National or Ethnic Language Fluency and the Quality of Relationship between Parents and Children in Multicultural Family in Korea (한국 다문화 가정 부모자녀 간 한국어 및 계승어 유창성의 일치도가 부모자녀 관계 만족도에 미치는 영향)

  • Korea has entered a multicultural society. As the number of multicultural family grows, identifying factors that facilitate family adjustment seems important. The relationship between parent and children can be a crucial predictor in family adjustment. Thus this study examined how the concordance/discordance in language fluency affects youth's satisfaction towards their parents. This research employed the 2012 National Survey of Multicultural Families (NSMF) using 9 to 24 year-old multicultural youth'(N=4314) data and that of their parents. Since some youth were from the same family, we used Multilevel Models to take into account youth-level data (level-1) and family-level data (level-2). The major findings are as follows: First, concordance/discordance in both national and ethnic language fluency is associated with the quality of relationship between parents and youth. Second, youth's satisfaction toward parents is highest when foreign parent and youth are fluent in Korean, as well as, when Korean parent and youth are fluent in ethnic languages. These findings can suggest directions for Korean multicultural policy to encourage fluency in both Korean and ethnic languages in multicultural families.

  • In order to facilitate the Korean government's efforts in integrating family planning and maternal & child health at the primary health care level (or township level), the Soon Chun Hyang College of Medicine, with the financial and technical assistance of WHO, has under-taken a service research project. The project has employed a quasi-experimental study design introducing interventions tat provide crucial factors lacking in the ongoing government programs such as midwives and qualified referral physicians. The study is being conducted in three locations, one control area and two study areas. Before introducing trained Nurse/Midewives into the study areas, a baseline prevalence survey was undertaken from 15 July 1981 to 10 August 1981 in selelcted townships of Seosan County. In this sample survey of bath the study and control areas, 2,484 eligible women (97% reponse rate) were interviewed to obtain benchmark data on basic evaluation indicators related to family planning and maternal and child health. The salients results were summarized as follows.: 1. CONTACT RATES WITH HEALTH WORKERS; During the year preceding the survey, 12% of women were visited by government health workers. The primary reason for such visits by health workers was family planning (45% of the visits). About 34% of the women visited the health centers during the year. The primary reason for visiting health centers was immunizations for their children (45% of the visits). 3. FAMILY PLANNING USE RATE; The baseline data showed little difference between women in the study area and the control area on contraceptive use. Approximately 59% were currently using some methods. However, among those current users, almost half were practicing less effective methods of birth control such as rhythm or withdrawal. Among other methods, the tubectomy was the most popular (16%), while use of the IUD, oral pill and condom together reached only 14%. 3. PRENATAL CARE RATE; About 75% of the women reported no prenatal care for their last births (the youngest child of each women), Additionally, among women received prenatal care, over half had only one visit. 4. ATTENDANCE AT DELIVERY; Most of the women surveyed (over 80%) were attended by a non-medical person during their last delivery. These figures are somewhat comparable to the national figure of 84% for remote areas. 5. POSTNATAL CARE; The proportion of women reporting postnatal care was only 4.5%, and postnatal care was not received by the majority of women surveyed. 6. CHILD HEALTH CARE: In contrast to the low rate of maternity care for women themselves, most women reported obtaining immunization care for their children. About 75% of the women obtained Polio and/or DPT, 58% BCG, and 44% Measles vaccine for their children. However, in terms of illness care, while 35% of the women stated that their youngest child had been sick during the month preceding the survey, only 28% of these women took their child to the clinic for treatment. 7. COMPLICATIONS OF PREGNANCY AND DELIVERY AND ABNORMALITIES IN THE NEWBORN; Among all last deliveries, 18% of the women had pregnancy complications and 9% of the women had complications during delivery About 5% of the women reported abnormality in their most recent newborn. 8. REPRODUCTION EFFICIENCY; PERINATAL MORTALITY AND INFANT MORTALITY Based on data from the pregnancy history in this survey, reproduction efficiency was estimated. Out of the 11,154 pregnancies reported by all women surveyed, foetal loss was 21% (almost 16% were induced abortions) and infant deaths before reaching one year old were 3.1%. The reproduction efficiency was, therefore, reduced to 76%. In terms of perinatal and infant mortality rates, the former was 40.2 per 1,000 total births and the latter was 39.3 per 1,000 live births. Both rates described J shaped relationships with age of mothers and parity, and they were also correlated with birth interval and mother's education. In summary, this baseline survey data indicated a need for (1) improving contraceptive practices with more effective methods to prevent unwanted pregnancies and (2) providing better services for maternal and child care to protect wanted pregancies. In the Korean rural setting. the author believes that the latter is more important as the value of each child has increased as a result of the family planning campaign for the past two decades. This calls for more effective integration of Family Planning and MCH programmes to meet the needs of the family in each stage of the child bearing and rearing period with deploying more qualified personnel than the current government program personnel.

