This project was to investigate cerebrovascular disease incidence relation to alcohol consumption, cigarette smoking and family disease history in the family members of normal and cerebrovascular disease patients, besides patients themselves. Alcohol consumption in the family members of cerebrovascular disease patients was significantly higher, especially in male with thirties, forties and fifties, than those of normal. Mean of daily cigarette smoking in the family members of cerebrovascular disease patients was found to be significantly higher, especially in males with forties and fifties, 1]lan those of normal families. Mean of monthly income of normal subjects was significantly higher than those of cerebrovasndar disease families(normal: $673, 000\pm$ 187, 000 won, cerebrovascular: $483, 000\pm213, 000$ won). A degree to be attacked with cerebrovascular disease was very higher in the family members of cerebrovascular disease patients than normal family. Such cases were much higher in father side than in mother side.

Purpose: Undertreatment of canter pain, especially due to the differences in the perception of pain between the patients and caregivers, is a well recognized problem. The purpose of this study were to determine if there exist differences in communication about pain intensity scores between patients and their family caregivers in Korea. Methods: A total of 127 patient-family caregiver dyads who have experienced canter pain participated in this study at a hospital in Seoul for six months. The data were obtained by fare to face interview with a structured questionnaire based on Brief Pain Inventory-Korean version and other previous researches. The clinical information for all patients was compiled by reviewing their medical records. Results: Patients' 'worst-pain for 24-hour' and 'right-now-pain' scores estimated by family caregivers were significantly higher than those by patient themselves. The degree of agreement between patients and family caregivers in the estimate of patients' 'worst-pain for 24-hour' intensity categories was 78.7% for 'severe pain', 40% for 'no pain', 27.5% for 'mild pain' and 22.9% for 'moderate pain'. In case of 'right-now-pain' intensity categories, the agreement was 50% for 'severe pain', 47.2% for mild pain, 46.3% for 'no pain', and 26.3% for 'moderate pain'. Conclusion: This study demonstrates that the degree of agreement between patients and family caregivers in the estimate of patients 'pain intensity categories was less than 50% except for 'severe pain'. The results indicate that Korean family caregivers tend to overestimate the canter pain intensity of their caring patients, especially, when a lancer patient has 'moderate' or 'mild pain'. Health Providers are advised to educate patient-family caregiver dyads to use a pain measurement scale to promote their agreement in pain Intensity stores. Further analyses and studies are needed to identify the factors and differences that influence their communication about pain intensity scores between patients and their family caregivers.
The aim of this study was to identify the levels of family members' burden and to describe factors influencing on famiCX*//8ly caregiver's burden when they are planned to transfer to general ward from the intensive care units (ICU). A descriptive survey was used with a convenience sampling of 101 family members of ICU patients at S university hospital in C city, Korea from August to November, 2010. The data were analyzed with SPSS 18.0 program. The level of object burden in family members had statistically significant differences with level of consciousness, number of patients' monitoring devices, and tracheostomy tube. The level of subject burden had statistically significant differences according to patients' gender, patients' education level, and relationship with patients. Transfer anxiety was showed statistically positive correlation with objective and subjective burden. We found transfer anxiety was statistically significant predictor of family caregiver's objective burden with explanatory power 12.7%. Family members' subjective burden were statistically influenced by patients' gender and transfer anxiety. These factors explained 23.8% out of total variance of family members' objective burden. The structured individualized method of transfer is recommended with further research of ICU families to further examine the dimension of transfer anxiety and how it affects family members' burden and patient outcomes.
This review aimed to analyze characteristics and performances of patient and family-centered care interventions and evaluate the convergence effectiveness. Randomized controlled trials were searched, selected, data extracted and quality-assessed using the Risk of Bias in 15 databases. Characteristics suggested from 21 studies were provision of information, education, communication and family and friend participation. There were 89 measurement variables of performances. Patient outcome was measured by mortality, length of hospitalization, etc., indicated as significantly improved in 18 studies. This review has provided evidence that patient and family-centered care improved experience and performance of diverse patients, families and health-care providers. There is need to convergence adopt patient and family-centered care and conduct evidence-based studies for improvement of quality of healthcare and patient safety in the future.
