Surgical Experience of Diffuse Esophageal Spasm - A report of 2 cases - (광범위 식도경련 -수술치험 2례-)

  • Diffuse esophageal spasm(DES) is a rare disease seen in 4% of all patients studied in an esophageal motility laboratory, and its diagnosis and surgical management is still controversial. Recently, we treated two patients by extended esophageal myotomy for diffuse esophageal spasm which was diagnosed by the clinical symptoms of patients, esophagoscopy, esophagography, and esophageal manometry. The successful result of treatments was proved with subsidence of previous clinical symptoms(dysphagia and chest pain), postoperative esophagography and esophageal manometry. We present the results together with the review of literatures.

Complete mouth rehabilitation with vertical dimension increase in patient with extremely worn dentition (심한 치아 마모를 보이는 환자에서 수직 고경 증가를 동반한 전악 구강 회복 증례)

  • Generalized severely worn dentition causes occlusal disharmony, esthetic problems, and temporomandibular joint disorders. In order to solve these problems, it is necessary to make a precise analysis of vertical dimension and treatment plans considering it. This case report demonstrates the complete mouth rehabilitation of a 58-year-old male patient with a lot of worn teeth by increasing vertical dimension. Provisional restorations were cemented and after 4 months of evaluation for patient's compliance, permanent prostheses were fabricated. With these treatments, functionally and esthetically satisfactory results were obtained.

  • This paper is proposed that implemental model of customer relationship management system for oriental hospital is designed by customer segmentation using personal information and medical record of outpatients in existing integrated medical information system database. Proposed model can be practical model at once, because it can construct by partial modification of existing medical information system without additional information technology and infrastructure. And, if we use the proper variable and method of customer segmentation according to marketing strategy, it can be flexible customer relationship management system not only improvement of customer satisfaction but also various marketing supports.

A Study on Anesthesia and Operating Room (OR) Nurses' Perception and Performance of Privacy Protection Behavior for Patients Undergoing General Anesthesia Surgery and Patients' Satisfaction with Operating Room Hospitalization Experience (프라이버시 보호 행동에 대한 전신마취 수술환자와 마취⋅수술실 간호사의 인식, 실천 정도 및 전신마취 수술환자의 입원경험 만족도 연구)

  • Purpose: This study aims to examine level of perception and performance of privacy protection behavior of anesthesia and operating room (OR) nurses for patients who underwent general anesthesia surgery. Methods: Data collection was conducted from August 2020 to January 2021 for a total of 101 participants, consisting of 49 patients and 52 nurses. Independent t-test and Pearson's correlation were conducted using SPSS 21. Results: Anesthesia and OR nurses showed the highest score in patient privacy, followed by patient information management, body privacy, and the lowest score in communication. There was a significant difference between the patient information and the communication. Conclusion: Anesthesia and OR nurses had the highest level of perception and performance of patient privacy protection behavior for body privacy, and the lowest for communication. In addition, there was a significant difference in patient information management and communication. In order to protect the privacy of patients undergoing general anesthesia surgery, efforts are needed to learn standardized nursing knowledge, attitudes, and practice.

A Case of Bronchoesophageal Fistula Treated by Bronchial Stent Insertion and Endoscopic Histoacryl Injection (기관 스텐트 삽입과 히스토아크릴 주입법으로 치료한 식도 누공 1례)

  • We report a bronchoesophageal fistula that treat with bronchial stent insertion and histoacryl injection. A 52-year-old man with esophagel cancer was transferred for dysphagia management. At the CT scan that underwent on admission, esophageal cancer with multiple lymph node metastasis was observed. At the gastroduodenoscopy and contrast study, bronchoesophageal fistula was observed. Recurrent stent insertion treatment was failed, and then, By the broncoscopy, covered stent was inserted to right bronchus, and By the endoscopy, fibrin glue and histoacryl was injected in the fistula opening. At the contrast study, contrast leakage was not observed, and the patient was discharged. But, at the 14 days after discharge, the patient was admitted to the emerency room because of cough symptom whenever he eat food. The patient was diagnosed with aspiration pneumonia, we were determined that it is unable to oral intake. The patient received a jejunostomy and antibiotic treatment for aspiration pneumonia. He was discharged after symptomatic improvement.

