Three-dimensional evaluation of lingual split line after bilateral sagittal split osteotomy in asymmetric prognathism

  • Objectives: The aim of this study was to evaluate the pattern of lingual split line when performing a bilateral sagittal split osteotomy (BSSO) for asymmetric prognathism. This was accomplished with the use of cone-beam computed tomography (CBCT) and three-dimensional (3D) software program. Materials and Methods: The study group was comprised of 40 patients (20 males and 20 females) with asymmetric prognathism, who underwent BSSO (80 splits; n=80) from January 2012 through June 2013. We observed the pattern of lingual split line using CBCT data and image analysis program. The deviated side was compared to the contralateral side in each patient. To analyze the contributing factors to the split pattern, we observed the position of the lateral cortical bone cut end and measured the thickness of the ramus that surrounds the mandibular lingula. Results: The lingual split patterns were classified into five types. The true "Hunsuck" line was 60.00% (n=48), and the bad split was 7.50% (n=6). Ramal thickness surrounding the lingual was $5.55{\pm}1.07$ mm (deviated) and $5.66{\pm}1.34$ mm (contralateral) (P =0.409). The position of the lateral cortical bone cut end was classified into three types: A, lingual; B, inferior; C, buccal. Type A comprised 66.25% (n=53), Type B comprised 22.50% (n=18), and Type C comprised 11.25% (n=9). Conclusion: In asymmetric prognathism patients, there were no differences in the ramal thickness between the deviated side and the contralateral side. Furthermore, no differences were found in the lingual split pattern. The lingual split pattern correlated with the position of the lateral cortical bone cut end. In addition, the 3D-CT reformation was a useful tool for evaluating the surgical results of BSSO of the mandible.

Three-dimensional intraoperative computed tomography imaging for zygomatic fracture repair

  • Objectives: Zygomatic complex (ZMC) fractures comprise up to 40% of all facial fractures. Misaligned bone fragments and misplaced fixation hardware traditionally detected postoperatively on plain radiographs of the skull might require re-operation. The intraoperative O-Arm (Medtronic, USA) is a three-dimensional (3D) computed tomographic imaging system. Materials and Methods: This retrospective single-center study evaluated the utility of O-Arm scanning during corrective surgeries for ZMC and zygomatic arch (ZA) fractures from 2018 to 2020. Three females and 16 males (mean age, 31.52 years; range, 22-48 years) were included. Fracture instability (n=6) and facial deformity (n=15) were the most frequent indications for intraoperative 3D O-Arm scan. Results: The images demonstrated that all fracture lines were properly reduced and fixed. Another scan performed at the end of the fixation or reduction stage, however, revealed suboptimal results in five of the 19 cases, and further reduction and fixation of the fracture lines were required. Conclusion: Implementation of an intraoperative O-Arm system in ZMC and ZA fracture surgeries assists in obtaining predictable and accurate results and obviates the need for revision surgeries. The device should be considered for precise operations such as ZMC fracture repairs.

How image-processing parameters can influence the assessment of dental materials using micro-CT

  • Purpose: The aim of this study was to evaluate the influence of voxel size and different post-processing algorithms on the analysis of dental materials using micro-computed tomography (micro-CT). Materials and Methods: Root-end cavities were prepared in extracted maxillary premolars, filled with mineral trioxide aggregate (MTA), Biodentine, and Intermediate Restorative Material (IRM), and scanned using micro-CT. The volume and porosity of materials were evaluated and compared using voxel sizes of 5, 10, and 20 ㎛, as well as different software tools(post-processing algorithms). The CTAn or MeVisLab/Materialise 3-matic software package was used to perform volume and morphological analyses, and the CTAn or MeVisLab/Amira software was used to evaluate porosity. Data were analyzed using 1-way ANOVA and the Tukey test(P<0.05). Results: Using MeVisLab/Materialise 3-matic, a consistent tendency was observed for volume to increase at larger voxel sizes. CTAn showed higher volumes for MTA and IRM at 20 ㎛. Using CTAn, porosity values decreased as voxel size increased, with statistically significant differences for all materials. MeVisLab/Amira showed a difference for MTA and IRM at 5 ㎛, and for Biodentine at 20 ㎛. Significant differences in volume and porosity were observed in all software packages for Biodentine across all voxel sizes. Conclusion: Some differences in volume and porosity were found according to voxel size, image-processing software, and the radiopacity of the material. Consistent protocols are needed for research evaluating dental materials.

