  • This research was conducted to evaluate the general dietary quality(the balance and diversity of food intake, nutrient intake and adequacy) of 12 to 36-month-old toddler, through examining their actual food intake. It was conducted for 664(male-357, female-307) healthy toddler who were outpatients from Mar. to Aug., 1999 at university hospitals and private hospitals in Seoul and kyunggi province. The anthropometric parameters were measured. The 24-hr recall method were utilized to analyze the nutrient intake by months, based on the classification of The Korea Child Gorwth Standard. As for the subjects of this research, we found that the younger the age, the more frequent the pattern of intake of only 3 kinds of food groups. The results are that 1) the group with 1 point of the DDS(Dietary Diversity Score) mostly took only foods from the milk group, 2) the group with 2 points only took foods from the grain and milk group, 3) the group with 3 points did not take foods from the fruit and vegetable group, and 4) the group with 4 points took just about all kinds of food groups except for the vegetable group. This means that most of them were not taking various kinds of food even after the age of 12 months, and that they took more than twice the recommended amount of foods from the milk group until the age of 20 months. Thus, one could find a tendency of dependance on the milk group for food intake. Food intake for the vegetable group was severely under the recommended allowance, regardless of the age. The intake of iron was less than two thirds of the recommended allowance, except for the group of 12 to 14-month-olds who had been fed with infant formula. The Nutrient Adequacy Ratio(NAR) of major nutrients(protein, calcium, phosphorous, iron, vitamin A, vitamin B$_1$, vitamin B$_2$, niacin, vitamin C) and the Mean Adequacy Ratio(MAR, shows the general quality of dietary intake) was 0.86$\pm$0.12, but the NAR of iron was 0.62$\pm$0.27, which was extremely low compared to those of other nutrients. Therefore, concerning the subjects of this research, the MAR value alone does not provide enough ground to conclude that the subjects dietary quality is satisfactory. • To assess the food habits, nutrient intake and diet quality of preschool children aged 1 to 6 in Pusan by their meals and snacks, dietary survey was conducted with 176 subjects by a questionnaire and 24hr recall method. 63.1% of the subjects often skipped meals and 72.1% of those skipped breakfast in the main. 94.5% had snacks over once per day. The mean energy intake was 1406.4kcal, with 57.7% of energy intake being supplied by carbohydrate, 13.7% by protein, 28.6% by fat. 54.9%, 71.7% and 26.5% of intakes of protein, calcium and iron were supplied respectively by animal food. Most of iron intake came from plant origin. The percent of calcium intake from animal origin was low significantly in a group aged 4 to 6(p<0.05). The mean intakes of calcium, iron and niacin of total subjects were below Recommended Dietary Allo wance(RDA) for Koreans. The mean intakes of iron and niacin of a group aged 1 to 3 were below RDA as well as those of energy, caicium, vitamin B2 and niacin of the group aged 4 to 6. For calcium, iron, vitamin A, niacin and vitamin C, proportions of subjects with intake levels less than 75% of RDA were all over 36%. For most nutrients, proportions of subjects with intake levels less than 75% of RDA were higher in the group aged 4 to 6 than in the group aged 1 to 3. Nutrient adequacy ratios(NAR) were the lowest in iron in case of the group aged 1 to 3 and in calcium in case of the group aged 4 to 6. NAR of iron increased and that of vitamin B2 decreased significantly along increase age(p<0.05). The mean adequacy ratios(MAR) were 0.82 for total subjects, 0.83 for the group aged 1 to 3 and 0.80 for the group aged 4 to 6. The indexes of nutritional quality(INQ) of calcium, iron and niacin of the group aged 1 to 3 were below 1 as well as those of calcium, vitamin B2 and niacin of the group aged 4 to 6. Both of the age groups received 31.6~59.0% of daily intakes of energy, fat, calcium, phosphrous, vitamin B2 and vitamin C from snacks. So snacks clearly play an important role in dietary intake. Both of the NARs of most nutrients and MAR were higher in snacks than in three meals. The INQs of calcium and iron from three meals and those of iron, vitamin A, vitamin B1 and niacin from snacks of the group aged 1 to 3 were all below 1. The INQs of calcium and vitamin B2 from three meals and those of vitamin A and niacin from snacks of the group aged 4 to 6 were all below 1. In conclusion, nutritional characteristics were different between the two age groups. More attention should be paid to improve adequacy of nutrient intake through meals. And dietary management for snacks is needed especially in the group aged 1 to 3.

