A Case of Extranodal Marginal Zone B-cell Lymphoma in Both Parotid Glands (양측 이하선에 발생한 림프절 외 변연부 B세포 림프종 1예)

  • Primary malignant lymphoma of the parotid gland is extremely rare entity and seldom described in the literature. Extranodal marginal zone B-cell lymphoma of mucosa associated lymphoid tissue(MALT lymphoma) is a relatively indolent disease and tents to remain localized for prolonged period of time. MALT lymphoma can be diagnosed after immunohistopathological study. Clinically, most MALT lymphomas are localized at the time of diagnosis and may be curable with local therapy alone, either surgery or radiotherapy. We present a case of MALT lymphoma in both parotid glands of patient who detected a left infraauricular huge mass as a first symptom and underwent surgical excision and immediate reconstruction using sternocleidomastoid myocutaneous flap.

  • Pulp and palm of the hand and heel of the sole are anatomically unique. Satisfactory reconstruction of these areas presents the plastic surgeon with many challenges and requires durable and sensible skin coverage, minimal donor morbidity and reliable operative procedure. We presents 7 clinical cases of sensate instep free flap transfer in this paper during the last 2 years. Three cases were soft tissue defects due to crushing and avulsion injury on the pulp of finger. 1 case was unstable scar and redundant flap after reconstruction of soft tissue dect of palm and 1 case was contracture of first web of hand. One case was a soft tissue defect due to avulsion injury on heel. Lastly, one case was chronic osteomyelitis with open wound on lateral malleolar area. Follow-up period ranged from 3 months to 2 years. Through the whole follow-up period, all flaps were viable and durable to persistant stress or weight bearing and were sensible enough to porotect the recocstructed area from injuries and maintain functions. In conclusions, the instep free flap should be considered as a valuable tool in reconstruction of hand and extremity requiring durability and sensation.

The Report for Atypical Vascular Variations in the Latissimus Dorsi Myocutaneous Flaps (광배근 피판의 비전형적인 혈관 해부학적 증례에 대한 보고)

  • It has been known that latissimus dorsi(LD) myocutaneous flap based on thoracodorsal artery is one of most useful method for microreconstructive surgery and the thoracodorsal artery of this flap has constant vascular anatomy. The retrospective study for anatomy of the thoracodorsal arterial system was performed at operative cases. The aim of this study was to document the anatomical variation of this pedicle clinically. 167 LD flaps were carried out from 1983 to 2002 in our clinic. We found unusual 7(4.2%) cases compared to standard textbooks of anatomy. One case was no vascular supply to LD muscle. In 2(1.2%) cases thoracodorsal artery was a typical branch of the subscapular artery but didn't branch to LD muscle, passed to lower serratus anterior muscle, and at this point, supplied vessel to LD muscle and it's vascular diameter was about 1mm diameter. The thoracodorsal artery arose from the axillary artery in 1.8% of cases(3 cases). One case had less than 1mm vascular diameter but a branch of subscapluar artery. It should be emphasized that we must elevate the latissimus dorsi flap after accurate cognition for the anatomy of thoracodorsal artery because the thoracodorsal arterial system is almost reliable but not uniform in rare cases.

  • The wrist and forearm are a frequently damaged area in high tension electrical injury as an input or output of the current. Electrical burns affecting the wrist and forearm may produce full thickness necrosis of the skin and damage deep vital structures beneath the eschar, affecting the local tendons, nerves, even bones and joints which result in serious dysfunction of the hand. From January 1997 to December 2001, we had treated 20 patients with high tension electrical burn in the wrist and forearm using anterolateral thigh free flap. Average follow up period were 24 months and we get satisfactory results both in functional and aesthetic aspects. This flap is considered useful in one-stage reconstruction of wide and large soft tissue defect combined with arterial injuries.

Reconstruction of Defect After Wide Excision of Malignant Soft Tissue Tumor of Limb Using Free Flap (유리피판을 이용한 사지 연부조직 악성종양 절제 결손의 재건례)

  • Purpose: Evaluation of results of free flap as a method of reconstruction in soft tissue defect after wide excision of soft tissue tumor of extremity. Materials and Methods: From 2000 through 2007, 11 patients received free flap surgery for soft tissue defect after wide excision operation for soft tissue tumor of limbs. Four cases were upper extremities and seven were lower extremities. Four subjects were diagnosed as squamous cell carcinoma, three as malignant melanoma, two as synovial sarcoma and one as malignant fibrous histiocytoma and alveolar soft part sarcoma. Donor sites of free flap varied with anterolateral thigh flaps in six cases, latissimus dorsi flaps in four, reverse forearm flap in one. By the method of doppler ultrasound, venous circulation was evaluated for the survival of each flap on the third, fifth and seventh day respectively after the operation. Results: 10 of 11 free flaps were successfully survived. Necrosis of free flaps in 1 cases occurred in case of anterolateral thigh flap. Conclusion: Free flap can be a useful method for reconstruction of soft tissue defect after wide excision of soft tissue sarcoma of extremity.

