The Reliability of Preoperative Simulation Surgery Planning for Distraction Osteogensis in Craniosynostosis Patients

  • Background Craniosynostosis management using distraction osteogensis represent a challenge for surgeons due to the great variability of the skull deformity even within the same etiology. The ability to apply the simulation surgery for improving the preoperative planning for distraction osteogensis could improve the results.Planning and Simulation 14 patients presented with craniosynostosis had been subjected to simulation surgery prior to real surgery. 3D CT scans was obtained upon patient admission. Adjustment of all skull position to Frankfort horizontal plane was done. 3 different distraction osteogensis plans were done for each patient according to the skull morphology. For each plane, movement for each bone segment was done according to the pre-planned distraction vectors. Also the distances of distractions were pre-determined according to the cephalic index as well as brain volume. Intraoperatively, we choose the most appropriate plan for the patient by the surgeon. At the end of distraction, 3D CT scan was obtained, and was compared to the simulation plan. Also the distance and the direction of distraction was compared to that of the plan. Accordingly, the distance was almost matching that of the simulation surgery, however the vector of distraction was not matched.Conclusion Preoperative stimulation planning for craniosynostosis patient is very valuable tool in the surgical management of craniosynostosis patients.

아말감 충전물간(充塡物間)의 결합강도(結合強度)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY ON THE BONDING STRENGTH OF THE JOINED AMALGAM RESTORATION)

  • The purpose of this study was to observe the tensile and bonding strength of the joined amalgam restoration. Amalgam alloys of fine-cut (F-type), spherical (S-type), and dispersed type (D-type) were selected in this study, and all specimens were divided into three groups according to the condensation methods as follows. Group I : the control group which condense the same kinds of mixed amalgam into the whole part of the mold respectively. Group II : the group which condense a mix of amalgam into one half of the mold, and then condense a new mix of amalgam into the rest half of the mold 15 minutes later. Group III : the group which condense a mixed amalgam into one half of the mold, and then condense a new mix of amalgam into the rest half of the mold 7 days later. All specimens were stored in incubator at $37{\pm}1^{\circ}C$ for seven days with immersing in saline solution before testing. The tensile and bonding strength of them were measured with Instron Universal Testing machine. The results were as follows: 1. In Group I, the order of tensile strength was F-type, S-type, and D-type. 2. In case of bonding of S-type + S-type, the difference of the bonding strength between Group II and III was not significant. (P> 0.05) 3. The bonding strength of F-type + S-type of Group II was marked the highest in value, and the lowest bonding strength was showed in bonded D-type + D-type of Group III. 4. In case of bonding with the different kinds of amalgam alloy in Group II, the specimen bonded to F-type was marked the highest bonding strength, and the specimen bonded with F-type was marked the lowest one. In Group II, the bonding strength of the specimens bonded with the same kinds of amalgam alloy was presented as the same order as that of Group I. 5. In Group III, the specimen connected with D-type marked the lowest bonding strength of all specimens. In Group III, the bonding strength of the specimens connected with the same kinds of amalgam alloy was the order of S-type + S-type, F-type + F-type, and D-type + D-type.

션트저항을 이용한 3상 인버터의 전압 변조지수 증대 (Improvement of Modulation Index in 3-phase Inverters using Shunt Resistors)

  • This paper has done a hardware-based approach to increase the modulation index in 3-phase inverters, unlike the conventional software algorithm-based approaches. The minimum required time to measure the currents in a three-phase inverters with shunt resistors has also been analyzed. By the analysis, the longest time in minimum required time is AD conversion time. To shorten the minimum required time, this paper proposed a sample-and-hold(S/H) circuit implemented at the inverter current signal output to retain the current signal. When the linear operation region of an inverter with S/H was compared with that without it, the modulation index was increased by 7.8 %. Inverters with S/H circuits can employ the traditional software algorithms, such as the voltage injection method or current restoration method, and it will yield further increase the modulation index.

