A Case Study Evaluating the Effectiveness of an Interdisciplinary Team-Based Integrated Medical Service Model in Improving the Quality of Life of a Fibromyalgia Patient and Caregiver through Psychological and Various Counseling Interventions (섬유근통 환자와 보호자의 삶의 질 증진을 위한 의·한 협진 기반의 통합의료서비스모델 적용평가 사례 연구: 심리 및 매체 상담개입을 중심으로)

  • Objectives: To evaluate the effectiveness of an integrated medical service model for a fibromyalgia patient and their caregiver, focusing on reducing pain and improving quality of life. Methods: A single-case study design was employed, involving a fibromyalgia patient and their primary caregiver treated at W University Hospital. The integrated medical service program, based on the Ministry of Health and Welfare's model, included medical consultations and complementary therapies such as psychological counseling, art therapy, music therapy, horticultural therapy, yoga, and meditation. The program was conducted weekly for 8 weeks, with each session lasting up to 100 minutes. Data collection involved both quantitative and qualitative assessments. Quantitative data included demographic surveys, psychological tests, health-related quality of life measures, pain indices, and sleep quality indices. Qualitative data were gathered through feedback evaluations and emotional assessments. Results: The patient showed improvements in mobility, self-care, daily activities, and anxiety/depression, with EQ-VAS scores increasing from 20 to 40 and pain perception decreasing from 67.41 to 42.58. The caregiver reported reduced anxiety/depression and an increase in EQ-VAS scores from 95 to 98. Both patient and caregiver exhibited emotional changes, with decreased depression and increased happiness. However, the patient showed an increase in fear and anger. Conclusions: The integrated medical service model positively impacted the emotional and psychological well-being of the fibromyalgia patient and their caregiver. Despite the limitations of a small sample size and a single-case study design, the findings suggest that an integrated approach can be beneficial. Larger-scale studies are needed to confirm and generalize these results.

  • The Incheon Metro Line 1 Geomdan Extension Phase 1 is the first project in South Korea where both a roadheader and TBM (Tunnel Boring Machine) are being used together. The shield TBM tunnel section is 1,057 m long, and is mostly composed of rock, including the section beneath the Airport Railroad and the Gyeongin Ara Waterway. A 7.8 m earth pressure balance shield TBM was used for tunnel excavation. The average monthly advance rate for both the North and South tracks is 239.1 m, with a maximum monthly advance rate of 334.5 m. This technical article comprehensively evaluates the productivity of the shield TBM, focusing on the TBM excavation performance. Above all, it aims to provide useful reference material for the successful execution of shield TBM tunnel construction.

Sharing Experiences in Selecting Clinical Outcome and Approving Validated Questionnaires : Insights from an Elderly Registry Study (노인등록연구 사례를 통한 임상평가지표 선정 과정 및 검증된 설문도구 승인 경험의 공유)

  • Objectives: Underpinned by the context of a Korean traditional medicine cohort study on healthy aging, this research primarily aims to guide the selection of Clinical Outcome Assessments (COAs) for elderly healthy aging patient registry research, offering insights into the selection process; and secondly, to streamline the resource-intensive process of obtaining permissions for validated COAs, benefiting future traditional Korean medicine clinical researchers. Methods : In this study, we identified outcomes through a review of previous studies, followed by a process involving expert consultations to select the final outcomes. Subsequently, for the selected outcomes that were Clinical Outcome Assessments (COAs) developed tools, we searched in commercial databases to confirm the availability of Korean versions and the necessity of obtaining permissions. Finally, we obtained permissions for their utilization and, when needed, acquired the original instrument questionnaire through payment. Results: Through a literature review of existing observational studies, a total of 57 outcomes were selected, with 19 of them identified as COA instruments. Upon verifying usage permissions for these 19 instruments, it was found that 17 required author-specific permissions, and among these, 2 needed a purchase as they were commercially available. Conclusion: This study provides a detailed overview of outcome selection and permission acquisition for elderly patient registry research. It underscores the importance of Clinical Outcome Assessment (COA) tools and the rigorous approval process, aiming to enhance research reliability. Continuous verification of COA information is essential, and future research should explore Core Outcome Set (COS) development through consensus-building approaches like Delphi studies.

