Incidental Breast Lesions on Chest CT: Clinical Significance and Differential Features Requiring Referral (흉부 전산화단층촬영에서 우연히 발견된 유방 병변: 임상적 중요성 및 진료 의뢰가 필요한 특징적 영상 소견)

  • Purpose: To evaluate the CT features of incidental breast lesions on chest CT and to suggest useful criteria for referral to a specialized breast unit. Materials and Methods: Between May 2009 and April 2014, enhanced chest CT examination reports containing the key word 'breast' were reviewed retrospectively. Patients who had incidental breast lesion and were referred to a specialized breast unit and then underwent pathological confirmation or follow-up over a 1-year period were included. Finally, 86 patients (all female, mean age, $48.9{\pm}12.6years$) were enrolled. Two radiologists evaluated lesion characteristics, including size, shape, margins, and enhancement. The correlations between the CT features and pathologies were evaluated, and the diagnostic accuracy of CT features in various combinations was assessed. Results: Among the CT features, irregular shape, non-circumscribed margin, and high contrast enhancement were different between malignant and benign lesions (p < 0.05). The combination of non-circumscribed margin and high contrast enhancement had the highest accuracy (97.7%). Conclusion: Reliable CT features for incidental malignant breast masses are irregular shape, non-circumscribed margin, and high contrast enhancement. The combination of non-circumscribed margin and high contrast enhancement could help distinguish incidental malignant breast lesions and indicate referral to a specialized breast unit.

Radiation Dose using Chest CT for Patients with Pneumoconiosis Complication - Comparison with International Guidelines - (진폐요양환자의 흉부 CT촬영에 사용된 선량 - 국내외 진단 참고 준위와 비교 -)

  • We report here on the results of evaluating the radiation doses using chest computed tomography (CT) for patients with pneumoconiosis complication. For the first time, we visited the 17 MIPs to evaluate the dose-length product (DLP, $mGy{\cdot}cm$), CT unit, and protocols of scanning and image reconstruction those is routinely used for treating patients with pneumoconiosis who have complication. All statistical analysis was performed using the Statistical Program for Social Sciences (SPSS ver. 19.0, Chicago, IL, USA). Mean of total DLP was $727.7mGy{\cdot}cm$, ranging from 272.0 to $1228.7mGy{\cdot}cm$. DLP from obtaining parenchymal lung images was significantly reduced than that from obtaining total lung images (555.9 vs. 707.2, p<0.001). Third quartile of total and pre-scanning DLP was 1036.1 and $504.1mGy{\cdot}cm$, respectively. Chest CT radiation doses for patients with pneumoconiosis complication are similar with korean diagnostic reference level as well as international guidelines.

Measurement of Radiation Dose of HR CT and Low Dose CT by using Anthropomorphic Chest Phantom and Glass Dosimetry (인체등가형 흉부팬텀과 유리선량계를 이용한 고해상력 및 저선량 CT의 선량측정)

  • The purpose of this study is to provide basic clinical data by evaluating images, measuring absorbed dose and effective dose by using high resolution CT and low dose CT by using anthropomorphic chest phantom and glass dosimeter. Tissue dose was measured by inserting a glass dosimeter into the anthropomorphic chest phantom. A 64-slice CT system (SOMATOM Sensation 64, Siemens AG, Forchheim, Germany) and CARE Dose 4D were used, and the parameters of the high resolution CT were 120 kVp, Eff. Scan parameters of mAs 104, scan time 7.93 s, slice 1.0 mm (Acq. 64 × 0.6 mm), convolution kernel (B60f sharp) were used, and low dose CT was 120 kVp, Eff. mAs 15, scan time 7.41 s, slice 3.0 mm (Acq. 64 × 0.6 mm), scan of convolution kernel B50f medium sharp. CTDIvol was measured at 8.01 mGy for high resolution CT and 1.18 mGy for low dose CT. Low dose CT scans showed 85.49% less absorbed dose than high resolution CT scans.

