The Comparison of Risk-adjusted Mortality Rate between Korea and United States (한국과 미국 의료기관의 중증도 보정 사망률 비교)

  • The purpose of this study was to develop the risk-adjusted mortality model using Korean Hospital Discharge Injury data and US National Hospital Discharge Survey data and to suggest some ways to manage hospital mortality rates through comparison of Korea and United States Hospital Standardized Mortality Ratios(HSMR). This study used data mining techniques, decision tree and logistic regression, for developing Korea and United States risk-adjustment model of in-hospital mortality. By comparing Hospital Standardized Mortality Ratio(HSMR) with standardized variables, analysis shows the concrete differences between the two countries. While Korean Hospital Standardized Mortality Ratio(HSMR) is increasing every year(101.0 in 2006, 101.3 in 2007, 103.3 in 2008), HSMR appeared to be reduced in the United States(102.3 in 2006, 100.7 in 2007, 95.9 in 2008). Korean Hospital Standardized Mortality Ratios(HSMR) by hospital beds were higher than that of the United States. A two-aspect approach to management of hospital mortality rates is suggested; national and hospital levels. The government is to release Hospital Standardized Mortality Ratio(HSMR) of large hospitals and to offer consulting on effective hospital mortality management to small and medium hospitals.

Effect of Oral Health Care on Swallowing Function and Swallowing-Quality of Life in Patients with Dementia (치매환자의 구강관리가 삼킴기능과 구강관련 삶의 질에 미치는 영향)

  • The purpose of this study was to evaluate the status of oral health care in dementia patients living in long-term care facilities and to investigate the effects of this on swallowing function and swallowing-quality of life. The subjects were 60 demented patients who were admitted to long-term care facilities in Gwangju and Jeonnam provinces. Their Oral Health Assessment Tool(OHAT), Clinical Dysphagia Scale(CDS), and Oral Health Impact Profile 14(OHIP-14) were measured. The results of this study were as follows: First, the effect of oral health care on dentition patients using long-term care facilities on swallowing function and swallowing-quality of life was significantly different according to oral care score (p<.05)(p<.01). Second, the relationship between oral health care and swallowing function of dementia patients and swallowing-quality of life. was examined. As a result, oral health care of dementia patients was affected by swallowing function (r=.405, p<.01) the swallowing-quality of life (r=.462, p<.01) and swallowing function were significantly correlated with swallowing-quality of life (r=.351, p<.01). Therefore, the oral health care of users of long-term care facilities was closely related to swallowing function and swallowing-quality of life.

Influence of Illness Perception and Physiological Indicators on Self Management of Hemodialysis Patients (혈액투석환자의 질병지각과 생리적 지표가 자기관리에 미치는 영향)

  • This study provides basic data for nursing intervention to increase self management of hemodialysis patients by identifying the relation among their illness perception, physiological indicators, and self management. The participants were 134 patients receiving hemodialysis at a general hospital in Seoul. Data were collected by using a structured questionnaire and medical records. The collected data were analyzed by using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient and multiple regression analysis with the SPSS/WIN 23.0 program. The significant factors influencing self-management of hemodialysis patients have been identified with 8 variables. The first one is registration for kidney transplantation (β=-.20, p=.034). Among sub items of illness perception are consequence (β=-.20, p=.031), treatment control (β=.19, p=.040), and illness coherence (β=-.18, p=.049). In addition, among physiological indicators are hematocrit (β=.38, p<.001), hemoglobin (β=.29, p=.005), BUN (β=-.25, p=.010), and phosphorous (β=.22, p=.033). These variables explained 26.3% for self-management of hemodialysis patients. In order for hemodialysis patients to improve their self-management, a concrete nursing intervention improving the treatment control among illness perception as well as improving the understanding of physical indicators needs to be provided.

