Prevalence of Multi-Antibiotic Resistant Bacteria Isolated from Children with Urinary Tract Infection from Baghdad, Iraq

  • Urinary tract infections (UTIs) are one of the most common infections in different age groups, including children. Bacteria are the main etiological agents of UTIs. The aim of the present study was to isolate, identify, and determine the antibiotic susceptibility of bacteria isolated from children with UTIs from Baghdad, Iraq. Three hundred and two urine samples were collected from children aged 6 months to 12 years. The samples were cultured on blood agar and MacConkey agar. The selected colonies were subjected to biochemical tests and antibiotic susceptibility analysis using the Vitek® 2 Compact automated microbial identification system. In this sample, 299 bacteria were identified, of which, 267 were gram-negative bacteria, and 32 were gram-positive bacteria. Escherichia coli (56%) was the most commonly isolated gram-negative bacteria, followed by Pseudomonas aeruginosa (14%), Enterobacter spp. (10.48%), Klebsiella pneumoniae (9.36%), Proteus spp. (7.8%), Acinetobacter baumannii (1.5%), and Morganella morganii (0.37%). Enterococcus faecalis (62.5%) was the most commonly detected gram-positive bacteria, followed by Staphylococcus aureus (37.5%). E. coli and P. aeruginosa were the most antibiotic-resistant bacteria. Among the tested antibiotics, meropenem showed 100% sensitivity, followed by imipenem (97.4%), amikacin (91.8%), and tobramycin (83.5%). In contrast, the high frequencies of resistance were observed with cefixime (93.2%), cefotaxime (78.7%), and ceftriaxone/cefotaxime (71.2%). In conclusion, carbapenems and aminoglycosides are highly recommended for the empirical treatment of UTIs, while, Quinolones, penicillins, and cephalosporins are not suggested. Frequent antibiotics susceptibility testing are warranted to determine the resistance pattern of UTI bacteria.

REGIONAL MICRO-SHEAR BOND STRENGTH TO DENTIN:EFFECTS OF DENTINAL HARDNESS, POSITION, AND REMAINING DENTIN THICKNESS (상아질의 경도, 위치 및 잔존 상아질 후경이 상아질에 대한 부위별 미세 전단결합강도에 미치는 영향)

  • The aim of this study was to measure the regional micro-shear bond strength of dentin bonding agents to dentin, and to investigate the relationship between the micro-shear bond strength and two dentinal characteristics ; Vickers hardness and remaining dentin thickness. Twenty-four freshly extracted, noncarious human molars were selected for this study. The materials tested in this study consisted of two commercially available dentin bonding agents (MAC-BOND, ONE-STEP) and two restorative light-cured composite resins (AELITEFIL, Z100). The occlusal or side surface of tooth crown was sectioned to expose dentin, and the exposed surface was finally polished with # 600 sandpaper. Four groups of application methods were used combining the filling materials and the dentin bonding agents. The composite resin-attached tooth specimens were embeded in a cold cure acrylic resin, and were cut with a low speed diamond saw to the dimension of 1mm $\times$ 1mm. Nine specimens were obtained from each tooth. The cut specimens were divided into three groups depending on the position of the dentin bonding surface. The micro-shear bond strength, remaining dentin thickness, and dentinal hardness were measured. Experimental results were then statistically analyzed with ANOVA. t-test, Scheffe test, and regression analysis. From this experiment, the following results were obtained : 1. In the case of occlusal surface bonding, the pooled micro-shear bond strength of ONST-AELIT group (16.62 MPa) was significantly higher than that of MACB-AELIT group (9.91 MPa) (p<0.05). However, there was no significant difference in the micro-shear bond strength depending on the dentin position (p>0.05). 2. In the case of side surface bonding of crown, the pooled micro-shear bond strength of four different bonding groups was not significantly different among each other (p>0.05). However, in three of the test groups (ONST-AELIT, MACB-Z100, ONST-Z100), the micro-shear bond strength to the lower 1/3(III) position was significantly lower than that to middle 1/3(II) position of surface (p<0.05). 3. In the ONST-AELIT bonding group, the pooled micro-shear bond strength to the occlusal surface was significantly lower than that to the side surface of crown (p<0.05). 4. There was no significant correlation between the micro-shear bond strength and dentin hardness / remaining dentin thickness (p>0.05).

