Evaluation of factors affecting sensory neural hearing loss (감각신경성 청력손실에 영향을 미치는 제 요인에 관한 연구)

  • Reports on a potential relationship between sensory neural hearing loss(SNHL) and cardiovascular or hematologic factor show that the results are controversial. A detailed analysis of risk factors in the development of SNHL was carried out in 3,050 non-noise exposed healthy worker. The mean hearing threshold of both ears at 500, 1000, 2000, 4000, 8000Hz was measured and the effect of age, possible cardiovascular risk factor and hematologic factor (blood viscosity and hemostatic factors) on SNHL were evaluated. first, each of these were associated with loss of hearing sensitivity when univariately and multivariatively analysed. In a multiple regression model, age, sex, body mass index, WBC and total cholesterol level were independently associated with the mean of hearing sensitivity decrease at 4000 and 8000Hz. Second, study subjects were divided into two group (normal vs SNHL) and we compare the possible risk between both groups, and analysed univariate and multivariative logistic model. In a multiple logistic regression model, age, sex, body mass index, WBC and total cholesterol level, total protein, platelet were independently associated with SNHL. Our results show that we have some reliable indices of susceptibility to SNHL using cardiovascular measures or biochemical factor, but future, more extensive studies are required.

Oral Health and Stress of College Women in Some Areas (일부지역 여대생의 스트레스와 구강건강)

  • The purpose of this study was to examine factors related to the oral health by stress in college women. This survey was conducted between June 16, 2014 and July 18, 2014 to investigate the correlation between stress and oral health Data were obtained from 393 women students of 5 colleges in Gwangju and Jeollanam-do. The oral health of college students is related with the stress which has a correlation between oral condition satisfaction(-) and inconvenient(+) of mastication and pronunciation (p<0.01). Also, factors affecting stress are discomfort during mastication and oral state satisfaction and discomfort during mastication. The discomfort during mastication showed the highest stress while the higher status of oral satisfaction showed lower stress (p<0.05). Therefore, college women stress is related with oral health. Colleges should consider developing a program which can appropriately manage and regulate stress to relieve their stress.

  • Recently, the cardiovascular disease has increased by causes such as westernization dietary life, smoking, and obesity. In particular, the acute myocardial infarction (AMI) occupies 50% death rate in cardiovascular disease. Following this trend, the AMI has been carried out a research for discovery of risk factors based on national data. However, there is a lack of diagnosis minor suitable for Korean. The objective of this paper is to develop a classifier for short-term relapse mortality prediction of cardiovascular disease patient based on prognosis data which is supported by KAMIR(Korea Acute Myocardial Infarction). Through this study, we came to a conclusion that ANN is the most suitable method for predicting the short-term relapse mortality of patients who have ST-segment elevation myocardial infarction. Also, data set obtained by logistic regression analysis performed highly efficient performance than existing data set. So, it is expect to contribute to prognosis estimation through proper classification of high-risk patients.

Risk Factors Influencing Stroke : using data from the 7th(2016-2018) Korea National Health and Nutrition Examination Survey (뇌졸중에 영향을 미치는 위험요인 : 국민건강영양조사 제7기(2016-2018) 자료를 활용하여)

  • The purpose of this study was to identify the risk factors influencing stroke. The research method was used two-stage stratified cluster sampling using data from the National Health and Nutrition Survey (2016-2018). This study subjects performed a complex sample logistic regression analysis on adults aged 19 or older. Risk factors affecting stroke were shown as follows: in age groups, odds ratio were conformed when people are in their 70s, the rate is 8.861 times higher; when they are in their subjective health conditions, the odds ratio are 4.501 times higher in the bad groups; odds ratio was 3.158 times higher in the group diagnosed with hypertension and 1.598 times higher in the group diagnosed with diabetes. The result of this study found that chronic diseases such as high blood pressure and diabetes and the management of senior citizens are important because stroke is dangerous and can be managed through prevention.

Living and Behavioral Differentials of the Elderly's Dementia in Daegu (대구시 노인들의 치매 실태 및 치매노인들의 생활 습관적 특성)

  • The main purposes of this study are to describe the elderly dementia, and to explore characteristics of the elderly dementia among the aged 65 and over in Daegu area. In this study, 1,120 subjects which were 0.6 percent of the population were selected and face-to-face interviews were conducted. The face-to-face interviews were conducted from July 1, 2004 to August 31, 2004. KMMSE was introduced to measure the status of the interviewees' dementia. The findings of this study are as follows: 8.9 percent of respondents turns out to be dementia patients. Among the dementia patients, 52.5 percent are mild dementia patients, 35.0 percent are mid-level symptoms and 12.5 percent are severe symptoms of dementia patients. Cross-tabulation analyses and logistic regression analysis were introduced to explore the demographic, living, and behavioral differentials of the elderly's dementia. According to the logistic regression analysis, it turns out that white educational attainment, participating in the leisure activities for both physical exercise and mental activities, having meals regularly, adequate amount of meals, and high economic status decrease the odds ratio of elder dementia among the elderly who are 65 and above in Daegu area and are statistically significant. On the other hand, age increases the odds ratio of the elder dementia of the elder among Koreans above 65 in Daegu area. Finally, policy implications and some suggestions for the improvement of the elder dementia are introduced and discussed.

