Endoscopic Balloon Dilatation in Children with Congenital and Acquired Esophageal Anomalies (소아의 선천성 및 후천성 식도 질환에서 내시경적 풍선 확장술)

  • Purpose: To evaluate the safety, efficacy and technical problems of the endoscopic balloon dilatation of esophageal anomalies in children. Methods: The medical records of 8 children treated by endoscopic balloon dilatation for esophageal anomalies over a 10-year period at Pusan National University Hospital were reviewed retrospectively. The balloon catheter (Maxforce TTS or CRE, Boston Scientific Co., USA) was positioned across the area of narrowing by direct visualization. The balloon was slowly inflated with normal saline to specified pressures for each balloon and maintained for 60 seconds and then deflated. After 60 seconds pause, the procedure was repeated with a larger sized balloon (increments of 1 mm for each subsequent dilation) till effective dilatation was confirmed by direct visualization without complications. Results: Three male and five female were included and their mean age was 4.2 years. A total of 27 (average of 3.2 per patient) dilatation were performed. Underlying diseases of patients are postoperative stricture of esophageal atresia in 3 cases, esophageal ring in 2 cases, achalasia, corrosive esophagitis and hypertensive LES in one case respectively. The size of initial dilating balloon was chosen on the basis of the diameter of the narrowing determined by endoscopy. The first dilation in patients with severe esophageal stricture was made with a 6 mm sized balloon. Complications observed were esophageal perforation and respiratory holding during the procedure in one case respectively. Successful outcome was seen in 6 patients (75%). Conclusion: Endoscopic balloon dilatation can provide a safe and effective mean of treating esophageal anomalies in children and should be considered the treatment of choice in the initial management of those cases.

PTV Margins for Prostate Treatments with an Endorectal Balloon (전립선 암의 방사선치료 시 직장 내 풍선삽입에 따른 계획표적부피마진)

  • Purpose: To determine the appropriate prostate planning target volume (PTV) margins for 3-dimensitional (3D) conformal radiotherapy (CRT) and intensity-modulated radiation therapy (IMRT) patients treated with an endorectal balloon (ERB) under our institutional treatment condition. Materials and Methods: Patients were treated in the supine position. An ERB was inserted into the rectum with 70 cc air prior to planning a CT scan and then each treatment fraction. Electronic portal images (EPIs) and digital reconstructed radiographs (DRR) of planning CT images were used to evaluate inter-fractional patient's setup and ERB errors. To register both image sets, we developed an in-house program written in visual $C^{++}$. A new method to determine prostate PTV margins with an ERB was developed by using the common method. Results: The mean value of patient setup errors was within 1 mm in all directions. The ERB inter-fractional errors in the superior-inferior (SI) and anterior-posterior (AP) directions were larger than in the left-right (LR) direction. The calculated 1D symmetric PTV margins were 3.0 mm, 8.2 mm, and 8.5 mm for 3D CRT and 4.1 mm, 7.9 mm, and 10.3 mm for IMRT in LR, SI, and AP, respectively according to the new method including ERB random errors. Conclusion: The ERB random error contributes to the deformation of the prostate, which affects the original treatment planning. Thus, a new PTV margin method includes dose blurring effects of ERB. The correction of ERB systematic error is a prerequisite since the new method only accounts for ERB random error.

  • 최근 영화나 드라마 같은 미디어 데어터가 폭발적으로 증가하면서, 다양한 언어로 번역된 자막 데이터도 증가하고 있다. 이러한 자막은 대부분 화면 하단이나 우측에 위치가 고정되어 나타내는 방식을 취하고 있다. 그러나 이 방식에는 몇 가지 한계점을 가지고 있다. 자막과 등장인물의 얼굴이 거리가 먼 경우, 시청자의 시선이 분산되어 영상에 집중하기 어렵다는 점과 청각장애를 가진 사람의 경우 자막만으로는 누가 말하고 있는 대사인지 혼동이 온다는 점이다. 본 논문에서는 만화에서 대사를 전달하기 위해 사용하던 말풍선을 동영상의 자막을 나타내는데 사용하는 새로운 자막 시스템을 제안한다. 말풍선을 사용하면 말꼬리로 화자의 위치를 가리키고, 시청자의 시선을 화자의 얼굴 근처에 집중시킴으로써 기존 자막이 갖는 한계점을 개선시킬 수 있다. 본 연구의 결과물을 검증하기 위해 사용자 평가를 실시했고, 기존의 자막 방식에 비해 시선의 안정성과 흥미성, 정확도에서 더 낫다는 결과를 얻을 수 있었다.