A Study on Tonsillectomy of the Elemeniary School Children in Busan Area (도시(都市) 국민학생(國民學生)의 편도선(扁桃腺) 절제율(切除率) 및 학부모(學父母)의 인식도(認識度))

  • In order to understand how the parents who have the children of elementary school actually recognising their children's tonsillectomy, and then, analyze its motivation, and how many children was treated tonsillectomy in our country, we hereby analyzed and intended for 3,882 of the children of elementary school in Susan area from 1st grade to the 6 years grade by the questionnaire through the parents of students. The viewpoint about the requirement of tonsil in the human body, the only 25.1 percent had responded: absolutely necessary. The respondents who responded absolutely necessary were highly ranked according to the urban, medium urban and then outskirt area. The advisory person who whishes, to be tonsillectomised was physicians 76.2 percent of the highest point. Operation had performed mostly the age of 6 year and had appeared the questionnaire through 6-9 year old, and they utilized their operational period was at the time of summer vacation of August 31.5 percent and winter vacation of December and January 16.7 percent respectively, and place of implementation was accounted for 87.0 Opercent at the hospitals and the motivation of tonsillectomy had motivated during one had been medical examination and advisory of the physician was appeared highest rate of 51.9 percent. In the case who's symptom of the cold and high fever somewhat improved was appeared as 79.6 percent and the other hand, 5.6 percent of them have responded not knowing they got better. The case that otitis media or chronic nasal inflammation had restored was 13.0 percent respectively, but 18.5 percent had responded their ailment was not knowing their improvement so well that ailed before operation. The sequela by aftermath of the tonsillectomy, seeing collectively, it appeared 16.7 percent of the person was responded they had sequela and the idea of operational result, 90.7 percent responded they had favoured their operation, and 3.7 percent regret their carelessness. As above mentioned, it is advisable to be operated by the degree of its symptoms and there is any complication existed or in case of absolutely necessary case.

Genetic Characterization for Human Enterovirus Isolated from Busan Region in Korea (부산지역에서 분리된 엔테로바이러스의 유행양상 분석 및 유전자형 연구)

  • Human enteroviruses (HEV) are considered one of the major infectious causes of central nervous system infections such as aseptic encephalomeningitis in pediatrics. This study was focused on providing information related to genetic characteristics and diversities of HEV which prevailed between 2007 and 2009 in Busan, Korea. A total of 2,743 specimens were collected from children and screened for isolation of HEV by cell culture and RT-PCR. Among the specimins, 240 isolates were grouped into 21 different HEV serotypes using VP1 RT-PCR. The major etiological agents were CV-A6 and CV-B2 in 2007, E-6 and E-30 in 2008 and CV-B1 in 2009. The occurrence of HEV infections was the most frequent in the summer (May to August, 188 cases, 78.3%). Most of the isolates were identified from specimens from children under 10 years old, with the highest occurrence in the 2 to 4 year old range (15.2%). However, there were no significant differences between male and female children for the isolates. For analyzing genetic characterization, VP1 gene was amplified by RT-PCR and sequenced. The phylogenetic tree was established by Clustal W method using DNASTAR software. Using the sequence analysis of the VP1 region, it was classified into 2 groups; HEV-A and HEV-B. The HEV-A group contained 6 serotypes and sequences of 31 isolates were compared within each serotype. The HEV-B group contained 10 serotypes and the sequences of 41 isolates were compared within each serotype. Homology analysis of the VP1 region showed that the identity scores of HEV-A and B isolates were different. In conclusion, genetic divergences were observed among the isolates from children between 2007 and 2009 in Busan.

