Feasibility and Potential of Reduced Port Surgery for Total Gastrectomy With Overlap Esophagojejunal Anastomosis Method

  • Purpose: Reduced port surgery (RPS) for gastric cancer has been frequently reported in distal gastrectomies but rarely in total gastrectomies. This study aimed to determine the feasibility of 3-port totally laparoscopic total gastrectomy (TLTG) with overlapping esophagojejunal (EJ) anastomosis. Materials and Methods: A total of 81 patients who underwent curative TLTG for gastric cancer (36 and 45 patients with 3-port and 5-port TLTG, respectively) were evaluated. All 3-port TLTG procedures were performed with the same method as 5-port TLTG, including EJ anastomosis with the intracorporeal overlap method using a linear stapler, except for the number of ports and assistants. Short-term outcomes, including the number of lymph nodes (LNs) harvested by station and postoperative complications, were analyzed retrospectively. Results: Clinical characteristics were not significantly different among the groups, except that the 3-port TLTG group was younger and had a lower rate of pulmonary comorbidity. There were no cases of open conversion or additional port placement. All operative details and the number of harvested LNs did not differ between the groups, but the rate of suprapancreatic LN harvest was higher in the 3-port TLTG group. No significant differences were observed in the overall complication rates between the 2 groups. Conclusions: Three-port TLTG with overlapping EJ anastomoses using a linear stapler is a feasible RPS procedure for total gastrectomy to treat gastric cancer.

  • Port e-Transformation would employ new technologies such as CRM, SCM, ERP to all the port facilities and improve the efficiency of management for ports. In this paper, firstly, we analyze classification organization of Port e-Transformation technology. Secondly, understand the current situation on each component of Port e-Transformation. Thirdly, present tasks for each component of it. Finally, propose problems and its solving methods for these tasks. However, rationalization of management for internal structure occurred during process of Port e-Transformation and new adjustment of laws related to port are not included in this paper.

A High-Isolation MIMO Antenna with Dual-Port Structure for 5G Mobile Phones

  • In this letter, a new dual-port Multiple-Input Multiple-Output (MIMO) antenna is introduced which has two independent signal feeding ports in a single antenna element to achieve smaller antenna volumes for the 5G mobile applications. The dual-port structure is implemented by adding a cross coupled semi-loop (CCSL) antenna as the secondary radiator to the ground short of inverted-F antenna (IFA). It is found that the port to port isolation is not deteriorated when an IFA and CCSL is combined to form a dual-port structure. The isolation property of the proposed antenna is compared with a polarization diversity based dual-port antenna proposed in the literature [9]. The operating frequency range is 3.3-4.0 GHz which is suitable for places where $4{\times}4$ MIMO systems are supposed to be deployed such as in China, EU, Korea and Japan at the band ${\times}$ (3.3 - 3.8GHz. The measured 6-dB impedance bandwidths of the proposed antennas are larger than 700 MHz with isolation between the feeding ports higher than 18 dB [1-2]. The simulation and measurement results show that the proposed antenna concept is a very promising alternative for 5G mobile applications.

A Study on the Reasonable Service Charges for Incheon North Port(berth 3) under the Private Investment Project

  • Recently, port development has been actively in progress through a private investment project. Incheon North Port functions as an auxiliary port for Incheon Port to solve its chronic demurral and freight congestion and to treat materials such as wood, iron material, feed materials. Service charges is one of important factors in the port choice to treat general goods. In general, service charges can be calculated by two methods such as cost accounting method and market price method. This study will review the characteristics of the Incheon North Port (berth 3), which is under the progress of the private investment project and estimate the reasonable service charges for general goods on the basis of market price. This will help a concessionaire to maximize the operation efficiency.

결절성 변화를 동반한 포도주색 반점의 치료 (Treatment of Port Wine Stain with Nodular Change)

  • Purpose: It's difficult to treat port wine stain with nodular change in Laser therapy, and many patients with port wine stain suffer from restriction in social activity in case that port wine stain located on face. We present our experience in excision of port wine stain and skin graft in two patients who had port wine stain with nodular change on their faces. Methods: After excision of discolorized skin and nodular changed lesion of port wine stain, thick split thickness skin graft was done. Results: There was no recurred nodular hypertrophy for 3 months and 3 years of follow up periods. Conclusion: Surgical treatments of port wine stain with nodular change can lead significant improvement of lesion and social intercourse.

글로벌 항만컨테이너터미널 경쟁력 제고 방안 (Analysis on Enhancement Plan of Competitiveness of Global Port Container Terminal)

