컴포머의 수화팽창에 관한 연구 (A STUDY ON THE HYGROSCOPIC EXPANSION OF COMPOMER)

  • 컴포머는 산성화된 이중 기능성 레진 단량체와 fluoro-alumine-silicate 글라스 입자로 구성되어 초기에는 복합레진과 같이 광원에 의해 중합되며 이차적으로 구강내 수분을 흡수하면서 글라스 아이오노머 시멘트와 유사한 산-염기 반응으로 중합되고 이의 결과로 물성의 저하없이 지속적으로 불소를 유리한다고 보고되고 있으며, 물분자를 흡수하여 수화됨에 따라 서서히 팽창하여 광중합시의 수축을 보상하고 미세누출을 최소화할 수 있다고 보고되고 있으나 아직 확립되어 있지는 않은 상태이다. 이에 본 연구는 현재 소아치과 임상에서 많이 사용되고있는 컴포머를 대상으로 초기의 중합수축에 의한 미세누출이 수화 평창에 의해 감소되는지 여부를 복합레진과 비교하기 위하여 5급 와동의 교합면측과 치은면측의 미세누출양을 1일과 30일 후에 측정한 결과 30일 후 컴포머의 치은면측에서 현저히 낮은 미세누출을 보였다.

5급 와동의 수복에 있어서 수종의 compomer의 미세 변연 누출에 관한 연구 (MICROLEAKAGE OF CLASS V COMPOMER RESTORATIONS)

  • The purpose of this study was to evaluate the microleakage of compomers. In this in vitro study, class V cavities were prepared on buccal and lingual surfaces of thirty extracted human premolars and molars on cementum cervical margin. The experimental teeth were randomly divided into five groups of six samples (12 surfaces) each, Group 1 : Dyract AP, Group 2 : F2000 compomer(non-etch), Group 3 : F2000 compomer(etch), Group 4 : Elan, Group 5 : Compoglass. The bonding agents and compomer were applied for each group following the manufacturer's instructions. After 500 thermocycling between $5^{\circ}C$ and $55^{\circ}C$, the 30 teeth were placed 1% methylene blue dye for 24 hours then rinsed with tab water. The specimens were embedded in clear resin, then sectioned buccolingually through the center of the restoration with a low speed diamond saw. The dye penetration on each of the specimen was observed with a stereomicroscope. The results of study were statistically analyzed using the Student-Newmann-Kaul' s Test. The results were obtained as follows. 1. All compomer restoration systems did not completely prevent marginal leakage. 2. In enamel margin, F2000 compomer(etch) group showed lower leakage value than other systems. Elan and Compoglass groups showed lower leakage value than Dyract AP and F2000 compomer(non-etch) groups (p<0.05). 3. In cementum margin, F2000 compomer(etch) and F2000 compomer (non-etch) groups showed lower leakage value than other systems. Dyract AP and Elan groups showed lower leakage value than Compoglass group (p<0.05).

5급 와동에서 수종의 상아질 접착제에 따른 미세누출의 비교 (THE COMPARISON OF MICROLEAKAGE OF CLASS V COMPOSITE RESIN RESTORATIONS WITH VARIOUS DENTIN BONDING SYSTEMS)

  • The purpose of this study was to evaluate microleakage of six current dentin bonding systems. In this in vitro study, class V cavities were prepared on buccal and lingual surfaces of thirty extracted human molars. Each margin was on enamel and dentin/cementum. Experimental teeth were randomly divided into six groups of 5 each. Group 1 : Scotchbond Multi-Purpose; Group 2 : Single Bond; Group 3 : Prime&Bond NT ; Group 4 : Clearfil Liner Bond 2 ; Group 5 : MAC Bond II ; Group 6 : One-up Bond F. The bonding agent and composite resin were applied to class V cavities according to manufacturer's directions. After thermocycling, the specimens were immersed in 0.5% basic fuchsin dye solution for 6 hours and sectioned longitudinally through the center of the restoration with a low speed diamond saw. The degree of microleakage was measured as the extent of dye penetration under the stereomicroscope at $\times$20. The data were analyzed using one way ANOVA. When significant differences found. multiple comparisons were made using Duncan's Multiple Range Test. The results of this study were as follows: 1. In all groups, leakage value seen at the enamel margin was significantly lower than that seen at the dentin margin(P<0.001). 2. At the enamel margin, none of the dentin bonding systems used in this study showed statistically significant difference in leakage values(P<0.05). 3. At the cementum margin, group 3 showed the highest leakage value, and others were decreased as group 5, 6, 4 in that order, and group 1, 2 showed the lowest leakage value. There was statistically significant difference between group 3 and the other groups except for group 5(P<0.05).

