A Study on the Construction of Pungcha Baji - Focusing on the Books Entitled "Hanbok Construction" - (풍차바지 제도에 관한 연구 - 한복구성학 책을 중심으로 -)

  • Based on instructions in the textbook "How to Construct Korean Costumes" which I authored, my university students were required to make Pungcha-Baji(Korean traditional pants for children) for one-year-old boys. While examining the students' construction, I found that the side seams of the vest and pants did not line up but were improperly twisted. It was found that the pants did not cover the child's backside because the width of the back was smaller than the width of the front when one of the side panels and the large center panel in the front was half the size of the hip and one of the side panels and the small panel in the back was four fifths of half the size of the hip. Although there were differences between the waist size of the pants and the bottom hem of the vest, the textbooks instruct that the amount of material and the number of pleats(i.e. 4 pleats) on the pants and the vest should be the same. Finding this mistake led me to investigate Pungcha-Baji construction in related textbooks. Thus with the textbook instruction, the side seams of the pants and the vest do not match and are improperly twisted. Hence, as a solution, the pleats should be made and adjusted after matching the side seams of the pants and the vest. The purpose of this study is to examine currently available Pungcha-Baji related textbooks and determine the correctness of their instructions and to ultimately provide correct construction methods for Pungcha-Baji pattern for academic purposes.

ALTERNATIVE ORAL REHABILITATION OF A CHILD WITH ECTODERMAL DYSPLASIA: A CASE REPORT (외배엽 이형성증 어린이의 구강 회복: 증례보고)

  • Ectodermal Dysplasia is a genetic disorder caused by the abnormal development of two or more structures derived from the ectodermal layer. As an aspect in dentistry, ectodermal dysplasia is characterized by hypodontia, conical or peg-shaped teeth, reduced salivary secretion and decreased vertical dimension. These unfavorable oral conditions make children to have difficulties with mastication, esthetics, and even in social activities. This case report presents an alternative oral rehabilitation treatment of a 4-year-old boy with ectodermal dysplasia. A removable space maintainer with artificial teeth in maxillary arch was delivered according to the initial treatment plan. However, the child failed to adapt the appliance because of his masticatory habit. Then a fixed-type space maintainer was delivered on the patient's esthetic demand and it restored function, esthetics and self-esteem of the patient. The treatment described here suggests that individually customized considerations are essential for the oral rehabilitation of a patient with ectodermal dysplasia.

  • Restless legs syndrome (RLS) is a common neurological sleep disorder in adults characterized by the following diagnostic criteria: an urge to move that is usually associated with unpleasant sensations and symptoms that are worse at rest, relieved by movement, and most severe at night. The definite diagnosis of RLS in children is stricter and consists of self-description of leg discomfort or the presence of 2 of 3 supportive criteria combined with 4 essential criteria for diagnosis in adults. RLS in childhood has often been misdiagnosed as growing pains or a part of normal development. As a result, physicians have often missed the chance for proper management. We diagnosed a case of RLS in a 5-year-old boy presenting with growing pains, whose mother was found to have had RLS since childhood. We confirmed RLS by using a polysomnograph, in which the indices of periodic limb movement syndrome (PLMS) and periodic limb movement during wakefulness (PLMW) were recorded to be compatible with RLS criteria. The patient's ferritin level was low normal, and his symptoms improved after taking iron supplements.

A Case Study on the Effect of the Art Therapy through the Self Expression and the Interpersonal Relations by a Schizophrenic Adolescent I (정신분열증 청년의 자기표현과 상호작용의 효과에 관한 미술치료 사례연구 I)

