Effects of Galantamine Treatment on Attention, Activities of Daily Living, and Neuropsychiatric Symptoms between the Patients with Pure Alzheimer's Disease and Mixed Dementia (갈란타민(Galantamine) 투여 후 순수 알츠하이머병 치매와 혼합성 치매 환자의 주의력, 일상생활능력 및 신경정신행동 증상에 대한 효과 평가)

  • Objectives : The purpose of this study was to compare the efficacy of galantamine treatment, especially attention ability between patients with pure Alzheimer's disease (AD) and Mixed dementia (MD) during a 24-week trial. Methods : A total of 40 patients were recruited for this 24-week study. The effect of galantamine on attention was measured using Seoul Computerized NeuroCognitive Function Test (SCNT) and frontal functions test of Seoul Neuropsychological Screening Battery (SNSB). Patients'activities of daily living using the Seoul-Activities of Daily Living (S-ADL) and the Seoul-Instrumental Activities of Daily Living (S-IADL) ; behavioral symptoms using the Korean version Neuropsychiatric Inventory (K-NPI) were measured at baseline and 24-week. Results : 17 pure AD patients and 23 MD patients were analyzed in this study. Attention as measured by SCNT was not significantly different from baseline after 24 weeks of treatment in both groups. There was no significant difference between two groups in mean change from baseline in the SCNT, S-ADL, S-IADL and K-NPI scores at 24-week. Conclusion : Galantamine showed a therapeutic effect on cognition, activities of daily living, neuropsychiatric symptoms in pure AD and MD. Furthermore, Galantamine may specifically help to maintain attention and it may have positive effects on other cognitive and functional abilities.

Therapeutic Effect of the Mixed Extract of Panax ginseng C.A. Mey. and Chaenomeles sinensis Koehne on the Injury of Brain Tissue in the Mice by Alzheimer's Disease (Alzheimer성 치매 유발 생쥐의 뇌조직 손상에 대한 인삼, 목과 혼합추출액의 치료 효과)

  • This study was conducted to investigate the effect of the mixed extract of P. ginseng C.A. Mey. and C. sinensis K. (Gin-CHF) on the infarction area of hippocampus in the mice with Alzheimer's disease induced by ${\beta}-amyloid({\beta}A)$. The Gin-CHF extract reduced the infarction area of hippocampus, and controlled the injury of brain tissue in the mice with Alzheimer's disease induced by ${\beta}A$. The Gin-CHF extract reduced the Tau protein, GFAP protein, and presenilin1/presenilin2 protein (immunohistochemistry) of hippocampus in the mice with Alzheimer's disease induced by ${\beta}A$. These results suggest that the Gin-CHF extract may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the Gin-CHF extract for Alzheimer's disease is suggested for further research.

  • In the present study, we assessed the effecs of fruit and vegetable bark resources rich in beneficial substances on the learning ability and memory improvement. It functions to inhibit the activity of acetylcholinesterase in brain tissue and improve cognitive functions in a simulation model of cognitive impairment induced by scopolamine. Hence, it can promote memory and learning ability. The formulation of the fruit and vegetable bark uses the extract of fruit and vegetable peels, which has long been used without causing toxicity in a wide range of food applications. Therefore, it can be used safely without a risk of side effects, even in the case of a long-term administration for the preventive purpose. Furthermore, the present study is a very beneficial study in the environment-friendly aspect in association with recycling of resources as it is based on the novel efficacies of fruit and vegetable barks which have been conventionally disposed as a refuse of fruits due to their poor sensory qualities despite the content of beneficial substances.

