방광요관 역류증 환아에서의 다양한 요단백의 배설 (Urinary Excretion of Various Urinary Proteins in Children with Vesicoureteral Reflux)

  • 목 적 : 방광요관 역류증이나 역류성 신병증을 가진 요로감염 환아에서 다양한 요단백(${\beta}_2$ MG, microalbumin) 또는 효소(NAG)의 배설을 측정하여 방광요관 역류의 정도나 역류성 신병증을 예측할 수 있는 지표 및 방광요관 역류증의 소실에 따른 이들 지표의 변화를 알아보고자 하였다. 방 법 : 요로감염증으로 진단 받았던 환아 중 방광요관 역류와 신반흔이 없었던 환아는 I군(n=30), 방광요관 역류증만 있었던 환아는 II군(n=12), 신반흔 및 방광요관 역류증을 같이 가지고 있었던 환아는 III군(n=34)으로 분류하여 24시간 또는 12시간 소변에서 ${\beta}_2$ MG, microalbumin, NAG를 측정하였다. 방광요관 역류의 정도가 낮은군(reflux grade I, II, III)과 높은군(reflux grade IV, V)으로 분류하여 두 군에서의 요중 ${\beta}_2$ MG, microalbumin, NAG 값을 비교하였다. 방광요관 역류를 가진 46명의 환아 중 16명에서 방광요관 역류가 완전히 소실되었고, 방광요관 역류증이 소실될 때까지 매년 24시간 또는 12시간 소변검사, VCUG, DMSA renal scan을 시행하였다. 결 과 : 1) 24시간 또는 12시간 소변에서의 microalbumin과 NAG는 III군에서 I군과 비교하여 의미있게 증가되어 있었고(microalbumin : $27.7{\pm}26.0mg/gCr$ vs $15.0{\pm}10.7mg/gCr$, P<0.05, NAG : $15.2{\pm}18.7U/gCr$ vs $3.4{\pm}2.2U/gCr$, P<0.05). ${\beta}_2$ MG는 각 군간의 통계적으로 의미있는 차이가 없었다. 2) 방광요관 역류의 정도에 따라서는 높은군(grade IV, V)에서의 NAG값이 낮은군(grade I, II, III) 보다 의미있게 증가되어 있었다($26.8{\pm}27.1U/gCr$ vs $7.6{\pm}3.8U/gCr$, P<0.05). 3) 방광요관 역류증의 소실에 따른 요단백 및 효소의 수치 변화에 있어서는 microalbumin과 NAG가 방광요관 역류증이 완전히 소실되었을 때 통계적으로 의미있게 감소하였다(P<0.05). 결 론 : 소아 요로감염 환아에서 역류성 신병증의 존재 여부 및 추적 관찰에서의 방광요관 역류의 소실여부를 예측하는데 microalbumin과 NAG의 측정이 유용하며, 특히 NAG는 방광요관 역류의 심한 정도를 예측하는데 유용한 지표가 될 수 있으리라 사료된다.

노인의 지각된 건강상태 및 건강개념과 건강증진 행위와의 관계 (The Relationship between Perceived health status, Health conception and Health promoting behavior in the Elderly)

  • This study was undertaken to grasp the relationship between perceived health status, health conception and health promoting behavior in the elderly. The subjects for this study were 157 old persons in Kyung-ju city. Data were collected by interview with questionnaires from August 24 to October 10, 1999. Analysis of the data was done by SPSS program using descriptive statistics, t-test, ANOVA, Pearson Correlation Coefficient, stepwise multiple regression. The results were summarized as follows 1. For the practice of health promoting behavior. the mean score was 2.52. The factor of the highest mean score was nutrition(3.02) and factor of the lowest mean score was exerciset(2.03). 2. According to age(F=3.35, P=.012), sex(t=2.26, P=.025), marital status (F=5.08, P=.007), education(F=4.22, P=.003), family numbed(F=5.31, P= .006), people living together (F=2.77, P=.044), economic status(F=7.10, P=.00l), average monthly pocket money (F=3.35, P=.012), there were differences of health promoting behavior. 3. According to age(F=3.40, P=.01l), sex(t=4.30, P=.000), education(F=8.55, P=.000), economic status(F=12.59, P=.000), average monthly pocket money(F=4.26, P=.003), present disease(t=-8.41, P=.000), there were differences of perceived health status. According to marital status(F=3.36, P=.037), education(F=2.89, P=.038), there were differences of health conception. 4. Performance in the health promoting behavior was significantly correlated with perceived health status(r=.272, P=.00l), health conception(r=.345, P=.000) 5. The combination of well-being health conception and average monthly pocket money explained $24.7\%$ of the variance of health-promoting behavior. On the basis of the above findings the following recommendations are made; Nursing interventions enhancing exercise or activity, accountability for health, stress management of the elderly must be provided. And Cost-effective health promoting program for the elderly must be developed.