  • This study is a descriptive research study to analyze factors affecting patient safety management activities by identifying shared leadership, organizational communication, and nursing service perceived by nurses to prepare basic data for theoretical and practical information and intervention measures. This study collected data from 155 clinical nurses in C region in G-do from July 17 to July 28, 2023, and a total of 154 copies were finally analyzed. Using the SPSS Win. 25.0 program, technical statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and hierarchical multiple regression were analyzed. As a result of analyzing variables affecting the subject's patient safety management activities with multiple regression using hierarchical selection, the higher the shared leadership, the higher the patient safety management. In order to efficiently perform nursing for nurses' patient safety management activities, research should be continuously conducted to develop specific intervention programs that can support patient safety nursing activities and verify their effectiveness

A Study on Demand and the Supply for Home-based Cancer Patient Management Projects of Public Health Centers (보건소 재기 암환자 관리사업에 대한 환자의 요구도 및 제공정도)

  • Purpose: As a part of the analysis of home-based cancer patients management of public health renters in cities, counties and districts across the nation, this study is to understand the degree of patient demands for that management and the degree and scope of the supply for the patient's demand. Methods: Developed the questionnaire which was constituted of degree of demand and supply for home-based cancer patient management and analyzed data centering on the frequencies and percentages by utilizing SPSS WIN 12.0. Results: The services provided through the home-based cancer patients management project include physical, emotional, spiritual and education/informative services. A survey was conducted for home-based cancer patients about these services, and its result showed that the degree of demand and supply was highest for emotional service, followed by education/informative service, spiritual service and physical service in the order of the demand-supply degree. When main items for each service were examined, it was found that: in the case of physical service, pain control was provided murk lower than its demand, while excretion disorder control and individual hygiene is provided murk more than its demand. In the case of emotional service, the degree of demand was overall higher than that of supply; spiritual service was provided appropriately to the degree of demand. Conclusion: This study examines the home-based canter patients management project of public health centers and compares and analyzes the degree of demand for patient services and the degree of services that are actually provided. The findings could be used as based data for the development of effective programs in future on the basis of actual demands of home-based cancer patients.

  • 우리나라는 노령화사회로 진행되어 감에 따라 노인성 만성질환이 증가하는 추세이다. 특히 노인성 만성질환 중 전립선비대증 질환의 경우 지속적인 관리가 필요한 질환이다. 전립성비대중의 경우 생명에 큰 지장이 없으면서 주기적으로 자주 의사를 찾아야 하는 번거로움이 있고, 또한 진단 및 투약결정이 상대적으로 복잡하지 않은 질환이다. 이 논문에서는 모바일 단말기를 이용하여 환자가 자신의 관찰데이터를 병원에 전송하도록 하고, 병원의 환자관리시스템에서 분석을 통해 환자를 모니터링하면서 의사의 직접진찰 시기를 조정할 수 있도록 설계된 전립선만성질환 환자에 대한 관리시스템에 대해서 소개한다.

Development of a Smart Application for Protecting Dementia Patients (치매환자의 보호를 위한 스마트 앱 개발)

  • The applications for considering their position and daily patterns of dementia patients have been developed in an early stage even if the older patients who have weaker or serious symptoms has increased in various forms. In this paper, we develop an android-based application which displays positions and pathways of patients on maps and provide messages in the cases of dangerous situations. Guardians need to register schedules including safe areas and personal information of their patients. This system registers behavior status categorized as normal or abnormal each position which is sent to a database. In particular, the deviation status is assigned in case the patients are not within the safe areas that their guardians registered on their schedule. The wandering status is assigned in case the patients are repeatedly passed by their pathways. This smart application contains the modules such as patient position sending, guardian and patient information, patient schedule and safe zone registration, position and behavior status registration, pathway display and message sending, and rescue request. This system sends the notification and alarm service providing normal and abnormal behavior with deviation and wandering status of patients respectively.

  • 최근, 임상연구는 유비쿼터스 헬스케어 서비스를 이용한 환자 개인의 맞춤형 의료 서비스에 관한 연구가 진행되고 있다. 유비쿼터스 헬스케어 서비스 시스템은 원격의 환자를 유무선 네트워킹 기술을 활용하여 "언제 어디서나" 이용 가능한 의료서비스를 제공할 수 있다. 이러한 기술들 덕택에, 의료 서비스가 의료기관 내에서 실-생활 영역으로 공간적인 확대와 특정 질병치료에서 평생 건강관리로 시간적인 확대가 가능해졌다. 본 논문에서는 유비쿼터스 환경에서 환자와 전문의가 실시간으로 상호작용을 통해 최적의 의료서비스 제공할 수 있는 유비쿼터스 헬스케어 서비스 기반의 과민성 방광 환자 관리 시스템을 설계하고 구현하였다.