치과용 Cone-beam CT를 이용한 상악동 골 이식 후 나타난 골 이식재의 부피 변화평가 (Evaluation of the Volume Changes of Grafted Bone Materials in Sinus Augmentation Procedure Using Dental Cone-beam CT)

  • 이 연구의 목적은 상악동 골증대술과 동반된 이식된 골의 3차원적인 형태를 평가하고 골이식 재료 및 임플란트의 종류, 수술 방법과 골흡수 정도 사이의 관계를 평가하는 것이다. 임플란트 식립을 위한 상악동 골증대술을 시행받은 환자들의 차트를 후향적으로 검토하였고 골이식재의 부피 및 부위의 방사선학적 분석을 시행하였다. 임플란트 식립 6개월 후 잔존 골 이식재의 비율로 측정된 부피 재생은 이식 재료의 형태 및 수술 방법과 연관성이 있었다. 원광대학교 산본 치과병원에서 15명의 환자에서 상악동 골이식술과 동반하여 53개의 임플란트가 식립되었다. 임플란트 식립 6개월 후 촬영된 컴퓨터 단층촬영에서 $Puros^{(R)}$ 와 다른 여러 가지 골이식재를 혼용한 그룹에비해 $Puros^{(R)}$ 이식재를 단독으로 사용한 그룹에서 더 높은 골흡수 정도를 보였다 (P<0.05). 그리고 수술 방법에 따른 골흡수 정도는 통계학적으로 유의성있는 차이를 보였다 (P<0.05). 골이식과 동시에 임플란트를 식립한 그룹에 비해 지연된 임플란트 식립을 시행한 그룹에서 더 높은 골이식재의 부피 변화율을 보였다. 후향적 자료 분석을 통해 $Puros^{(R)}$ 와 다른 여러 가지 골이식재를 혼용한 그룹에 비해 $Puros^{(R)}$ 이식재를 단독으로 사용한 그룹에서 더높은 골흡수 정도를 보였다. 또한 골이식과 동시에 임플란트를 식립한 그룹에서 지연 식립한 그룹보다 더 낮은 흡수율을 보여주었다.

진료실 CAD-CAM에 의한 컴퓨터 가이드 임플란트 수술과 즉시 임시보철치료: 증례보고 (Computer-guided implant surgery and immediate provisionalization by chair-side CAD-CAM: A case report)

  • 치과와 기공실을 오가며 복잡하게 진행되는 기존 CAD-CAM 컴퓨터 가이드 제작 방식을 단 순화하기 위하여, 진료실에 설치한 바로가이드 시스템(VARO Guide, CAD, Pre-Guide, VARO-mill, NeoBiotech, Seoul, South Korea)을 통하여 Top-Down 개념과 수복 주도개념을 적용하여 1-day 수술과 임시보철을 완료하는 증례를 보고하고자 한다. Pre-Guide를 통해 환자의 중심위에서 교합을 채득하고 CT를 촬영하며, 진료실 내 컴퓨터에서 CAD를 통해 바로가이드를 1시간 내에 제작하였다. 미리 제작된 광중합 레진이 담긴 Pre-Guide 트레이를 술부에 위치시킨 후 중심위에서 CT를 촬영하였다. CT 데이터가 진료실 내 컴퓨터로 전송되면 수복 위치를 정한 후 이어서 3차원적으로 턱뼈에서 임플란트를 식립할 위치를 디자인하여 STL 파일로 추출하였다. 그리고 진료실에 설비된 밀링 기계(VARO-mill)에 Pre-Guide를 고정하여 수술용 가이드를 15분 내에 제작한다. 이 가이드로 환자는 한번 내원하여 임플란트를 계획한 위치에 안전하고 정확하게 수술해 줌과 더불어, 초기고정력이 양호한 임플란트에 대해서는 즉시임시치아까지 장착해 주어 환자에게 즉시 기능이 가능하도록 할 수 있었다.