In Korea surrogate medical decision makings happen without legal grounds. The purpose of this article is to research the issues in preparing policies for decision-making on behalf of unrepresented patients. As aspects of comparative law, there are two approaches. One of them is to regulate default surrogate list. If no agent or guardian has been appointed, some legislatures provide that members of patient's family who is reasonably available, in descending order of priority of not, may act as surrogate: (1) the spouse, unless legally separated; (2) an adult child; (3) a parent; or (4) an adult brother or sister. If none of them is eligible to act as surrogate, some legislatures allow close friends to make health-care decisions for adult individuals who lack capacity. On the other hand there are other legislatures which provide no surrogate decision maker list but oblige the responsible authority to determine with advice of family members or friends of the patient. In the end the first approach can not guarantee that the surrogate decision maker like family members or friends will determine in the best interest of the patient.
A comprison was made regarding perceived family support among patients with somatoform disorders, psychosomatic disorders and depressive disorders. The subjects included 49 patients with somatoform disorders, 43 patients with psychosomatic disorders, and 50 patients with depressive disorders. Perceived social support-family scale was used to measure the extent of family support. The patients with somatoform disorders were significantly lower in family support than the patients with psychosomatic disorders. However, no significant differences were found between patients with somatoform disorders and those with depressive disorders, as well as between patients with psychosomatic disorders and those with depressive disorders. Patients with older age had significantly higher scores on family support than those with younger age. Married patients were significantly higher in family support than unmarried ones. These results suggest that low family support may be associated with either the etiology or the sequelae of somatization. Thus, it is emphasized that the role of family support is essential in evaluation and treatment of somatization. In addition, longitudinal studies will be required to investigate the causative role of low family support in somatization.
Objectives : To evaluate the relation of familial history of alcoholism and plasma level of ${\beta}$-endorphin, ethanol, ${\beta}$-endorphin, cortisol and blood glucose were compared in 48 male alcoholics and 29 normal controls. Methods : Subjects are divided into two groups by family history of alcoholism. Blood samples were obtained before and after 0.75mg/kg of ethanol consumption at 7th admission day. Results : 1) The ratio of family history positive to negative of the patient group was 2 to 1. 2) The age at admission of positive family history group was younger than negative group. 3) There was no significant difference in change of plasma ethanol level among three groups. 4) There was no significant difference in change of plasma ${\beta}$-endorphin level among three groups. 5) There was no significant difference in change of plasma cortisol level among three groups. 6) There was no significant difference in change of fasting blood sugar level between two patient groups.
Purpose: The purpose of this study was to compare the need for hospice care between families of children and adults with cancer. Methods: The data was collected from 190 families of children and adult with cancer using self-rating questionnaires from December, 2004 to February, 2005. Data was analyzed using SPSS/Win program by Mean, SD and t-test. Results: The mean score of the need for hospice care in families of children with cancer was greater significantly than in families of adults with cancer (t=-2.126, P=.035). The scores of two factors among the five factors evaluated for the need for hospice care were different significantly. The mean score of control of major terminal physical symptoms' in families of children with cancer was greater significantly than the mean score of adults with cancer (t=-2.165, P=.032). The mean score of 'spiritual care to prepare for death' in families of adults with cancer was greater significantly than the mean score of children with cancer (t=-2.380, P=.018). Conclusion: For improving the quality of life for both patients and families, the hospice service program needs to consider the life cycle of patients.

This study was conducted to identify families' perception and attitude toward application of physical restraint(PR) in general neurological wards. The study is based on subjects from 70 family members who have cared patients applying PR. Data was collected using self-report questionnaires and reviewing of patients' medical records. 68.6% of subjects were female and the mean age was 47.9 years old. Family members were either sons, daughters or spouses. The questionnaires analyzed families' perceptions based on a score of 5, the results showed a mean score of 4.2, and items related to maintaining therapeutic tools scored the highest. The application of PR with two hands was perceived by families as more important than PR applied with one hand. In families' attitude toward PR, there was general agreement that families had the right to decide to apply and remove PR. Continuous education and information regarding PR is required to support families.
