Factors Influencing Medication Adherence in Patients with Hypertension: Based on the 2008 Korean National Health and Nutrition Examination Survey (고혈압 환자의 약물복용 이행에 영향을 미치는 요인: 2008년 국민건강영양조사를 이용하여)

  • Purpose: The purpose of this study is to examine factors influencing medication adherence in patients with hypertension. Methods: This study carried out a secondary analysis of data from the 2008 Korean National Health and Nutrition Examination Survey (KNHANES). Stratified sampling was used to select a participant sample that was representative of patients with hypertension throughout the country. Using the SPSS/WIN 18.0 program, data were analyzed using descriptive statistics, $x^2$ test, t-test, and logistic regression. Results: Of the patients with hypertension, 8.8% had showed non-adherence to medication. Medication adherence was associated with age, spouse, Medicare insurance, number of other diseases, and current smoking status. The cases with older age, a spouse, Medicare insurance, higher number of other diseases, and no current smoking status showed significantly high medication adherence. Conclusion: Nursing interventions and further studies are needed to achieve high levels of medication adherence based on factors influencing medication adherence such as age, spouse, Medicare insurance, number of other disease, and current smoking status.

The Effect of the Weight of a Sandbag on the Sheath Region after a Transcatheter Arterial Chemoembolization (간동맥 화학색전술 후 시술부위의 모래주머니 적용무게에 따른 효과)

  • Purpose: This study aimed to investigate the differences in exudate and bleeding incidence and the changes in back pain and discomfort based on the weight of a sand bag applied to the femoral puncture site after hepatic transcatheter arterial chemoembolization. Methods: This quasi-experimental study comprised 82 patients randomly divided into three different groups. Experimental group 1 patients had a 600g sandbag, experimental group 2 patients an 800g sandbag, and control group patients a 1900g sandbag, on femoral access sites post procedure. The three groups of patients were assessed on level of exudate and bleeding and asked about back pain and discomfort at 30, 60, 120, 180 and 240 minutes after the procedure. Results: There were no significant differences in exudate and bleeding between the three groups before and after application of the sandbag post procedure. Both back pain and discomfort were significantly lower in the experimental groups than in the control group. Conclusion: There was no significant effect on bleeding and exudation due to the weight of the sandbag. The lowest level of back pain and discomfort was found in the group with the lightest weight (600 g). Therefore, the use of lighter-weight sandbags to prevent post procedure vascular complications is proposed.

Chartacteristics of Water-bottom Reflection Coefficients in Bransfield Strait, Antarctic Peninsula (남극 브랜스필드 해협의 해저면 반사계수 특성)

  • Reflection coefficients of the seafloor have been calculated from the amplitude ratio of secondary to primary water bottom reflection in seismic data obtained from Bransfield Strait, Antarctic Peninsula. Test processing for the coefficients shows that moving average is effective to reduce severe fluctuation of the coefficient measured at each point. Relationship between the coefficients and the properties of water bottom is analyzed to illuminate geological environment. In the central Bransfield Basin, the magnitude of reflection coefficients decreases as it is distant from the sedimentary sources. Reflection coefficients range from 0.12 to 0.2 near the continental slope of the basin, and from 0.1 to 0.12 in the basin floor. In the western Bransfield basin, reflection coefficients between 0.2 to 0.3 are obtained from the area eroded by glacial movement. On the volcanic structures near Deception Island, the coefficients show relatively high values more than 0.2. Paleo-geological structures uplifted by tectonic movement and outcropped by glacial erosion have relatively high coefficients.

  • An unrecorded variety, Viola betonicifolia var. albescens (Nakai) F. Maek. & T. Hashim., was reported from Jeonju, Imsil, and Jinan in Jeollabukdo province, Korea. Viola betonicifolia var. albescens shares several characteristics with its related species V. patrinii Ging. (i.e., white flowers, long triangular blades, winged petioles, green pedicels, and a rounded sepal appendage), but is distinct from V. patrinii, which has a glabrous winged petiole, colorless upper petals, glabrous upper petals, and higher elevation habitats, in that it has a pubescent winged petiole, striate-colored upper petals, bearded upper petals, and lower elevation habitats. It is also distinguished from V. lactiflora Nakai, which has glabrous wingless petioles, red pedicels, and a toothed sepal appendage.

Effects of Green and Black Korean Teas on Lipid Metabolism in Diet-Induced Hyperlipidemic Rats (한국산 녹차와 홍차가 고지혈증 유도 쥐에 있어서 혈청 지질 대사에 미치는 영향)

  • The study investigated the serum lipid metabolism and enzyme activities of Korean teas for their preventative activity against chronic disease and obesity. Male Sprague-Dawley rats were raised for 8 weeks on four experimental diets: normal diet, hyperlipidemic diet, and hyperlipidemic diet to which green and black teas (2% each) were added. Various biological actions, including lipid metabolism and enzyme activities of the serum, were investigated. Diet-induced, hyperlipidemic rats fed with green and black teas, showed significant decrease in food efficiency ratio, triglyceride, total lipid, and phospholipid compared to control, i.e. the normal and diet-induced, hyperlipidemic rats. Total cholesterol, LDL-cholesterol, Al(atherogenic index), LHR, VLDL-cholesterol, ester-cholesterol, and free-cholesterol also showed a significant decrease. However, there was no significant difference between the tea-fed, diet-induced, hyperlipidemic dieted groups. HDL-cholesterol concentration was increased significantly in the tea-dieted and normal groups compared to the control. There was a little difference in lipase activity between the normal and control groups, although green and black tea-dieted experimental groups were both increased compared to the control. The contents of total lipid, triglycerides, and total cholesterol were decreased in the normal and experimental groups compared to the control. The GOT, GPT, ALP and LDH serum enzyme activities of the experimental groups were significantly reduced compared to those of the control groups.

