Cervicofacial infection in a Nigerian tertiary health institution: a retrospective analysis of 77 cases

  • Objectives: Infection involving the orbit, zygomatic space, lateral pharyngeal space, or hemifacial and oral floor phlegmon is referred to as cervicofacial infection (CFI). When diagnosis and/or adequate treatment are delayed, these infections can be life-threatening. Most cases are the result of odontogenic infections. We highlight our experiences in the management of this life-threatening condition. Materials and Methods: This was a retrospective study of patients who presented with CFI from December 2005 to June 2012 at the Oral and Maxillofacial Surgery Clinic or the Accident and Emergency Unit of Ahmadu Bello University Teaching Hospital (Zaria, Nigeria). The medical records of all patients who presented with either localized or diffuse infection of the maxillofacial soft tissue spaces were retrospectively collected. Data collected was analyzed using SPSS version 13.0 and are expressed as descriptive and inferential statistics. Results: Of the 77 patients, 49 patients (63.6%) were males, a male to female ratio of 1:7.5. The ages ranged from two years to 75 years with a mean of $35.0{\pm}19.3$ years, although most patients were older than 40 years. The duration of symptoms prior to presentation ranged from 6 to 60 days, with a mean of $11.0{\pm}9.4$ days. More than 90% of the patients presented to the clinic within the first 10 days. The most commonly involved anatomical space was the submandibular space (n=29, 37.7%), followed by hemifacial space (n=22, 28.6%) and buccal space (n=7, 9.1%). Ludwig angina accounted for about 7.8% of the cases. Conclusion: CFI most commonly involves the submandibular space, typically affects individuals with a low level of education, and is influenced by traditional medical practices. Despite improved health care delivery, CFI remains a significant problem in developing countries.

Ordering of Alloy 690 Steam Generator Tubings in a Nuclear Power Plant (원자력발전소 증기발생기 Alloy 690 전열관 재료의 규칙화 반응)

  • Considering the case in the United States where most nuclear power plants with an initial design life of 40 years continue to operate until 60 or 80 years after undergoing material soundness evaluation, it is time to plan a more robust long-term operation strategy for nuclear power plants in Korea. There are some reports that SRO/LRO might be formed when Alloy 690 is heat treated for 10,000 hours to 100,000 hours at 360 to 450 ℃. The possibility of LRO formation in Alloy 690 steam generator tubings of Kori nuclear power plant unit 1 (Kori-1) was investigated using existing research papers. The mechanism in which SRO/LRO occurred was also surveyed. Alloy 690 was found to be more likely to cause ordering than Alloy 600 in terms of alloy composition. The ordering could be evaluated through changes in material properties. However, it is difficult to evaluate it from a microstructural point of view. The likelihood of LRO in Alloy 690 of the Kori-1 plant operated at 320 ℃ for 19 years seemed to be low in terms of time and exposure temperature.

Current status of professor resignation at a local medical school and their post-resignation employment opportunities (일개 지방 의과대학의 교수 사임 현황과 사임 이후 근무 형태)

  • Universities are essential in the educating medical school students and create an excellent educational environment for them. We checked the status of personnel turnover at a local medical school to determine the form of entry into the workplace after resignation. According to their intention, the total number of resignations was 54, with individuals being 42.22±5.92 years old (32-64 years old) and having 5.53±4.20 years of service. Resignations under 3 years were 20, and under 15 years or more were nine. By year, the number of professors resigned the most at 11 in 2020 and seven in 2022. Regarding present employment after resignation, 28 individuals (51%) moved to other universities or university hospitals, eight (14%) to general hospitals, 15 (27.8%) to clinics or employment, and three (5.6%) elsewhere. As a result of the analysis, by dividing into the metropolitan areas including Seoul, the area excluding the metropolitan area, and the Jeju area based on the area where a bachelor's degree was obtained, 65.5% and 42.9% of the metropolitan and non-metropolitan area were transferred to universities and university hospitals, respectively. Further, the Jeju area was employed in general or private hospitals. The resignation of a professor from a local medical school varies slightly depending on the demand for medical care in other regions, including metropolitan areas, but tends to continue. Therefore, it is necessary to determine directions for continuing education.

