Panoramic radiological study to identify locally displaced maxillary canines in Bangladeshi population

  • Purpose : This study was performed to determine the prevalence of maxillary canine impaction on a basis of a single panoramic radiograph in Bangladeshi population. Materials and Methods : A random sample of seven hundred panoramic radiographs was collected from the patient record of a dental clinic. All the selected panoramic radiographs were taken from January 2009 to August 2010 by a single panoramic radiograph machine with the same exposure time (19 seconds) for all radiographs. One hundred and twenty panoramic radiographs were excluded to minimize the selection bias. In a dim lit room, an observer assessed the radiographs on a standard radiographic light box. The position of the impacted maxillary canine was recorded in line with the longitudinal axis of a tooth using the edge of a metal ruler. Data were subsequently put on SPSS 11.5 software and chi-square (${\chi}^2$) tests were applied to find out the association. Results : Among 580 panoramic radiographs it was found that impacted maxillary canines were present in only 7 (1.2%) radiographs. A statistical significant difference was found between the age of the patients and the vertical position of the impacted canines (p=0.000) and between the age of the patients and the horizontal position of the impacted canines (p=0.003). Conclusion : The prevalence was found to be low compared with the present study from the limitation of panoramic image. Further study needs to include three-dimensional imaging modality.

상악동저와 상악 대구치 치근의 위치 관계: 파노라마방사선사진과 Cone beam형 전산화단층영상의 비교 (Comparison of panoramic radiography and cone beam computed tomography for assessing the relationship between the maxillary sinus floor and maxillary molars)

  • Purpose: This study compared panoramic radiography and cone beam computed tomography (CBCT) for evaluating the relationship between the maxillary sinus floor and the roots of maxillary molars. Materials and Methods: Paired panoramic radiographs and CBCT images from 97 subjects were analysed. This analysis classified 388 maxillary molars according to their relationship to the maxillary sinus floor on panoramic radiograph and CBCT. Correlations between these two radiographic techniques were examined. Results: Maxillary molar roots that were separate from the sinus floor showed the same classification in 100% of the cases when using these two imaging techniques. The corresponding percentage for such roots that were in contact with the sinus floor was 75%. When roots overlapped the maxillary sinus floor on panoramic radiographs, only 26.4% of maxillary first molars and 60.0% of second molars showed protrusion of roots into the sinus with CBCT. Conclusion : The results of the study suggest that roots projecting into the sinus on panoramic radiographs require a three-dimensional image in order to analyze the proximity of their apex to the sinus floor.   • The introduction of cone-beam computed tomography(CBCT) and computer software in orthodontics has allowed orthodontists to provide more accurate diagnosis and treatment. The most common use of CBCT imaging allows orthodontists to visualize the precise position of supernumerary or impacted teeth, especially impacted canines. In doing so, the exact angulation of impaction and proximity of adjacent roots can be evaluated by orthodontists, allowing them to choose vector forces for tooth movement while minimizing root resorption. Even though 2-dimensional panoramic images can be used to view the position of the impacted canines, they have limitations because it is not possible to evaluate the impacted tooth position 3-dimensionally. An accurate knowledge of root position improves the determination of success in orthodontic treatment. Nowadays, considering the fast pace of technological development, a combination of intraoral scanning, digital setups, custommade brackets and wires, and indirect bonding may soon become the orthodontic standard. In this paper, this will be discussed along with the digital models.

3D Dental-CT를 이용한 상악 매복 과잉치의 진단 : 증례보고 (DIAGNOSIS AND EXTRACTION OF IMPACTED SUPERNUMERARY TEETH IN THE MAXILLA WITH 3D DENIAL-CT)

  • 과잉치는 법랑기(enamel organ)가 치배형성을 지속하거나 과도한 세포증식을 함으로써 발생한다. 과잉치는 상악 전방부에서 종종 발견되며, 유치열기와 혼합치열기에서 다양한 문제를 야기한다. 과잉치는 형태에 따라서 보충치(supplemental tooth)와 흔적치 (rudimental tooth)로 나눌 수 있다. 보충치는 정상 절치와 크기 및 형태가 유사하나, 흔적치는 비정상적인 형태와 작은 크기를 갖고 있다. 보충치는 영구 측절치 부위에서 호발하며 정상 전치와 유사한 형태와 크기를 갖고 있으므로, 발치시 주의를 기울여야 한다. 본 3증례는 상악 전방부에 정상 전치 형태의 과잉치를 갖는 증례들이다. 모든 증례에서, 치근단 교합, 파노라마 방사선 사진 등의 기존 방사선 사진 촬영 외에 3 Dimensional Dental-Computed Tomography(3D Dental-CT)를 촬영하여 진단하였다. 기존 일반 방사선 사진들(conventional radiographs)과 3D Dental-CT를 비교시, 일반 방사선 사진들은 2차원적인 영상만을 제공하여 상의 해석에 있어서 술자의 주관이 개입되는 한계가 있다. 반면 3D Dental-CT는 과잉치의 정확한 위치와 형태에 대한 객관적 인 정보를 제공하며, 좀 더 보존적 인 수술을 시행하고, 수술 시간과 스트레스를 줄이는데 유용하다. 그러므로 일반 방사선 사진 촬영 후 과잉치의 위치가 불분명하고 인접 영구치의 손상이 의심되는 경우 3D Dental-CT를 이용한 진단이 고려된다.

악관절장애진단에 있어서 동시다층개별화측방단층촬영의 진단효과 (THE DIAGNOSTIC EFFECT OF THE SIMULTANEOUS MULTIFILM INDIVIDUALIZED LATERAL TOMOGRAPHY IN THE DIAGNOSIS OF THE TEMPOROMANDIBULAR DISORDERS)

  • This study was designed to evaluate the diagnostic effect of the simulatneous multifilm individualized lateral tomography in the diagnosis of the temporomandibular disorders. The subjects consisted of 29 patients with symptoms of the temporomandibular disorders. The panoramic view, oblique lateral transcranial radiograph (OLTC) (Hirozontal angulation 0°, Vertical angulation 29°), submentovertex view, and simultaneous multifilm individualized lateral tomographs (SMFI) in centric occlusion (2.5㎜ thickness difference, 5 layers) were taken for the patients. This study compared the findings from each radiographs in the determining of mandibular condylar position with dual linear measurement of the subjective closest posterior and subjective closest anterior interarticular space and in the determining of bony changes on the studied 30 temporomandibular joints (TMJ) with symptoms of the temporomandibular disorders. The results were as follows: 1. The distribution of condylar position of OLTC and 5 layers of SMFI depended on the radiographs (p<0.05). The condylar position and the distribution of condylar position of OLTC were more similar to lateral sections of the SMFI than mesial sections, and in the distribution of the condylar position of SMFI, the more lateral sections of SMFI, the more concentric 2. positions. There were 10 cases in which all layers showed the same condylar position as that of OLTC. There were 3 cases in which no layers showed the same condylar position as that of OLTC. 3. In the SMFI of 30 Temporomandibular joints studied, there is 13 cases in which all five layers represented the same condylar position in the same TMJ and 11 cases in which 4 layers represented the same condylar position in the same TMJ and 6 cases in which 3 layers represented the same condylar position in the same TMJ. So at least 3 layers of SMFI represented same condylar position in the same TMJ. 4. The bony changes were not detected in conventional radiographs on the temporomandibular joint and the bony changes were not detected in simultaneous multifilm individualized lateral tomographs. SMFI provided a meams for a three dimensional visualization of the shape, the position and the extent of bony changes of TMJ.