  • Objectives : This article aims to find how effective the art therapy is on the improvement of the self expression and the interpersonal relations with others, as medium for verbal communication of schizophrenic patients. Methods : The subject of this study was one schizophrenic patient who had been treated with drugs for five years in W university hospital, as well as counseled by an art therapy oo counsel office in J city. The art therapy had been performed the total 25 sessions, fifty minute per session, one session per week, from November 28 in 2008 to April 27 in 2009. The test was by HTP. Results : The results of this study are as follows: First, in the pretest of HTP, the subject showed a little lethargy, painting very simple house-shaped figures and too simple rectangular figures corresponding to those painted by 4 or 5-year-old children. In the post-test, however, the structures of houses were described concretely and three-dimensionally, and the descriptions of people were changed into concrete figure, by which we could see the improvement effect on the self expression and the interpersonal relations. Second, in the earlier stage, there was little verbal self expression, and little self respect, staring into space and showing passive attitude without positive assertion and expression. In the later stage, however, there were positive assertion and expression, even showing sense of humor. Lastly, in the earlier stage, there were few interpersonal relations. In the later stage, however, eye contacts and the interpersonal relations were increased during conversation and in the art work process, respectively. Conclusions : These results suggest that the art therapy can be effective on improvement of the self expression and the interpersonal relations with others, as medium for verbal communication of schizophrenic patients.

Investigating the Relationship among Co-Parenting, Maternal Parenting Stress, and Preschoolers' Anxiety and Hyperactivity (부모공동양육 및 어머니의 양육스트레스와 유아의 불안 및 과잉행동 간의 관계)

  • The main purpose of this study was to examine the relationship among co-parenting, maternal parenting stress, and preschoolers' anxiety and hyperactivity with a sample of 155 mothers with 3 to 4 year old children (83 boys and 72 girls) living in Seoul. They completed a questionnaire on co-parenting, maternal parenting stress, and preschoolers' anxiety and hyperactivity. The results were analyzed by means of correlations and regressions. Co-parenting was positively correlated with preschoolers' anxiety and hyperactivity but affectionate, integrated co-parenting was negatively correlated with preschoolers' anxiety and hyperactivity. Maternal stress of parental suffering, dysfunctional interaction, and difficult temperament were positively related to preschoolers' anxiety and hyperactivity. Conflicting co-parenting was positively correlated with maternal stress of parental suffering, but affectionate and integrated co-parenting was negatively correlated with maternal stress of parental suffering and difficult temperament. Furthermore, maternal parenting stress mediated the relationship between co-parenting and preschoolers' anxiety and hyperactivity. Especially, maternal stress of parental suffering tended to play a perfectly mediating role between conflicting and integrated co-parenting and preschoolers' anxiety, between conflicting co-parenting and preschoolers' hyperactivity. Maternal stress of difficult temperament tended to play a perfectly mediating role between integrated co-parenting and preschoolers' anxiety and between conflicting co-parenting and preschoolers' hyperactivity. These results clearly indicate that maternal parenting stress plays a crucial role in the levels of preschoolers' anxiety and hyperactivity.

  • Pulmonary lymphangiomatosis is a very rare pulmonary lesion with an aggressive potential that occurs mainly in newborns, infants and young children of both sexes. It is characterized by pulmonary abnormalities of lymphatic system, showing an increased number of complex anastomosing lymphatic channels in the pleura, in the subpleural interlobular septa, and along the bronchovascular lymphatic route and uniformly fatal. We report a case of lymphangiomatosis behaving like lymphangioleiomyomatosis in a 26-year-old woman.

A Case of Ceftriaxone Associated Biliary Sludge (Ceftriaxone과 연관된 소아의 담도 오니(biliary sludge) 1례)

  • Ceftriaxone, an effective third generation cephalosporin with a wide range of antimicrobial activity, has become widely used by pediatricians for a variety of bacterial infections including meningitis. It has been associated with the development of sludge or stone in the gallbladder of some patients treated with this drug. Ceftriaxone associated biliary sludge has unusual acoustic characteristics and resembles gallstone. The sludge can cause symptoms such as cramping abdominal pain, and disappears after stopping ceftriaxone administration. Because of these seemingly confusing observations, it is important for the clinicians to recognize these findings that ceftriaxone treatment can cause. We report a case of ceftriaxone associated biliary sludge in Korean children. A 6-year-old girl who was treated for meningitis with ceftriaxone (100mg/kg/day) developed cramping upper abdominal pain from 5th hospital day. Physical examination, liver function tests and X-ray revealed no specific abnormal findings. But abdominal ultrasound revealed high amplitude echogenic sludge with prominent post-acoustic shadow in gallbladder and its diameter was 1.5cm. We stopped ceftriaxone administration and tried conservative care. Abdominal cramping pain subsided after 3 days of ceftriaxone removal. Second abdominal ultrasound confirmed the disappearance of sludge at 3 weeks later.