Study on Frailty Profiles and Associated Factors in Later Adulthood (노년기 허약 유형과 영향요인에 관한 연구)

  • The purpose of this study was to identify frailty profiles based on physical, psychological, and social domains of functioning and to examine the associated factors showing the differences among frailty profiles. Respondents were 70 years and older(n=403) and latent class analysis was applied to determine the optimal subgroups based on Tilberg Frailty Indicators which comprised of three domains(the physical, psychological, and social domain). Also, we performed multinominal logistic regression analysis to find out factors making differences among frailty profiles. Latent class analysis(LCA) identified three distinct types: multi-frail type(27.0%), psychologically frail type(26.8%), inadequate support type(46.2%). All three types had common difficulties in dealing with daily life problems and did not receive enough help with theses difficulties. Based on the results of the LCA three-class models, people in multi-frail type accumulated problems in physical and psychological domains and had partially social domain. On the other hands, psychologically frail type showed a relatively high anxiety disorder and depression. Lastly, people in inadequate support type reported the lack of helps, but they were relatively healthy. Comparing these groups with inadequate support type, people with multi-frail had lower educational level, poor nutritional management status and were less likely to participate in labor market. People in psychologically frail type were more likely to be male, to live in big cities rather than middle and small cities, and less likely to smoke. Based on these results, our results showed the multifaceted concept of frailty among Korean elderly people and we suggested several implications for preventing frail process.

A Study on the Influencing Factors of High Risk Drinking by Gender in Single Adult Households (성인 1인 가구의 성별에 따른 고위험 음주 영향요인에 관한 연구)

  • This study sought to analyze factors influencing high-risk drinking in single-person households. For this, data from the 2018 community health survey were used. Subjects were 32,389 adults above the age of 19 in single-person households. For the data analysis, high-risk drinking groups were extracted according to the high-risk drinking rate index of the survey to arrive at influencing factors and differences in health-related and sociodemographic characteristics. The IBM SPSS 25.0 software was used for analysis and a complex sampling design was applied. The results showed that the high-risk drinking rate of Korea's single-person households was 15.0% (male: 25.8%, female: 5.8%) and age, education under high school level, service-industry employees, smokers, people with depression, high blood pressure, and irregular breakfast eaters appeared as common elements for both genders. Stress appeared to only affect males while being diabetic only affected females. High-risk drinking was higher for males in their 30~40s and women in their 20~30s. The younger generation showed the highest numbers in high-risk drinking and factors like stress or depression appeared to be influencing factors for high-risk drinking. Hence, mental health programs along with customized health policies through health forms and lifestyle changes will be required to lower the high-risk drinking rates of single-person households.

The Study on Hypertension Cure Rate Management Centering around Wellness Local Community : With GwangJu as a Central Figure (웰니스 지역사회 중심의 고혈압 치료율 관리 방안에 관한 연구 : 광주광역시 중심으로)

  • This study was conducted to identify the factors of hypertension treatment in Gwangju and to establish a hypertension cure rate management plan by using local community health surveys to provide the hypertension cure rate management plan centering around the wellness local community. The research collected 13,714 Gwangju research data among a total of 685,820 local community health surveys of KDCA (Korea Disease Control and Prevention Agency) from 2017 to 2019. Among the data, 2,941 subjects, those with diagnosed hypertension aged over 30, were selected and analyzed through SAS 9.4, SAS Enterprise Miner 15.1. The results are as follows. The differences in hypertension diagnosis cure rate in Gwangju based on the subjects' socioeconomic characteristics were shown in gender, age, marital status, level of educational attainment, economic activity status, and monthly income. The significant differences in hypertension cure rate based on health behavior characteristics were shown in current smoking, monthly alcohol consumption, high-risk drinking, breakfast, recognition of good health level, diabetes and treatment, annual unmet medical needs, and annual health center use. As a result of the logistic regression analysis and interactive decision tree analysis to identify the factors affecting hypertension treatment, the research found that the factors that appear are age, marital status, diabetes and treatment, and annual unmet medical needs. Accordingly, to increase the recognition of the importance of hypertension treatment to people of young ages and not to develop complications, public health-educational effort in Gwangju is needed with an effective preparation plan.

