A Clinical Study on the Physiotherapy of Congenital Muscular Torticollis (선천성 유아 사경의 물리치료에 대한 임상적 고찰)

  • Torticollis is a deformity of the neck that shows tilting of the head toward the affected side and rotation of the chin toward the opposite side. In many cases with this condition, unilateral tightness of the sternocleidomastoid muscle is found frequently with fibrous tumor. The analysis of 43 cases of congenital muscular torticollis and result of their physical therapy are reported. The research was from Jan. 1, 1990 to Dec. 31, 1995 at Ulsan Dong Kang Hospital. The results obtained were as follows. 1. There were 30 males(69.8%) and 13 females(30.2%). Age distribution of patients showed below 2 month 20 cases(46.5%), above 1 year 7 cases(16.3%), 5~6/11~12 months 4 cases(9.3%), and 7~8 month 2 cases(4.6%). 2. 25 cases(58.1%) were on the right and 18 cases(41.9%) on the left side. Torticollis was mild to moderately severe in mild 22 cases(51.1%), moderate 18 cases (41.9%) and severe 3 cases(7%). 3. There were normal spontaneous delivery in 36 cases(83.7%), Cesaean section in 4 cases(9.3%) and breech presentation in 3 cases(7%). Sequence of birth, the 1st born babys were 30 cases(69.8%) and the 2nd born babys were 13 cases(30.2%). 4. Above fair result of physiotherapy were 36 cases(83.7%). 5. Of the 43 cases with fellow up, above fair 32 cases(74,4%) and poor 11 cases(25.6%) showed. As conclusion, passive stretch exercise of sternocleidomastoid muscle with early torticollis diagnosis seems more successful in recovery than no exercise.

A Study of 80 Cases of Empyema (농흉의 임상적 고찰)

  • The authors made a clinical study of 80 cases of empyema who were diagnosed and treated at department of chest surgery, St. Mary`s Hospital, Chatholic Medical College, during the period of May.l964 through April.1969 and compared the empyema of infant and children with that of adults. 1. In age and sex ditribution, infant was 6 cases, childhood 22 cases and adult 52 cases. The ratio of male to female was 2.2:1. There`s a little difference in infant-childhood but prominence of males over females in adults was being 3. 3:1, in its ratio. 2. The cardinal symptoms were cough [61.3%], fever [60.0%] and dyspnea [52.8%]. The leukocytosis were observed in 83.7% of all cases, 96.2% of infant-childhood and 76.9% of adults. The hemoglobin level showed subnormal in 82.1% of infant-childhood and in 55.8% of adults. 3. Most frequent lesion to predisposing factor of empyema was pneumonia [43.7%],being prominent in infants children [64.3%] to that of adult 4. The Pathogenic organism by culture in 75 cases of empyema were staphylococuss [48%], streptococuss[9.3%], Gram[-] bacilli [9.3%], Klebsiella[2.7%], pneumococcus[4.0%], E. coli [5.4%] and no growth 21.3% in over all. Among the cases of empyema. staphlocal origin was 62.9% in infant-childfood and 39.6% adults. 5. Staphylococci were most susceptible to erythromycin [86. 1%], Kanamycin [75.0%], albamycin [61.7%] and neomycin [52.8%] but most resistant to penicillin, Chtoramphenicol and terramycin. 6. In the treatment of empyema, of 53 cases were closed thoracotomy drainage and the remainder of cases by open thoracotomy, decortication, thoracoplasty and pleuropneumonectomy. we could attain favourable results by only the closed thoracotomy in infant-childhood, 28 cases. 7. The mortality rate was 6.3% in over all; adult 3 cases, infant and children 2 cases. 3 cases of these, were due to staphylococcal infection.

  • Dsysphagia or difficulty in swallowing is a symptom wich indicates the presence of disease or dysfunction. Because the cause & the original site are variable, it is significant to study the clinicostatistical analysis on 133 cases with complant of dysphagia during the last one year in the department of Otolaryngology, College of Medicine, Seoul National University. The results are as follows; 1) Among the total O.P.D. patients (6313 cases), patients complaining the dysphagia are 2.1%. 2) Male to female sex ratio is 1. 3 to 1, slightly predominent in male and average are 35.7 years. 3) The duration of chief complaint is about 251 days in average, but excluding the 8 cases with more than year of symptom, it is about 40 days. In detailed analysis of the duration; 67 cases (50.4%) occurred within 1 weak: 24 cases (18.0%), 1 week to 1 month: 34 cases (25.6%), 1 month to 1 year: 8 cases (6.0%), more than 1 year. 4) As to the site of origin, the oropharyngolaryngeal origin 106 cases (76.7%) are 4 times as many as the esophageal origin 27 cases (20.3%). 5) As to the etiological classification: inflammation 67 cases (50.4%), tumor 28 cases (21.0%), corrosive agent 14 cases (10.5%), foreign body 11 cases (8.3%) in order. 6) As to the disease entity: pharyngitis and tonsillitis 42 cases (31.6%), stomatitis 14 cases (10.5%), corrosive esophagitis 14 cases (10.5%), laryngeal tumor 12 cases (9.0%), peritonsillar abscess 9 cases (6.8%), esophageal foreign body 7 cases (5.2%), tongue tumor 7 cases (5.2%), tonsillar tumor 6 cases (4.5%), (pharyngeal foreign body 4 cases (3.0%)in order.

