Comparison of Spine and Lower Extremities Range of Motion Between in Elderly Fallers and Nonfallers (넘어지는 노인과 안 넘어지는 노인의 척추와 하지의 관절가동범위의 비교)

  • 이 연구의 목적은 노인들 중, 넘어지는 사람들(fallers)과 안 넘어지는 사람들(nonfallers)의 경부, 체간, 하지의 관절가동범위를 측정하여 비교하는 것이다. 본 연구대상은 남자 6명, 여자 22명, 나이 65세에서 88세의 노인으로 독립적으로 활동할 수 있는 사람들을 두 개의 집단으로 한 사람씩 면접을 통해 분류하였다. 대상자 선정 이전 일년 동안 한번 이상 넘어진 경험이 있는 사람들을 넘어진 사람들로 규정, 그리고 넘어진 경험이 없는 사람들을 안 넘어진 사람들로 정의하였다. 관절가동범위는 각도기로 측정하였고 미국 정형외과 학회(American Academy of Orthopaedic Surgeons)의 표준화된 가동 범위 측정방법을 사용하였다. 넘어진 사람들과 안 넘어진 사람들의 연구 결과의 차이를 알아보기 위하여 양측 독립성 t-검정을 사용했다. 연구결과, 넘어진 사람들과 안 넘어진 사람들간의 관절가동범위 중 흉요부의 굴곡, 신전, 우측굴곡, 좌측굴곡, 그리고 우측회전과 좌측회전, 고관절부의 신전, 내전, 외측회전과 내측회전, 그리고 족관절의 배굴에서 유의한 차이가 있었다. 이와 같은 결과는 체간, 고관절부 그리고 발목의 유연성이 노인들의 균형을 유지하는데 영향을 준다고 말할 수 있다.

The Effects of Range of Motion of Lower Limb on Gait time of Height of High Heeled Shoes in Gait (보행 시 하이힐 굽 높이에 따라 보행시간이 하지관절 가동범위에 미치는 영향)

  • The purpose of the study was to compare the differences among phases accoring to the gait time on the heel height during gait, investigate the effect on ROM of the lower limb on gait time. Ten female college students in their 20s participated in the study, and variables were calculated through 3D gait analysis on height of heel. As a statistical method, one-way ANOVA was performed for the differences between the three heel heights, and multiple regression analysis was performed to determine the effect of gait time on the ROM. As a result of the study, phase 2, the higher the heel, the longer the gait time, but phase 3, the higher the heel, the shorter the gait time. As a result of analyzing the effect of gait time on the ROM of the lower limb, in phase 2, the greater the ROM for the ankle and knee joint in 1 cm, and for the ankle joint in 5 cm, the longer the gait time. In phase 3, the greater the ROM for the hip joint in 1 cm, the longer the gait time, and the smaller the ROM for the ankle joint in 10 cm, the longer the gait time. Therefore, in the case of high-heeled shoes, it is suggested that the control of the ankle joint is important.

The Effects of Electrical Stimulation Method on Muscle Strength, Pain, Range of Motion, Fear Avoidance-Belief in Patients with Anterior Cruciate Ligament Reconstruction (전기자극방법이 앞십자인대 재건술환자의 근력과 통증, 관절가동범위 및 공포-회피반응에 미치는 영향)

  • The purpose of this study was to investigate the effect of neuromuscular electrical stimulation (NMES) and transcutaneous electrical nerve stimulation (TENS) on muscular strength, pain, range of motion and fear-avoidance in anterior cruciate ligament reconstruction (ACLR) patients. The subjects of this study who received ACLR after arthroscopic injury were divided into two groups: NMES (n=14) and TENS (n=14). Each group received intervention after routine physical therapy. Each electrotherapy intervention was applied regularly for a total of 12 times, 3 times a day (30 minutes) for 4 weeks. The evaluation by intervention method included isokinetic muscle strength in flexion and extension of the knee joint, pain, range of motion of the knee joint, and fear-avoidance beliefs. The results showed significant improvement in pain, range of motion, and fear-avoidance beliefs in both groups (p<0.05). However, there was a significant improvement in isokinetic knee flexion and extension strength only in the NMES group (p<0.05). There was no significant difference between the two electrotherapy methods in muscle strength, pain, range of motion, and fear-avoidance beliefs. These study results suggest that NMES and TENS can be effective intervention methods for pain, range of motion, and fear-avoidance response in patients with ACLR, but to improve muscle strength NMES may be more effective.

