The Effects of Intravenous Methylprednisolone Pulse Therapy by Mendoza Protocol in Primary and Secondary Nephrotic Syndrome (일차성 및 이차성 신증후군에서 Mendoza Protocol에 의한 Intravenous Methylprednisolone Pulse Therapy의 효과)

  • Purpose : Since Mendoza(1990)'s report that long term methylprednisolone pulse therapy by Mendoza protocol (MP therapy) is a good treatment option in focal segmental glomerulosclerosis(FSGS), there have been reports of the effects of this therapy in steroid-resistant nephrotic syndrome. However, no studies have been performed on the effects of MP therapy in steroid- dependent nephrotic syndrome and secondary nephrotic syndrome. In this study, we investigated the effects of long term MP therapy in primary and secondary nephrotic syndrome in which previous treatment options were not effective. Methods : We chose 10 children who were diagnosed with steroid-dependent minimal change nephrotic syndrome(SD-MCNS), who had shown frequent relapse during the immunocompromised or cytotoxic therapy Period, and 6 children with FSGS and 5 children with secondary nephrotic syndrome children, who had shown no response during the previous therapy period. We treated these patients according to Mendoza protocol involving infusions of high doses of methylprednisolone, often in combination with oral cyclophosphamide for 82 weeks. Results : In all the 10 children with SD-MCNS, complete remission was visible on average of $18{\pm}9$ days after MP therapy was started. However, all these children relapsed during or after MP therapy. In these children, the mean relapse rate prior to MP therapy was $2.1{\pm}1.0$ relpases/year, which was reduced to $1.4{\pm}0.9$ relapses/year during MP therapy(P>0.05) and rose to $2.7{\pm}1.0$ relapse/year after MP therapy. Of the 6 children with FSGS, 4 children($67\%$) showed complete remission, of whom 3 children($50\%$) remained in the remission status during the follow up period, $1.2{\pm}0.7$ years, after the end of MP therapy. 2 children($33\%$) showed no response. All of the 5 children with secondary nephrotic syndrome showed remission and remained in the remissiom status during the follow up period, $1.7{\pm}0.6$ years The only side effect of MP therapy was transient hypertension in 10 children of ail subjects during the intravenous infusion of methylprednisolone. Conclusion : We conclude that although long term MP therapy is not effective in the treatment of SD-MCNS, it is an effective therapy against intractable FSGS and secondary nephrotic syndrome.   • Background : The paradoxical response refers to an enlargement of old lesions or unexpected new ones during apparently adequate antituberculous therapy. This response has been reported in cases of intracranial tuberculoma, tuberculous lymphadenopathy, tuberculous pleurisy and pulmonary tuberculosis. However, there are few reports on its frequency and clinical characteristics. Materials and Methods : This study enrolled 205 patients who were treated with first line antituberculous agents for more than 6 months. We retrospectively studied 155 patients with pulmonary tuberculosis and 57 patients with pleural tuberculosis (7 patients had both) from July 1998 to March 2000. The patients were divided into the paradoxical response group and the non-paradoxical group. The clinical characteristics of the paradoxical response group and the non-paradoxical group. The clinical characteristics of the paradoxical group were investigated. Statistical analysis was done with an independent sample T-test and Chi-squared test. Results : 29 of the 205 patients(14.1%) had paradoxical response. Among the 29 patients, there were 19 pulmonary tuberculosis, 8 tuberculous pleurisy(2 patients had both). Paradoxical response appeared 32 days (mean 35 days in pulmonary tuberculosis, mean 25 days in tuberculous pleurisy) after the beginning of chemotherapy. The duration to regress less than half of initial chest lesion was 114 days in pulmonary tuberculosis and 124 days in tuberculous pleurisy, respectively. Most common clinical manifestation of paradoxical response patients was coughing in both pulmonary tuberculosis and tuberculous pleurisy. Male sex, high blood WBC count and high level of pleural fluid LDH were related with paradoxical response. Conclusion : These findings suggest that presponse usually appears 1 month and disappears within 4 months after the beginning of anti-tuberculous chemotherapy. Paradoxical response was relatively correlated with male sex, high blood WBC count and high level of pleural fluid LDH.