Clinical Experience in Open Heart Surgery - A review of the ninety cases - (개심술 치험 90)

  • From June 1984 to Aug. 1986, 90 cases of open heart surgery were performed at the department of cardiothoracic surgery of Kosin Medical College. There were 63 cases of congenital cardiac anomalies and 27 cases of acquired heart diseases. The sex ratio of congenital and acquired heart diseases were represented as 1.4:1 and 1:1.5 respectively. The age distribution was ranged from 7/12 to 56 years old. Among the 63 congenital cardiac anomalies, 49 cases of acyanotic group and 14 cases of cyanotic group were noted. In 49 cases of acyanotic group, 32 VSD, 15 ASD, 1 PS, and 1 RCA-LV fistula were noticed. In 14 cases of cyanotic group, 7 TOF, 1 TOF (Dextrosardia) combined with IVC interruption, 1 Triology of Fallot, 2 Pentalogy of Fallot. 1 DORV, 1 TA with PDA, VSD, ASD and Left SVC and 1 TAPVC were included. Of the 27 acquired heart diseases, 13 mitral, 3 aortic , 6 bival, 3 triplevalvular diseases and 2 LA myxomas were noted. Overall mortality were 11 cases, which included 2 cases of acyanotic heart diseases, 6 cases of cyanotic heart diseases and 3 cases of acquired hear diseases.

Open Heart Surgery in the First Half of 1978: A Report of 112 Cases (1978년도 상반기 개심술 : 112례 보고)

  • Last year in this department 100 cases of open heart surgery were done annually. This year 200 cases of open heart surgery were scheduled. During the first 6 months of this year 112 open heart surgery cases were done with 13 deaths [11.6%]. There were 72 cases of cougenital malformation with 9 operative deaths [12.5%], consisting of 23 acyanotic cases with one death [4.5%] and 49 cases of cyanotic cases with 8 deaths [16.3%]. Out of 40 tetralogy of Fallot, 6 cases expired [15%]. For 39 cases of acquired valvular heart disease and one Ebstein anomaly valves were replaced with 4 operative deaths [10%]. Single valve replacement in 33 with 3 operative deaths and double valve replacement in 7 cases with one death were noted. Two patients expired among 28 mitral valve replacement cases [7.1%]. Among 7 double valve replacement patients, consisting of 3 mitral and aortic and 4 mitral and tricuspid valve replacement one case expired. In a case of Ebstein anomaly, tricuspid valve was replaced with plication of atrialized right ventricle successfully. The operative result was excellent.

A Clinical Study on 29 Cases of Vocal Cord Paralysis caused by Neoplasm (종양에 의한 성대마비 29예에 대한 임상적 고찰)

  • The authors observed the clinical status of 29 patients with vocal cord paralysis caused by tumor from April, 1983 to September, 1993 at Department of Otorhinolaryngology, Yongdong Severance hospital, Yonsei University College of Medicine. The results were as follows: 1) In the kinds of neoplasms, the most frequent were lung Ca. with 13 cases(44.8%), followed by 8 cases by thyroid Ca., 3 cases by neurogenic tumor, 2 cases by mediastinum tumor, cervical esophagus Ca., tracheal Ca., glomus jugulare were 1 case each. 2) In sex distribution, there were 18 cases of males and 11 cases of females with the male to female ratio being 1.8:1. In age distribution, most of the cases(10 cases ; 34.5%) were in the 7th decade. 3) In chief complaints, most of the cases(17 cases : 58.6%) had hoarseness only and aspiration, stridor, dyspnea, cough, dysphagia were present in some cases. 4) In site of the paralysed vocal cord, 21 cases were in the left cord. 5 cases in the right cord and 3 cases in the both cords. 5) In the position of paralysed vocal cord, most of the cases(23 cases : 79.3%) were in the parmedian position.

Clinical Observation of 127 Cases of Wounds of Chest in Viet-Nam War (월남전에서 치험한 흉부손상 120례에 대한 임상적 고찰)

  • During the 35 month period from November 1966 to November 1967 and from June 1971 to March 1973 I had experienced 127 cases of non fatal wounds of chest in Viet-Nam. .Among these 127 cases, 62[45.4%] were gun shot wounds, 49[35.8%] were shrapnel wounds and the other were traffic accident. stab wounds and miscellanous. Approximately 21% of gun shot wound were perforating and 79% were penetrating but all cases of shrapnel wounds were penetrating. Of these 127 cases. 90% evacuated to hospital within 6 hours and average time 2.5 hours. The tranfusion requirement of these cases ranged from zero to 36 pints of whole blood with an average of 2.600cc. Initial intrathoracic findings were hemopneumothorax and hemothorax mostly. and the incidence of open thoracotomy was 9.5%[12cases] and closed thoracotomy was 82.8%[104cases], which were contrast to the reports from Korean conflict. I had experienced 24 cases with complication, such as large hematoma in lung parenchyme[8 cases], atelectasis[4 cases], pyothorax [3 cases], pneumonia [3 cases], fibrothorax [3 cases], pleural effusion [2 cases] and wound infection [2 cases]. Mortality rate for entire group was 4.7% but the cases associated with brain injury was 100%, with spinal cord injury was 50%, with large vessel 50%, and abdominal injury was 33.3%, and nobody died solely of thoracic injury.

