  • This paper is for the a study on home healthcare system for a patient using SAW Tag. This system is useful when there's a patient in home. Many patients are wounded in restroom. But restroom is very private place and nobody want to be disturbed here. The place where the patient is most injured in the house is the toilet, and it is the point where the privacy of the individual should be kept and the patient's condition should be checked and cope with in case of emergency. And using saw tag, we can calculate position and ID through reflected rf signal. Therefore we propose a general home healthcare system. The management system utilized a seat bed system to monitor at-home patients. The system mounts a bio-signal measuring unit on a toilet seat, measures a bio-signal from the bio-signal measuring unit, and the measured bio-signal is transmitted to the main server through the user's smart phone. With proposed system, we can expand operation area to silver town where many patients are cared. In this paper we developed sensor system and saw tag positioning system and showed the result.

  • Recently, various medical services are provided with the advance of IT. However, there is a problem that a third party would exploit medical information when the information is sent and received through wired or wireless connection. In this paper, a patient information management scheme using group index information for the third party not to illegally exploit a patient's medical information without his consent is proposed. This proposed scheme creates index information in each hierarchical level to be used with access information so that not only medical staff can have access to patient's medical information hierarchically but also it can manage access level in groups. The scheme aims to enable the medical staff to minimize the time spent to analyze the type of disease and to prescribe for it so that they can improve patients' satisfaction. Plus, the scheme aims to improve work efficiency by minimizing the medical staff's workload according to the authority to access patients' medical information.

A Review of Self-Monitoring of Blood Pressure for Self-Management of Hypertension (고혈압 환자의 혈압자가측정에 관한 문헌고찰)

  • 연구목적: 고혈압환자의 효과적인 자가관리 간호중재를 개발하는데 필요한 기초자료로 혈압자가측정(self-monitoring of blood pressure)에 대한 특성, 장점, 범위 및 제한점을 파악한다. 연구방법: 1996년부터 2005년까지 PubMed에 수록된 고혈압환자의 자가관리 방법중 혈압자가측정방법이 포함된 외국의 연구를 중심으로 체계적인 고찰을 실시하였다. 전체 69개 자가관리방법을 적용한 논문 중에서 혈압자가측정이나 가정에서의 혈압측정(home monitoring)에 대한 45개 논문을 선정하여 분석하였다. 연구결과 전반적으로 혈압자가측정 방법은 고혈압환자의 혈압조절을 향상시키는 효과가 유의한 것으로 나타났다. 환자가 혈압을 자가측정한 방법에서 평균 혈압, 수축기 혈압, 맥압이 유의하게 감소한 것으로 보고되었다. 장기적 프로그램에서 혈압조절율이 향상되거나, 이완기혈압의 감소효과도 보고되었다. 혈압자가측정의 기준치는 연구마다 일치하지는 않았지만 135/85mmHg 수준이 정상혈압의 상한수준으로 제시되었다. 일부 연구에서는 가정 내 혈압측정이 24시간혈압측정을 대체하는 방법으로 추천되었다. 보다 정확한 혈압자가측정을 위해서 신뢰성 있는 혈압측정기구의 사용과 정확한 측정방법이 환자들에게 교육될 필요가 있다. 결론: 혈압자가측정은 고혈압환자의 혈압조절에 유용한 방법으로 고찰되었다. 또한 고혈압의 진단, 혈압치료와 고혈압 약물복용 행위를 향상시키는 교육적 효과도 긍정적으로 나타났다. 따라서 고혈압환자를 위한 효과적인 자기조절프로그램에 혈압자가측정법을 포함하는 것이 추천되며, 추후 한국에서 시도된 관련 연구에 관한 문헌고찰의 비교연구가 필요하다.

Effects of Respiratory Infectious Disease Simulation-based Education using Standardized Patient for Nursing Student's of the Knowledge, Clinical Nursing Competency (표준화환자를 이용한 호흡기감염 시뮬레이션 교육이 간호대학생의 지식, 임상수행능력에 미치는 효과)

  • The purpose of this study is to analyze the effects of simulation education using standardized patients on respiratory infectious disease knowledge and clinical performance of nursing students. A single-group before-and-after design for the nursing of infectious respiratory patients using standardized patients from March 2 to June 15, 2020, targeting 112 senior nursing students. Respiratory infectious disease education program 'knowledge of lung infectious diseases', 'hand washing', 'wearing a mask', 'encouraging patients and caregivers to wear masks', 'intravenous injection', '3-way injection', 'surgical aseptic technique', 'disinfecting medical devices' , 'contaminated linen management', 'infected person management manual', etc., and 10 educational tasks were performed, and consisted of lectures, technical training, simulation using standardized patients, and debriefing. After simulation education using standardized patients, students' knowledge and clinical performance skills on respiratory infectious diseases showed significant improvement, and it was expected that it could be used for various infection control practices.

  • Self management of chronic asthma is of great importance, since the disease could lead the patient into an emergent situation. In the present study, we describe implementation of a personal digital assistant(PDA) based asthma management system for personal application including symptom and medication to prevent from the potential exacerbation of the disease. The software program was written by the Visual C++ tool in the mobile computing environment and Object Store was applied for data management. User friendly GUI environment was provided for the patient to input his/her daily condition and self treatment such as medication for successful management. The input screen design substituted for keyboard input to a mouse in order to easy to select an item and minimize the keyboard input. The implementation results of this system., Real-time data collection and process were possible and be able to have been carried effectively out a continuous symptom, a medication of asthma patients, risk management.