  • The purpose of this study was to investigate the effects of the nutrition therapy and drug(simvastatin) treatment on the levels of blood lipids for the hyperlipidemic patients according to genetic polymorphism of apo CIII. Subjects of the study consisted of 43(male: 9, female: 34 )hyperlipidemic patients registered to Kyung Hee Medical Center, Intakes of nutrients for the subjects were determined by 24-hr recall method through a personal interviews. The subjects were instructed to take the hypolipidemic and hypocholesterolemic diets. Compliance was monitored through food records, personal interview, and body-weight measurements. The patients changed their basal diet containing 40. 1g fat(182.7mg cholesterol./day) to a diet containing 30.7g fat(139.2mg cholesterol/day) after 12 weeks of nutrition therapy. The distribution of genotypes for apo CIII by Msp I was 32.5% of TT, 50.0% of TC, and 17.5% of CC. aT the beginning of the study, the level of blood triglyceride was the highest for patients with TT and TC. Following the nutrition therapy, patients with CC exhibited the largest drop of blood triglyceride. Following simvastatin treatment with nutrition therapy, blood LDL-cholesterol decreased in most of the patiens with TT. Meanwhile the distribution of genotypes for apo CII by the Fok I was 10.0% of TT, 47.5% of TC, and 42.5% of CC. The level of blood triglyceride was the highest for patients with CC. Following the nutrition therapy, patients with TC exhibited a significant drop for the blood triglyceride. Following simvastatin treatment with nutrition therapy the level of blood LDL-cholesterol decreased in most of the patients with CC. We concluded that dietary habits and food have been changed by nutrition therapy. And blood lipid levels have been decreased by the restriction of intakes of energy, fat and cholesterol. There was variation in the levels of blood lipids according to apo CII polymorphisms. The level of blood lipids responded to nutrition therapy and drug treatment in different was according to genetic polymorphisms. Accordingly, the choice of individualized therapy based on the patient\\`s genetic polymorphism is very important for effective therapy(Korean J Nutrition 33(8) : 813-823, 2000)

  • The purpose of this study was to determine the zinc status of preschool children in Ulsan. The study was conducted in 95 children aged 3 to 6 years by investigating the anthropometric indices and assessing the biochemical analysis. The blood was analysed to assess serum zinc and alkaline phosphatase. And a questionnaire for dietary intakes using 24-hr recall method were performed by mothers of 95 subjects. The dietary intakes of children were analysed to determine the prevalence of inadequate and excessive intakes of zinc with Dietary Reference Intakes for Koreans (KDRIs). WHL (Weight-Length Index) and Kaup index were used to define obesity. The overall prevalence of overweight and obese subjects were 14.7% and 6.3% by WHL, were 15.8% and 13.7% by Kaup index. The mean intakes of zinc by children aged 3${\sim}$5y and 6y were 5.5 ${\pm}$ 1.4 mg/d (75.7% RDA) and 6.7 ${\pm}$ 2.0 mg/d, respectively, that was the level exceeding the estimated average requirement (EAR) and the recommended intake (RI) of Korean Dietary Reference Intakes. Less than 1.3% and 7.9% of children had usual zinc intakes below EAR and RI of KDRIs, respectively. The percentages of children with intakes exceeding the tolerable upper intake level (UL) were 2.6%. The zinc nutritional status by biological assay was found that mean serum zinc and alkaline phosphatase (ALP) of total subjects were 64.0 ${\pm}$ 8.4 ${\mu}$g/dl and 72.8 ${\pm}$ 14.9 U/L, there was not a significant difference between boys and girls. The range of serum zinc level was 45${\sim}$89 ${\mu}$g/dl and children with a low serum zinc concentration by several cut-off points were 18.9${\sim}$55.8%, especially. Serum zinc level was positively correlated to the intakes of calorie, calcium, fiber, iron, zinc, zinc/kg and height (p<0.001). Serum ALP was positively correlated height, weight and WLI. The zinc intake of children aiso showed a positive correlation with height and weight. These results indicate that there were significant correlations between the zinc status and growth of preschool children. Preschool children in Ulsan have dietary zinc intakes that exceed the new DRIs. The present level of intake does not seem to pose a health problem, but if zinc intakes with fortified foods and supplements were considered, the amount of zinc consumed by children may become excessive.