  • Verrucous carcinoma is a variant of highly differentiated squamous cell carcinoma and has a predilection for the oral cavity and larynx. It is known to be of good prognosis due to its low regional and distant metastatic potential. Peak incidence is at ages 40-69 while predominantly occuring in males and its association with tobacco smoking has been implicated. The verrucous carcinoma have accordingly been treated by radiotherapy or surgical procedures but there has been considerable controversy regarding the effect of radiotherapy due to the possibility of anaplastic transformation Laser surgery and partial resection are among the commonly used surgical procedures but laser surgery has not yet shown results in improving phonetic abilities. Authors experienced two cases of verrucous carcinoma of the larynx treated by partial laryngectomy and false vocal cord advancement flap who showed no recurrence of disease under stroboscopic examination after two year follow up and when compared to preoperative aerodynamic studies, the expiratory air pressure was decreased and mean air flow rate increased which improved glottal efficiency markedly.

Restoration of the Injured Fingertip with Eponychial Cutaneous Flap (손톱위 피부피판을 이용한 손톱형태 복원의 치험례)

  • The restoration of the fingernail is not simple in case of amputated distal fingertip which involved the pulp and nail. The nail should maintain a length of at least 2 mm from the eponychium for an adequate grip and decent appearance. Various methods to reconstruct the fingernail bed are available. The nail bed graft from amputated finger or great toe, and free onychocutaneous flap are commonly used. The nail bed of the injured tip tends to be atrophied, deformed and failed as a graft. And the great toe is often turned down as a donor. We have restored satisfactorily the nail beds of three injured finger tips with eponychial cutaneous flaps. The pulps were reconstructed with either a reverse dorsal digital island flap or free pulp graft. Repeated again. A mean follow- up was six months. The nail grew up to the average of 3.7 mm. All patients were satisfied with the length of the nail and met with good cosmetic results. An eponychial cutaneous flap is useful to restore the nail of the distal fingertip amputation. The procedure is relatively simple and morbidity is minimum.

Capsular Flaps for Correcting Implant Palpability in Secondary Augmentation Mammoplasty (유방성형술의 재수술에서의 보형물 만져짐 교정을 위한 피막 피판)

  • Purpose: Among reasons for reoperations in augmentation mammoplasty, palpable implant, due to thin skin is relatively common, but not easy to correct, especially if thin skin area is wide. The capsule around the implant is a physiologic response to foreign body, naturally formed, and suitable for use as a flap because of its high vascularity. Authors report that capsular flap is very effective and successful method for correction of implant palpability in secondary breast augmentation. Methods: From September 2007 to September 2008, the capular flaps were performed on 5 patients having palpable and wrinkling breast implants due to very thin skin among the cases on whom secondary augmentation mammaplasty had been performed. After the capsular flap was elevated according to thin skin area, the capsular flap was turned down or over to cover the thin skin area and made the thin skin area thick. Results: Post - implant palpable breast wrinkling had been successfully corrected by capsular flaps and breast implants were not palpable any more during the follow - up period(average 9.2 months). All patients who suffered from deformed breast were satisfied. Conclusion: Authors suggest that the capsular flap is a ideal, effective and useful method in management of implant palpability.

A Case Report of Hypospadias Repaired by Radial Forearm Free Flap (요측전완유리피판을 이용한 요도하열의 교정 1례 보고)

  • Purpose: Urethral reconstruction following failed hypospadias repair is difficult because of a severe shortage of the skin on ventral surface of the penis. We experienced a successful reconstruction of a failed hypospadias repair using the radial forearm free flap. So we present our case. Methods: A 16 - year - old boy had a proximal shaft hypospadias and had been taken a transverse preputial island flap but it completely broke down. There was no spare skin on the shaft of the penis. So we reconstructed a neo - urethra and the ventral skin of the penis with the radial forearm free flap. Results: Postoperatively he made relatively good process. The urethral catheter was maintained for 10 days and self - voiding was started. The suprapubic catheter was removed at the postoperative 30th day after control of cystitis. The postoperative appearance of the penis is acceptable and a straight erection is achieved. Conclusion: In the scarred penis with hypospadias, the radial forearm free flap transfer is a reliable reconstructive method for the neo - urethra and the ventral skin of the penis.

Reconstruction of Defective Type Congenital Cleft Earlobe with S-shaped Flap (새로운 S자형 피판을 이용한 결손형 선천성 이수열의 재건)

  • Purpose: Congenital cleft earlobe is relatively rare malformation and defective type congenital cleft earlobes are reconstructed with mainly local flap methods rather than primary closure or z - plasty. Various methods are introduced but many of these remain visible scars or require complex operative techniques. We designed a new and simple method of reconstruction for defective type cleft earlobe. Methods: On the posterior surface of the auricle and mastoid area, S - shaped line was drawn continuously. One arc is for turnover hinge flap to make the anterolateral surface of the earlobe, and the other is for transposition flap to reconstruct the posterolateral surface. The donor site of the transposition flap was closed primarily. Results: Four patients were operated by S - shaped flap design method. They were all female and two were right side and others were left. We obtained aesthetically satisfactory postoperative results with inconspicuous scars at the posterior side of the auricle. In one case, minor revision was performed because of insufficient blood supply of the hinge flap. Conclusion: We can reconstruct defective type cleft earlobe with new, simple S - shaped design for hinge flap and transposition flap.