치과용 접착제(Dental adhesive)를 사용한 제5급 복합레진수복물의 변연누출에 관한 실험적 연구 (IN VITRO STUDY ON THE MARGINAL LEAKAGE OF THE CLASS V COMPOSITE RESTORATION WITH DENTAL ADHESIVE)

  • This study was undertaken to evaluate the degree of the marginal leakage of composite restoration with 3 brands of dental adhesives by means of the dye penetration at the enamel and dentinal margins. 150 cavities of class V were prepared on the buccal and lingual surfaces of 75 extracted anterior and premolar teeth, which were devided into 3 groups. The cavities were filled with composite resin, Silar$^{(R)}$ (3M) and Heliosit$^{(R)}$ (Vivadent) after application of the dental adhesives, specifically Scotchbond$^{(R)}$ (3M) which is essentially composed with halophosphorus ester of Bis-GMA, Dentin Adhesit$^{(R)}$ (Vivadent) which is polyurethane resin, and Enamel Bond$^{(R)}$ (3M) which is a product of Bis-GMA with low viscosity at internal surfaces and margins of the cavities. All specimens were immersed in $37^{\circ}C$, 0.5% methylene blue solution for 24 hours after thermocycling at $4^{\circ}C$ and $60^{\circ}C$, embedded in acrylic resin, and sectioned with diamond disk into two parts. The sectioned specimens observed with the light microscope. The following results were obtained: 1. The group filled with Scotchbond$^{(R)}$-Silar$^{(R)}$ the other two groups at the enamel margins. 2. No significant difference in the degree of the marginal leakage had appeared between Dentin Adhesit$^{(R)}$-Heliosit$^{(R)}$ group an d Enamel Bond$^{(R)}$-Silar$^{(R)}$ group at the enamel margins. 3. Severe marginal leakage with penetration of dye to the floor of cavity had appeared from the all three groups and no significant difference in the degree of marginal leakage existed between the three groups at the dentinal margins.

공초점레이저주사현미경을 이용한 심미수복재와 상아질의 접착계면에 관한 연구 (A CONFOCAL LASER SCANNING MICROSCOPIC STUDY ON THE INTERFACE BETWEEN TOOTH COLORED RESTORATIVE MATERIALS AND DENTIN)

  • The purpose of this study was to evaluate on the interfacial morphology between dentin and restorative materials. In this in vitro study, the cavity wall restorated with 3 different kinds of tooth colored restorative materials [resin-modified Glass Ionomer cement (Fuji II LC), composite resin (Z-100), compomer (Dyract)]. The thirty extracted human molar teeth without caries and/or restorations are used. The experimental teeth were randomly divided into three groups of ten teeth each. In each group, Wedge shaped cavities (width: 3mm, length: 2mm, depth: 1.5mm) were prepared at the cementoenamel junction on buccal and lingual surfaces. The adhesive of composite resin were mixed with rhodamine B. Primer of composite resin, Prime & Bond 2.1 of Dyract and liquid of Fuji II LC were mixed with fluorescein. In group 1, the cavity wall was treatment with dentin conditioner, and then restorated with Fuji II LC. In group 2, the cavity wall was treatment with Prime & Bond 2.1 and then restorated with Dyract. In group 3, the cavity wall was etching with 10% maleic acid, applied with primer and bonding agent and then restorated with Z-100. The interface between dentin and restorative materials was observed by fluoresence imaging with a confocal laser scanning microscope. The results were as follows : 1. In Glass ionomer group, adaptation of resin modified Glass-ionomer restoration against cavity wall is tight, but the crack formed inside of restoration were observed. 2. In Dyract group, the penetration of resin tag is shorter and the width of hybrid layer is narrower than composite resin group. 3. In Z-100 group, primer penetrated deeply through dentinal tubule. Also bonding agent was penetrated along the primer, but the penetration length is shorter than primer part, and in 3-D image, the resin tag is conical shape and lateral branch is observed.

