Diagnosis of a Trocar Site Mass as Omental Herniation after Laparoscopic Gastrectomy

  • A trocar site hernia is a rare complication. We report a patient who had an abdominal wall mass at a previous trocar site after laparoscopic distal gastrectomy. It was diagnosed as omental herniation and fat necrosis. We conclude that patients with trocar site masses exhibiting fat density on a computed tomography scan could be followed up without surgery, and that fascial defects located at 10-mm or larger trocar sites should be closed whenever possible to prevent hernia formation.

A Review Study on Qigong Interventions for Performance Enhancement in Korean Journal Articles: A Preliminary Study for Developing Qigong Therapy for Elite Athletes (수행능력 향상을 위한 국내 기공 연구 고찰: 스포츠 현장에 적용 가능한 기공 프로그램 개발을 위한 예비 연구)

  • Objectives: To investigate the research trends of qigong interventions to make influence on performance enhancement. Methods: Korean databases were searched for papers related to studies of qigong interventions to assist subjects perform better. Results: Twelve articles published in Korean Journals from 2000 to 2017 were reviewed. Among the retrieved papers, eight were applied with breathing exercises, including Dan-jeon breathing training, one applied with brain breathing meditation with DoIn exercise, one applied with qigong meditation, one applied with qi-training, and one applied with brain breathing training. RoBANS was used for assessment. The quality of selected articles was generally low. 10 of the 12 articles on qigong reported that experimental group showed statistically significant increase of performance enhancement both directly and indirectly. Conclusions: Reviewing 12 articles, this study established that qigong interventions might be beneficial for enhancing performance especially in sports. However, more scientifically designed study on qigong therapy are needed to prove the efficacy and effectiveness.

  • 1. Objectives In oriental medicine, there is no method to diagnosis Sasang constitution of school aged children. The objective diagnostic method is necessary to improve the health condition of children. The method must be able to reduce time and to correct subjective error. So the purposes of this study are developing the questionnaire to diagnosis Sasang constitution of school-aged children. 2. Methods The questionnaire consists of selected substance of ${\ulcorner}$Dongyisusebowon${\lrcorner}$, characteristic questionnaire of children and questionnaire for the Sasangin Diagnosis Questionnaire(for adult). Experts, element school children and a scholar on Korean literature revise the questionnaire. 3. Results and Conclusion Experts examed pre-questionnaires and selected the final questionnaries by CVI 0.8 at propriety of contents. Sasangin Diagnosis Questionnaire (SDQ) for Child consists of 84 questions (24 questions for child, 48 questions and 12 questions for doctor).

  • A patient with right upper limb weakness was diagnosed with idiopathic polyneuropathy and received a series of Korean Medicine including acupuncture, electroacupuncture, bee venom acupuncture, and administration of herbal medicine BacJung-hwan for 17 days of hospitalization period. The progression of the weakness was measured by Hand grip tester, neuralgia and numbness were measured by the NRS(Numerical Rating Scale) scores. After treatment, the right grip strength improved from 12kg to 35kg and the right upper limb neuralgia of the NRS5 was improved to NRS2. The present case study suggests that the potential effects of Korean Medicine treatment for idiopathic polyneuropathy.

A Clinical Study on Patients of Scoliosis (척추측만증의 임상적 관찰)

  • Objective : This study is designed to evaluate the clinical effect of the oriental medical treatment (Ex: chuna, physical theraphy, acupuncture, moxibustion, herb medicine, etc.) on patient of scoliosis. Methods : We investigated 12 patients with scoliosis who visited to Department of Acupunture & Moxibustion, Tae gu Oriental Hopital of Kyung San University from January 2000 to September. 12 patients had a diagnosis of scoliosis by X-ray. We treated 12 patients by the oriental medical treatment (Ex: chuna, acupuncture, physical theraphy, moxibustion, herb medicine, etc.)and they were evaluated with post treatment X-ray findings. Results : 1. We investigated 6 female and 6 male patients. The most commom distribution of age was 30's. 2. In the scoliosis angle : before treatment, the average scoliosis angle was $13.56^{\circ}$ but after treatment, it was decreased to $9.31^{\circ}$ and the mean correctability was 33.47%. 3. The more servious the symptoms were, the lesser the correction rate was. It was decreased in the order of Gr I, Gr II, Gr III and Gr IV. 4. The larger the scoliosis angle and the degree of rotation were, the lesser the correction rate was. Conclusion : These results suggest that the oriental medical treatment (Ex: chuna, acupuncture, physical theraphy, moxibustion, herb medicine, etc.) was effective treatment modality on patient of scoliosis.