Inhibitory Effect of Bee Venom Herbal Acupuncture Solution on Acetylcholinesterase in PC 12 Cells (PC12 세포에서 봉독약침액의 Acetylcholinesterase 억제효과)

  • 목적 : Acetylcholine은 콜린과 아세트산의 에스테르로 인체에서 중요한 신경전달물질로 Acetylcholine-sterase(AChE)라는 효소에 의해 분해된다. Alzheimer's disease, ataxia, myasthenia gravis, Parkinson' disease 등의 질환에 AChE 억제제가 사용되어 왔으며 최근 한약재의 AChE 억제 효능에 관한 연구들도 진행되고 있다. 봉독은 관절염, 통풍 등의 질환에 응용되어 왔으며 진통효과 및 항염증작용에 대한 임상적, 실험적 연구가 많이 보고되어 왔으나 AChE 억제효과에 대한 연구는 아직까지 보고된 바 없다, 본 연구에서는 봉독약침액과 봉독의 과민반응 유발항원 중 하나인 Phospholipase A2 억제효능이 있는 것으로 알려진 상백피를 혼합한 상백피봉독약침액의 AChE 억제효과를 알아보았다. 방법 : PC12 세포주에서 추출한 AChE와 0.1, 0.01 and $0.001mg/m{\ell}$ 농도의 봉독약침액 및 상백피봉독약침액을 60분간 반응시켰다. 효소면역측정법(ELISA)을 이용하여 흡광도를 10분, 30분, 60분 경과시 각각 측정한 후 효소활성저해도(%)를 계산하였다. 효소활성저해도(%) = [(Cc - Ce)/Cc] ${\times}$ 100 Cc : 대조군 흡광도, Ce : 실험군 흡광도 결과 : 1. 봉독약침액은 0.1, 0.01, $0.001m{\ell}/mg$의 농도에서 30분 경과 후부터 유의성 있는 억제효과를 나타내었다. 2. 상백피봉독약침액은 $0.1m{\ell}/mg$ 농도에서 10분 경과 후부터 유의성 있는 억제효과를 나타내었고, $0.01m{\ell}/mg$ 농도에서 30분경과 시 유의성 있는 억제효과를 나타내었다. 3. 봉독약침액과 상백피봉독약침액의 AChE 억제효과 비교에서 봉독약침액의 억제효과가 상백피봉독약침액 보다 뛰어났다. 요약 : 봉독약침액과 상백피봉독약침액의 AChE 억제효과를 확인하여 두 군 모두 유의성 있는 결과를 얻을 수 있었다. 앞으로 알츠하이머병이나 치매와 같은 신경퇴행성 질환에 대한 봉독의 임상적 활용 및 보다 넓은 범위에 대한 연구가 필요할 것이라 사료된다.

Association Between Psychiatric Medications and Urinary Incontinence (정신과 약물과 요실금의 연관성)

  • Urinary incontinence (UI), affecting 3%-11% of males and 25%-45% of females globally, is expected to rise with an aging population. It significantly impacts mental health, causing depression, stress, and reduced quality of life. UI can exacerbate psychiatric conditions, affecting treatment compliance and effectiveness. It is categorized into transient and chronic types. Transient UI, often reversible, is caused by factors summarized in the acronym DIAPPERS: Delirium, Infection, Atrophic urethritis/vaginitis, Psychological disorders, Pharmaceuticals, Excess urine output, Restricted mobility, Stool impaction. Chronic UI includes stress, urge, mixed, overflow, functional, and persistent incontinence. Drug-induced UI, a transient form, is frequently seen in psychiatric treatment. Antipsychotics, antidepressants, and other psychiatric medications can cause UI through various mechanisms like affecting bladder muscle tone, altering nerve reflexes, and inducing other conditions like diabetes or epilepsy. Specific drugs like lithium and valproic acid have also been linked to UI, though mechanisms are not always clear. Managing UI in psychiatric patients requires careful monitoring of urinary symptoms and judicious medication management. If a drug is identified as the cause, options include discontinuing, reducing, or adjusting the dosage. In cases where medication continuation is necessary, additional treatments like desmopressin, oxybutynin, trihexyphenidyl, or amitriptyline may be considered.