2급 와동 복합레진 인레이 충전 후 변연누출에 관한 연구 (A STUDY ON THE MARGINAL LEAKAGE OF CLASS II COMPOSITE RESIN INLAY)

  • The purpose of this study was to evaluate the microleakage of class II composite resin inlays and compare them with the conventional light-cured resin filling restorations. Class II cavities were prepared in 60 extracted human molars with which cervical margins were located below 1.0mm at the cemento-enamel junction using No. 701 tapered fissure carbide bur. All of the prepared cavities were restored as follows and divided into 6 groups. Group I and 2 were restored using direct filling technique and group 3,4,5 and 6 were restored using direct inlay technique that was cemented with dual-cured resin cements. group I: Cavities were restored with light-curing composite resin, Brilliant Lux. group 2. Cavities were restored with light-curing composite resin, Clearfil PhotoPosterior. group 3: Cavities were restored with Clearfil CR Inlay and heat treated at $125^{\circ}C$ for 7 minutes. group 4: Cavities were restored with same material as group 3 and heat treated at $100^{\circ}C$ for 15 minutes. group 5: Cavities were restored with Brilliant (Indirect esthetic system) and heat treated at $125^{\circ}C$ for 7 minutes. group 6: Cavities were restored with same material as group 5 and heat treated at $100^{\circ}C$ for 15 minutes. All specimens were polished with same method and thermocycled between $6^{\circ}C$ and $60^{\circ}C$, then immersed in a bath of 2.0% aqueous solution of basic fuchsin dye for 24 hours. Dyed specimens were sectioned longitudinally and dye penetration degree was read on a scale of 0 to 4 by Tani and Buonocore's method 45). The results were as follows: 1. Microleakage was observed rather at the cervical margins than at the occlusal margins in all groups. 2. Composite resin inlay groups showed significantly less leakage than direct filling groups at the cervical margins (p < 0.001). 3. In composite resin inlay groups, there was no significant difference in microleakage between specimens by heat treating temperature and time (p > 0.05). 4. There was no significant difference in leakage between each groups at the occlusal margins (p > 0.05).

V 급 와동 변연부 형태 및 복합레진 종류에 따른 변연 누출에 관한 실험적 연구 (An Experimental Study on the Effects of the Margin Designs and Composite Resins on the Marginal Leakage of Class V Resin Restorations)

  • The primary aim of this study was to assess the effects of the margin designs and composite resins on the marginal leakage of Class V resin restorations. 60 Class V cavities with $90^{\circ}$ butt joint, 60 with $45^{\circ}$ bevel joint, and 60 with concave joint were prepared on the labial surfaces of 38 extracted anterior teeth and the buccal and lingual surfaces of 71 premolars, and classified as the butt group, bevel group and concave group, respectively. After completion of the cavity preparation, Heliosit${(R)}$ was filled in 20 butt joint cavities, 20 bevel joint cavities, and 20 concave cavities. And Hi-Pol$^{(R)}$ and Palfique$^{(R)}$ were filled in the same manner, respectively. After finishing, all specimens were subjected manually to 50 thermal cycles at $4^{\circ}C$, $37^{\circ}C$, and $60^{\circ}C$. They were immersed in 0.5% methylene blue solution for 24 hours, and buccolingually sectioned with sectioning machine. The sectioned specimens were examined under the light microscope. The results indicated the following: 1. The bevel group showed the least amount of marginal leakage compared with the other two design groups, but the statistical analysis showed no significant difference at the 95% confidence level. 2. Hi-Pol$^{(R)}$ group showed the most severe marginal leakage compared with the other composite resin materials, but there was no significant difference in the groups. 3. The bevel-Heliosit$^{(R)}$ subgroup, which was filled with Heliosit$^{(R)}$ in the beveled cavities, showed the least amount of marginal leakage, but there was no significance in all the subgroups.

비우식성 치경부 병소와 5급 와동 수복물이 치주조직에 미치는 영향 (Effects of non-carious cervical lesions and class V restorations on periodontal conditions)