Coronary Artery Bypass Graft Surgery in Patients 70 Years of Age and Older (70세 이상 고령 환자에서의 관상동맥우회술)

  • Background: There has been an increase in the number of elderly patients considered for coronary artery bypass grafting (CABG). Recently, there were many satisfactory reports of coronary artery bypass grafting (CABG) in old age due to the development in operative technique and postoperative management. We evaluated operative and follow-up results of patients 70 years of age and older compared to 60 years old. Material and Method: We retrospectively studied the cases of 74 consecutive patients 70 years or older (group A) who underwent a elective CABG from January 2000 to December 2003 and compared that of relatively young age group (group B, 60-69 years old). We compared preoperative characteristics, operation technique, postoperative results that effect outcome, also we investigated late mortality and cardiac events at follow-up periods. Result: Preoperative demographic and clinical characteristics of two groups were not different, except preoperative renal dysfunction(serum creatinine: $\geq$1.4 mg/dl) (group A 17, 23$\%$ vs group B 14, 9$\%$) (p=0.024). There was no difference of the mean number of distal anastomosis and the left ventricular ejection fraction in group A decreased significantly from 53.7$\pm$13$\%$ preoperatively to 49.9$\pm$ 12$\%$ postoperatively (p=0.02), but not changed in group B. There was no difference at operative mortality rate and postoperative major morbidity rate, but wound problem of saphenous vein harvest site was significantly higher in group A than group B (6.8$\%$ vs 0.7$\%$, p=0.02). The mean follow up duration was 24.3$\pm$13 months and the cumulative survival were 95.4$\%$ at 2 year and 79.9$\%$ at 4 year in group A and 95.4$\%$ at 2 year and 90.1$\%$ at 4 year in group B (p=ns). Conclusion: We conclude that age is not a factor of determination when we decide about operation because coronary artery bypass grafting in elderly more than 70 years old can be performed with a low mortality rate and acceptable morbidity rate.

  • 인슐린의존형당뇨병 환자의 혈당조절의 방법으로 먼저 식이요법과 운동이 권장되며, 이러한 방법으로 혈당조절이 만족스럽지 않을 때 경구혈당강하제를 사용하는 것은 혈당조절과 합병증 예방에 중요한 역할을 할 수 있다. 국내에서는 경구혈당강하제로 설폰요소제, 비구아나이드제인 메트포르민(metformin) 및 알파글루코시다제(${\alpha}$-glucosidase) 억제제인 아카보스(acarbose) 등이 사용되고 있다. 새로 개발중인 트로글리타존, 지방산 산화 억제제 등의 약제는 아직 임상에서 널리 사용되고 있지는 않다.

  • 상윤상후두부분적출술은 과거 후두전적출술을 실시하던 환자들에서 선택적으로 사용하여 만족할만한 종양학적 결과 및 후두의 생리적인 기능을 보존 할 수 있어 점차 널리 보급되고. 있다. 이 술식후의 기능변화나 보상기전등에 대하여는 잘 알려지지 않았으나 정상인과 비교하여 음성지표들의 변화가 보고되고 있다. 이 수술은 원발병소의 위치에 따라 CHEP, CHP 방범으로 재건할 수 있으며 종양의 범위에 따라 일측의 피열연골과 윤상연골의 앞부분을 제거하는 extended procedure등이 사용되고 있다. (중략)

  • 성문하 협착과 기관 협착이 동반된 경우 이를 치유할 몇가지 방법이 시도되고 있으나 만족할만한 효과를 얻지는 못하고 있다. 저자들은 성문하와 기관협착이 동반된 27세 남자환자에서 기관 및 성문하 협착부 전면을 제거후 쇄골에 흉쇄유돌근(Sternocleidomastoid muscle: SCM)이 붙은채로 쇄골의 절반을 절제하여 수질부위를 제거후 협부점막을 부착하였다. 이렇게 얻어진 Vascularized clavicle을 성문하와 기관의 전면부 재건에 이용하여 좋은 결과를 얻었기에 문헌고찰과 함께 이를 보고하고자 한다.
