Three Cases of Acute Benign Paroxysmal Positional Vertigo Improved after Korean Medicine Treatment (한방 치료 후 호전된 급성기 양성 발작성 두위성 현훈 치험 3례)

  • Objectives : The purpose of this study is to report three cases of Korean medical treatment for acute benign paroxysmal positional vertigo(BPPV). Methods : This study was conducted with three acute benign paroxysmal positional vertigo patients who hospit alized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean medical Hospital. Three patients were treated with Korean medical treatment including herbal medicine(Banhabaeckchulchunma-tang), acupuncture, p harmacopuncture(Hwangryunhaedok-tang). After treatment, we evaluated subjective symptoms. Results : The results of three patient showed remarkable improvements on subjective symptoms after the treatment. Conclusions : It is considered that Korean medical treatment can be applied to the treatment of BPPV.

Banhabaeckchulchunma-tang for Benign Paroxysmal Positional Vertigo: A Systematic Review and Meta-analysis (양성 발작성 자세변환성 현훈에 대한 반하백출천마탕(半夏白朮天麻湯)의 효과: 체계적 문헌고찰과 메타분석)

  • Objectives : The purpose of this study is to evaluate the effect of Banhabaeckchulchunm a-tang(BBCT) for Benign Paroxysmal Positional Vertigo(BPPV) Methods : We searched randomized controlled trials(RCTs) which assess the effect of BB CT for BPPV through 8 electronic databases from their inception to July 2021. RevMan 5.4 was used to evaluate the risk of bias. Results : 12 RCTs with 901 subjects were included. The BBCT treatment group had significantly higher total effetive rate(TER) than the western medicine treatment group(P=0.0001), and the Dizziness Handicap Inventory(DHI) score(P=0.003), traditional chinese medicine syndrome(TCM syn.) score(P<0.00001), and Visual analog scale(VAS)(P=0.0006) were significantly lower than the western medicine treatment group. The combined treatment of BBCT and canalith repositioning procedure(CRP) group had significantly higher TER than only CRP treatment group(P=0.02), and there was no significant difference in DHI score(P=0.12). TG(P=0.006) and TC(P=0.04) were significantly lower, and ApoA1 was significantly higher(P=0.0001). There was no significant difference in LDL(P=0.24). Conclusions : These results demonstrate that BBCT could be effective for BPPV especially after CRP. However, because of limits of included studies such as high heterogeneity between the literatures, unclear risk of bias, insufficient reports of adverse events(AEs), a well-designed RCTs with a low risk of bias in more diverse countries are needed in the future.

Patients Treated with Acupuncture on Splenius Muscle and Combined Korean Medicine for Acute Dizziness Caused by Whiplash Injury: Three Case Reports (현훈을 동반한 편타성 손상 환자에 대한 판상근 아시혈 침치료 중심의 한방복합치료 치험 3례)

  • Objective: The purpose of this study was to report the effect of acupuncture on the splenius muscle and the effect of combined Korean medicine treatment on acute dizziness in patients due to whiplash injury. Methods: Three patients with acute dizziness were treated with acupuncture of the splenius muscle and combined Korean medicine. A numeric rating scale (NRS) and the Korean dizziness handicap inventory (K-DHI) were checked every day from admission to discharge to confirm symptom improvements. Results: In the acute stage of dizziness caused by whiplash injury, acupuncture of the splenius muscle and combined Korean medicine improved the NRS and K-DHI scores. Conclusions: For patients with acute dizziness caused by whiplash injury, a combination of acupuncture on the splenius muscle and Korean medicine treatment may be a solution.

Treatment of Dizziness with Polycythemia Vera by Korean Medicine: Case Report (한방치료로 호전된 진성적혈구증가증으로 환자의 현훈 치험 1례)

  • Objectives: Patients with polycythemia vera can experience many symptoms, including dizziness, headache, and fatigue, caused by a high red blood cell mass and blood viscosity. Some symptoms persist even after appropriate treatment. The purpose of this study was to examine the effect of Korean medicine on dizziness following polycythemia vera treatment. Methods: A patient diagnosed with polycythemia vera was treated with Korean medicine, including pharmacopuncture, acupuncture, and herbal medicine. We used a numeral rating scale (NRS) and the Dizziness Handicap Inventory (DHI) to observe the effects of the treatment. Results: After treatment, the hematocrit and hemoglobin increased to within normal range, and the NRS scores for dizziness and fatigue, subjective discomfort, and DHI scores decreased compared with the preadmission values. Conclusions: This report shows that Korean medicine helped to relieve dizziness in a patient with polycythemia vera. However, we need further studies to establish treatment regimens for polycythemia vera symptoms.