Prevalence of pre-obesity and above and its associated factors in adult women: an analysis of the 2020 Korea National Health and Nutrition Examination Survey

  • Purpose: This study was conducted to determine the prevalence of pre-obesity (overweight) and above in adult women and to identify associated factors. Methods: Data were obtained from the eighth Korea National Health and Nutrition Examination Survey (KNHANES VIII-2), conducted in 2020. The sample comprised 2,288 women aged 19-64 years who participated in the KNHANES VIII-2. Data were analyzed using complex sample design analysis with SPSS version 20.1. Results: The prevalence of pre-obesity and above among adult women was 46.5%, with 18.6% classified as having pre-obesity and 27.9% as having obesity. A higher prevalence of pre-obesity and above was observed in women aged 50-59 years (odds ratio [OR]=1.67, p=.019) or 60-64 years (OR=1.80, p=.029); women whose highest educational attainment was high school (OR=1.28, p=.018) or middle school or less (OR=1.60, p=.017); those in middle-income households (OR=1.55, p=.005); those engaging in muscle-strengthening activities less than 2 days per week (OR=1.37, p=.019); and those sleeping less than 6 hours per night during the week (OR=1.37, p=.025). Conclusion: As nearly half of all adult women have either pre-obesity or obesity, prevention and management strategies must target both groups. Interventions should be prioritized for women in their 50s and older, as well as those with low education or income levels. Additionally, receiving adequate sleep of 7 hours or more and engaging in muscle-strengthening activities at least 2 days per week are important components of obesity management.

  • Objectives: The aim of this study was to investigate the effects of periodontal disease in menopausal women. Methods: We used data from the 7th (2016-2018) Korea National Health and Nutrition Examination Survey. The subjects of the study were 2,643 women aged 40 to 59 years who participated in the periodontal examination and answered a menses questionnaire. The data were analyzed using complex samples multivariate logistic regression to investigate the effects related to periodontal disease in menopausal women. Results: After adjusting for general characteristics, oral health behaviors, and health status, those aged 50-59 years had odds ratio (OR) for periodontal disease of 2.52 compared to those aged 40-49 years. Compared to those who brushed 3 or more times a day, those with less than 3 brushings a day had OR of 1.48. Those who smoked had a 3.00 higher risk of periodontal disease than those who were non-smokers. Further, those with glycosuria had a 2.26 higher risk of periodontal disease than those without glycosuria. Conclusions: In order to promote the oral health of menopausal women, it is suggested that comprehensive and systematic oral health education should be implemented considering various variables.

Canine Lymphoma as a Possible Human Lymphoma Model: A Case-Series Study

  • Canine lymphoma (cL) is the most common hematopoietic cancer in dogs. Various determinants have been evaluated to find the predisposing factors in both human and canine lymphoma. Due to common risk factors and similar pathways, cL is considered a potential model for non-Hodgkin lymphoma (NHL) in humans. In this case-series study, major hospitals in Tehran consented to take part in this study and between the years of 2020-2022, provided us with 52 cL cases which were approved by the attended pathologist. We designed a questionnaire and collected information about the dogs and their owners. Most of the owners were women, young (younger than 50 years old), had at least diplomas and interestingly were housewives or househusbands. Male dogs with middle to old age (more than 6 years) were mostly referred. The most common characteristics were neutered, normal BCS, purebred, urban but not industrial residence, previous tobacco smoke exposure but no history of previous fungicide or pesticide exposure. Also, most of them did not have any previous autoimmune or immunosuppressive diseases. Presented characteristics should be considered risk determinants but to approve their validity, they should be further evaluated in epidemiological studies.

  • A period study of the semi-detached eclipsing binary system W Delphini based on the extensive series of minimum timings covering more than a century(109 years) indicates a cyclic(O-C) variation of the system. This variation can be explained as due either to (1) stellar magnetic activity cycles of the cool subgiant G5 secondary component of the binary with a subsurface magnetic field equals to 3 kG, or (2) a long-term orbital period increases with a rate of $1.68{\times}10^{-8}$ day/cycle caused by a mass transfer rate of $4.9{\times}10^{-8}M_{\odot}yr^{-1}$ from the less to more massive component modulated by a light time effect due to a hypothetical third body with period of $53.4{\pm}1.06$ years. The former explanation is more recommended than the later one since the obtained third body mass value($M_3=1.58\;M_{\odot}$) is quite large but it can not manifest itself observationally and also it cannot be a white dwarf. In the contrary, from the magnetic activity point of view, the obtained characteristics are in good consistent when applying Applegate(1992) mechanism. However, further precise photometric and CCD observations for minima timings with brightness determinations are needed to confirm the present solution.

Cost-effectiveness Analysis of Cervical Cancer Screening Strategies Based on the Papanicolaou Smear Test in Korea

