Developing Framework of Tobacco Curriculum Integrating Related Subjects From Elementary Through High School

  • 연구배경: 최근 청소년 흡연의 저연령화 문제가 심화되고 있어 청소년 흡연 대책마련이 절실히 요구되고 있다. 그 중에서도 흡연예방교육은 근본적인 문제를 해결할 수 있는데, 우리나라의 교육여건상 흡연예방교육을 위하여 할애할 수 있는 시간과 독립성, 교육내용상의 질을 한꺼번에 개선하기는 매우 힘들다. 이과 같은 문제를 해결하기 위하여 본 연구는 기존의 교과 과정의 구조에 체계적인 연계성 이론을 적용하여 통합적인 흡연예방 프로그램을 구상하게 되었다. 연구방법: 수직적/수평적인 연계성 이론에서 통합성, 접합성, 연결성의 3가지 영역을 적용하기 위한 방법은 다음과 같다. 1) 지식, 태도, 기술, 행동에 따른 일반적 가이드 라인을 재조성하였다. 2) 효과적인 흡연예방을 위한 사회/심리적인 요인을 기존 교과와 같이 보완할 수 있도록 개발하였다. 3) 기존 교과서의 내용을 분석하여 흡연예방과 관련된 효과와 단원을 파악하여 2)과 3)을 접합하였다. 4) 연결성은 거시적인 관점에서 지금까지 개발된 모든 요소들이 관련되어 있음을 보여 주어야 하므로, 본 연구는 가이드라인, 강화내용, 각 교과 단원이 모두 연결되어 있음을 보여주는 표 제시로 완성하고자 하였다. 이 연구를 위하여 각 교과를 대표하는 교과서 총 111권이 검토되었다. 연구결과: 청소년 흡연예방에 효과적이라고 알려진 사회/심리적인 모든 요인들은 사회, 윤리, 도덕 등의 교과에서 충분히 다루고 있음을 알 수 있었다. 그러나, 흡연과 직접적으로 관련되어 있지 않기 때문에 흡연예방교육과 상관이 없다고 여겨질 수 있다. 이러한 단점을 보완하기 위하여 각 관련 교과와 단원, 흡연예방관련지도를 제안하는 교사용 보완 지도자료 지침서가 개발되었다. 흡연예방교육에 필요한 모든 교육적인 요소들이 수직적(초중고등학교), 수평적(각 교과간)으로 연계되어 있음을 보여 주었다. 연구결론: 독립적인 흡연예방 프로그램을 개발하기 보다는 이와 같이 기존의 커리큘럼에 구조적으로 통합할 수 있는 흡연예방 프로그램이 비용-효율적일 수 있다.

The Effect of Smoking Prevention and Smoking Cessation Program using Smoking Cessation Supporters on the Smoking Knowledge and Beliefs and Self-efficacy of Smoking Cessation of Nursing Students (금연서포터즈를 활용한 흡연예방 및 금연프로그램이 간호대학생의 흡연 지식 및 신념, 금연 자기효능감에 미치는 효과)

  • This study was conducted to evaluate the effect of smoking prevention smoking cessation program using smoking cessation supporters on the smoking knowledge and beliefs and self-efficacy of smoking cessation of nursing students. The smoking prevention and smoking cessation program developed for this study consisted of general education on smoking (8 times for 30 minutes each time) and the activities of college student smoking cessation supporters. The 30 college student smoking cessation supporters consisted of 6 people per team, a total of 5 teams, and conducted online video promotions by team, smoking cessation promotion and campaign activities, recruiting and mentoring for smoking cessation challengers. As a result of the study, smoking prevention and smoking cessation programs using smoking cessation supporters were effective in increasing nursing students' smoking knowledge and changing their beliefs about smoking negatively. In particular, by understanding the awareness and overall situation of smoking among nursing students, and operating a smoking prevention and smoking cessation program, it made them realize that smoking prevention and secondhand smoke among non-smokers are threatening the health of others. It contributed to the spread of the smoking cessation culture in the university by providing a way to motivate people to quit smoking and to maintain their success in quitting smoking.