Electro-Acupuncture for Cervical Vertigo: A Systematic Review and Meta-analysis (경추성 현훈에 대한 전침 치료: 체계적 문헌고찰 및 메타분석)

  • Objectives The objective of this study was to appraise the effect of electro-acupuncture (EA) for the treatment of cervical vertigo. Methods We performed a literature search using nine electronic databases up to the end of November 2020. We included randomized controlled trials (RCTs) which appraised the effect of EA in the treatment of cervical vertigo. The risk of bias was evaluated from the Cochrane risk of bias tool. Results Thirteen appropriate RCTs were included and analyzed. The efficacy rate of the EA group was statistically higher compared with the control group treated with western medicine (p<0.00001). EA treatment significantly improved efficacy rate when combined with western medicine than western medicine alone (p<0.0001). Also EA used with combined therapy (traction, traditional chinese medicine, injection) significantly improved efficacy rate compared with combined therapy alone (p=0.02). Meta-analysis of thirteen studies demonstrated positive results for using EA for the treatment of cervical vertigo. Conclusions Based on results, EA treatment could be effective in cervical vertigo. However, it should be noted that the studies included in this research are regionally biased and small in number. Further well-designed research should be required.

A Case Report of Osuyubujaijung-tang on the Recurrence of Breast Cancer in a Patient Complaining of Coldness and Dizziness (유방암 재발 환자의 장궐 및 현훈이 오수유부자이중탕으로 호전된 치험 1례)

  • A 63-year-old female patient was admitted to the hospital with complaints of coldness in the head and right femur, persistent vertigo, dry mouth, vexation, general weakness, anorexia, and hot flashes. After being diagnosed with left-sided ductal carcinoma in 2017 and undergoing partial resection, she was recommended radiotherapy, chemotherapy, and hormone therapy, but she refused and did not receive any treatment. At a checkup in November 2022, the left-sided invasive ductal carcinoma recurred. She underwent partial resection and was scheduled to receive radiotherapy two months later. During hospitalization, the patient received traditional Korean medicine treatment, including Korean herbal medicine (Osuyubujaijung-tang), acupuncture, moxibustion, and cupping. Subjective symptom changes were evaluated daily. At discharge, the patient's symptoms improved, and her condition varied with the presence of Aconitum carmichaelii. This study suggests that "Osuyubujaijung-tang" cared the coldness and vertigo caused by "Han-dam (寒痰)" in patients with Soeumin visceral syncope pattern (少陰人 臟厥症) and that A. carmichaelii played a major role in this outcome.

A Case Report on Cervicogenic Dizziness and Headache Treated with Korean Medicine in a Hospitalized Patient (한방병원에 입원한 경추성 현훈 및 두통 환자에 대한 한방 치료 1례)

  • Objective: The purpose of this study is to report the efficacy of using Korean medicine treatment with cervicogenic dizziness and headache. Methods: We used Korean medicine treatments (herbal medicines, acupuncture, pharmaco-acupuncture, and Chuna) to treat a hospitalized patient with cervicogenic dizziness and headache. The treatments were assessed using the Korean Dizziness Handicap Inventory (K-DHI) to evaluate the degree of dizziness. The Numerical Rating Scale (NRS) was used to evaluate headaches and post-traumatic neck pain. General health status was evaluated using the European Quality of Life Five Dimensions (EQ-5d) scale. Results: The patient's NRS and EQ-5d scores improved after treatment. The change in the K-DHI scores showed that the patient's cervicogenic dizziness symptoms were relieved. Conclusions: The results of this study suggest that Korean medicine therapies may be effective for treating cervicogenic dizziness and headaches.