Leukocyte count and hypertension in the health screening data of some rural and urban residents (일부 농촌과 도시의 건강선별조사 자료로 본 백혈구수와 고혈압과의 관계)

  • We used the health screening data of some rural and urban residents to examine the cross-sectional association between leukocyte count and hypertension. The 206 male and 203 female rural residents were selected by multi-stage cluster sampling method in Kyungsan-Kun area of Kyungbuk province in 1985 and 600 urban residents were selected by the same sampling method as the rural residents in Daegu city of the same province in 1986 compatible with age-sex distribution of Daegu city of 1985 census, but of whom 384 actually responded. The rest of 600 were replaced by age and sex with those who were members of the medical insurance plan visiting the health management department of the university hospital to get the biannual preventive medical checkups. Excluded in the analysis were those having hypertensive history, diseases and extreme outlying values of the screening tests, leaving 373 rural and 571 urban residents. Leukocyte count was measured with ELT-8 Laser shadow method and the unit $cells/mm^3$, Blood pressures were determined with an aneroid sphygmomanometer with pre-standardized method and hypertensives were defined as those showing systolic blood pressure more than 140mmHg and/or diastolic blood pressure more than 90mmHg. Total residents pooled (N=944) showed a significant difference between hypertensives and normotensives ($6965.93{\pm}1997.01\;vs\;6490.61{\pm}1941.32,\;P=0.00$) and in rural residents was noted the similar significant difference (P=0.03). None of significant differences were noted in any stratum stratified by residency and sex. Compared to the lowest quintile of WBC, 2/5 quintile showed odds ratio 0.99 (95% Confidence interval, Ci 0.62-1.59), 3/5 quintile 1.41 (95% CI 0.90-2.21), 4/5 quintile 1.76 (95% CI. 1.14-2.72), and highest quintile 1.80 (1.15-2.82) in the total residents. Likelihood ratio test for linear trend for it indicated a significant trend ($X^2_{trend}=5.53,\;df=1,\;P<0.05$). There were no other significant odds ratios compared to the lowest quintile of WBC in strata stratified by residency and sex. The odds ratios in total residents which had showed significant odds ratios became nonsignificant and of reduced magnitude after controlling age, frequency of smoking and drinking with multiple logistic. regression. In each stratum, it changed magnitudes of odds ratios slightly and unstably. None of the trend tests showed any significant trend. These results suggest that the Friedman et al's finding of association between leukocyte count and hypertension may be due to an statistical type I error resulting from the data dredging in an exploratory study, in which more than 800 variables were screened as possible predictors of hypertension.

The Associated Factors of Health Examinations Behaviors among Some Elderly Persons in Urban and Rural Areas (일부 도시·농촌지역 고령자의 건강검진 수진행동에 관련된 요인)

  • Objectives: We investigated the factors related to health examination behaviors, sociodemographic aspects and lifestyles of elderly persons with different social backgrounds, and compared sexual and regional differences in urban and rural elderlies. Methods: The total study subjects(464 individuals) from urban(236) and rural areas(228), recruited by a stratified cluster random sampling were interviewed and examined about their sociodemographic profiles, daily lifestyles, subjective health status, conditions concerning use of medical resources, hearing acuity, visual acuity and ADL(activity of daily living), and whether they receive health examination or not. For statistical analysis, Chi-square test was used for sexual and regional comparisons among the groups who have been given a health examination and the one who have not. Results: In urban areas, the rate of having underwent health examination was 54.5% in men and 46.9% in women, and in rural areas, it was 59.8% in men and 42.7% in women, showing its higher rate in men than in women in both areas. For regional differences between the group who have taken a health examination and the one who have not, there was a significant difference in terms of age, family pattern, current job, monthly household income, owning a house, drinking status, eating habit, subjective health status, whether they have taken outpatient medical service for the recent 3 months or not, anxiety for the health, and IADL conditions according to whether the community is rural or urban. In multiple regressions, the influential factors on the health examination behaviors were selected such as having their own house, their family doctor, amnesia, urinary incontinence and chronic disease in urban districts. But in rural districts, the variables were selected such as having or not of their family doctor, urinary incontinence, anxiety for the health, educational level, their own house and chronic disease. Conclusions: It is suggested that the approach to the health examination of an older patient requires substantial consideration of highly variable individual sociodemographic characteristics involving regional attributes as well as their daily life styles, subjective health status, status of performing health examination, physical health status and ADL conditions.