신라시대 교량 월정교 교각기초의 복원안정성 연구 (A Study on the Stability of Foundation for Piers of WoljungGyo Bridge Built in Ancient Silla)

  • 신라의 수도인 경주에 서기 760년에 건설된 것으로 알려진 월정교에 대한 복원공사가 최근 완료되었다. 고문헌에 따르면, 월정교는 축조 이후 고려시대인 서기 1280년까지 약 520년 동안 사용된 것으로 보인다. 월정교지에서는 양측 교대와 4개의 교각기초 및 한두단의 교각석이 발견되었다. 발견된 교각기초 중 하나는 상당한 부등침하가 일어난 상태였다. 따라서 월정교의 안전한 복원을 위해서는 부등침하의 원인을 밝히는 것이 매우 중요하다. 이에 본 연구에서는 월정교 복원공사에 앞서, 지층상태 파악을 위한 시추조사, 2차원 지반강성 평가를 위한 표면파시험, 지지력 평가를 위한 대형평판재하시험 등 광범위한 현장조사를 수행하였다. 지반조사 결과를 바탕으로 기초안정성 평가를 위한 3차원 유한요소해석을 실시하였다. 연구결과, 월정교 교각기초의 하부지반은 월정교 복원에 따른 하중에 대해 지지력 및 침하에 충분한 안전율을 가진 것으로 평가되었다. 따라서 교각기초 하부지반에 대한 추가적인 보강은 필요하지 않은 것으로 판단되며, 복원에 앞서 부등침하가 발생한 교각기초 하부에 자갈을 이용한 평탄화 작업은 실시할 필요가 있다.

Minimal invasive horizontal ridge augmentation using subperiosteal tunneling technique

  • Background: The goal of this study was to retrospectively evaluate the prognosis of minimal invasive horizontal ridge augmentation (MIHRA) technique using small incision and subperiosteal tunneling technique. Methods: This study targeted 25 partially edentulous patients (10 males and 15 females, mean age $48.8{\pm19.7years$) who needed bone graft for installation of the implants due to alveolar bone deficiency. The patients took the radiographic exam, panoramic and periapical view at first visit, and had implant fixture installation surgery. All patients received immediate or delayed implant surgery with bone graft using U-shaped incision and tunneling technique. After an average of 2.8 months, the prosthesis was connected and functioned. The clinical prognosis was recorded by observation of the peri-implant tissue at every visit. A year after restoration, the crestal bone loss around the implant was measured by taking the follow-up radiographs. One patient took 3D-CT before bone graft, after bone graft, and 2 years after restoration to compare and analyze change of alveolar bone width. Results: This study included 25 patients and 39 implants. Thirty eight implants (97.4 %) survived. As for postoperative complications, five patients showed minor infection symptoms, like swelling and tenderness after bone graft. The other one had buccal fenestration, and secondary bone graft was done by the same technique. No complications related with bone graft were found except in these patients. The mean crestal bone loss around the implants was 0.03 mm 1 year after restoration, and this was an adequate clinical prognosis. A patient took 3D-CT after bone graft, and the width of alveolar bone increased 4.32 mm added to 4.6 mm of former alveolar bone width. Two years after bone graft, the width of alveolar bone was 8.13 mm, and this suggested that the resorption rate of bone graft material was 18.29 % during 2 years. Conclusions: The bone graft material retained within a pouch formed using U-shaped incision and tunneling technique resulted with a few complications, and the prognosis of the implants placed above the alveolar bone was adequate.

무치악 환자에서 완전 디지털 시스템을 활용한 All-on-6 임플란트 고정성 보철물 수복 증례 (All-on-6 implant fixed prosthesis restoration with full-digital system on edentulous patient: A case report)

  • 무치악 환자에서 잔존 치조골을 최대의 효율로 활용해 임플란트 고정성 보철물을 수복하는 치료법 중 하나로 All-on-6 개념을 사용할 수 있으나, 치료과정에서 복잡함과 번거로움이 동반된다. 나날이 발전해 온 디지털 시스템을 활용한다면 무치악 환자의 임플란트 식립부터 보철물 제작 및 수복까지 높은 효용성으로 활용할 수 있다. 본 증례에서는 76세 상악 무치악 환자의 일체형 나사 유지형 고정성 임플란트 보철 수복(1-piece design, screw retained implant fixed prosthesis)에 대해 진단부터 임플란트 식립 그리고 보철물 제작까지 전 과정에서 디지털 시스템을 활용하였다. 술전 진단에서 구강직접스캔(Direct intraoral scan)을 통해 환자의 정보를 데이터화 하였으며 이를 토대로 임플란트 식립 수술 가이드와 식립직후 장착할 즉시 임시보철물을 디자인 하였다. 최적의 잔존 치조골 위치에 식립된 6개의 임플란트 위치정보에 따라 정밀하게 제작된 즉시 임시보철물을 수복하여 술 후 사용 가능하도록 하여 환자의 불편감을 최소화하였다. 디지털 시스템을 활용하여 환자의 요구사항, 안정된 수직고경과 교합, 심미성을 고려하여 제작한 새로운 임시보철물을 거쳐 최종보철물을 디자인하고 제작하였다. 디지털 시스템을 활용하여 무치악 환자의 효율적이고 정확한 임플란트 식립부터 보철물 제작까지 전 과정에 있어서 복잡한 임플란트 치료과정을 단순화하여 환자의 불편감을 최소화시키고 기능적이고 심미적인 결과를 얻어 보고하는 바이다.