Nutritional Assessment and Management in Long-term Care Insurance's Home Visit Care Service (경기도 일부 노인의 노인장기요양보험 방문요양서비스의 영양상태 평가 및 영양관리 현황)

  • The purpose of the study was to examine nutritional management and nutritional care demands of home-visit care service and the nutritional status of serviced elderly. The survey was carried out from $5^{th}$ to $21^{st}$ of January, 2011 among 37 In-Home Service institutions. The number of elderly surveyed by 143 care helpers was 281. Analysis was performed for 203 subjects (male: 44, female: 159). The age distribution was from 50 to 99 years. The grading by long-term care insuranceshowed 18,2% on level 1 and 2, and 81.8% on level 3. For the disease treatment, the proportion of implementing diet treatment turned out to be very low. The proportion of subjects living with their children was 45.3%, and living alone was 29.1%. The proportion of home-visit care among types of In-Home Service was 70%. Subjects who needed additional necessary diet management service turned out to be 40.9%. Diet management was the most necessary services from all levels. MNA (specifiy MNA) scores were significantly (p < 0.001) associated with BMI, mid-arm circumference (MAC), calf circumference (CC), intake problems, and weight loss during the last 3 months. For studied elderly, 45.3% were malnourished, and 46.8% were at the risk of malnutrition by MNA score. Based on the result of this survey, the nutritional care was not systematic on the In-Home Service. Prevention and management of the disease was much better than the treatment to improve the quality of life. We conclude that nutrition management is necessary for the elderly. To improve nutritional status of elderly in home care services, systematic nutrition management should be implemented.

A STUDY ON THE MICROBIAL CONTAMINATION OF DENTAL UNIT AND ULTRASONIC SCALER (덴탈유니트의 핸드피스 및 초음파 치석 제거기의 미생물 오염에 관한 연구)

  • The risk of cross-contamination in dental clinic is very high. Those who are engaged in dental clinic are exposed to various microorganisms in saliva and blood of patient. Potential possibility of cross-contamination of patient to patient, patient to dentist, dentist to laboratory technician always exist, which is important in the view of public health. It is well known that microorganisms may cause cross-contamination by suck-back of microorganisms into the water supply line or air supply line of dental unit and sprayed back into the next patient's oral cavity. The majority of microorganisms coming from dental unit are water microorganisms from the main water supply which have colonized the tube within the units and multiplied in the relatively warm and stagnant conditions. The purpose of this study is to measure the extent of microbial contamination of dental unit and ultrasonic scaler, to evaluate that dental unit water supply is suitable for drinking water, and to assess the effect of flushing on reduction of microbial contamination of dental unit and ultrasonic scaler. In the first experiment, water samples(50ml) from 20 dental units and 10 ultrasonic scalers in Seoul National Univ. Hosp. were tested for the presence of coliform. The samples were filtered by membrane filtration technique.(Microfil system, Millipore Co. U. S. A.) The filter was then placed onto MacConkey agar plate and the plates with filter on it were incubated aerobically at $37^{\circ}C$ for 5 days. The colors and shapes of colonies were examined if those were coliform. To verify the presence of coliform, the colonies were inoculated into phenol red lactose broth and incubated aerobically at $37^{\circ}C$ for 2 days. The fomation of gas was observed. In the second experiment, water samples from 20 handpieces, 10 ultrasonic scalers and 30 A/W syringes after 0, 2, 4, 6 min. flushing respectively were taken. $200{\mu}l$ water samples were spreaded on Brain Heart Infusion agar plate and the plates were incubated aerobically at $37^{\circ}C$ for 5 days. The number of colony was counted. The results obtained were summarized as follows 1. The water from dental unit and ultrasonic scaler was not suitable for drinking water. 2. No coliform was founded in dental unit and ultrasonic scaler water supply. 3. The number of colony of dental unit and ultrasonic scaler was highest in the group of o min. flushing(p<0.05). 4. There was no statistically significant difference in the extent of microbial contamination among handpiece, ultrasonic scaler and A/W syringe (p>0.05). 5. The number of colony was lowest in the group of 4 min. flushing, but there was no statistically significant difference among 2, 4, 6 min. flushing groups.(p>0.05) 6. It is recommended to flush dental unit water line for 4 min. after use on each patient.