  • Purpose: The non-carious cervical lesion(NCCL) is a loss of tooth structure at the neck of affected teeth that is unrelated to tooth caries. The reported prevalence of NCCL varies from 5% to 85%. Prevalence and severity of lesions have been found to increase with age. They are becoming more significant as people live longer and become more aware of the importance of oral health. The purposes of this study were first, to examine the periodontal conditions associated with NCCL, and second, to investigate the clinical effects of class V restorations of NCCL on periodontal tissues. Materials and methods: The sample size was 982 teeth of 50 subjects(25 male, mean age $52{\pm}7$) who were seen at the Department of Periodontology, Pusan National University Hospital. At the baseline examination, clinical periodontal parameters were measured. After the initial examination, 24 patients who were absent from hypersensitivity were selected. The teeth with NCCL were randomly divided into the test and control groups. The teeth in the test group were restored with flowable resin; the control teeth were not restored. Six months later, the clinical examinations were repeated. The data were analyzed using the SPSS program. Results: The results were as follows: 1) NCCL occurred on 45.8% of examined teeth. The percentage of affected teeth was higher in maxillary and premolar teeth. 2) The shallow saucer type was the most common. 3) Teeth with NCCL had more gingival recession, lower attachment level, and higher incidences of bleeding on probing(BOP) and plaque than NCCL-free teeth. 4) Six months later, gingival recession, attachment level, the percentages of BOP and plaque in the test group were lower than in the control group(p<0.05). Conclusion: NCCLs were more found in maxillary teeth, especially in premolar teeth. The results suggest that the restoration of NCCL could affect some periodontal parameters favorably.

제 2 급 와동에 충전된 Compome의 미세누출에 관한 연구 (MICROLEAKAGE OF COMPOMER IN CLASS II CAVITY)

  • To evaluate the micro leakage of compomer, 4 materials were divided into 4 groups of 15 cavities each.(Group 1: Z-100, Group 2: Dyarct AP, Group 3: Fuji II LC, Group 4: Compoglass) After the class II cavities were prepared using carbide bur No. 553, all specimen were restored by incremental filling technique. In group 3, Z-100 was filled with a base of a light curing glass-ionomer. After 7 days, all specimens were thermocycled between $5^{\circ}C$ and $55^{\circ}C$ for 500 cycles, followed by placement in 50% silver nitrate dye for 2 hours at $37^{\circ}C$. After rinsed in distilled water, these teeth were immersed in photodeveloping solution and exposed to fluorescent light for 6 hours. Teeth were then washed in distilled water to remove the photodeveloping solution, sectioned mesio-distally and evaluated. The results were as follows : 1. In the cervical portion, there was significant difference between Fuji II LC and other groups(Z-100, Dyract AP, Compoglass), Fuji II LC had the least value.(p<0.05) 2. In the cervical portion, there was not significant difference among Dyract AP, Z-100 and Compoglass. 3. In the occlusal portion, there was not significant difference among Dyract AP, Z-100 and Compoglass. From the results above, In enamel, microleakage of compomer such as Dyract AP and Compoglass resemble to that of composite resin. It is thought that it is due to characteristics of composite resin portion of compomer. But in dentin, microleakage of compomer is higher than that of resin modified glass ionomer cement, it is thought that in compomer, acid-base reaction is not developed with dentin.

쐐기 형태의 5급와동에서 수복재료에 따른 변연 미세누출의 비교 (COMPARISON OF MARGINAL LEAKAGE OF WEDGE-SHAPED CLASS V CAVITY ACCORDING TO RESTORATIVE MATERIALS)

  • The purpose of this study was to assess the effects of restorative materials on the marginal leakage of wedge-shaped class V cavity. The study was performed in vitro in 25 defect-free permanent, extracted teeth. Wedge-shaped class V cavities were prepared and then the teeth were randomly selected and restored according to the following. Group A : restoration with Tetric Ceram(composite resin) Group B : restoration with Tetric flow(flowable resin) Group C : restoration with Compoglass after acid etching(compomer) Group D : restoration with Compoglass(compomer) Group E : restoration with Fuji II LC improved(resin-modified GIC) After thermocycling, the specimens were immersed in 5% basic fuchsin solution for 6 hours and sectioned longitudinally through the center of the restoration. The degree of marginal leakage was measured as the extent of dye penetration under the stereomicroscope. The data were analysed using one-way ANOVA. When significant differences found, multiple comparisons were made using Duncan's Multiple Range Test. The results were as follows: 1. The occlusal margins of all groups except for Fuji II LC improved showed lesser leakage than gingival margins and there was statistically significant difference(p<0.05). 2. At the occlusal margins, group A, B showed same marginal leakage scores, and others were decreased as group C, D, E in that order. There were statistically significant difference between group A, Band group D, E, group C and group E(p<0.05). 3. At the gingival margins, group B, C showed same marginal leakage scores, and others were decreased as group A, D, E in that order. But there was statistically significant difference between group B, C and group E(p<0.05). 4. In the Compoglass restoration, acid-etching technique was beneficial for marginal sealing ability at all of margins. But there was no statistically significant difference (p>0.05). In the restorations for wedge-shaped class V cavities, resin restoration with acid etching technique is recommended.