Pseudomembranous colitis in children: Experience of a university hospital in Korea (소아 가막성 대장염: 단일 대학병원의 경험)

  • Purpose : Pseudomembranous colitis (PMC) occurs rarely in children, but its incidences are increasing due to frequent antibiotic use. We investigated the incidence and clinical characteristics of PMC accompanied by bacterial enteritis-like symptoms in children. Methods : Between November 2003 and July 2007 at the Department of Pediatrics, Dongsan Medical Center, we analyzed the medical records of consecutive patients who received antibiotics in the past 1 month, developed bacterial enteritis-like symptoms, and were diagnosed with PMC based on sigmoidoscopy examination and histological findings. Results : Among 22 patients who underwent sigmoidoscopy and biopsy examinations, 11 (50%) were diagnosed with PMC. These 11 patients were aged 2 months-12 years, among whom 5 patients (45.5%) were less than 1 year old. The clinical symptoms were bloody diarrhea (28.6%), abdominal pain or colic (28.6%), watery or mucoid diarrhea (23.8%), vomiting (9.5%), and fever (9.5%). The antibiotics used were penicillins (55.6%), macrolides (27.8%), cephalosporins (11.1%), and aminoglycosides (5.6%). The period of antibiotic use was 3-14 days. The interval between the initial antibiotic exposure and the onset of symptoms was 5-21 days. The results of stool examination of all patients were negative for Clostridium difficile toxin A. Patient distribution according to the degree of PMC was as follows: grade I, 18.2% (2 cases); grade II, 27.3% (3); grade III, 36.4% (4); and grade IV, 18.2% (2). PMC did not recur in any case.Conclusion : PMC is not a rare disease in children. If pediatric patients receiving antibiotics manifest symptoms like bacterial enteritis, PMC should be suspected. Endoscopy and biopsy should be applied as aggressive diagnostic approaches to detect this condition.

Chronic Dialysis in Infants and Children Under 2 Years of Age (2세 미만 만성 신부전 환아에서의 만성 투석)

  • Purpose : Dialysis in children with chronic renal failure presents with many difficulies. The purpose of this study is to find an improved method in chronic dialysis in infants and children less than 2 years of age by analyzing the experience with 10 cases. Methods : A retrospective review of the medical records of 10 patients(6 boys and 4 girls) was conducted. The patients had chronic renal failure and underwent chronic dialysis at Samsung medical center from March 1999 to February 2007. Results : At Initiation of dialysis, the median age was 3 months old(22 days-20 months), the median body weight was 3.75 kg(2.2-10.3 kg), and the median serum creatinine level was 4.3 mg/dL(2.0-11.4 mg/dL). The median duration of dialysis was 29.5 months(3-62 months). Dysplastic kidney disease was the most common underlying renal disease. Two patients were treated with hemodialysis, 4 patients with peritoneal dialysis, and 4 patients eventually switched dialysis modality. Nine of the 10 patients took erythropoietin and anti-hypertensive drugs. At the end of the follow up period, 1 patient received kidney transplantation, 2 patients died due to sepsis, and 5 patients were treated with peritoneal dialysis. Two patients were lost to follow up. The most common complication of dialysis was infection. Achieving vascular access and maintaining proper catheter function were the most important factors in treating patients with hemodialysis. The growth status of patients was aggravated after 6 month of dialysis but improved after 1 year of dialysis. Patients showed better growth on peritoneal dialysis than hemodialysis. Conclusion : Chronic dialysis can be performed successfully in infants and children under 2 years of age. Vascular access was the main limitation of hemodialysis, and infection was the common problem in both hemodialysis and peritoneal dialysis. To improve the patients survival rate and quality of life, major efforts should be directed toward the prevention of infection and preservation of catheter function.   • The EEG asymmetry of prefrontal lobe has been researched to analyze the affect such as depression or anxiety and the behavioral characteristics such as BIS(Behavioral Inhibition System)/BAS(Behavioral Activational System) on the basiss of alpha band. However there has been several proposals about the importance of beta asymmetry pointing out the limitation of alpha asymmetry. Here, for 44 numbers of 4-year-old children, by comparing alpha asymmetry and beta asymmetry with BIS/BAS results respectively, we propose the way to classify the characteristics of affect and behavioral trend. We also found that BIS is related with beta-asymmetry, while BAS is related with alpha-asymmetry. As for the affective trait, we use the alpha asymmetry following the other results. Our results seem to be consistent with the other results that the synapse formation of young children is more active at the hemisphere than the right hemisphere.