  • GTO의 경쟁력 제고 방안 요인 중 대분류 세 가지 요인에 대한 분석결과는 "주관적인 요인"이 4.21점, "경영환경적인 요인"이 3.94점, 그리고 "정부 정책적인 요인"이 3.90점이었다. 세 가지 요인 중 주관적인 요인의 평균평점이 월등히 높아 결국 GTO의 경쟁력을 제고하기 위해서는 GTO 스스로 물동량을 확보하고, 항만물류서비스를 제고하는 등의 노력을 하는 것이 가장 중요하다는 것을 나타내고 있다. 하지만 경영환경적인 요인과 정부 정책적인 요인의 평균평점은 거의 동일하여, 이 두 가지 요인은 GTO의 경쟁력 제고에 거의 동일하게 영향을 미치고 있다는 결론이다. 즉, 주관적인 요인의 평균평점이 월등히 높다는 것은, GTO의 경쟁력을 제고하기 위해서는 항만 컨테이너터미널을 운영하는 GTO 스스로가 가장 우선적으로 (1) 로컬 및 환적 컨테이너 물동량을 확보하고, (2) 신속성 정시성 안정성 등 항만물류서비스를 제고하며, (3) 생산성을 높여 저렴한 요율을 적용하여야 하고, (4) 선사 유치 및 전략적 제휴 등을 통해 물동량을 확보하는 것이 가장 중요하다는 것을 의미한다. 한편 경영환경적인 요인(3.94점)이 정부 정책적인 요인(3.90) 보다 약간 높지만 거의 동일한 평균 평점을 기록하고 있는데, 이것은 경영환경적인 요인, 즉 세계경제의 발전 및 컨테이너 물동량의 증가, 컨테이너선사의 발전 및 Alliance, 항만의 인지도, 중추항만으로서의 기능 등도 GTO의 경쟁력을 제고 하는데 상당히 중요하며, 이와 더불어 선진적인 항만개발정책, 선전진적인 요율 정책, 환적물량 확보 등을 위한 인센티브 정책 등 정부 정책적인 요인도 GTO의 경쟁력을 제고 하는데 상당한 영향을 미치고 있는 것으로 보인다.

고객 세분화에 따른 항만 선호도 비교분석 (The Estimating of Port Preference according to Customer′s Segmentation)

  • 본 연구에서는 항만의 주요고객인 선사를 대상으로, 국외선사와 국내선사로 세분화하여 속성의 중요도와 항만 선호도 차이를 비교분석하였다. 컨조인트 분석 결과 국내선사와 국외선사 간의 항만선호도에서 분명한 차이를 보였으며, 이러한 차이는 기항지와 환적항에서 동시에 나타났다. 국외선사들은 환적항으로서 상하이항을 가장 높게 평가했지만, 국내선사들은 부산항을 상하이항보다 높게 평가했다. 또한, 기항지 선호도 비교에서도 비슷한 결과를 나타냈다. 이러한 연구결과는 국내외 선사에 따라 항만 선호도가 다르기 때문에 집단별로 구분하여 평가할 필요가 있음을 의미한다. 또한, 이를 바탕으로 타항만에 비해 경쟁위위에 있는 부산항의 선호도를 유지하기 위해서는 지속적인 목표(Target) 마케팅이 필요하며, 주요 목표(Target)는 항만의 주요 고객인 선사임을 암시한다.

명시선호(Stated Preference) 방법에 의한 인천남외항 컨테이너 물동량 추정 (Estimating Container Traffic of New Incheon Outer-South Port Using Stated Preference Methodology)

  • Traditional traffic forecast has employed regression analysis or time-series analysis based on past trends of explanatory variables. However, not existing but planned port facilities do not have historical data for traffic estimation. Consequently, arbitrary traffic allocation has been subject to researcher's intuition. In this paper, container throughput at New Incheon Outer-South Port will be estimated using stated preference(SP) and sample enumeration methodology on the basis of survey data about the choice behaviors of port users in a theoretical situation. In the SP survey, shippers, freight forwarders and carriers were required to answer a choice between two alternative ports: Busan and Incheon. Although total 27 scenarios of questionnaires were constructed with 3 levels of 3 explanatory variables, each interviewee was asked to answer for just 9 scenarios chosen at random. A binary choice logit model was applied to the survey data. The elasticity of travel time is estimated to be very high, implying that building New Incheon Outer-South Port could be effective in relieving the congestion of the Kyungin corridor. The analysis result shows that increasing service level at Incheon Port would bring in the substantial diversion of container cargo in the Capital region to Incheon Port from Busan Port.

군산항만의 발전전략에 관한 연구-대 중국교역을 중심으로- (A Study on Development Strategies for Kunsan Port : Focused on trade with China)

  • The establishment of the WTO system means that the global age of trade has officially arrived. Since the integration of the world economy brings about the free movement of goods and services between nations, it is inevitable that sea-bound freight will continue to increase. A recent World Bank report says that China and Korea will be the first and seventh largest economic Powers, respectively, by the year 2020. In particular, the Korea peninsula has a geo-political advantage in being developed as a major Northeast Asian container center. Moreover China's swift uprising needs new order of trade for economy belt in Northeast Asia. Therefore, it can be said that Kunsan Port. which has already been designated as a free trade zone, has greater potential to rise as a regional beach-head port and main region for foreign investment. As such. Kunsan Port will play a major role in accelerating the emergence of the West Sea in international trade. There are several strategies for developing Kunsan Port into a central container port: 1) develop Kunsan port Into an import/export front base f3r multinational corporations, 2) develop Kunsan container port into the core composite container-linked central port, 3) attract foreign investment to the Kunsan Free Trade Area so as to promote balanced development among the regions of Korea, 4) support the central government and local governments to accelerate the coming of the "West Sea Era." These recommendations call for urgent implementation.