  • The purpose of this study is to provide the information of the aged olds for which basic data are almost no available. The subjects were divided on the base of age 75. The elderly under 75 were named 'the young olds' and over 75 were 'the aged olds'. The aim of this research is to promote health and to improve nutrition, and the survey was made for health promotion behaviors, habits against health risk, dietary management status and diet intake. And it was conducted by 24 hr-recall method and analyzed by nutrients intake. 242 subjects were collected in 12 cities or counties in Gyeonggi Province and 20 elderly people (10 male and 10 female) were selected out of 1 village in each district. Questionnaire about health behavior and dietary management was carried out by interview method through regional home extension workers. Statistical analyses were made by SAS (version 8.1) and Chi-square tests and General Linear Models were used. Out of the subjects the aged olds over 75 was 31.9%, elementary school educated (93.5%), with spouse (40.3%), with adult children(28.6%), monthly living cost of 500-1,000 thousand won(40.3%). Mean age was 78.82 years compared with 69.75 years of the young olds. 46.8% of the aged olds used monthly pocket money over 1000 won and it was lower than 63.3% of the young olds. Only half of the aged olds had regular exercise of walking (77.8%) or with athletic equipment (17.8%). However, the young olds did more frequent walking (82.1 %) and less exercise with athletic equipment (4.8 %), which was significantly different. Kinds of disease were different with the young or the aged olds, as more proportion of cardiovascular disease(37.9%) for the young olds and joint lumbago neuralgia(41.6%) for the aged olds. Dietary management was good (3 meals per day: 93.4%, fixed mealtime: 72.4%, and regular amount: 79.9%). But there was significant difference in side dish varieties and kinds of snacks; for the aged olds only 8% had over 5 sorts (compared with 18.8% of the young olds) and the kinds of snacks were cookie, candy, juice, carbonated beverage for the aged olds (compared with noodle, milk, soybean-milk for young olds). The ratio of nutrients intake (energy, riboflavin and niacin) with RDA was significantly higher for the aged olds than that of the young olds. The surveyed subjects had no difficulties in Activities of Daily Living (ADL), but some of the aged olds had difficulties in Instrumental Activities of Daily Living (IADL) like housekeeping, using transportation, going shopping and making phone calls. These results suggest that low quality of life is linked with low economic status of the rural elderly and congregate meal at village hall would be required because of the lack of side dishes variety for the aged olds. And nutrition education program about good snacks and exercise practice would be needed for the aged olds. By operating nutrition education program the aged olds would enjoy better quality life maintaining or ameliorating IADL abilities.

  • The purpose of this study is to search how to promote health and improve nutrition and health care of the elderly people in rural area. Behaviors for health promotion and habits against health risk were surveyed. Dietary management was analyzed for surveyed nutrient intake by 24 hr -recall method. 242 subjects were collected in 12 cities or counties in Gyeonggi Province and 20 elderly people (10 male and 10 female) were selected out of 1 village in each district. Questionnaire for health behavior and dietary management was carried out by trained interviewers. Statistical analyses were made by SAS (version 8.1) and Chi-square tests and General Linear Models were used. Characteristics of the elderly people were 61-74 year-olds (68.2%), elementary school educated (78.4%), with spouse (51.7%), monthly living cost of 500-1,000 thousand won(43.4%), and monthly pocket money of 50-100 or 100-200 thousand won(33.5%, 26.5%). 41.4% of the subjects checked up medical examination regularly. The alcohol drinking status was significantly different according to gender: high no-drink rate of female (52.5%) and low no-drink rate of male (25.6%). Kinds of disease were different according to gender: higher proportion of cardiovascular disease(46.3%) and diabetes mellitus(8.1 %) in male and joint lumbago neuralgia(44.4%) and osteoporosis(8.6%) in female. Gastrointestinal complaints were nausea (69.0%) and chronic indigestion (17.8%). Constipation (12.0%) and vomiting (4.3%) were more frequent in female. Dietary management was good (3 meals per day: 93.4%., dining with family: 72.4%, regular mealtime: 72.4%, and 3-4 times of snacks per week: 44.9%) except side dish taking of 3-4 kinds only. However, almost one-third of the female elderly ate alone (30.6%) which was well compared with one-fifth of the male (19.7%). And food and nutrients intake were not significantly different according to gender except that male elderly's intake of energy and protein was lower than that of female's. The surveyed subjects had no difficulties in Activities of Daily Living (ADL), but some female elderly had some difficulties with Instrumental Activities of Daily Living (IADL) like working at home, using transport, and going shopping. These results suggest that low quality of life linked with low economic status of the rural elderly and that congregate meal at village hall would be required for the female elderly eating alone. For the undernourished male elderly, it would be needed to provide snacks and to establish nutrition and health surveillance system.