A Descriptive Study Of School Children's Knowledge, Attitudes And Practices Regarding Smoking (중학생들의 흡연에 대한 지식, 태도, 그리고 행동에 관한 기술적 연구)

  • The goal of this study is to explore different risk factors for smoking and look at the relationships between knowledge, attitudes, and behavioral intentions regarding smoking among schoolchildren, in order to reduce teenage smoking. To achieve this goal a self-administered questionaire regarding smoking was provided to schoolchildren in the 7th and 8th grades in one junior high school in Jerusalem. The schoolchildren were exposed to 10-12 hours of a smoking prevention intervention program. The questionaire focused primarily on the personal characteristics, social environment, knowledge, attitudes, practices, and behavioral intentions of the children. Crosstabs were performed on each variable to determine if significant associations exist among the different variables. The statistical computer, package, SPSS PC, was used to manipulate the data along with Chi-square test. The findings were as follows : About 11% of the children aged 12-14 have ever smoked or are smoking currently, and about 24.0% of those who ever smoked started smoking at the age of 10. Boys smoke more(p<.01), poorer students(by self-perception of school performance) smoke more, and those who had peers who were smokers were more likely to smoke(p<.05). The percentage of the children who reported that either father or mother smoked was about 30%, but no statistical association was found between parental smoking and children's smoking, although trends were noted in the expected direction, i.e. more smokers among children of smokers. Only 1.1% of the children intended to smoke in the future, and 98.0% of the children indicated that they can or they might be able to withstand social pressure. Seventy percent of the children demonstrated medium to high knowledge about smoking, Males, 8th graders, better students, and those without friends who smoke had higher social pressure showed more negative attitudes(p<.01). Those with non-smoking siblings showed more negative behavioral intentions regarding smoking(p<.01), and better students showed more negative behavioral intentions. Those who had higher knowledge scores showed more negative attitudes towards smoking, but not significantly so. Those who had very negative behavioral intentions showed highly significant negative attitudes towards smoking(p<.01).

The Effects of a Staged Smoking Prevention Program for Primary School Children (초등학생을 위한 단계별 흡연예방 프로그램의 효과)

  • The subjects of this study were 415 children in the first to third grade selected from 4 primary schools. Among them, 212 children belong to the experimental group, and 203 children were in the control group. The program was provided respectively to first, third and fifth grade children for 2, 4 and 6 teaching-hours from 2013.1.1 to 2013.12.19. To measure the effects of the program, the knowledge and attitudes on/toward smoking, self-concept were used as the research tools. Before and after program-implementation, there were significant differences in knowledge of smoking, but no differences in attitudes and self-concept. A smoking prevention program for primary school children should be applied step-by-step in conjunction with the regular education curriculum. In addition, the program should aim for long-term effects rather than short-term effects.

The Relationships between Health Behaviors, Mental Health and Smoking among Korean Adolescents (한국 청소년의 건강행태 및 정신건강과 흡연의 관련성)

  • This study, as a secondary analysis research, bases its source upon on raw data from the on-line survey of the Korean adolescents' health behaviors in 2015. This study was conducted to clarify the relevance of Korean adolescents' smoking with their health behaviors and mental health, and thus further to provide basic sources of effective education for their smoking prevention and for program development. The results of the study showed that Korean adolescents' smoking was significantly affected by the adolescents' sex, grades, school achievements, types of schools, allowances, economic status, parents' educational background (p<0.001). Meanwhile, smoking based on the characteristics of health behaviors was significantly different according to alcohol consuming experience, families' and friends' smoking, physical activity and subjective health status, and BMI (p<0.001). Smoking characterized by mental health showed the significant difference in their stress recognition, subjective sleep patterns, depressed experiences, suicidal ideation, and subjective sense of happiness (p<0.001). Based on all the factors regarding youth smoking, there should be development and provision of educational program for smoking prevention and cessation, including healthy behavior and mental health improvement for Korean adolescent.