Preparation and Characterization of Solid Dispersions of Eprosartan with Hydrophilic Polymers (친수성 고분자를 이용한 에프로살탄 고체분산체의 제조 및 특성 분석)

  • In this study, we developed and optimized hydrophilic polymer based solid dispersion formulations (SDs) for enhancing the aqueous solubility of eprosartan, one of poorly soluble drugs, that has been broadly used for the treatment of high blood pressure. Poly(ethylene glycol) (PEG) and poly(vinyl pyrrolidone) (PVP) based SDs were prepared by hot melting and solvent evaporation methods and the drug/polymer composition varied in the range of 1:1~1:5 with or without poloxamer 407 (P407) as a polymeric surfactant. The SDs prepared by solvent evaporation showed more reduced crystallinity than ones by hot melting, and PVP based SDs showed more enhanced solubility and lower crystallinity than PEG based SDs. Furthermore, it was observed from DSC and PXRD analysis that the SDs with P407 (drug:polymer: P407 = 1:5:1) demonstrated no crystallinity and the most enhanced solubility (more than 3~4 times).

Effect of Fermentation Conditions on the Production of Lovastatin by Aspergillus terreus (Aspergillus terreus의 발효조건이 lovastatin 생산에 미치는 영향)

  • The biosynthesis of lovastatin, a cholesterol lowering agent formed by the filamentous fungus Aspergillus terreus, was examined in a 2.5 L jar fermenter. In batch bioreactor cultures conducted at various agitation rates, 400 rpm showed the best result in terms of lovastatin production. Notably, the effect of pH on lovastatin biosynthesis was found to be significant: when the pH was controlled at around 5.8 during the whole fermentation period, lovastatin concentration reached 598 mg/L, which is much hihger than the amounts obtained by pH-uncontrolled and pH 7.4-controlled fermentations. In addition, both L-histidine and L-tryptophan were observed to be favorable amino acids for the enhancement of lovastatin production when 6 g/L of the respective amino acids were supplemented at the beginning of the fermentation period. By further optimization of the production media and the physical environment, lovastatin production was increased to 836 mg/L (3.5 mg/L/hr) which is approximately 10 times higher than the productivity of the basic control culture.

  • The roots of Paeonia lactiflora (PL) has been traditionally used as analgesic, spasmolytic and tonic in Korea, China, and Japan. As part of a search for herbal medicine to treat diabetes and obesity, we confirmed hypoglycemic effect of PL through high fat diet-induced obese and diabetic mice experiments in vivo. Treatment of ethanolic extract from PL led to a significant decrease in glucose level, which is comparable to that of an antidiabetic drug metformin. In addition, PL selectively stimulates the transcriptional activities of both peroxisome proliferator-activated receptor $(PPAR){\alpha}$ and ${\gamma}$, and inhibits enzymatic activity of protein tyrosine phosphatase 1B (PTP1B), which are predicted to be therapeutic target in treatment of type2 diabetes and obesity. Especially, the n-hexane fraction (Hx) from PL ethanol extract showed more potent activities on $PPAR{\alpha}$ and than others and exihibited moderate inhibitory activity against PTP1B.

  • In-situ $CaCO_3$ formation onto recycled wood pulp was studied to improve optical properties and ash attachment to the fiber furnish in papermaking. We controlled initial reaction temperature of in-situ $CaCO_3$ formation method from $30^{\circ}C$ to $50^{\circ}C$. It was found that the attachment of newly formed $CaCO_3$ to recycled fibers, old newspaper (ONP) in this case, was stronger than that of ground calcium carbonate (GCC, mean dia. $2.4{\mu}m$) addition case, but was not much different among those formed at different temperature. Morphologies of newly formed $CaCO_3$ were changed according to the reaction temperature. More aragonite shape was seen at higher temperature. In-situ $CaCO_3$ formation increased brightness and lowered ERIC value of ONP sheet greatly at the same level of ash contents when compared to GCC addition method, but gave equivalent ERIC and brightness when compared to those of the precipitated calcium carbonate (PCC) addition method. However, tensile strength of the handsheets of the in-situ $CaCO_3$ formation method were much greater than those of the PCC addition method.

  • In-situ $CaCO_3$ formation on recycled wood pulp was studied to improve optical property and filler attachment to the fiber furnish in papermaking. We tried to attach calcium oxide (CaO) to the recycled fibers, old newspaper (ONP) in this case, by using selected polymers before in-situ $CaCO_3$ formation reaction on fibers, and then, $CO_2$ was injected to the furnish until all the CaO on fiber surfaces was consumed. It was found that the attachment of newly formed $CaCO_3$ to recycled fibers became stronger by attaching CaO to the fibers before in-situ $CaCO_3$ formation reaction. It was expected that the polymers used for the attachment of calcium source to the fiber furnishes helped to keep the newly formed $CaCO_3$ strongly attached to the fiber surface as well as to retain the impurities associated with calcium source and recycled fibers, if any. In-situ $CaCO_3$ formation gave higher brightness and much less ERIC value in ONP sheet than the case when the equivalent amount of GCC was added to the furnish.