컴퓨터 단층 촬영 영상에서의 전이성 척추 종양의 정량적 분류를 위한 라디오믹스 기반의 머신러닝 기법 (Radiomics-based Machine Learning Approach for Quantitative Classification of Spinal Metastases in Computed Tomography)

  • Currently, the naked eyes-based diagnosis of bone metastases on CT images relies on qualitative assessment. For this reason, there is a great need for a state-of-the-art approach that can assess and follow-up the bone metastases with quantitative biomarker. Radiomics can be used as a biomarker for objective lesion assessment by extracting quantitative numerical values from digital medical images. In this study, therefore, we evaluated the clinical applicability of non-invasive and objective bone metastases computer-aided diagnosis using radiomics-based biomarkers in CT. We employed a total of 21 approaches consist of three-classifiers and seven-feature selection methods to predict bone metastases and select biomarkers. We extracted three-dimensional features from the CT that three groups consisted of osteoblastic, osteolytic, and normal-healthy vertebral bodies. For evaluation, we compared the prediction results of the classifiers with the medical staff's diagnosis results. As a result of the three-class-classification performance evaluation, we demonstrated that the combination of the random forest classifier and the sequential backward selection feature selection approach reached AUC of 0.74 on average. Moreover, we confirmed that 90-percentile, kurtosis, and energy were the features that contributed high in the classification of bone metastases in this approach. We expect that selected quantitative features will be helpful as biomarkers in improving the patient's survival and quality of life.

Three-dimensional morphometric analysis of mandibule in coronal plane after bimaxillary rotational surgery

  • Background: The aim of this report is to present a new reference for aesthetic mandible surgery using three-dimensional cone-beam computed tomography-based treatment planning for orthognathic surgery which can be implemented in surgical planning and perioperative procedure. Methods: To make an objective standard for evaluating aesthetic mandibular outline, we make an aesthetic scoring criteria with consideration of asymmetry, broad mandibular border line, and prominent mandibular angle. Two maxillofacial surgeons and two orthodontists rated their aesthetical evaluation from 1 to 5. Experimental group consisting of 47 female and 38 male patients who had rotational orthognathic two-jaw surgery from 2010 to 2011 were chosen according to aesthetic scoring done by two maxillofacial surgeons and two orthodontists. A high aesthetic score (${\geq}16$) means the facial contour is symmetric, with no broad and narrow aesthetic mandible frontal profiles. Control A group consisted of ten female and ten male patients who had no orthognathic surgery experience and low aesthetic score (${\geq}10$). Control B group consisted of ten female and ten male patients who had no orthognathic surgery experience and had anaesthetic mandibular frontal profile and a high aesthetic score (${\geq}16$). The three-dimensional image of the patient was taken from dental cone-beam CT (DCT) scanning (experimental group and control A group: 6 months DCT after surgery, control B group: 1st visit DCT). Each DCT was reformatted to reorient the 3D image using 3D analyzing program (OnDemand3D, cybermed Inc, CA, USA). After selection of 12 landmarks and the construction of reoriented horizontal, vertical, and coronal reference lines, 15 measurements were taken in 3D analysis of frontal mandibular morphology. Afterwards, horizontal and vertical linear measurements and angular measurements, linear ratio were obtained. Results: Mean $Go^{\prime}_{Rt}-Me^{\prime}-Go^{\prime}_{Lt}$ angular measurement was $100.74{\pm}2.14$ in female patients and $105.37{\pm}3.62$ in male patients. These showed significant difference with control A group in both genders. Ratio of $Go^{\prime}_{Rt}-Go^{\prime}_{Lt}-Me^{\prime}$ length to some linear measurements (ratio of $Me^{\prime}-Cd^{\prime}_{Rt}Cd^{\prime}_{Lt}$ to $Me^{\prime}-Go^{\prime}_{Rt}Go^{\prime}_{Lt}$, ratio of $Me^{\prime}-Go^{\prime}$ to $Me^{\prime}-Go^{\prime}_{Rt}Go^{\prime}_{Lt}$, ratio of $Go^{\prime}_{Rt}-Go^{\prime}_{Lt}$ to $Me^{\prime}-Go^{\prime}_{Rt}Go^{\prime}_{Lt}$) showed significant difference with control A group in both genders. Conclusion: This study was intended to find some standard measurement of mandible frontal view in 3D analysis of aesthetic patient. So, these potential measurement value may be helpful for orthognathic treatment planning to have more aesthetic and perspective outcomes.