Effects of Early Passive ROM Exercise on ROM of Lower Extremities and Foot Edema in Hemiplegia Patients with Stroke (조기 수동적 하지 관절운동이 뇌졸중 후 편마비 환자의 하지 관절가동범위와 발부종에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of early passive range of motion (ROM) exercise on ROM of lower extremities and foot edema in hemiplegic patients after stroke. Methods: The data were collected between August 2009 and April 2010 from 11 patients in the experimental group and 13 in the control group. The passive ROM exercise was performed twice a day for 2 weeks. Results: In the experimental group, ROM of lower extremities (flexion of hip, flexion of knee and ankle) increased significantly compared to that of the control group. There was no significant difference in foot edema between the two groups. Conclusion: The results indicated that the early passive ROM exercise can improve the ROM of lower extremities, but not the foot edema in patients after stroke.

The Effect of Running Speed and Slope on the Lower Extremity Biomechanics (달리기 속도와 경사가 하지관절의 생체역학에 미치는 영향)

  • This study analyzes the effects of changes in running velocity and slope on the biomechanical factors of the lower limb joints. For this purpose, 15 adult males in their 20s ran according to changes in running speed (2.7, 3.3 m/s) and slope ( -9°, -6°, 0°, 6°, 9°) on the treadmill, and their running characteristics (stride length, stride frequency). The range of motion of the lower limb joint and the vertical ground reaction force were greater in UR (p <.05), and the moment of the lower limb joint, braking force, thrust and load factor was large in DR (p <.05). In joint power, the ankle joint was greater in DR, and hip joint was greater in the UR (p <.05). These results show that the injuries of the ankle joint will be greater than other cases when running DR at a speed of 3.3 m/s.

Effects of Knee Balance Taping Therapy on Knee Pain and Range of Motion in the Elderly (슬관절 밸런스 테이핑 요법이 노인의 무릎 통증과 관절가동범위에 미치는 효과)

  • This study purposed to verify the effects of knee balance taping therapy on knee pain and knee range of motion in the elderly with knee pain. The Quasi-experimental design, nonequivalent control group pretest-posttest was used in this study. Data collected from December 5 to 10, 2014. The participants were older than 60 with knee pain visiting senior community centers. Non-medicated elastic tapes were applied around the knee in the experimental group(n=21) and were not applied in the control group(n=19). Knee pain and range of motion were measured 3 times (before, after 1 hour, after 24 hours). Data were analyzed using ${\chi}^2$ test, Fisher's exact test, Mann-Whitney test, t-test, Repeated measure ANOVA with the SPSS version 21.0 program. The experimental group showed significantly decreased knee pain (F=34.03, p<.001) and increase in knee range of motion (F=7.83, p=.006). The results of this study confirm that knee balance taping therapy is an effective intervention for reducing pain and improving range of motion in elderly with knee pain.

Effect of a 12-week Shoulder Stability Rehabilitation Program on the Range of Motion and Muscle Strength of Baseball Players with Shoulder Instability (12주의 어깨 안정성 회복 프로그램이 견관절 불안정성 야구선수의 관절가동범위, 근력에 미치는 영향)

  • This study investigated the effect of a 12-week rehabilitation program on the range of motion (ROM) and muscle strength of high school baseball players with shoulder instability. We allocated 12 players with shoulder instability to the rehabilitation group and 13 players without shoulder instability to the normal exercise group. Covariate analysis (ANCOVA) was performed to assess the ROM of the internal (IR) and external (ER) rotational joints before and after participating in the 12 weeks of rehabilitation, and two-way ANOVA was performed to assess isokinetic muscle strength. The statistical significance level was set at p<.05. The IR ROM of the dominant (D) shoulder with instability and non-dominant (ND) shoulder was significantly increased before and after the rehabilitation program. The total ROM of the D shoulder with instability significantly increased after rehabilitation. IR isokinetic strength significantly improved at an angular velocity of 180°/s after rehabilitation. These results indicate that the rehabilitation program used in this study could be effective in improving ROM and muscle strength in patients with shoulder instability. However, due to the limited results, additional research on the premise of extending the rehabilitation period is necessary.