Application of Micro-BLISA in Serodiagnosis of Fascioliasis in Cattle (간질증의 효소면역학적 진단)

  • Fascioliasis in cattle is one of the most common and very serious trematode diseases in Korea. In the present study, the enzyme linked immunosorbent assay (ELISA) was applied in the diagnosis of fascioliasis using antigen of Fasciela hepatica, perokidase of conjugate anti-cattle Is G and orthophenylenediamine as a substrate by micro-method technique of Volley et at. (1976b) and MacLaren (1978) with a slight modification. Results obtained from the present study are as follows. 1. In assay for optimal dilution of stock antigen, the antigen (protein contents; 0. Bmgymz) was diluted from 1150 to 1/600 with carbonate buffer (pH 9.6), and then absorbance values were measured with 1/100 diluted sera. The regression equations between the OD values of ELISA and dilution of antigen were log Y: -0.181-0.00127X in infected sera, and log Y: -0.578-0. 000879X in normal sera. The significantly higher (p<0.05) OD value was observed in the former. 2. In assay for optimal dilution of sera, the sera were diluted from 1125 to 1/400 with in PBSJ Tween 20 (pH 7.4), and absorbance values were measured with 1/200 diluted antigen. The regression equation between the OD values of ELISA and dilution of sera were log Y: -0.1540-0.0007238X in infected sera and log Y: -0.4834-0.00116X in normal sera. The former was higher than the latter (p<0.05). 3. In the 27 cases of negative intradermal test, OD values of the ELISA are $0.447{\pm}0.144$, the 95% confidence interval (Mean+2 H SD) of the values was 0.735, and there was no case over the values. Therefore, the sensitivity of the antigen to diagnose fascioliasis was 100% in the negative case. The OD value 0.7 which is designed as a criterion (detection level of positive one) is useful for the performance of the ELISA in fascioliasis. 4. According to the OD value of criterion in the regression equations, the optimal dilutions of stock antigen and serum were 1/250 and 1/100, respectively. 5. In the 58 cases of fascioliasis from which the adult could be found in the bile ducts, the OD value was $0.846{\pm}0.224$. The 75% (44 cattle) among them had higher value with compared to the criterion, and the 60% (20 cattle) of the cases of proliferative cholangitis of 33 cattle which had been infected previousely with Fasciola sp. is higher than the criterion. 6. Prevalence of fascioliasis was 43.4% in the application of the ELISA to 272 cattle which were reared in Jeonbug district.

The way of system operating is pull-down Menu one, It can be processing very efficiently. 5. Date processing in system, we can manage memory volume of computer verlr efficiently using of dynamic allocation variables. 6. We can make it very easy to edit & revise the input data, processed data is saved to diskette in 2 files (TDF, THF) , these are semipermanent preservation.

  • Development of high-speed Internet services and the increased supply of mobile devices have become the key factor for the acceleration of ubiquitous technology. WiBro system, formed with lP backbone network, is a MBWA technology which provides high-speed multimedia service in a possibly broader coverage than Wireless LAN can offer. Wireless telecommunication environment needs not only mobility support in Layer 2 but also mobility management protocol in Layer 3 and has to minimize handover latency to provide seamless mobile services. In this paper, we propose a fast cross-layer handover scheme based on signal strength prediction in WiBro environment. The signal strength is measured at regular intervals and future value of the strength is predicted by Exponential Smoothing Method. With the help of the prediction, layer-3 handover activities are able to occur prior to layer-2 handover, and therefore, total handover latency is reduced. Simulation results demonstrate that the proposed scheme predicts that future signal level accurately and reduces the total handover latency.

