT.

A Study on the Development and usefulness of the x/y Plane and z Axis Resolution Phantom for MDCT Detector (MDCT 검출기의 x/y plane과 z축 분해능 팬텀 개발 및 유용성에 관한 연구)

  • The aim of this study is to establish a new QC method that can simultaneously evaluate the resolution of the x/y plane and the z-axis by producing a phantom that can reflect exposure and reconstruction parameter of MDCT system. It was used with Aquilion ONE(Cannon Medical System, Otawara, Japan), and the examination was scanned using of 120 kV, 260 mA, and the D-FOV of 300 mm2. It produced new SSP phantom modules in which two aluminum plates inclined at 45° to a vertical axis and a transverse axis to evaluate high contrast resolution of x/y plane and z axis. And it changed factors such as the algorithm, distance from gantry iso-center. All images were reconstructed in five steps from 0.6 mm to 10.0 mm slice thickness to measure resolution of x/y plane and z-axis. The image data measured FWHM and FWTM using Profile tool of Aquarius iNtusion Edition ver. 4.4.13 P6 software(Terarecon, California, USA), and analysed SPQI and signal intensity by ImageJ program(v1.53n, National Institutes of Health, USA). It decreased by 4.09~11.99%, 4.12~35.52%, and 4.70~37.64% in slice thickness of 2.5 mm, 5.0 mm, and 10.0 mm for evaluating the high contrast resolution of x/y plane according to distance from gantry iso-center. Therefore, the high contrast resolution of the x/y plane decreased when the distance from the iso-center increased or the slice thickness increased. Additionally, the slice thicknesses of 2.5 mm, 5.0 mm, and 10.0 mm with a high algorithm increased 74.83, 15.18 and 81.25%. The FWHM was almost constant on the measured SSP graph for evaluating the accuracy of slice thickness which represents the resolution of x/y plane and z-axis, but it was measured to be higher than the nominal slice thickness set by user. The FWHM and FWTM of z-axis with axial scan mode tended to increase significantly as the distance increased from gantry iso-center than the helical mode. Particularly, the thinner slice thickness that increased error range compare with the nominal slice thickness. The SPQI increased with thick slice thickness, and that was closer to 90% in the helical scan than the axial scan. In conclusion, by producing a phantom suitable for MDCT detectors and capable of quantitative resolution evaluation, it can be used as a specific method in the management of research quality and management of outdated equipment. Thus, it is expected to contribute greatly to the discrimination of lesions in the field of CT imaging.

Surgical Treatment of Ebstein Anomaly (Ebstein 기형의 외과적 치험)

  • Background: Ebstein anomaly is a rare congenital disease distinguished by its unique deformity in tricuspid valve and right ventricle & atrium. In its surgical treatment , tricuspid valve reconstruction and valve replacement are well known method, but various surgical methods were suggested. Material and Method : From January 1984 to December 1995, 8 patients with Ebstein anomaly underwent surgical correction. Age and sex distribution, clinical symtoms, radiologic findings, preoperative studies, operative findings, operative methods and its results were analyzed. Result: The sex ratio was 5 to 3(male : female). Patients' ages were averaged 17.6(2-28) years. In all cases, it showed typical deformities of the tricuspid valve. Associated anomalies were permenant foramen ovale, atrial septum defect, pulmonary stenosis. Surgical procedures included tricuspid valve replacement(n=4) and tricuspid valve reconstuction(n=4). Two cases of sinus tachycardia and complete AV block occured postoperatively. There were two hospital death and no late death. All survivors are in NYHA class I or II with median follow up of 64.8 months. 2-D echocardiogram disclosed improvement tricuspid regurgitation during the follow up period. Conclusion: Even though operative method of Ebstein anomaly should be decided according to each anatomical characteristics, we recommended that tricuspid valvuloplasty and plication can be one of the good methods method in the selective cases.

Multi-camera Calibration Method for Optical Motion Capture System (광학식 모션캡처를 위한 다중 카메라 보정 방법)

  • In this paper, the multi-camera calibration algorithm for optical motion capture system is proposed. This algorithm performs 1st camera calibration using DLT(Direct linear transformation} method and 3-axis calibration frame with 7 optical markers. And 2nd calibration is performed by waving with a wand of known length(so called wand dance} throughout desired calibration volume. In the 1st camera calibration, it is obtained not only camera parameter but also radial lens distortion parameters. These parameters are used initial solution for optimization in the 2nd camera calibration. In the 2nd camera calibration, the optimization is performed. The objective function is to minimize the difference of distance between real markers and reconstructed markers. For verification of the proposed algorithm, re-projection errors are calculated and the distance among markers in the 3-axis frame and in the wand calculated. And then it compares the proposed algorithm with commercial motion capture system. In the 3D reconstruction error of 3-axis frame, average error presents 1.7042mm(commercial system) and 0.8765mm(proposed algorithm). Average error reduces to 51.4 percent in commercial system. In the distance between markers in the wand, the average error shows 1.8897mm in the commercial system and 2.0183mm in the proposed algorithm.