인공관절치환수술 노인의 섬망 발생 영향요인 (Influencing Factors of the Incidence of Delirium in Elderly Patients with Arthroplasty)

  • Purpose: This retrospective chart review study was carried out in order to examine the frequency of delirium, and to identify the risk factors associated with the development of delirium in elderly patients with arthroplasty. Methods: Data were collected from medical records of patients who received arthroplasty during one and half year in a hospital. Three hundred sixty five patients were selected for the study. The onset of delirium was reviewed based on the result of psychological doctor's consultation or nurse's assessment with Confusion Assessment Method and delirium onset risk factors were examined. Data were analyzed with descriptive statistics, t-test, $x_2$-test, Fisher's exact test and logistic regression analysis. Results: Delirium occurred with 31 patients (8.5%) out of 365 patients who underwent arthroplasty. There were statistically significant differences between incidence of delirium and gender ($x_2$=6.11, p=.025), age ($x_2$=32.81, p<.001), hearing difficulty ($x_2$=8.08, p=.012), albumin level of preoperational day (t=-3.43, p<.001), albumin (t=-2.20, p=.028) and hemoglobin level (t=-2.83, p=.005) of operational day. Age and gender were the most significant predictive factors with regard to the incidence of delirium. Conclusion: Through understanding of these results, nurses will be able to identify those patients who may be at risk for developing delirium in early stages.

Association Between Screen Overuse and Behavioral and Emotional Problems in Elementary School Children

  • Objectives: This study identified the association between excessive exposure to screen media and behavioral and emotional problems in elementary school students. Methods: A total of 331 parents of children aged 7-10 years were recruited from "The Kids Cohort for Understanding of Internet Addiction Risk Factors in Early Childhood (K-CURE)" study. Children's demographics, household media ownership, screen time, and behavioral/emotional problems were assessed using a parental questionnaire. Children's behavior/emotional problems were measured using the Korean version the of Child Behavior Checklist (K-CBCL) score. Results: The total K-CBCL score in the screen overuse group was 51.18±9.55, significantly higher than 47.28±10.09 in the control group (t=2.14, p=0.05). For each subscale, the externalization score (51.65±10.14, 48.33±8.97, respectively; t=2.02, p<0.05), social problem score (55.41±6.11, 53.24±5.19, respectively; t=2.27, p<0.05), and rule breaking behavior score (55.71±6.11, 53.24±5.19, respectively; t=2.27, p<0.05) were significantly higher in the screen overuse group than in the control group. In addition, the screen overuse group also had a significantly higher usage rate than the control group, even if limited to smartphones, not only on weekdays (3.56±2.08, 1.87±2.02, respectively; t=-4.597, p<0.001) but also weekends (1.62±0.74, 1.19±0.83, respectively; t=-3.14, p=0.003). Conclusion: The study suggested that screen media overuse patterns in children in Korea are particularly relevant to the excessive use of smartphones and are related to higher risks of emotional and behavioral problems.