  • 흉부 CT 영상은 각종 흉부 질환의 진단에 널리 사용되고 있으며, 영상 분석 기술을 이용하여 암이나 다양한 형태의 종양 등의 관심 영역(region-of-interest)을 자동으로 탐지하는 기법들이 최근 많이 연구되고 있다. 본 논문에서는 흉부 CT 영상에서 연속된 여러 장의 2D 영상의 변화를 분석하여 중요 관심 영역인 노듈(nodule)을 찾는 방법을 제안한다. 인접 영상에서의 노듈의 연속성과 크기의 변화, 모양, 밝기의 특징을 분석하여 노듈을 검출하였고, 실험 결과, 상당한 정도의 정확도를 보였다. 향후 추가적인 연구를 통하여 실제 환경에서도 활용 가능할 것으로 기대된다.

Comparison of Clinical Significance Between Chest CT Scan and Bronchoscopy Prior to Bronchial Artery and Outcome of Embolization in Patients with Hemoptysis (객혈환자의 기관지동맥 조영술 전 흉부 전산화 단층촬영과 기관지내시경 검사의 유용성 비교 및 색전술 성적)

  • Background : Emergency management in hemoptysis is bronchial artery angiography and embolization. This study was designed to investigate the accuracy of localization of bleeding site by simple roentgenogram, computed tomography(CT) and bronchoscopy prior to embolization and to evaluate the outcome of embolotherapy. Method : We retrospectively evaluated 50 patients performed bronchial artery embolization(BAE), admitted to tertiary university hospital due to hemoptysis. Results : The most common causes were pulmonary tuberculosis, old tuberculous related parenchymal damage, aspergilloma, and bronchiectasis. The success rate of BAE within one month was 90%; within 3 months was 88%; during follow up period of mean 11.6 months was 76%. The concordant rate of simple roentgenogram with angiographic outcome in terms of bleeding site is 70%; in chest CT 80%; in bronchoscopy 81%; in combined information of simple roentgenogram and CT 83%; in combined information of simple roentgenogram and bronchoscopy 78%. Conclusion : The diagnostic accuracy for the bleeding site was similar between chest CT and bronchoscopy, showing high diagnostic yield. The success rate of BAE was comparative to prior studies. Further study will be needed in a large scale in near future.

Correlation Analysis of between Patient and Equipment Factors and Radiation Dose in Chest Low Dose and Abdominal Non-contrast CT (흉부 저선량 및 복부 비조영 CT 검사에서 환자 및 장비 인자와 선량과의 상관관계 분석)

  • This paper is to establish a basis for a dose reduction strategy by confirming correlations with the factors that may affect the radiation dose based on the dose records in low-dose chest CT and abdominal non-contrast CT. In order to find out the causes of unnecessary exposure, the correlation between seven factors (age, gender, height, weight, BMI, patient status [inpatient and outpatient], and use of dose modulation) and CT dose were identified. Logistic regression was used as the statistical analysis for correlation verification. In the low dose chest CT, as the higher values of height and BMI and dose modulation off were associated with lowering the risk exceeding Diagnostic Reference Levels(DRL) (odds ration<1, p<0.05). However, as woman compared to man and the higher values of weight were associated with highering the risk exceeding DRL (odds ration>1, p<0.05). In the abdomen CT, as dose modulation off were associated with lowering the risk exceeding DRL (odds ration<1, p<0.05). Therefore It is necessary to conduct research on the relationship between various factors affecting radiation exposure and patient radiation dose for reducing the dose.

Quantification of Experimentally Induced-Pleural Effusion in Beagle Dogs: Radiography versus CT and Ultrasonography (비글견에서 실험적으로 유발한 흉수의 정량 평가: 방사선, 컴퓨터단층촬영 및 초음파 검사 비교)