Reconstruction of Stereo MR Angiography Optimized to View Position and Distance using MIP (최대강도투사를 이용한 관찰 위치와 거리에 최적화 된 입체 자기공명 뇌 혈관영상 재구성)

  • Purpose : We studied enhanced method to view the vessels in the brain using Magnetic Resonance Angiography (MRA). Noticing that Maximum Intensity Projection (MIP) image is often used to evaluate the arteries of the neck and brain, we propose a new method for view brain vessels to stereo image in 3D space with more superior and more correct compared with conventional method. Materials and Methods: We use 3T Siemens Tim Trio MRI scanner with 4 channel head coil and get a 3D MRA brain data by fixing volunteers head and radiating Phase Contrast pulse sequence. MRA brain data is 3D rotated according to the view angle of each eyes. Optimal view angle (projection angle) is determined by the distance between eye and center of the data. Newly acquired MRA data are projected along with the projection line and display only the highest values. Each left and right view MIP image is integrated through anaglyph imaging method and optimal stereoscopic MIP image is acquired. Results: Result image shows that proposed method let enable to view MIP image at any direction of MRA data that is impossible to the conventional method. Moreover, considering disparity and distance from viewer to center of MRA data at spherical coordinates, we can get more realistic stereo image. In conclusion, we can get optimal stereoscopic images according to the position that viewers want to see and distance between viewer and MRA data. Conclusion: Proposed method overcome problems of conventional method that shows only specific projected image (z-axis projection) and give optimal depth information by converting mono MIP image to stereoscopic image considering viewers position. And can display any view of MRA data at spherical coordinates. If the optimization algorithm and parallel processing is applied, it may give useful medical information for diagnosis and treatment planning in real-time.

The Evaluation of Quantitative Accuracy According to Detection Distance in SPECT/CT Applied to Collimator Detector Response(CDR) Recovery (Collimator Detector Response(CDR) 회복이 적용된 SPECT/CT에서 검출거리에 따른 정량적 정확성 평가)

  • Purpose Recently, with the spread of SPECT/CT, various image correction methods can be applied quickly and accurately, which enabled us to expect quantitative accuracy as well as image quality improvement. Among them, the Collimator Detector Response(CDR) recovery is a correction method aiming at resolution recovery by compensating the blurring effect generated from the distance between the detector and the object. The purpose of this study is to find out quantitative change depending on the change in detection distance in SPECT/CT images with CDR recovery applied. Materials and Methods In order to find out the error of acquisition count depending on the change of detection distance, we set the detection distance according to the obit type as X, Y axis radius 30cm for circular, X, Y axis radius 21cm, 10cm for non-circular and non-circular auto(=auto body contouring, ABC_spacing limit 1cm) and applied reconstruction methods by dividing them into Astonish(3D-OSEM with CDR recovery) and OSEM(w/o CDR recovery) to find out the difference in activity recovery depending on the use of CDR recovery. At this time, attenuation correction, scatter correction, and decay correction were applied to all images. For the quantitative evaluation, calibration scan(cylindrical phantom, $^{99m}TcO_4$ 123.3 MBq, water 9293 ml) was obtained for the purpose of calculating the calibration factor(CF). For the phantom scan, a 50 cc syringe was filled with 31 ml of water and a phantom image was obtained by setting $^{99m}TcO_4$ 123.3 MBq. We set the VOI(volume of interest) in the entire volume of the syringe in the phantom image to measure total counts for each condition and obtained the error of the measured value against true value set by setting CF to check the quantitative accuracy according to the correction. Results The calculated CF was 154.28 (Bq/ml/cps/ml) and the measured values against true values in each conditional image were analyzed to be circular 87.5%, non-circular 90.1%, ABC 91.3% and circular 93.6%, non-circular 93.6%, ABC 93.9% in OSEM and Astonish, respectively. The closer the detection distance, the higher the accuracy of OSEM, and Astonish showed almost similar values regardless of distance. The error was the largest in the OSEM circular(-13.5%) and the smallest in the Astonish ABC(-6.1%). Conclusion SPECT/CT images showed that when the distance compensation is made through the application of CDR recovery, the detection distance shows almost the same quantitative accuracy as the proximity detection even under the distant condition, and accurate correction is possible without being affected by the change in detection distance.