  • Recently, Ubiquitous innovation is being promoted so that they can support the uHealthcare provide management to human's health. It is thus necessary to conduct such research on the medical care environment where there is a high demand for utilization of status information. In the current situation, there is a lack of research on measures of security processing and monitoring patient status information produced from rapid growth of infra within Medical environment. This study suggests a solution of using RFID to gather patient information such as inpatient information, location of treatment room, progress of patient, humidity, temperature, and diagnostic status, after which the information are protect and processed using security level method.

Lifestyle, Diet, Self-care, and Diabetes Fatalism of Diabetic Patients with and without Diabetic Foot (당뇨병성 족부질환 여부에 따른 당뇨병 환자의 생활습관, 식습관, 자가관리 및 Diabetes fatalism)

  • Objectives: This study was to determine diabetes fatalism of diabetic patients with and without diabetic foot and its association with lifestyle, diet, and self-care. Methods: The subjects were diabetic patients with (male/female 48/21) and without diabetic foot (male/female 33/26). We administered the questionnaires which were designed to determine diabetes fatalism, lifestyle, diet, and self-care. Diabetes fatalism was determined by Diabetes fatalism scale (DFS), which consisted of total 12 items in three subscales namely, emotional stress, religiou spiritual coping, and perceived self-efficacy. Results: The patients with diabetic foot had undesirable diets more frequently (1.37 and 0.91 days/week respectively) and their desirable diets (2.74 and 3.61 days/week respectively) and foot care (4.61 and 5.53 days/week respectively) were less frequent than those without diabetic foot (p < 0.05). An item analysis of the 12 DFS items revealed a Chronbach' ${\alpha}$ of 0.614 and 0.869, respectively in diabetic patients with and without diabetic foot. Perceived self-efficacy related DFS of subjects without diabetic foot was positively associated with smoking (r=0.350, p<0.01), undesirable diet (r=0.295, p<0.05), and drinking (r=0.257, p<0.05), while its negative association with exercise (r=-0.224, p<0.088) and foot care (r=-0.247, p<0.059) did not reach to statistical significance. Conclusions: This work was the first study reporting the potential usefulness of DFS, especially perceived self-efficacy related subscale as a predictor of lifestyle, diet and self-care on the Korean diabetic patients, at least those without severe diabetic foot to screen those who should be the first target for diabetes education.

A Study on Practice of Protective Actions for Medical Information - A comparison between hospital administrators and occupational therapists - (의료정보 보호행동 실천에 관한 연구 - 병원행정관리자와 작업치료사를 비교 -)

  • Attempts were made in this paper to compare the practice of protective actions for information of patients' medical treatment between hospital administrators who do not make direct contact with patients and occupational therapists who usually do. The comparison between jobs in charge showed that occupational therapists did not practice much protective actions for information of patients' medical treatment ($3.52{\pm}.809$) compared to hospital administrators ($3.92{\pm}.724$), even though the former had received regular education about protection, management and supervision of patients' medical information more often ($3.17{\pm}1.129$) than the latter ($3.16{\pm}1.037$). In spite of the fact that occupational therapists were exposed frequently to the danger of revealing medical information in the process of their job performance through talks and communications with patients, they displayed relatively little concern for and awareness of keeping information of medical treatment from being leaked by them. It is thus suggested to promote awareness of medical staff to protect medical information by means of flexible educational system for each occupational group, periodical monitoring, continuing public relation, training and quality control for protection of medical information, as well as routine self-examination of such practice.

Satisfaction and Needs of Cancer Patients and Caregivers under Home-based Cancer Patients Management in Jeju (제주지역내 재가암환자관리를 받은 환자와 보호자에게 제공되는 프로그램 만족도와 요구)

  • This study was conducted to improve the quality of home-based cancer management programs through a survey of satisfaction and needs of the program provided by a regional cancer center. From March 2015 to October 2015, we conducted face to face surveys of patients and caregivers enrolled in home-based cancer management. A total of 101 patients, 59 patients and 41 caregivers, were enrolled. Breast cancer was the main cancer and 51 (86%) patients and 36 (85%) caregivers were satisfied with the number of visits for home-based cancer management. For the service application route, 22 patients (37.29%) obtained information through the cancer center publicity paper and 11 caregivers (26.19%) received recommendations from acquaintances. Except for treatment provided directly to the patients, psychological counseling was the most preferred, and satisfaction regarding picnic and cancer education were also high. Satisfaction with the program provided by regional cancer center was relatively high ($4.14{\pm}1.21$ on a 5-point scale). Additionally, satisfaction of the patients was higher than that of the caregivers, but this difference was not significant ($4.29{\pm}1.11$, $3.93{\pm}1.31$, p = 0.141). Self-esteem was higher among caregivers than patients, but this difference was not significant. To improve the quality of life of cancer patients and caregivers, it is necessary to develop customized programs considering patients' economic situations and need for psychological counseling.

  • Grisel's syndrome is a non-traumatic subluxation of the atlantoaxial joint with an inflammatory condition in the adjacent soft tissues. Due to the instability of the cervical spine, careful airway management is crucial to prevent potential cervical spinal cord injury following airway manipulation. We successfully secured the patient airway using a supraglottic airway device (I-gel) in a patient who had previously diagnosed with Grisel's syndrome. The operation was successfully completed, and the patient recovered without any neurological complications. I-gel can be a good option for airway management during general anesthesia in a patient diagnosed with Grisel's syndrome.