Vitamin D Improves Intestinal Barrier Function in Cirrhosis Rats by Upregulating Heme Oxygenase-1 Expression

  • Intestinal barrier dysfunction always accompanies cirrhosis in patients with advanced liver disease and is an important contributor facilitating bacterial translocation (BT), which has been involved in the pathogenesis of cirrhosis and its complications. Several studies have demonstrated the protective effect of Vitamin D on intestinal barrier function. However, severe cholestasis leads to vitamin D depletion. This study was designed to test whether vitamin D therapy improves intestinal dysfunction in cirrhosis. Rats were subcutaneously injected with 50% sterile $CCl_4$ (a mixture of pure $CCl_4$ and olive oil, 0.3 mL/100 g) twice a week for 6 weeks. Next, $1,25(OH)_2D_3$ ($0.5{\mu}g/100g$) and the vehicle were administered simultaneously with $CCl_4$ to compare the extent of intestinal histologic damage, tight junction protein expression, intestinal barrier function, BT, intestinal proliferation, apoptosis, and enterocyte turnover. Intestinal heme oxygenase-1 (HO-1) expression and oxidative stress were also assessed. We found that vitamin D could maintain intestinal epithelial proliferation and turnover, inhibit intestinal epithelial apoptosis, alleviate structural damage, and prevent BT and intestinal barrier dysfunction. These were achieved partly through restoration of HO-1 and inhibition of oxidative stress. Taken together, our results suggest that vitamin D ameliorated intestinal epithelial turnover and improved the integrity and function of intestinal barrier in $CCl_4$-induced liver cirrhotic rats. HO-1 signaling activation was involved in these above beneficial effects.

Comparative study on the effects of micro- and nano-sized zinc oxide supplementation on zinc-deficient mice

  • Background: Zinc (Zn) is an essential cofactor for physiological homeostasis in the body. Zn oxide (ZnO), an inorganic compound that supplies Zn, exists in various sizes, and its bioavailability may vary depending on the size in vivo. However, comparative studies on the nutritional effects of micro-sized ZnO (M-ZnO) and nano-sized ZnO (N-ZnO) supplementation on Zn deficiency (ZnD) animal models have not been reported. Objectives: This study investigated the nutritional bioavailability of N-ZnO and M-ZnO particles in dietary-induced ZnD mice. Methods: Animals were divided into six experimental groups: normal group, ZnD control group, and four ZnO treatment groups (Nano-Low, Nano-High, Micro-Low, and MicroHigh). After ZnD induction, N-ZnO or M-ZnO was administered orally every day for 4 weeks. Results: ZnD-associated clinical signs almost disappeared 7 days after N-ZnO or M-ZnO administration. Serum Zn concentrations were higher in the Nano-High group than in the ZnD and M-ZnO groups on day 7 of ZnO treatment. In the liver and testis, Nano-Low and Nano-High groups showed significantly higher Zn concentrations than the other groups after 14-day treatment. ZnO supplementation increased Mt-1 mRNA expression in the liver and testis and Mt-2 mRNA expression in the liver. Based on hematoxylin-and-eosin staining results, N-ZnO supplementation alleviated histological damage induced by ZnD in the testis and liver. Conclusions: This study suggested that N-ZnO can be utilized faster than M-ZnO for nutritional restoration at the early stage of ZnD condition and presented Mt-1 as an indicator of Zn status in the serum, liver, and testis.