제 5급 와동에서의 단일용기 상아질 접착제와 자가 산부식 접착제의 상아질에 대한 침투도 평가 (A CONFOCAL MICROSCOPIC STUDY ON DENTINAL INFILTRATION OF ONE-BOTTLE ADHESIVE SYSTEMS AND SELF-ETCHING PRIMING SYSTEM BONDED TO CLASS V CAVITIES)

  • Objective : The purpose of this study was to evaluate the resin infiltration into dentin of one-bottle adhesive systems and self-etching primer bonded to Class V cavities using confocal laser scanning microscope(CLSM). Material and Methods : Forty Class V cavities were prepared from freshly extracted caries-free Human teeth. These teeth were divided into two groups based on the presence of cervical abrasion: Group I, cervical abrasion : Group II, wedge-shaped cavity preparation. Resin-dentin interfaces were produced with two one-bottle dentin bonding systems-ONE COAT BOND(OCB; Coltene$^R$) and Syntac$^R$SPrint$^{TM}$(SS; VIVADENT)-, one self-etching priming system-CLEARFIL$^{TM}$ SE BOND (SB : KURARAY)- and one multi-step dentin bonding system-Scotchbond$^{TM}$Multi-Purpose (SBMP, 3M Dental Products)-as control according to manufacturers' instructions. Cavities were restored with Spectrum$^{R}$(Dentsply). Specimens were immersed in saline for 24 hours and sectioned longitudinally with a low-speed diamond disc. The resin-dentin interfaces were microscopically observed using CLSM. The quality of resin-infiltrated dentin layers were evaluated by five dentists using 0~4 scale. Results : Confocal laser scanning microscopal investigations using primer labeled with rhodamine B showed that the penetration of the primer occurred along the cavity margins. Statistical analysis using one-way ANOVA followed by Duncan's Multiple Range test revealed that the primer penetration of the group 2(wedge-shaped cavity preparation) was more effective than group 1(cervical abrasion) and that of the gingival interfaces was more effective than the occlusal interfaces. In the one-bottle dentin bonding systems, the resin penetration score of OCB was compatible to SBMP, but those of SS and self-etching priming system, SB were lower than SBMP.

상아질 접착제의 미세누출에 관한 연구 (A STUDY ON THE MICROLEAKAGE OF DENTIN BONDING SYSTEMS)

  • 본 연구는 여러가지 상아질 접착제들의 법랑질 변연과 상아질 변연에서의 미세누출을 비교 평가하기 위해 이루어졌다. 발거된 건전한 영구치의 협면과 설면에 5급 와동을 형성하고 형성된 50개의 와동을 무작위로 5개의 군으로 나누어 각군에 Adper$^{TM}$ Scotchbond$^{TM}$ Multi-purpose Plus Adhesive (SM; 3M ESPE, USA), Adper$^{TM}$ Single bond 2 (SB; 3M ESPE, USA), Clearfil$^{TM}$ SE Bond (SE; Kuraray Medical Inc., Japan), Adper$^{TM}$ Prompt$^{TM}$ L-Pop$^{TM}$ (PL; 3M ESPE, USA), GBond$^{TM}$ (GB; GC Co., Japan)를 적용하고 Filtec$^{TM}$ Z350 A3 (3M ESPE, USA)로 충전하였다. 시편을 $37^{\circ}C$ 증류수에 24 시간 보관한 후 $5^{\circ}C$$55^{\circ}C$에서 10초씩 1000회 열순환하고 2% methylene blue 용액에 24시간 넣어 염색한 뒤, 주수 하에 치아를 협설로 절단하여 색소의 침투 정도를 화상분석현미경을 이용하여 침투깊이를 측정한 뒤 각 군간의 미세누출 정도를 비교하여 침투깊이에 따라 $0{\sim}3$점으로 분류하여 다음과 같은 결과를 얻었다. 1. 법랑질 변연에서의 미세누출은 PL(0.85), SB(0.55), GB(0.50), SM(0.35), SE(0.25) 순이었으며 PL은 SM 및 SE 와 통계학적으로 유의한 차이를 나타냈다(p<0.05). 2. 상아질 변연에서의 미세누출은 GB(2.10), SE(1.45), PL(1.40), SB(1.05), SM(0.70)순이었으며 GB는 SB 및 SM 과 통계학적으로 유의한 차이를 나타냈다(p<0.05). 3. 각 실험재료 군내에서 법랑질 변연에 비해 상아질 변연에서 미세누출이 더 크게 나타났으며, SE, PL, GB에서 통계학 적으로 유의한 차이를 나타냈다.(p<0.05)