  • This study was performed to quantify the pleural effusion in radiography, ultrasonography and computed tomography(CT) and to evaluate and compare the usefulness of these methods. Normal saline of 10 ml/kg was infused into the pleural space until a final loading volume of 60 ml/kg body weight was reached in six Beagle dogs. The radiographic examination was performed for the detection and quantification of pleural effusion. On the ultrasonographic study, the maximum perpendicular distance was measured between the surface of the lung and the thoracic wall to evaluate pleural effusion. On the CT image, pleural effusion was evaluated as the perpendicular distance to the thoracic surface in the maximum pleural effusion volume on any transverse images with soft tissue window. Statistical analysis was performed using linear regression test. The volume of pleural effusion and measurements of radiography and ultrasonography had no statistical relationship. However, a significant correlation was identified between the volume of pleural effusion and the depth at right ($r^2=0.715$), left ($r^2=0.745$), and mean right and left depth ($r^2=0.844$) on the CT images. All of the thoracic radiographs, ultrasonography, and CT are useful in recognition of pleural effusion. In quantification of pleural effusion, the CT measurement method is superior to radiographic and ultrasonographic measurements.

Analysis of Image Quality and Scan Dose when Applying Reconstruction Algorithm Changes to Chest CT Scans (흉부 CT 스캔에서 재구성 알고리즘 변화적용 시 화질과 스캔 선량 분석)

  • In this study, among chest CT examination conditions, the tube voltage was changed to 100 and 80 kVp and the reconstruction algorithm was changed to FBP, ASIR-V, and DLIR to compare and analyze changes in examination dose and image quality. As a result, when applying ASIR-V and DLIR at a tube voltage of 100 kVp, which is lower than the existing tube voltage, the dose is lowered while achieving image quality most similar to that when applying 120 kVp and FBP. especially, DLIR reconstructed images had excellent SNR and CNR at all tube voltages. In addition, the SSIM index was analyzed to be closest to 1, showing the highest similarity to the original image. Therefore, when performing repeated chest CT examinations, the application of DLIR can reduce the examination dose by about 29.7%, which is expected to help solve some of the biggest problems with CT examinations, namely radiation exposure due to the examination.

Estimate of Radiation Doses in MDCT Using Patient Weight (MDCT 검사에서 환자 체중을 이용한 선량 평가)

  • The purpose of this study provides measurements of radiation dose from MDCT of head, chest, abdomen and pelvic examinations. A series of dose quantities that are measured of patient weight to compare the dose received during MDCT examinations. Data collected included: weight together with CT dose descriptors, volume CT dose index (CTDIvol) and dose length product (DLP). The effective dose was also estimated and served as collective dose estimation data. Data from 1,774 adult patients attending for a CT examination of the head (n=520) or chest (n=531) or abdomen (n=724) was obtained from spiral CT units using a same CT protocol. Mean values of CTDIvol was a range of 48.6 mGy for head and 6.9, 10.5 mGy for chest, abdomen examinations, respectively. And mean values of DLP was range of 1,604 $mGy{\cdot}cm$ for head, 250 $mGy{\cdot}cm$ for chest, 575 $mGy{\cdot}cm$ for abdomen examinations, respectively. Mean effective dose values for head, chest, abdominal CT were 3.6, 4.2, and 8.6 mSv, respectively. The degree of CTDIvol and DLP was a positive correlation with weight. And there was a positive correlation for weight versus CTDIvol ($r^2$=0.62), DLP ($r^2$=0.694) in chest. And head was also positive correlation with weight versus CTDIvol ($r^2$=0.691), DLP ($r^2$=0.741). We conclude that CTDIvol and DLP is an important determinant of weight within the CT examinations. The results for this study suggest that CT protocol should be tailored according to patient weight.

The efficacy of computerized tomographic scan for chest trauma (흉부외상 환자에 대한 전산화 단층촬영의 효용성)

  • Computerized tomography(CT) is an effective technique in the initial evaluation of the abdomen and head following blunt trauma. To evaluate the efficacy of CT of the thorax, a retrospective study comparing early thoracic CT scanning with initial chest roentgenogram (CXR) was carried out on 134 patients with blunt trauma on the chest. Among 134 patients, 45 patients had normal initial chest roentgenogram and 24 patients showed normal CT findings. Sensitivities of diagnosing pneumothorax and pleural effusion by CXR were low (46.2 and 62.9% respectively), whereas 71.4%(45/63) of patients had thoracostomy only by CXR. Although sometimes abused, CT of the thorax is effective in the initial diagnosis.