유용미생물(EM) 기비시용 처리가 오이 생육 및 수량에 미치는 영향 (Effect of Basal Application of Effective Microorganisms on the Growth and Yield of Cucumber)

  • This study was aimed to determine the effect of basal application of Effective Microorganisms (EM) on the grow and yield of cucumber. For treatments, the EM was applied to soil with fertilizer composed with $N-P_2O_5-K_2O$-manure (24.0-16.4-23.8-2,000kg) in the 1.0 strength (defined as EM+1S), 2/3 strength (defined as EM+2/3S), 1/2 strength (defined as EM+1/2S), without fertilizer (defined as EM), or only fertilizer in the 1.0 strength (defined as 1S). In result, there was no significant differences of organic substance content and pH with the EM treatment. While the EC (Electric conductivity) concentration was decreased, plant-available P (phosphorus) was markedly increased. Chlorophyll content was highest in the treatment of EM+standard application rate for both semi-forcing and retarding culture. In contrast, no significant difference was found in plant height and internode length under the fertilizer treatment. Weekly harvested number of cucumber was highest at the treatment of EM+standard application for the semi-forcing culture, while it was 3.6 at the EM+1/2 application for the retarding culture. Weekly yield was greatest at the EM+standard application treatment and decreased with the decrease of fertilizer application rate. In addition, weekly yield was significantly reduced in the treatment of EM. There was no significant difference in yields by production time with the fertilizer applications?. Yield was increased with temperature for the semi-forcing culture, while consistent pattern was maintained for the retarding culture.

Bacillus cereus IAM1072에 의한 올리고당 생산의 최적조건 (Optimum Production Condition of Oligosaccharide by Bacillus cereus IAM1072)

  • 이 연구는 Bacillus cereus IAM1072의 배양액에서 직접 올리고당을 생산하기 위해 행하였다. 올리고당 생성 배양조건은 접종비 3%, pH, 7.0, 온도 $30^{\circ}C$, 배지액량 40ml, 배양시간 24시간이었으며, 올리고당 생성에 가장 효과적인 최적 배지조성은 12% soluble starch, 1.5% perptone, 0.25% $K_2HPO_4$이었다. 생성된 올리고당 중 $G_5$ 올리고당을 정제하였다. Methylation을 한 후 GC 분석을 한 결과 $G_5$ 올리고당은 glucose 단위가 5개로 구성된 ${\alpha}-1,4-glucosidic$ 결합한 올리고당이었다.

자외선 B 조사에 의한 마우스 피부 ATPase 양성 가지세포의 변화 및 녹차투여의 효과 (The Change of ATPase-positive Dendritic Cell and the Effect of Green Tea in Mouse Skin by Ultraviolet B Irradiation)

  • In this study we assessed the influences of ultraviolet (UV) light B radiation on epidermal ATPase-positive dendritic cell (DC) and the effect of green tea treatment in ICR mouse. The extent of changes following 200 mJ/$cm^2$ (0.5 mW/sec) was studied at 0, 6, 12, 18, 24, 30 or 36 hours after exposure. SBCs were decreased by 6 hours after irradiation. There was tendency to decrease from 6 hours to 24 hours and had little further change from then to 36 hours after irradiation. The mice that received 0, 50, 100, 200, 300 or 400 mJ/$cm^2$ of UVB were examined 24 hours after irradiation. The DCs were decreased as the radiation dose increases from 100 to 400 mJ/$cm^2$. The frequency of UVB (200 mJ/$cm^2$)-induced DC decrease was reduced by treatment of green tea (i.p. and topical application, p<0.01).

Rhodotorula glutinis K-24가 생산하는 Invertase Isozymes군에 관한 비교 연구 (Comparative Studies of Invertase Isozymes Produced by Rhodotorula glutinis K-24)

  • 세포내와 세포벽에 invertase가 존재하며 catabolite inhibition을 받지않고 세포외로 구성적으로 invertase를 생산하는 Rh. glutinis K-24를 공시균주로 선택하여 세포내와 세포벽 invertase를 정제한 후 정제효소의 효소화학적 성질을 밝혔다. 세포내 invertase는 DEAE-Sephadex A-50에 의한 Batchment, DEAE-Sephadex A-50 column chromatography, gel filtration on Sephadex G-200, isoelectric focusing 등의 조작에 의해 Disc 전기영동상 단일한 효소단백질까지 정제되었다. 세포벽 invertase는 B. pumilis에 의해 생산되는 세포벽 용해효소를 작용시켜 invertase를 용출 시킨 다음 DEAE-Sephadex A-50 column chromatography, gel filtration on Sephadex G-100의 조작에 의해 부분 정제하였다. 세포내와 세포벽 Invertase의 분자량은 각각 약 310,000과 67,000이었고 세포내 경우는 subunit 분자량이 약 70,000이었다. 세포내와 세포벽 효소의 반응 최적 pH와 온도는 모두 4.0와 6$0^{\circ}C$였으며 온도 안정성도 7$0^{\circ}C$까지 거의 비슷하게 나타났다. 그러나 Km값은 세포내 효소의 경우는 4.3x$10^{-3}$M이었고 세포벽 효소의 경우는 2.1x$10^{-2}$M이었다. 그외 Inhibitor에 대한 효소의 저해양식도 양효소에 비슷한 결과를 나타내었다.