New Record of the Brassy Trevally, Caranx papuensis (Carangidae, Perciformes) in Jeju Island of Korea (한국 제주 연안에서 채집된 전갱이과(Carangdiae) 어류, Caranx papuensis의 첫 기록)

  • A single specimen belonging to the family Carangidae was first collected by angling in Seogwi-dong, Seogwipo-si, Jeju-do on 16 October 2023. This individual was identified as Caranx papuensis Alleyne & MacLeay based on morphological traits as following: lateral line gently curving below the first dorsal fin, presence of scaleless area on the thorax, and gill rakers 26. A total of 619 base pairs sequences of the mitochondrial DNA cytochrome c oxidase subunit I region was analyzed, and we found it closely matched to the Japanese C. papuensis (K2P distance=0.54%). We propose its new Korean name "Hwang-jul-jeon-gang-i" based on a yellow band along the lateral line.

Effect of Body Size on Feeding Physiology of an Intertidal Bivalve, Glauconome chinensis Gray (Glauconomidae)

  • To determine the effect of body size on the clearance rate and ingestion rate of small intertidal bivalves, Glauconome chinensis, feeding experiments were conducted on individuals of 12 different size classes, from 4 to 16 mm in shell length. Relationships between morphological parameters were also determined. The clearance and ingestion rates of G. chinensis responded similarly to their body size, ranging from 1.3 to 28.2 mL/hr/ind. and from 24.0 to 458.5, ${\mu}gC/hr/ind$., respectively. Both rates increased significantly (p<0.001) as shell length increased from 4 to 9 mm, although neither rate changed significantly when shell length was in the range from 12 to 16 mm. The weight-specific clearance rate $(CR_w)$ and ingestion rate $(IR_w)$ decreased with increasing body size, with values from 1.0 to 3.1 L/hr/g and from 17.9 to 51.3 mgC/hr/g, respectively. The $CR_w$ of G. chinensis was intermediate compared to those of larger bivalve species. The clearance rate (CR) relative to flesh dry weight (FDW) of G. chinensis were fitted well to the power function: $CR=0.43\times(FDW)^{0.71}\;(r^2=0.89)$. The exponent of fitting equation (0.71) of G. chinensis was higher than those of Mytilus edulis (Walne, 1972), Crassostrea gigas (Walne, 1972), and Placopecten magellanicus (MacDonald and Thompson, 1986).

Effect of IgY on Periodontitis and Halitosis Induced by Fusobacterium nucleatum

  • Fusobacterium nucleatum is a morbific agent in periodontitis and halitosis. Egg yolk antibody (IgY) was obtained from egg yolks from chickens stimulated with F. nucleatum. This study was to assess the effectiveness of IgY on periodontitis and halitosis caused by F. nucleatum in vitro and in vivo. The growth of F. nucleatum was inhibited (p <0.05) by different concentrations of IgY in vitro and the results of a Halimeter show volatile sulfur compounds (VSCs) were reduced to $904{\pm}57ppb$ at a concentration 40 mg/ml of IgY. The changes of fatty acids of F. nucleatum were determined using GC-MS. The scores for odor index of rat saliva were decreased. The major constituent of volatile organic compounds (VOCs) including short-chain acids decreased 46.2% in 10 mg/ml IgY, ammonia decreased 70% in 40 mg/ml IgY, while aldehydes and olefine ketones were almost unchanged. The ELISA assay revealed that IL-6 and TNF-${\alpha}$ were decreased after 4 weeks' IgY treatment. Morphometric (X-ray) and histological analyses (HE) showed that IgY reduced alveolar bone loss and collagen fibers became orderly in rat models. As a result, IgY may have the potential to treat periodontitis and halitosis.