  • Experimental studies were implemented to evaluate clinical efficacy of 'Daeshiho-Tang' and to validate Oriental medicine description of the recipe which has been widely used in metabolic diseases and febrile diseases. Following results were obtained; 1. Suppressive action was shown on convulsion due to cerebrocortical causes but no such action was noted either myelic or diencephatic causes in mice. 2. Analgesic and antipyretic actions were recognized in mice, rats and rabbits. 3. Antispasmodic actions were recognized on contraction of the ileum induced by $BaCl_2$ and histamine in mice and guinea-pigs. 4. Transport accelerating action of the small intestine was gained in mice.

  • In this paper, a cross-layer resource allocation mechanism is proposed for wireless multimedia service. In particular, a game theory based on quality of service (QoS) for multimedia users is introduced to deal with the fairness of network resource allocation in wireless networks. Moreover, the channel states of wireless users are additionally regarded under the cross-layer design in WiMAX environment. In details, the bargaining solution is adopted to discover the efficient and fair resource allocation strategy for multimedia service in considering QoS in the peak signal-to-noise ratio (PSNR) and the channel states in the carrier-to-interface ratio (CINR). The proposed mechanism is illustrated and evaluated by simulation results of transmitting video sequences in WiMAX environment.

  • Solar assisted heat pump with freon circulating system has been developed. Revising the cool chamber(flower storage) with the solar thermal hot water producing system, the more amount of hot water can be produced, which can be even used for room panel heating. The compressor was cooled by water jaket instead of air cooled so that the system energy efficiency was improved quite well.

  • To understand the role of intestinal microflora in the biological effect of functional herbs, which have been used in Korea, Japan, and China as traditional medicines, and suggest new bioactive compounds transformed from herbal constituents, the metabolic activities of the functional herb components (ginsenoside Rb1, crocin, amygdalin, geniposide, puerarin, ginsenoside Re, poncirin, hesperidin, glycyrrhizin, and baicalin) toward their bioactive compounds (compound K, crocetin, benzaldehyde, genipin, daidzein, ginsenoside Rh1, ponciretin, hesperetin, 18b-glycyrrhetic acid, and baicalein) were measured in fecal specimens. The metabolic activities of these components were $882.7{\pm}814.5$, $3,938.1{\pm}2,700.8$, $2,375.5{\pm}913.7$, $1,179.4{\pm}795.7$, $24.6{\pm}10.5$, $11.4{\pm}10.8$, $578.8{\pm}206.1$, $1,150.0{\pm}266.1$, $47.3{\pm}58.6$, and $12,253.0{\pm}6,527.6\;{\mu}mol/h/g$, respectively. No differences were found in the metabolic activities of the tested components between males and females, although these metabolic activities between individuals are extensively different. The metabolites of functional herb components showed more potent cytotoxicity against tumor cells than nonmetabolites. These findings suggest that intestinal microflora may activate the pharmacological effect of herbal food and medicines and must be the biocatalytic converter for the transformation of herbal components to bioactive compounds.

Effects of Acupuncture at ST36 on Blood Pressure and Endothelial Dependent Vasodilation in Hypertensive Patients (족삼리(足三里) 자침이 고혈압환자의 혈압과 내피세포 의존성 혈관확장반응에 미치는 영향)

  • Objectives : The objective of this study was to assess the effect of acupuncture applied at the ST36 point on blood pressure and endothelial dependent vasodilation in hypertensive patients. Methods : 24 hypertensive patients were recruited and randomized to a study group (12 subjects) or a control group (12 subjects). Both groups took FMD (endothelial-dependant, flow-mediated dilation) measurement and then acupuncture needles were inserted at ST36 for the study group. In the control group, they took sham acupuncture as a control. FMD was rechecked after 10-min acupuncture treatment. Blood pressure was measured before and after acupuncture treatment. Results : FMD increased significantly in the study group after acupuncture (9.5${\pm}$2.0% to 11.1${\pm}$2.2%), but not in the control group. In both groups, there were no changes in blood pressure and heart rate. Conclusions : Acupuncture on ST36 appears to improve endothelial dysfunction of hypertensive patients and this might result from inducing activation of endothelium-derived nitric oxide.