Poly(3-Hydroxybutyric Acid)와 Chitosan 블렌드 필름의 기체 투과도, 유지 투과도 및 생분해도 (Gas and Lipid Permeabilities and Biodegradability of Poly(3-Hydroxybutyric Acid)/Chitosan Blend Film)

  • PHB/chitosan 필름의 수분 투과도는 PHB 필름께서 280.25 g/m$^2$ㆍ24 hr로 가장 높았고 chitosan의 함량이 증파할수록 수분 투과도는 낮아지는 경향을 보였다. PEG가 첨가된 PHB/chitosan 필름에서도 PHB-P 필름의 수분 투과도가 294.5 g/m$^2$ㆍ24 hr로 가장 높았고, chitosan의 함량이 증가할수록 수분 투과도는 낮아졌다. 산소 투과도의 경우 PHB 필름이 0.026${\times}$$10^{-10}$$cm^3$(STP) cm/$cm^2$ㆍsecㆍcmHg로 가장 높았으며 chitosan의 첨가 비율이 증가할수록 산소 투과도는 낮아졌다. 또한 가소제인 PEG가 첨가되지 않은 필름이 PEG가 첨가된 필름보다 산소 투과도가 더 낮았다. 유지 투과도에서는 PHB와 PHB-P를 제외한 필름들이 24시간동안 유지론 통과시키지 않아 유지 차단성이 우수한 것으로 나타났다. 필름의 생분해도 측정시 필름을 첨가하지 않은 대조군에 비해 필름이 첨가된 실험군의 산소 소모량이 높아 필름이 생분해되고 있음을 확인할 수 있었으며 필름의 PHB의 비율이 높을수록 분해도가 높은 것으로 나타났다.

Tolerability and Effect of Early High-Dose Amino Acid Administration in Extremely Low Birth Weight Infants

  • Purpose: The aim of this study is to examine the tolerability and effect of early high-dose amino acid administration in extremely low birth weight infants (ELBWIs). Methods: This retrospective cohort study included ELBWI (birth weight <1,000 g, n=142). Biochemical, nutritional, and neurodevelopmental data were compared between infants who received conventional low amino acid (LAA; 1.5 g/kg/day) and those who received high amino acid (HAA; 3 g/kg/day) within the first 48 hours after birth. Neurodevelopmental data included weight, height, and head circumference at discharge, 12 to 14 and 18 to 24 months of corrected age and the Korean Bayley Scale of Infant Development II (K-BSID-II) score at 18 to 24 months of corrected age. Results: The HAA group demonstrated higher peak plasma albumin ($3.0{\pm}0.4$ vs. $3.2{\pm}0.5$, P<0.05) and lower serum creatinine ($1.7{\pm}0.9$ vs. $1.4{\pm}0.8$, P<0.05) during the first 14 days than the LAA group. Full enteral feeding was achieved significantly earlier in infants in the HAA group than in infants in the LAA group ($46.2{\pm}23.0days$ vs. $34.3{\pm}21days$, P<0.01). There was no difference between the two groups in the z score changes in all growth indicators from birth to discharge and at 12 to 14 and 18 to 24 months of corrected age, as well as in the K-BSID-II score at 18 to 24 months of corrected age. Conclusion: Aggressive administration of amino acids during the first 2 days of life in ELBWI was well tolerated and correlated with earlier full enteral feeding, but did not improve growth and neurodevelopment.