Effects of a Balance Taping on Pain and ROM of the Knee Joint in the Elderly with Lower Limbs Arthritis (밸런스테이핑이 하지 관절염 노인의 하지통증과 무릎관절가동범위에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of balance taping on pain and Range of Motion (ROM). ROM of the knee joint in the elderly with lower limbs arthritis. Methods: This study was a nonequivalent pretest-posttest design, quasi-experimental study. An experimental group of 35 elderly and a control group of 35 elderly were assigned. Subjects responses to the Visual analogue Scale (VAS) and range of motion scores as measured by a joint goniometer. Data were analyzed by, t-test and repeated measures ANOVA. Results: In the experimental treatment group, the pain score was significantly decreased from 6.46 to 2.09 after balance taping (p<.001). In addition, knee joint ROM of the experimental group was significantly increased from 114.80 to 121.92 after balance taping (p=.003, p<.001, p<.001). Conclusion: This is a very important study as it is an excellent treatment that nurses can utilize and these data support the efficacy of the method.

  • 가상현실과 증강현실의 대중화로 사람의 동작을 실시간 3D 애니메이션으로 구현하는 연구가 활발히 진행 중이다. 특히 Microsoft에서 키넥트 (Kinect)를 개발함에 따라 저렴한 가격에 부가적인 장치 필요 없이 간단한 조작만으로도 3D 모션 정보 취득이 가능해졌다. 하지만 키넥트 카메라는 마커 기반 모션 캡쳐 시스템에 비해 관절 정보의 추정 성능이 뒤떨어져 낮은 정확도를 보이는 단점을 지니고 있다. 이에 본 논문에서는 키넥트 카메라를 이용해 사람의 관절 정보를 취득하고 이것에 관절 가동 범위 (Range of Motion, ROM)를 적용하여 비정상적인 동작을 보정하는 시스템을 제안한다. ROM을 구하는 방법으로는 수행자가 모든 관절에 대해 회전 운동을 수행한 뒤 관절들의 회전 운동 정보를 취득, 분석하여 정상적인 ROM을 설정하고 실험으로부터 사람의 동작이 개선되는 것을 확인하였다.

Evolution of Exercise Performance and Medical Assessments in U-12 Youth Football Players (U-12 유소년 축구선수의 운동수행력 및 의학적 평가)

  • This study aimed to examine the evolution of exercise performance and medical assessments in U-12 youth football players. Participants were recruited from an youth football players (YFG, n=15; $11.2{\pm}0.9yrs$) and normal youth (CON, n=15; $11.5{\pm}0.6yrs$). Both groups were tested for exercise performance (health related physical fitness, skill related physical fitness, functional movement screen; FMS and Y-balance) and medical assessments (lower extremity alignment, range of motion; ROM and manual muscle test; MMT). YFG showed a positive effects on pacer in health related physical fitness, 50-m run and Single-leg standing with eyes closed in skill related physical fitness, active straight leg raise and rotary stability in FMS, non-dominant composite score in Y-balance, respectively compared to CON. However, YFG showed a negative effects on Q-angle of dominant and non-dominant in lower extremity alignment, dorsiflexion of non-dominant, plantarflexion of dominant and non-dominan in ROM, hip extension and abduction, knee flexion and extension of dominant, knee flexion and extension of non-dominant in MMT, respectively compared to CON. These results indicated that U-12 youth football players have shown excellent exercise performance, but medical assessments has proved negative effects.