General Anesthesia for Dental Treatment of a Pediatric Patient with Marfan Syndrome (마판 증후군 소아 환자의 전신마취 하 치과치료)

  • Multiple caries in a pediatric patient often requires dental treatment under general anesthesia, especially when the patient is suffering from a systemic disease. The patient was a 6-year-old boy with Marfan syndrome and needed extensive dental treatment. Marfan syndrome is an inherited disorder resulting from mutations in Fibrillin-1 gene. Patients are known to have mainly cardiovascular, ocular, and musculoskeletal problems. Although clinical symptoms of the syndrome are age-realted, thus hindering early diagnosis of the disease in young children, our patient had been confirmed by a gene study at a younger age. Medical history of the patient revealed moderate to severe mitral regurgitation and aortic root dilatation, which required mitral valve replacement surgery with a mechanical valve. As a result, the patient was taking warfarin post-operatively and changes in medication had to be made before the dental treatment. Also, prophyalctic antibiotics had to be given before the treatment for prevention of (to prevent the) infective endocarditis. With careful control of the medications and bleeding tendency, general anesthesia and the treatment were done successfully without any complications.

A Case Report of Cranial Fasciitis in an Adolescent Male (청소년 시기의 두개 근막염 1례: 증례보고)

  • Purpose: Cranial fasciitis is a rare type of benign tumor that occurs mostly in children younger than 6 years. It arises from the deep fascia, periosteum, or fibromembranous layer that covers fontanelles. The etiology is unknown, although prior trauma has been postulated to be an underlying cause. There is a 2:1 male predominance. Despite its rapid growth, this tumor has a benign clinical course and can be cured by total excision. Methods: A 16-year-old male presented with a 3 cm-sized palpable mass in the left lateral eyebrow region that he first noticed 4 months before presentation. The mass had grown rapidly since it was first noticed. Preoperative brain computed tomography showed a well-demarcated mass approximately 3 cm in size extending from the subcutaneous layer to the periosteum. Preoperatively, the presumed diagnosis was a dermoid cyst. An operation was performed with the patient under general anesthesia. The subcutaneous mass was completely excised by periosteal dissection. Results: Histological diagnosis revealed the presence of cranial fasciitis. After 20 months of follow-up, there have been neither complications nor evidence of local recurrence besed on clinical examination. Conclusion: Although cranial fasciitis is quite rare, it should be considered in the differential diagnosis for lytic skull lesions in patient whose clinical presentation suggests this possibility. This condition could be occasionally mistaken for malignant or locally aggressive lesions. To prevent local recurrence, curettage of the underlying bone is recommended for patients with bone involvement.

Caries Management of High-Risk Children by Caries Risk Assessment (우식위험평가에 의한 우식 고위험 유아의 치아우식 관리)

  • The purpose of this study was to classify risk groups according to Caries Risk Assessment (CRA) and to investigate the effect of caries prevention program after 1 year of caries prevention intervention program in 6-year-old infants with high caries risk. The subjects were selected based on responses to CRA questionnaires. At the first visit, oral examination, Cariview, bacteria and saliva flow test were performed. The caries risk group was classified accordingly. The subjects were given fluoride application and oral health education every four months and evaluated the same as the first visit after 1 year. As a result of classifying the risk level according to CRA, more than 80% of the subjects were in the high or extreme high risk. The dft index was increased in all risk groups after the intervention. There was a significant difference between the before and after intervention (p<0.05). The Cariview score showed a slight decrease after the intervention in the moderate and high risk groups. As a result of the evaluation of bacteria test, Streptococcus mutans were decreased to ${\geq}10^5CFU/ml$ saliva after intervention in all groups. Lactobacilli were decreased after intervention in high risk and extreme high risk groups. As a result of saliva flow, there was significant difference between caries risk groups before and after intervention (p<0.05). In conclusion, regular caries management has been shown to influence caries risk factors in high-caries risk children. Also, it is necessary to find out periodical dental risk management system which is suitable for domestic situation through the related studies.