  • The purpose of this study was to evaluate the clinical and computed tomographic features of 7 cases of maxillofacial gunshot injuries in the suicidal patients visited the emergency room, Capital Armed Forces General Hospital. The obtained results were as follows : 1. The gunshot wounds were directed from submental area to dorsum of nose(3 cases), frontal area(1 case), orbit(1 case), infraorbital area (1 case), and lateral to nasal wing(l case).   • Branchial cleft cyst is rarely encountered congenital neck disease. It is commonly believed that the branchial anomaly is persistance of remnant of the embryologic branchial apparatus. Among the patients visited Kyung Hee Medical Center with neck mass and inflammatory sign from January, 1980 to Aprial, 1994, we reviewed 26 cases of branchial cleft cysts confirmed by histopathologic findings with retrospective study on clinical aspects. The results are as follows: 1) There was no sex difference(14 male and 12 female), and most common between 2nd and 4th decade(21 cases, 80%). 2) Palpable mass was most common complaint(21 cases). In physical exam, the mass was non-tender, mobile and soft in most cases(over 80%). 3) The most common lesion site was anterior triangle in 15 cases. Along the SCM level, 21 cases were in upper 1/3, 2 cases in middle 1/3 and 3 cases in lower 1/3. 4) According to Bailey's classification, type II were 20 cases(76.9%), type I 5 cases(19.2%) and type III 1 case. 5) Among 28 cases, fistulous tract was found in 6 cases: one was complete type and 5 were incomplete type. 6) Among 26 cases before operation, 10 cases were diagnosed as branchial cleft cyst, 6 cases tuberculosis, 3 cases parotid tumor and 2 cases thyroglossal duct cyst. 7) Type of lining epithelium in histopathologic finding was stratified squamous epithelium in 22 cases(84.6%), mixed type in 4 cases(15.4%).

A Statistical Study of the Foreign Bodies in the Food and Air Passages for past 4 years and 5 months (과거 4년 5개월간 본교실에서 취급한 식도 및 기도이물의 통계적 고찰)

  • The statistcal study was done on 81 cases of the foreign bodies in the food passages (73 cases) and air passages (8 cases), who had visited the Dept. of Otolaryngology of Kyung Hee Medical Center and treated endoscopically, from Nov. 1971 to Mar 1976. The results were as follows, 1) The incidence of esophageal foreign bodies (73 cases) in order of frequency was coin (61 cases; 83.6%), bone (6 cases; 8.2%), food particles (3 cases; 4.1%), discs (2 cases), and other metals (l case). The incidence of foreign bodies in the air passages (8 cases) was food particles (3 cases; 37.5%), pointed metal (2 cases; 25%), plastic pencil cap (l case), plastic ring (1 case), and wood piece (1 case) in order. 2) In sex distribution, 51 cases (62.9%) were males and 30 cases (37.1%) were females, and the ratio between males and females was about 1.7 : 1.3. In the age incidence, 69 cases (82.5%) of all foreign bodies were under 5 years of age, and especially, coin cases were 59 cases (96.7%) and the highest was 8 years of age, lowest was 11 months of age. 7 cases (87.5%) of all airway foreign bodies were under 10 years of age. 4) The ratio between the food passages(73 cases) and the air passages (8 cases) was about 9 : 1. In the location of the foreign bodies of the food passages, 66 cases (90.4%) were in the first esophageal narrowing, 5 cases (6.9%) in the second narrowing, and 2 cases (2.7%) in the third narrowing. In the air passages, the frequent sites were trachea (5 cases; 62.5%), larynx (2 cases), and bronchus (1 case) in order. 5) In duration of lodgement, 63 cases (86. 3%) of esophageal foreign bodies were removed within 24 hours, and one case was removed within 16 days. The number of cases lodged within 24 hours were 4 cases (50%). in the air passages and one case was removed within 18 days. 6) In treatment, 37 cases (50.7%) of esophageal foreign bodies were extracted by modified upper esophagoscopy, 33 cases (42.5%) by esophagoscopy under the local anesthesia, 3 cases (4.1%) by esophagoscopy under the general anesthesia. 8 cases of the coin cases were passed into the stomach during the endoscopic examination. Laryngeal foreign bodies (2 cases; 25%) were extracted by laryngoscopy under the local anesthesia. 2 cases (40%) of tracheal foreign bodies by Bronchoscopy with tracheotomy under the local anesthesia, 3 cases (60%) by Bronchoscopy under the general anesthesia, and bronchial foreign body (1 case) by open thoracotomy under the general anesthesia, at the Dept. of Chest surgery.