  • This study was conducted to evaluate the potential of indigenous vesicular-arbuscular mycorrhizal fungi(VAMF) in the rhizosphere soil of horticultural crops grown under greenhouse and open-field condition, in the southern area of Kores. Soil samples collected from the rhizosphere of some sellected horticultural crops, such as cucumber, hot pepper, lettuca, tomato and eggplant grown under greenhouse or open-field condition. All tested crops are considered as mycorrhizal plants. The infection rate of horticultural crops investigated ranged from 38% to 70%, hot pepper and eggplant grown under greenhouse condition showed the highest infection being 66.0% and 70.0%, respectively. Spore densities were from 4.8 to 20.0g-1 on dried soil basis. Spore densities of VAMF in the rhizosphere soils under greenhouse condition were higher than that of open-field conditions. The highest distribution of spores in diameter ranged from $75{\mu}m$ to $106{\mu}m$ in the rhizosphere soil of lettuce, cucumber and tomato while those in hot pepper and eggplant ranged from $75{\mu}m$ to $250{\mu}m$. Glomus sp.-type spores predominated in the slightly acid soil(pH 6.3), while Acaulospora sp.-type spores greatly predominated in the very strongly acid field(pH 4.9).

Effect of a 6-month Low Sodium Diet on the Salt Taste Perception and Pleasantness, Blood Pressure and the Urinary Sodium Excretion in Female College Students (6개월간의 저염식이 여자 대학생의 짠맛에 대한 인지와 기호, 혈압 및 뇨 나트륨 배설량에 미친 영향)

  • The study aim was to examine the effect of sensory responses of subjects after 6-month dietary sodium reduction with the aid of nutritional education. Fourteen female college students voluntarily restricted their sodium intake for 6 months, during which time they received nutritional education on the low sodium diet. As a control group, 10 students, whose anthropometric measurement, sodium intake behavior, and blood pressure were not different from those of the experimental group, were maintained on a normal diet. For the sensory responses of subjects, the salt taste perception and pleasantness for graded (0.15-1.3%) NaCl solutions were measured by a 9-point hedonic scale. The optimum sodium concentration, urinary sodium excretion, and blood pressure were measured. All the measurements were done at the beginning and end of the experiment. The sensory evaluation revealed an absence of any difference between the two groups in salt taste perception and pleasantness responses at the beginning. After 6-month adaptation, the experimental group subjects showed higher responses to low NaCl solution (0.15, 0.3, 0.5%) in salt taste perception and pleasantness evaluation while the control group subjects exhibited the opposite response. The optimum sodium concentration was reduced from 105.6 mmol to 80.7 mmol (p = 0.015) and the urinary sodium excretion was also reduced from 1,398 mg to 906 mg (p = 0.041) only in the experimental group. Systolic blood pressure was significantly reduced in the experimental group, although there was no correlation between the urinary sodium excretion and blood pressure. The optimum sodium concentration was negatively correlated with the urinary sodium excretion (r = 0.418, p = 0.053), indicating that adaptation to low sodium diet can reduce sodium intake. Further study on the individual responses of subjects on a low sodium diet by periodical evaluation may provide useful data for setting the duration needed to stabilize a lowered appetite for sodium.

Optimization of Ethanol Extraction Conditions from Glasswort (Salicornia herbacea) Using Response Surface Methodology (반응표면분석법을 이용한 퉁퉁마디 에탄올 추출조건의 최적화)

  • Response surface methodology (RSM) was used to monitor the characteristics of ethanol extracts from glasswort (Salicornia herbacea). A central composite design was used to investigate the effects of the independent variables of sample ratio, extraction temperature, and ethanol concentration on the dependent variables color, sugar, salinity, yield, electron donating ability, and total polyphenol content of extracts. The maximum $^{\circ}Brix$ (8.46) was obtained under specific extraction conditions, with a sample ratio of 7.04 g/100 mL, an extraction temperature of $89.01^{\circ}C$, and an ethanol concentration of 34.29% v/v. At a sample ratio, extraction temperature, and ethanol concentration of 7.00 g/100 mL, $89.15^{\circ}C$, and 34.14% v/v, respectively, the salinity was 7.35%. When the sample ratio, extraction temperature, and ethanol concentration were 5.56 g/100 mL, $68.61^{\circ}C$, and 99.14% v/v, respectively, the maximum electron donating ability was 86.10%. A maximized total polyphenol content of 1,140.15 mg/100 g was found with the following conditions: sample ratio of 8.6 g/100 mL, extraction temperature of $64.19^{\circ}C$, and ethanol concentration of 71.74% v/v. Overall, the optimal ranges of extraction conditions for effective components of glasswort were 3.38.5.33 g/100 mL sample ratio, $55.87-76.96^{\circ}C$, and 25.00.67.31% v/v ethanol.