  • Background: Despite the increasing number of screening examinations performed for cervical cancer utilizing the Papanicolaou smear test (Pap test), few studies have examined whether this strategy is cost-effective in Korea. Objective: This study was conducted to evaluate the cost-effectiveness of cervical cancer screening strategies incorporating the Pap test based on age at the start and end of screening as well as screening interval. Materials and Methods: We designed four alternative screening strategies based on patient age when screening was started (20 or 30 years) and discontinued (lifetime, 79 years). Each strategy was assessed at screening intervals of 1, 2, 3, or 5 years. A Markov model was developed to determine the cost-effectiveness of the 16 possible cervical cancer screening strategies, and this was evaluated from a societal perspective. The main outcome measures were average lifetime cost, incremental quality-adjusted life-years (QALYs), and incremental cost-effectiveness ratio (ICER). Results: Compared with various strategies comprising younger starting age, discontinuation age, and longer screening intervals, strategies employing annual screening for cervical cancer starting at a target age of 30 years and above were the most cost-effective, with an ICER of 21,012.98 dollars per QALY gained (with a Korean threshold of 30,000,000 KRW or US$27,272). Conclusions: We found that annual screening for cervical cancer beginning at a target age of 30 years and above is most cost-effective screening strategy. Considering the potential economic advantages, more intense screening policies for cervical cancer might be favorable among countries with high rates of cervical cancer and relatively low screening costs.

Characteristics of 240 Chinese Father-child Pairs with Malignant Disease

  • To obtain a screening and early detection reference for individuals who have a family history of cancer on the paternal side, we collected and analyzed data from 240 pairs in which both fathers and their children were diagnosed with cancer. Disease categories of fathers and sons were similar to that of the general population of China, whereas daughters were different from general female population with high incidence of breast cancer and gynecological cancer. Sons were more likely than daughters to have the same type of cancer, or to have cancer in the same organ system as their fathers (P < 0.0001). Sons and daughters developed malignant diseases 11 and 16 years earlier than their fathers, respectively (P < 0.0001 for both sons and daughters). Daughters developed malignant diseases 5 years earlier than sons (P < 0.0001). Men with a family history of malignant tumors on the paternal side should be screened for malignancies from the age of 45 years, or 11 years earlier than the age of their fathers' diagnosis, and women should be screened from the age of 40 years, or 16 years earlier than the age at which their fathers were diagnosed with cancer. Lung cancer should be investigated in both men and women, whilst screening should focus on cancer of the digestive system in men and on breast and gynecological cancer (ovary, uterine and cervical cancer) in women.

A STUDY OF CLINICAL RESULTS ON STERI-OSS ENDOSSEOUS IMPLANTS (Steri-Oss 임플랜트의 임상 결과에 관한 연구)

  • This investigation evaluated patients who received Steri-Oss implants from the Dental Hospital of Chosun University during the period from March 1989 to August 1997. 346 fixtures of 127 patients were included in this study. The results were as follows ; 1.The follow-up period was defined as the period between the surgical placement of the implants and the last follow-up examination. The mean follow-up period was $2.17{\pm}1.21$ years. 2.The period between fixture installation and second surgery was $0.71{\pm}0.44$ years in the maxilla and $0.46{\pm}0.21$ years in the mandible. 3.The number of fixtures which were installed in the upper jaw(112) was less than that in the lower jaw(234) and in the posterior region(260) was more than in the anterior region(86). 4.The length of fixture which was most frequently used was 12 mm and least was 8mm. Screw implants were installed more than cylindrical implants. 3.8mm implant was the most common implans, followed by 4.5mm and 3.25mm. 5.The number of augmentation cases was more than that of non-augmentation cases and the rate of augmentation cases in the maxilla was more than that in the mandible. 6.Implant restorations for partial edentulos patients(94cases) were more than single- tooth implant restorations(33cases) or implant restorations for complete edentulos patients(10cases). 7.Free-standing prostheses for partially edentulous patients were more commom than any other type of connection between implants and natural teeth. 8.Plaque Index($0.95{\pm}0.74$) and Gingival Index($0.31{\pm}0.52$) were very similar around the natural teeth and reflected an acceptable level of plaque and gingivitis control. Mean value for keratinized mucosa index($1.93{\pm}1.20$) remained fairly constant around level 2(1-2 mm keratinized epithelium). 9.Patients were generally satisfied with implant in terms of comfort, function, speech and esthetics. 10.There was not a statistically significant differences in overall survial rate between implants placed in the maxilla (91.5%) and those placed in the mandible (93.8%). Fourteen implants lost before the prosthetic rehabilitation and eleven implants lost following variable periods in function after the prosthetic phase of the treatment. 11.Cause of implant failures was exfoliation or removal of fixture due to non-osseointegration before the prosthetic rehabilitation or due to fracture of fixture, masticatory pain after the prosthetic rehabilitation. 12.The survival rate of Steri-Oss implants using the Kaplan-Meier statistical analysis was 93.8% at 2 year and 86.6% at 5 year, In all cases, implant losses occured predominantly in the healing period. There was a steep decline in the rate of implant loss after the first year. 13.The survival rate of Steri-Oss implants in the anterior region was 94.8% at 2 year and 94.8% at 5 year and that in the posterior region was 92.8% at 2 year and 75.9% at 5 year. In conclusion, this study revealed a number of parameters and guidelines for achieving an optimal success rate in osseointegration.