Evaluation for Optimization of CT Dose Reduction Methods in PET/CT (PET/CT 검사 시 CT 피폭선량 감소 방법들의 최적화 평가)

  • Purpose Various methods for reducing radiation exposure have been continuously being developed. The aim of this study is to evaluate effectiveness of dose reduction, image quality and PET SUV changes by applying combination of automatic exposure dose(AEC), automated dose-optimized selection of X-ray tube voltage(CAREkV) and sinogram affirmed iterative reconstruction(SAFIRE) which can be controled by user. Materials and Methods Torso, AAPM CT performance and IEC body phantom images were acquired using biograph mCT64, (Siemens, Germany) PET/CT scanner. Standard CT condition was 120 kV, 40 mAs. Radiation exposure and noise were evaluated by applying AEC, CAREkV(120 kV, 40 mAs) and SAFIRE(120 kV, 25 mAs) with torso phantom compare to standard CT condition. And torso, AAPM and IEC phantom images were acquired with combination of 3 methods in condition of 120 kV, 25 mAs to evaluate radiation exposure, noise, spatial resolution and SUV changes. Results When applying AEC, CTDIvol and DLP were decreased by 50.52% and 50.62% compare to images which is not applying AEC. mAs was increased by 61.5% to compensate image quality according to decreasing 20 kV when applying CAREkV. However, CTDIvol and DLP were decreased by 6.2% and 5.5%. When reference mAs was the lower and strength was the higher, reduction of radiation exposure rate was the bigger. Mean SD and DLP were decreased by 2.2% and 38% when applying SAFIRE even though mAs was decreased by 37.5%(from 40 mAs to 25 mAs). Combination of 3 methods test, SD decreased by 5.17% and there was no significant differences in spatial resolution. And mean SD and DLP were decreased by 6.7% and 36.9% compare to 120 kV, 40 mAs with AEC. For SUV test, there was no statistical differences(P>0.05). Conclusion Combination of 3 methods shows dose reduction effect without degrading image quality and SUV changes. To reduce radiation exposure in PET/CT study, continuous effort is needed by optimizing various dose reduction methods.

Gamma Ray Detection Processing in PET/CT scanner (PET/CT 장치의 감마선 검출과정)

  • The PET/CT scanner is an evolution in image technology. The two modalities are complementary with CT and PET images. The PET scan images are well known as low resolution anatomic landmak, but such problems may help with interpretation detailed anatomic framework such as that provided by CT scan. PET/CT offers some advantages-improved lesion localization and identification, more accurate tumor staging. etc. Conventional PET employs tranmission scan require around 4 min./bed position and 30 min. for whole body scan. But PET/CT scanner can reduced by 50% in whole body scan. Especially nowadays PET scanner LSO scintillator-based from BGO without septa and operate in 3-D acquisition mode with multidetectors CT. PET/CT scanner fusion problems solved through hardware rather than software. Such device provides with the capability to acquire accurately aligned anatomic and functional images from single scan. It is very important to effective detection from gamma ray source in PETdetector. And can be offer high quality diagnostic images. So we have study about detection processing of PET detector and high quality imaging process.

Evaluation of Total Lung Volume and Density using Multi-Detector Computed Tomography in Normal Dogs (정상견에서 다중채널 컴퓨터단층촬영술을 통한 폐용적과 밀도의 평가)

  • This study was performed to quantitatively assess the normal lung volume and density according to the position by multi-detector computed tomography (MDCT) in dogs. Helical CT of the thorax was performed on 4 different positions with dorsal, left lateral, right lateral and ventral recumbency in 6 Pekingese and 6 Maltese dogs. During CT scanning, dogs were kept hyperventilated. Through the 3-dimensional reconstruction of CT images, the lung parameters were measured as the volume and density of the left, right including accessory lobe, and total lung. 3D images represented the different lung shape between Pekingese and Maltese dogs. Their difference of total lung volume and total lung density was not significant on the each position in both breeds. Right lung volume was significantly higher than left. The difference of left and right volume was $66.91{\pm}25.1$ ml. Linear relationship was shown between body weight and lung volume of ventral recumbency position. The dependent lung had higher density and lower volume than nondependent lung in both breed dogs. The volume of nondependent lung was not changed compared with the volume on ventral or dorsal recumbency. The total lung volume measured with MDCT is correlated with the lung density, and the lung density is useful to predict the normal total lung volume.