A Study on the Sailing Speed of Ancient Ships - especially on the average speed and the effect of the wind, the tide, and the man-power at the oar - (고대 선박의 항해속도 연구 - $\ll$고려도경$\gg$을 중심으로-)

  • Xu-Jing(徐競) an official of the Song(宋), a medieval Kingdom of China, wrote a book titled $\ll$Koryo Tu Jing(高麗圖經)$\gg$ which explains his travel to the Koryo as a member of diplomatic mission in 1123. $\ll$Koryo Tu Jing$\gg$ is the record of his personal experience in Koryo with many explanatory illustrations and especially contains 5 months' voyage record of his diplomatic fleet. His fleet set sail at a port located in the Ding Hai Xian(定海縣), Ming Zhou(明州) via a few islands of Koryo [Hyup Kye San(俠界山) , the Kun San Do(群山島) , the Ja Yon Do(紫燕島) , the Keup Su Mun(急水門) in Kang Hwa Gun(江華郡) and the Hap Gul(蛤窟) ] and finally arrived the Port Ye Song Hang(禮成港) . According to the Xu-Jing's record his fleet sailed the sea with the help of the favorable seaward winds and tides as the usual way of ancient sailing. The Xu- Jing's Fleet sailed the sea between the Mei Cen(梅岑), Ming Zhou(明州) of China and the Hyup Kye San(俠界山) of Koryo from about 5:00 a.m., May 24th(of the lunar calendar) to about 5:00 p.m., June 2nd. At this section, the average speed of the seaward winds was 19.45km/h and the average speed of the fleet which sailed only by the power of the winds was 6.29km/h. This means that 32.3% of the favorable seaward winds' speed was equal to the speed of the ancient fleet which sailed only by the power of the favorable seaward winds. The fleet sailed the sea between the Ja Yon Do(紫燕島) and the Keup Su Mun(急水門) from about 9:00 a.m., June 10th to about 1:00 p.m., the same day. At this section the fleet sailed by the power of tides in addition to the favorable seaward winds without oaring. The average speed of the winds was not different from that of former section and the average speed of the tides was 1.937km/h. And at this section the average speed of the fleet increased by 0.41km/h than that of the former section. This means that 21.1% of the speed of the tides was equal to the increased speed of the ancient fleet by virtue of the tides. The fleet sailed the sea between Keup Su Mun(急水門) and the Hap Gul(蛤窟) from about 1:00 p.m., June 10th to about 3:00 p.m., the same day. At this section, there were no seaward winds and the fleet sailed only by the powers of tides and oaring. And at this section, the tide increased the average speed of the fleet by 0.3114km/h and the fleet could sail at the speed of 4.3km/h. So we can conclude that the average speed of ancient fleet without any influences of the seaward winds and tides was 3.98 km/h. We can make use of the various sailing speeds of ancient fleets when judging their maritime activities. If we make use of the various sailing speeds of the ancient fleets as calculated in this article, we will be able to get various important informations about the certain ancient fleet's maritime maneuver. For example, we can infer the sailing routs of a certain fleet and the time when the fleet passed a certain spot by making use of the various sailing speeds of the ancient fleet. In this article I did not take account of the shapes of ships that consist of the ancient fleets and the sizes of the various ships and fleets. It was because that such factors would not change the foresaid conclusions seriously.

Transcatheter Arterial Chemoembolization Combined with Interferon-α is Safe and Effective for Patients with Hepatocellular Carcinoma after Curative Resection

  • Objectives: Intrahepatic recurrence is the major cause of death among patients with hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) after curative surgical resection. Several approaches have been reported to decrease the recurrence rate. The objective of our study was to compare the clinical effects of transcatheter arterial chemoembolization (TACE) combined with interferon-alpha (IFN-${\alpha}$) therapy on recurrence after hepatic resection in patients with HBV-related HCC with that of TACE chemotherapy alone. Methods: We retrospectively analyzed the data from 228 patients who were diagnosed with HBV-related HCC and underwent curative resection between January 2001 to December 2008. The patients were divided into TACE (n = 126) and TACE-IFN-${\alpha}$ (n = 102) groups for postoperative chemotherapy. The TACE regimen consisted of 5-fluorouracil (5-FU), cisplatin (DDP), and the emulsion mixed with mitomycin C (MMC) and lipiodol. The recurrence rates, disease-free survival (DFS), overall survival (OS), and risk of recurrence were evaluated. Results: The clinicopathological parameters and adverse effects were similar between the 2 groups (P > 0.05). The median OS for the TACE-IFN-${\alpha}$ group (36.3 months) was significantly longer than that of the TACE group (24.5 months, P < 0.05). The 3-and 5-year OS for the TACE-IFN-${\alpha}$ group were significantly longer than those of the TACE group (P < 0.05) and the recurrence rate was significantly lower (P < 0.05). The TACE and IFN-${\alpha}$ combination therapy, active hepatitis HBV infection, the number of tumor nodules, microvascular invasion, liver cirrhosis, and the BCLC stage were independent predictors of OS and DFS. Conclusions: The use of the TACE and IFN-${\alpha}$ combination chemotherapy after curative hepatic resection safely and effectively improves OS and decreases recurrence in patients with HBV-related HCC who are at high risk. Our findings can serve as a guide for the selection of postoperative adjuvant chemotherapy for patients with HBV-related HCC who are at high risk of recurrence.