A Study on Anxiety in College Freshmen (불안척도에 의한 대학신입생의 정신건강 평가)

  • The authors studied anxiety, using Zung's Self-Rating Anxiety Scale(SAS), in the subjects of 3,499 male and 1,335 female college freshmen of Yeungnam University. The authors collected the reports of SAS during the periods from January to February, 1986, and applied ANOVA and t-test on anxiety scores in order to compare them between various psychosocial factors, and sexes. The results are as follows: There was significant difference in the mean averages of total anxiety scores between male and female students: male students scored $32.91{\pm}7.70$, female students scored $34.48{\pm}6.00$, (P<0.001). The anxiety scores relating to the items of sweating, apprehension, restlessness, and insommia were relatively higher in both groups. The anxiety scores relating to the items of faintness, mental disintegration, tremors, dizziness were lower in both groups. Thirty-nine male students(1.1%) showed seriously high anxiety scores of 50 or higher, while twenty-one female students(1.6%) showed the same scores. So the authors inferred that the features of anxiety symptoms were much the same in our country, but female showed more various symptoms and higher level of anxiety than males. Male students attending in pharmacy showed higher level of anxiety scores(P<0.01). There was a strong tendency toward higher anxiety scores in the students who were dissatisfied with their home atmosphere, colleges, department and familiarity of parents, and those who had pessimistic views of self image in the past, present, or future, in both groups(P<0.001).

Osteogenic Differentiation of Bone Marrow Stem Cells Using Thermo-Sensitive Hydrogels (온도감응성 수화젤을 이용한 골수간엽줄기세포의 골분화 유도)

  • Poly (ethylene glycol)-based diblock and triblock thermo- sensitive polyester copolymers were investigated for application on tissue engineering and injectable biomaterials in drug delivery system due to their nontoxicity, biocompatibility and biodegradability. We synthesized the diblock copolymers consisting of methoxy poly (ethylene glycol) (MPEG) (Mn=750 g/mole) and poly $(\varepsilon-caprolactone)$ (PCL) by ring opening polymerization of $\varepsilon-CL$ with MPEG as an initiator in the presence of HCl $Et_2O$. The effect of diblock copolymers on in vivo osteogenic differentiation of rat bone marrow stromal cells (BMSCS) with and without the presence of osteogenic supplements (dexamethasone) was investigated. Thin sections were cut from paraffin embedded tissues and histological sections were stained by H&E, von Kossa, and immunohistochemical staining for osteocalcin. In conclusion, dexamethasone containing thermo- sensitive hydrogel might be improved osteogenic differentiation of BMSCs. We expect the osteoinduction effect to be excellent when it uses stem cell or other osteogenic materials.

Clinical Study of Blood Flow Velocity Using Transcranial Doppler Ultrasound on Cerebral Arteries in Dizziness Patients (경두개 도플러 초음파를 이용한 현훈 환자의 뇌혈관 혈류 측정에 관한 임상적 연구 (중대뇌동맥, 추골동맥 및 기저동맥을 중심으로))

  • Objectives : This study was designed to research whether transcranial doppler ultrasound(TCD) can yield a suitable diagnosis or prognosis for dizziness. Methods : To evaluate the blood flow, we measured the mean velocity(Vm), peak systolic velocity(Vs) and pulsatility index(PI) of the middle cerebral artery(MCA), vertebral artery(VA) and basilar artery(BA) in 54 dizziness patients and 44 normal subjects. To investigate the difference between dizziness patients and normal subjects, we selected 54 dizziness patients diagnosed normal by Brain MRI, and 44 normal subjects who did not have any symptoms of hyperlipemia, hypertension, diabetes melitus, heart disease, or smoking, diagnosed normal by Brain MRI. 30-69 year olds were divided into 30-49 year olds, 50-59 year olds and 60-69 year olds. Results : In normal subjects, there was a significantly higher Vm, Vs in the MCA and BA, and Vs in the VA of female subjects, and there was a significant difference in PI in the MCA and BA increasing with age. In comparison with normal subjects, dizziness patients had a significantly lower Vm in the BA. Male dizziness patients had significantly lower PI in the MCA. 30-49 year olds had significantly lower Vm in the BA. Conclusions : The significant difference in blood flow velocities in the BA between dizziness patients and normal subjects suggests that dizziness is related to blocking of blood flow in the BA and that diagnosis and prognosis can be made through TCD. More detailed study of dizziness patients will be needed.
