  • The study of radiation-hazard in the human skin tissue is carried out by direct irradiating to experimental animals. The influences of a radiation to the animal's skin tissue are analyzed from this experiment. However, this also accompanies losses in terms of both time and economy. In this study, we simulated human tissue by using a swine skin tissue. The depth of the swine skin tissue for the experiment is determined, and the amount of the direct radiation below this skin depth is analyzed numerically. The amount of the radiation occurred by exposure below the skin tissue can be inferred. Moreover, it is possible to use only cells effectively and animal experiments to analyze the body-hazard by radiation.

  • Tracheostomy is one of the oldest surgical procedures in medical history. The indications for a tracheostomy include the relief of an upper airway obstruction, long-term mechanical ventilation, and decreased airway resistance to help wean the patient from mechanical ventilator support. Unfortunately, tracheostomy is also associated with a number of problems including, bleeding, infection, pneumothorax, and tracheal stenosis. A pneumothorax is an uncommon complication of a tracheostomy, and can result from direct injury to the pleura or positive pressure ventilation through a dislocation of the tracheostomy tube. We report an uncommon case of a tracheostomy-induced bilateral tension pneumothorax with a review of the literature.

  • Bronchogenic cyst is rare lesion that arises from maldevelopment of the primitive foregut, and is usually found in the lung and mediastinum. Cutaneous or subcutaneous bronchogenic cyst is rare and occur unusually in the shoulder region. We report here a case of 20-month-old boy with a bronchogenic cyst on his left shoulder region. He underwent incisional biopsy and curettage for a tender cystic mass at a department of dermatology, diagnosed as a epidermal inculsion cyst. But, additional cystic mass was palpated during wound care. MRI showed a well-defined subcutaneous cystic mass. The excised cyst was lined with pseudostratified ciliated columnar epithelium with occasional goblet cells and diagnosed as a bronchogenic cyst.

A Telemetry System using Intra-body Communication for Neural Prosthesis (체내 통신을 이용한 신경 보철용 원격 통신 시스템)

  • Intra-body communication' is a wireless communication technology that uses a body as a transmission medium for electrical signals. Generally, an 'earth ground' is used to create an electric field for operating the system; however this operating method could not apply to telemetry for implanted neural prosthetic devices. So this paper suggests a newly designed intra-body communication for neural prosthetic devices. A floating system which has a couple of electrodes with body was studied to remove an influence of the 'earth ground'. We found that 10MHz is the most suitable carrier frequency in skin experiments and over 3MHz in subcutaneous experiments. The system has been applied to a current stimulator circuit for cochlear implant that uses pulse width modulation (PWM) method at 480kbps rate successfully.

  • A 2-month-old, intact female Alaskan Malamute was presented for evaluation of dyspnea and subcutaneous emphysema of the neck following biting wound. Two small punctured skin lesions and crepitus on the neck were found during physical examination. Radiographs revealed pneumomediastinum and subcutaneous emphysema. Confirmative diagnosis of tracheal laceration was made by identifying the tracheal wall discontinuity on the computed tomography. CT is a valuable tool to diagnose an external traumatic tracheal injury, and more specifically, to assess the location and the extent of laceration, and it leads to early definitive treatment and reduction of the incidence of complications.

  • Subcutaneous emphysema on the head, neck, and pneumomediastinum are, abnomal but well-documented, presence of air in the subcutaneous tissue and mediastinum, and can be diagnosed by palpation of the soft tissues, lateral or crosstable cervical radiograph and upright chest radiograph. The common clinical features of cervical emphysema and pneumomediastinum were facial and cervical swelling, presence of crepitation on palpation of the soft tissue, and retrosternal pain. Subcutaneous emphysema may arise from use of high-speed air turbine drills, facial trauma, trachea bronchial tear, endotracheal intubation, mechanical ventilation, chest injury, tracheostomy, following Lefort I osteotomy, and spontaneously. Symptoms of subcutaneous emphysema and pneumomediastium are generally self-limiting and eventually subside with conservative therapy. As we report a case of traumatic subcutaneous emphysema and pneumomediastinum after facial injury with clinical presentation and treatment consideration.

Basal Cell Carcinoma in a Domestic Hedgehog (국내 고슴도치에서 발생한 기저세포암종)

  • A 1-year-old female hedgehog was presented with subcutaneous mass in the right forelimb. Grossly, an elevated subcutaneous mass approximately 4 cm in diameter was located in forelimb. Central dark brown area was surrounded by peripheral milky white zone in the cut surface of mass. Histopathologically, numerous neoplastic cells formed irregular cords or sheets in dermis. Most of neoplastic cells showed palisading features to basement membrane with or without central caseous necrosis. These neoplastic cells showed invasive tendency to adjacent tissues. According to immunohistochemistry, the neoplastic cells demonstrated strong positive signals for pancytokeratin, and negative for vimentin. Based on the gross, histopathology and immunohistochemistry, this mass was diagnosed as basal cell carcinoma in a hedgehog.