Cultural Characteristics of Rhizoctonia cerealis Isolated from Diseased Wheat Fields and Evaluation of the Resistance of Korean Winter Cereal Crops (밀 잎집눈무늬병원균(Rhizoctonia cerealis)의 배양적 특성과 국내육성 맥류 품종의 저항성 평가)

  • It was identified as a sharp eyespot (Rhizoctonia cerealis) that the isolates from abnormal symptoms in wheat that showed yellowing leaves, necrotic spot on stem base and dead tillers. These isolates have slower growth property and fewer mycelia than Rhizoctonia solani AG-1(1A) (KACC 40106). They showed binuclear cell, same media cultural and DNA characteristics to R. cerealis. They caused same symptoms on leaves and stem base appeared in artificial inoculation test, comparing to diseased wheat fields and also affect to maturing of kernels. They have optimal growth temperature and acidity on the artificial media as $20{\sim}25^{\circ}C$ and pH 5~7, respectively. In the investigation of varietal resistance of Korean winter cereal crops to sharp eyespot, there was no resistant in wheat cultivars that all materials infected over 20% diseased ratio. 12 cultivars including 'Anbaekmil', however, considered to moderate resistance with 20 to 30% infection ratio. The others crops using in feeding, whole crop barley, oat, rye and triticale were resistant below 15% diseased degree except the rye that showed over 50% infection rate. It was the first evaluation to sharp eyespot resistance for the Korean feeding crop cultivars. Most tested Korean barley cultivars for malting and food were moderate and susceptible to the sharp eyespot. Only 3 hulled barley, 'Tapgolbori', 'Albori' and 'Seodunchalbori', showed resistance with less than 10% diseased ratio. All tested naked barley cultivars showed susceptible response to the disease.

Change of Protein and Amino Acid Composition During Chungkook-Jang Fermentation Using Bacillus Licheniformis CN-115 (Bacillus licheniformis CN-115 균주를 이용한 청국장 제조 과장에 있어서 단백질 및 아미노산의 변화)

  • Chungkook-Jang was produced by fermenting Bacillus licheniformis CN-115. The changes of chemical composition, enzyme activity, and amino acids during the fermentation were investigated. The proximate composition was shown irregular fluctuation phenomenon during the fermentation, but only the moisture tended some reducing during the fermentation just after steaming. The content of amino nitrogen was increased radically after the 36 hours of fermentation and became the highest level at 18.072 mg/g at the 60 hours of it. In accordance with the fermentation of Chungkook-Jang, pH got to the 8.39 at 60 hours with increasing, protease activity was increased according to the fermentation and acid and neutral protease activity was reduced after being reached at the highest activity at 48 hours. The most suitable pH was 6.5 and temperature was $35^{\circ}C$ for dissolution-activated of protein in the process of fermentation of Chungkook-Jang. The content of water soluble protein and the content of salt soluble protein were increased at continuously according to the fermentation time of Chungkook-Jang the largest quantity. The molecular weight of water soluble protein of Chungkook-Jang fermented for 48 hours was about 19,000. The amino acids of water soluble protein just after steaming were totally 16 kinds and proline was amino acid and them was in series by glutamic acid and serine in that ordered. The amino acids salt soluble protein, just after steaming were totally 16 kinds and was the largest quantity phenylalanine, glutamic acid and aspartic acid and aspartic acid in that order.

Effects of the Hizikia Mineral Salts on Blood Pressure and Mineral Metabolisms in Spontaneously Hypertensive Rats and Normotensive Rats (톳유래의 무기성분강화염이 본태성 고혈압쥐와 정상쥐의 혈압과 미네랄대사에 미치는 영향)

  • This study was conducted to evaluate the effects of the salts fortified with Hizikia component on blood pressure and mineral composition in spontaneously hypertensive rats (SHR/NCrj) and normotensive rats (WKY/NCrj). The two species rats were assigned to three groups and were fed with drinking water to which Hizikia salts were added for 6 weeks. The final blood pressures (BP) among groups of SHR were increased to 12, 4, and 21%, respectively. In terms of urine and fecal minerals, the $Na^+$ contents in two species were significantly higher in the control than in the salt groups, and vice versa with regard to the $ K^+$ contents. The amount of excretion mineral was higher in urine than in feces. The $Na^+$, $Ca^{++}$ and $Mg^{++}$ contents of the liver in SHR were higher in the control than in the salt groups (p<0.05). The $Na^+$ content of the kidney in SHR was higher in the control than in the salt groups, but the content was shown an opposite trend in WKY. The $K^+$ contents were higher in the low salt group (p<0.05). These results may indicate that the salts fortified with the Hizikia component may be useful in lowering systolic blood pressure owing to the maintenance of positive mineral metabolism.