Antioxidant Effect and Component Analysis of Cardiospermum halicacabum Leaf Extracts (풍선덩굴 잎 추출물의 항산화 효과 및 성분 분석)

  • In this study, the antioxidant effect and component analysis for extract and fractions of Cardiospermum halicacabum leaf were investigated. All experiments were performed with 50% ethanol extract, ethyl acetate fraction and aglycone fraction obtained from dried C. halicacabum leaf. The yields of extract and fractions were 16.4, 0.9 and 0.3% per dried powder, respectively. DPPH (1,1-phenyl-2-picrylhydrazyl) radical scavenging activity ($FSC_{50}$) of ethyl acetate fraction ($92.5{\mu}g/mL$) was the greatest radical scavenging activity, but lower than (+)-${\alpha}$-tocopherol ($8.9{\mu}g/mL$). In reactive oxygen species (ROS) scavenging activity (total antioxidant capacity, $OSC_{50}$) on ROS generated in $Fe^{3+}-EDTA/H_2O_2$ system, aglycone fraction ($4.2{\mu}g/mL$) was the highest total antioxidant capacity and similar to L-ascorbic acid ($1.5{\mu}g/mL$). The cellular protective effects of C. halicacabum leaf extract and fractions on the $^1O_2$-induced cellular damage of human erythrocytes were exhibited at all concentration-dependent ($5.0-25.0{\mu}g/mL$). Especially, aglycone fraction (${\tau}_{50}$, 76.4 min) in $25.0{\mu}g/mL$ showed the most protective effect among extracts. Components of the ethyl acetate fraction obtained from C. halicacabum extracts were analyzed by TLC, HPLC chromatogram and LC/ESI-MS. Results showed that the ethyl acetate fraction contained some flavonoids, such as apigenin-7-O-glucuronide, apigenin-7-glucosdie and quercitrin hydrate. These results suggest that the extracts and fractions of C. halicacabum leaf may be applied as antioxidant functional cosmetic raw materials.

  • 최근 휴대전화 문자스팸 차단 강화로 인한 풍선효과로 휴대전화 음성스팸이 지속적으로 증가하고 있고, 2015년 3월부터는 음성스팸 건수가 문자스팸 건수를 역전해서 급속도로 증가하고 있어 효율적으로 음성스팸을 차단할 수 있는 시스템 구축의 필요성이 대두되고 있다. 이에, 음성스팸 신고번호와 음성스팸 가상 트랩(실제 존재하지 않는 가상의 휴대전화번호를 적용)을 통해 탐지된 내용을 기반으로 음성스팸 전화번호 차단리스트를 생성하고 이를 이통사에게 제공하여 일정기간 동안 음성호를 차단하는 음성스팸 실시간 차단시스템을 설계하고자 한다.

Efficacy of Intra-Aortic Balloon Pump in Postcardiotomy Cardiogenic Shock (개심술 후 인공 심폐기 이탈 시 동맥내 풍선 펌프 사용의 유용성)

  • Intra-aortic balloon pump (IABP) is well known for its hemodynamic benefit but still has its own complications. Proper use of IABP is the best way to obtain maximum benefit with low complication rate. Material and Method: Twenty one(men 10, female 11) patients were included in this study among the 100 consecutive adult cardiac surgery patients in our hospital. Eighteen(85.7%) were ischemic heart disease patients. They all received IABP therapy due to postcardiotomy cardiogenic shock according to the well-known indications. Their preoperative conditions, intraoperative factors including hemodynamics, postoperative conditions and IABP-related complications were analyzed. Result: Nineteen patients(90.5%) were successfully weaned from IABP. There were 2 patients of operative death and the mortality rate was 9.5%. Duration of IABP use was 40.7$\pm$24.3 hours. There were 2 cases(9.5 %) of IABP-related vascular complications that required surgical intervention. Conclusion: We concluded that IABP could be used effectively and safely for postcardiotomy cardiogenic shock patients with low complication rate.

  • The conventional surgery method of thrombectomy of venous thrombi from the deep veins of the lower extremity was the use of Forgarty balloon catheter. The catheter is inconvenient due to the presence of the balloon and prohibiting venous valves within the venous trees. With the use of a stone-forceps(Fig. 1), thrombi within iliofemoral vein could be easily removed without the obstacle of the valves because the instrument keeps valves open. This instrument is also useful in monitoring the back-flow from the iliac vein. Thrombi within the veins below the level of inguinal incision are removed successfully only by effective manual compression of the calf and thigh muscles. 1 recommend operating on the iliac vein first rather than the lower venous tree.