한국인 20대 청년 인체분절의 관성특성에 관한 연구 (Biodynamic Characteristics of Korean Male in Twenties-Mass, Center of Mass and Moment of Inertia Characteristics of Body Segments)

  • The body segment parameters of twelve young male Korean were measured to compare with the results of foreign cadaver studies. A human body was assumed to have fourteen body segments. The mass of each segment was measured with a water immersion test and the mass center of a segment was determined on the balance platform by changing postures. In the case of Korean, because of the difference in body proportion, the mass center of whole-body is located further from the distal end of head(Korean : 44.9% vs. Caucasian : 41.2%), and the mass center of each segment also located in different proportional locations. The existing regression equations, which can estimate segment mass based upon the anthropometric dimensions, estimates segment mass (the mass of shank) for Korean with 13% error. Therefore, it is not recommended to estimate the mass, and the moment of inertia of body segment of Korean based on the existing equations. However, the density information of body constituents was similar enough to apply it to Korean density. It was validated by the comparison between the results of the direct immersion method and 3-dimensional volume reconstruction of segment form the cross sectional images of CT-scan. The average body density measured form twelve subjects was $1.035{\;}kg/m^3$ and showed deceasing trendency.

활로 4 징증의 완전교정술후 비관혈적 방법에 의한 추적관찰 (Non-invasive Evaluation of the Patients after Total Correction of Tetralogy of Fallot)

  • Between January 1984 and December 1986, sixty nine patients, aged 16 months to 25 years [mean age 10.05*6.40 years], underwent total correction of tetralogy of Fallot in Kyungpook national university hospital. In 66 hospital survivors, 30 patients were followed up for 12 to 48 months [mean 30.10*10.26 months]. These 30 patients were classified in two groups, TAP [transannular patch] and Non-TAP group. There were 9 patients in TAP group, and 21 in Non-TAP group. There were no significant differences between two groups in terms of age at operation, follow up duration, ACC time, and bypass time. All patients were evaluated by two dimensional echocardiography, Doppler echocardiography, standard 12-lead electrocardiography, and plain chest X-ray. Right ventricular systolic pressure, pulmonary arterial systolic pressure, pressure gradient between the right ventricle and the pulmonary artery, presence or absence of pulmonary regurgitation and its grading, fractional shortening of the left ventricle, and Qp/Qs in case of remnant ventricular septal defect were obtained by echocardiographic examination. Cardiothoracic ratio was measured by plain chest film, and ventricular dysrrhythmia was detected by electrocardiogram. Comparing the data between two groups, there was significant difference in incidence of postoperative pulmonary regurgitation [p< 0.05], 100%[9/9] in TAP group and 47.6 %[10/21] in Non-TAP group, but all the regurgitations were not severe. There were no significant differences in other comparisons, despite of higher incidence of cardiomegaly in TAP group [CT ratio: 59.3*5.3% VS 54.7*6, 4 %].

Opportunistic investigation of vascular calcification using 3-dimensional dental imaging

  • Purpose: Given the growing use of cone-beam computed tomography (CBCT) scans, this study assessed radiation exposure from these scans in the context of national guidelines and recommended dose limits. Materials and Methods: The current literature was reviewed to quantify the benefit of opportunistic diagnosis of carotid artery calcification relative to the potential risk of radiation-induced cancer. Results: The average radiation from CBCT at its largest field of view and highest resolution possible amounts to a reasonable but still low ionizing radiation exposure. This exposure is comparable to 22 days of background radiation and is notably lower than the radiation exposure from medical CT scans. According to the risk assessment analysis, the risk of stroke events involving internal and external carotid artery calcification (CAC) was 202 and 67 per 100,000 individuals, respectively. In contrast, the estimated risk of radiation-induced cancer associated with CBCT was notably lower, at 0.6 per 100,000. Conclusion: The present study advocates for a comprehensive assessment of CBCT scans encompassing the areas of the internal and external carotid arteries by a knowledgeable professional, given the potential advantages of early detection of vascular abnormalities. Dental professionals who take scans involving these areas need to be mindful of reporting these findings and refer patients to their primary care physician for further investigation.