Knowledge and Attitude Towards Tobacco Smoking among 13-15 Year-Old School Children in Viet Nam - Findings from GYTS 2014

  • Studies have shown that smoking is a learnt behavior, often initiated during adolescence. This paper aims to describe tobacco-related knowledge, attitude and associations among school adolescents aged 13-15 with exposure to anti-smoking information. Using data from the Global Youth Tobacco Survey (GYTS) in Viet Nam, 2014, knowledge was measured through 4 questions about tobacco use, and attitude was assessed through 3 questions on personal, social and environmental aspects. Students giving most anti-tobacco responses to all questions were considered as having correct knowledge or appropriate attitude or both. Access to anti-smoking information was determined by exposure to any media messages on tobacco control during the past 30 days and teaching in school about the danger of tobacco use during the past 12 months. A substantial percentage of students thought that being near others who smoke might be harmful to them and smoking is harmful to health (89.4% and 89.6% respectively). However, only 46.4% reported that it is definitely difficult to quit smoking and 66.9% thought that smoking for only 1 or 2 years, once stopped, is harmful to health. Slightly more than half of the respondents reported appropriate attitude that young smokers have fewer friends than others and smoking makes them less attractive and less comfortable at social events. Noticing anti-smoking messages in the media together with having lessons in school about the dangers of tobacco substantially increased the likelihood of having correct knowledge, appropriate attitude and both. Despite relatively high awareness about smoking harms, effective educational communication is still highly needed to improve the level of comprehensive knowledge and an appropriate attitude regarding tobacco use.

A STUDY OF THE TRAUMATIC INJURIES IN THE PRIMARY DENTITION (유치열기의 치아 외상에 관한 연구)

  • The purpose of this study was to analyze the traumatized primary teeth and investigate the occurrence, types, area, cause, place of trauma to primary teeth and seasonal variation in a hospital population, and to evaluate the interval between injury and treatment. A total 411 children with 745 traumatized teeth participated in this study from 1998 to 2000. The results were as follows : 1. There were slightly more boys than girls giving a male-to-girl ratio of 1.74:1.0. The 1-2, 2-3 year old boys and girls had the highest number of traumatic injuries to their primary dentition. 2. The most common injury was concussion. 3. The majority of traumatized teeth are the upper primary central incisors. 4. Falling down was the most common cause of injury in both sexes. The most of the injuries occurred inside home. 5. 46% of the children visited the dental office within 24 hours after traumatic injury. 6. The most common month was October(12.7%), and June(12.2%), May(11.7%) were followed.

A Study on the Appropriate Manpower Estimation according to the Evaluation of the Blood Collection Workload of Medical Technologists (임상병리사의 채혈 업무량 평가에 따른 적정 인력 산정에 관한 연구)

  • This study explored the method of determining the appropriate size of the workforce according to the assessment of the workload of medical technologists (also called medical and clinical laboratory technologists, and medical and clinical laboratory scientists) in order to present a standard production model for the appropriate manpower in blood collection rooms. The eleven university hospitals selected for this study had between 600 and 2,000 beds. The 14-steps standard blood collection time was 4 minutes and 8 seconds for the outpatients aged between 20 to 60 years old (57%) except for children and the elderly (43%). Assuming that there were 8 hours per day for mechanically collecting blood, the maximum number of blood donations by one clinical laboratory scientist was analyzed to be 100 cases. In conclusion, it is appropriate to have fewer than 100 cases of daily blood collection by a medical technologist engaged in blood collection. Since the proper number of blood collection workers (100% of blood collection work)=the number of annual working days/(one day's work hours/time per case)×the number of working days per year, then the proper number of blood collection workers (one day's work hours)=the number of working days per year/100×the number of working days).