  • The purpose of this study was to evaluate the eating behaviors and dietary quality of various aged adults. This study conducted through questionnaire and dietary survey by 24-hr recall to 850 adults residing in Samcheok. Subjects were divided into four groups according to the age in male and female, which included 20~29 yrs group (male n = 94, female n = 66), 30~49 yrs group (male n = 201, female n = 216), 50~64 yrs group (male n = 88, female n = 96), and $\geq65$ yrs group (n = 46, female n = 43). In male, 20~29 yrs group had a larger proportion of subjects who skipped breakfast, lunch and dinner compared to other groups (p < 0.001, p < 0.05, p < 0.01). The daily food and energy intakes in $\geq50$ yrs group were significantly lower than those of < 50 yrs. Protein and fat intakes of 20~29 yrs group were significantly higher than those of the other groups. But ND (Nutrient Density) of minerals as like calcium, iron and plant iron in 20~29 yrs group were significantly lower than those of other groups. The KDDS (Korean's Dietary Diversity Score) of 20~29 yrs group, 30~49 yrs group, 50~64 yrs group and $\geq65$ yrs group in male were 4.1, 3.8, 3.7 and 3.5 respectively and there was significant difference (p < 0.001). In female, skipping breakfast and dinner were significantly higher in 20~29 yrs group than in other groups (p < 0.001, p < 0.001). And the use of nutrient supplementation was higher in 30~49 yrs group than that of other groups (p < 0.05). The daily energy intakes was higher in 20~49 yrs group than that of $\geq65$ yrs group. The ND (Nutrient Density) of animal protein, vitamins A and $B_1$ in 20~29 yrs group were significantly lower than those of other groups. The KDDS of 20~29 yrs group, 30~49 yrs group, 50~64 yrs group and $\geq65$ yrs group in female were 4.0, 3.9, 3.9 and 3.6 respectively and $\geq65$ yrs group was significant lower than other groups. According to these results, $\geq65$ yrs and 20~29 yrs group had more dietary habit problems and poor nutrition status than 30~64 yrs group. The results of this study revealed that nutritional management and education for adequate meals should be emphasized in adults and elderly.

  • The purpose of this study was to investigate nutrient intake and food behavior according to the nutrition knowledge of middle school students residing in Seoul and Kyunggi-do, Korea. Anthropometric measurements and questionnaires, including 24-hr recall of dietary intake, were collected from 543 male and female middle school students. They were assigned to one of five groups according to their nutrition knowledge : very high group (VHG ; 90 - 100 score), high group (HG ; 80 - 90 score), normal group (NG ; 70 - 80 score), low group (LG ; 60 - 70 score) and very low group (VLG ; < 60 score), and comparisons were made. The mean age of the subjects was 13.7 years old. The mean height, weight, and BMI of male and female students were 161.9 cm, 52.6 kg and 20.2 kg/$m^2$, 157.0 cm, 50.4 kg, and 20.4 kg/$m^2$ respectively. Female students skip breakfast and dinner more frequently than male students do. Male students skipp lunch and eat fast foods more frequently than females do. Protein, calcium, vitamin B$_1$, vitamin B$_2$, niacin, vitamin C, fat, and animal protein intakes in females are shown to be related to their nutrition knowledge. Calcium, protein, animal protein, vitamin B$_2$and niacin intakes are significantly lower in the VLG than in the others. However, vitamin B$_1$and vitamin C intakes are significantly lower both in VHG and VLG. Fat intake in VHG is lowest. Nutrition knowledge of male students is correlated with mothers knowledge, nutrition attitude and nutritional status. On the other hand, in female students, nutrient consumption was lowest in subjects whose nutrition knowledge was highest and lowest. Therefore, nutrient consumption is affected by nutrition knowledge. However, in female students, possibly due to wrong information on diet or prejudice and outlook, nutrient consumption was low even when they scored high in nutrition knowledge. In conclusion, nutrition knowledge of male students is affected by the mothers nutritional knowledge and attitude. Therefore, nutrition education for mothers is very important. In male students, as their nutrition knowledge is low, their nutrient consumption is affect. These results indicate nutrition education and correct information for body image, balanced diet, regularity of meals and food selection for middle school students are required at both school and home.

  • The purpose of this study was to investigate the effect of nutrition counseling on improving metabolic syndrome (MS) risk factors. Sixty-eight subjects were grouped according to their numbers of MS risk factors. Subjects who have three or more risk factors of MS were defined as "High risk", subjects who have two risk factors of MS were defined as "low risk", and subjects who have below two risk factors of MS were defined as "no risk" group. All groups finished nutrition counseling every three weeks for 12 weeks. Anthropometric, dietary assessments (24 hr-recall) and blood samples were measured at 0 and 12 weeks nutrition counseling. After 12 weeks of intervention, anthropometric data (weight, BMl, body fat (%), and waist/hip ratio) were significantly decreased (p < 0.05) in all groups. Daily consumption of calorie was decreased significantly (p < 0.05) in the group of low risk and high risk. Blood level of fasting plasma glucose was significantly decreased (p < 0.001) in all groups after 12 weeks of intervention. Significantly, the fasting plasma glucose level was returned to normal range in the high risk group. The number of people who have three or more risk factors of MS (high risk group) was decreased from 25 to 12. Sum of MS Criteria decreased from 85 to 52 in the group of MS and decreased from 143 to 99 in all groups. These results indicate that nutrition counseling for male workers at the worksite proved to be helpful by reducing the risk factors of MS and thereby reducing the risk of cardiovascular disease. Continuing and systematic nutritional management programs should be developed and implemented for male workers at the worksites.