Identifying and Predicting Adolescent Smoking Trajectories in Korea (청소년기 흡연 발달궤적 변화와 예측요인)

  • The purpose of this study is two-fold: 1) to identify different adolescent smoking trajectories in Korea; and 2) to examine predictors of those smoking trajectories within a social developmental frame. Data were from the Korea Youth Panel Survey(KYPS), a longitudinal study of 3,449 youths followed since 2003. Using semi-parametric group-based modeling, four smoking trajectories were identified: non initiators, late onsetters, experimenters, and escalators. Multinomial logistic regressions were then used to identify risk and protective factors that distinguish the trajectory groups from one another. Among non smokers at age 13, late onsetters were distinguished from non initiators by a variety of factors in every ecological domain. Among youths who already smoked at age 13, escalators who increased their smoking were distinguished from experimenters who almost desisted from smoking by age 17 by self-esteem and academic achievement. Finally, implications for youth welfare practice from this study were discussed.

A meta-analysis of adolescent psychosocial smoking prevention programs in the United States: Identifying factors associated with program effectiveness (사회 심리 이론에 근거한 학교 흡연 예방 프로그램의 메타분석: 미국 사례와 Explanatory Variables)

  • Adolescent psychosocial smoking prevention programs have been successful, but limited in the magnitude of program effects. The present study is the secondary analysis after the previous study estimated mean effect sizes in smoking knowledge, attitudes, skills, and behaviors with treatment variables. Regardless of overall program effect estimations that other meta.analysis studies have done, this study is conducted to identify explanatory variables that are likely to increase program effects. A decrease of adolescent smoking behaviors is associated with the following factors: a. Younger students ($5^{th}-7^{th}$) than older students ($8^{th}-12^{th}$). b. Research methodology using true experimental design, quasi experimental design with equivalence between groups, use of random assignment, 10% or less attrition rate, use of a no treatment control group, high implementation fidelity, and/or acceptable instrumentation reliability. c. Programs using trained peer leaders, targeting cigarette smoking only, implementing 10 or more treatment sessions and/ or providing booster sessions.

A Study on the Effects of the Home-Linked Indirect Smoking Prevention Program for Early Childhood (유아기 간접흡연예방 가정연계교육 프로그램의 효과)

  • This study was conducted to prove the effectiveness of home-linked indirect smoking prevention education in early childhood in improving the awareness, skills, attitudes and willingness to protect themselves from smoking. 208 5-year-old children were recruited from three kindergartens located in Seoul and Gyeonggi-do. Children in the experimental group received home-linked indirect smoking prevention education. Children in the comparative group, received indirect smoking prevention education in kindergarten. Children in the control group received general health education. The results revealed that all groups showed significant differences between pretest and posttest in awareness of second-smoke, attitudes and willingness to stop smoking. However, the skills to protect oneself from second-smoke showed a significant difference in the experimental group and the comparative group. The values of changes among the groups showed high increases in the order of experimental, comparative, and control groups. This shows that home-linked education had considerable positive effects on indirect smoking prevention.

The Effects of Motivation Enhancement for smoking cessation Program on Middle School Students' Knowledge on smoking, self-efficacy, and Daily amount of Smoking (금연동기강화상담 프로그램이 중학교 남학생의 흡연지식, 자기효능감 및 일일 흡연량에 미치는 영향)

  • This study was conducted to evaluate the effects of a Motivation Enhancement for Smoking Cessation Program on middle school students' knowledge regarding smoking, self-efficacy, and daily amount of smoking. The study used a nonequivalent control group pre-post-test design and included 47 students who smoke from two middle schools located in city U as subjects. Subjects were separated into two groups, an experimental group with 23 students and a control group with 24 students. Data were collected from February 19, 2018 to May 11, 2018 and analyzed using the IBM SRSS/win 24.0 program to conduct a t-test, ${\chi}^2$-test, Fisher's exact test, and a paired t-test. After subjects went through the Motivation Enhancement for Smoking Cessation Program, their knowledge regarding smoking (t= 3.38, p=0.002), self-efficacy (t= -3.37, p=0.002), and their daily amount of smoking (t=3.38, p=0.002) showed significant changes. The results indicate that the Motivation Enhancement for Smoking Cessation Program is effective at increasing subjects' knowledge regarding smoking and self- efficacy while decreasing their daily amount of smoking. Therefore, to induce cessation of adolescent smoking, Motivation Enhancement for Smoking Cessation Programs tailored to each student should be held.