Application study of random forest method based on Sentinel-2 imagery for surface cover classification in rivers - A case of Naeseong Stream - (하천 내 지표 피복 분류를 위한 Sentinel-2 영상 기반 랜덤 포레스트 기법의 적용성 연구 - 내성천을 사례로 -)

  • Understanding the status of surface cover in riparian zones is essential for river management and flood disaster prevention. Traditional survey methods rely on expert interpretation of vegetation through vegetation mapping or indices. However, these methods are limited by their ability to accurately reflect dynamically changing river environments. Against this backdrop, this study utilized satellite imagery to apply the Random Forest method to assess the distribution of vegetation in rivers over multiple years, focusing on the Naeseong Stream as a case study. Remote sensing data from Sentinel-2 imagery were combined with ground truth data from the Naeseong Stream surface cover in 2016. The Random Forest machine learning algorithm was used to extract and train 1,000 samples per surface cover from ten predetermined sampling areas, followed by validation. A sensitivity analysis, annual surface cover analysis, and accuracy assessment were conducted to evaluate their applicability. The results showed an accuracy of 85.1% based on the validation data. Sensitivity analysis indicated the highest efficiency in 30 trees, 800 samples, and the downstream river section. Surface cover analysis accurately reflects the actual river environment. The accuracy analysis identified 14.9% boundary and internal errors, with high accuracy observed in six categories, excluding scattered and herbaceous vegetation. Although this study focused on a single river, applying the surface cover classification method to multiple rivers is necessary to obtain more accurate and comprehensive data.

Exploring the Changes in Elementary Teachers' Modeling pPCK for Science Modeling Instructions Through Knowledge Exchange of the Refined Consensus Model (개선된 합의 모델(RCM)의 지식 교환을 통한 초등교사의 모델링 pPCK 변화 탐색)

  • The purpose of this study was to explore changes in elementary teachers' modeling pPCK (personal PCK) resulting from knowledge exchange within the realms of PCK described in the Refined Consensus Model (hereafter RCM). For this purpose, a professional learning community (hereafter PLC) was established for three elementary school teachers to facilitate knowledge exchange specifically focused on science modeling instructions. The study then analyzed the CoRe (content representations) written by the research participants twice to explore any changes in modeling pPCK (personal PCK). In addition, the discourse shared by the participants in the PLC and the data from the in-depth interviews were also analyzed using grounded theory research methods. The results of the study showed that there was no significant change in knowledge of the science curriculum in elementary teachers' modeling pPCK, but there were changes in orientations toward teaching science, knowledge of students' understanding in science, knowledge of instructional strategies and representations, and knowledge of assessment of science learning. Furthermore, the analysis of PLC discourse and in-depth interviews showed that modeling instructions reflection (ePCK; enacted PCK) and educational research-based modeling cPCK (collective PCK) influenced these changes in teachers' modeling pPCK. Accordingly, this study suggests recommendations for pedagogical approaches aimed at improving teachers' modeling PCK.

How to build an AI Safety Management Chatbot Service based on IoT Construction Health Monitoring (IoT 건축시공 건전성 모니터링 기반 AI 안전관리 챗봇서비스 구축방안)

  • Purpose: This paper conducts IoT and CCTV-based safety monitoring to analyze accidents and potential risks occurring at construction sites, and detect and analyze risks such as falls and collisions or abnormalities and to establish a system for early warning using devices like a walkie-talkie and chatbot service. Method: A safety management service model is presented through smart construction technology case studies at the construction site and review a relevant literature analysis. Result: According to 'Construction Accident Statistics,' in 2021, there were 26,888 casualties in the construction industry, accounting for 26.3% of all reported accidents. Fatalities in construction-related accidents amounted to 417 individuals, representing 50.5% of all industrial accident-related deaths. This study suggests implementing AI chatbot services for construction site safety management utilizing IoT-based health monitoring technologies in smart construction practices. Construction sites where stakeholders such as workers participate were demonstrated by implementing an artificial intelligence chatbot system by selecting major risk areas within the workplace, such as scaffolding processes, openings, and access to hazardous machinery. Conclusion: The possibility of commercialization was confirmed by receiving more than 90 points in the satisfaction survey of participating workers regarding the empirical results of the artificial intelligence chatbot service at construction sites.

Denali Inferior Vena Cava Filter Retrieval: Complications and Success Rates (Denali 하대정맥 필터 제거: 합병증과 성공률)

  • Purpose The present study was to evaluate the outcomes of Denali filter retrieval. Materials and Methods We retrospectively reviewed 143 patients who received Denali filter insertion from September 2015 to April 2020. Ninety-seven patients who required removal of the filters were include in this study. Filters were retrieved with either standard or advanced techniques. Venography before and after retrieval was obtained to evaluate technical success, complications and duration of filter insertion. Results All 97 filters were retrieved successfully without complications. Ninety-two (94.8%) were retrieved with standard technique and 5 filters (5.2%) required the advanced technique. There were two cases with a filter angle greater than 15 degrees. Inferior vena cava penetration was shown in 17 patients (17.5%) on venography but was not associated with contrast media extravasation after filter removal. Conclusion The Denali filter showed a high rate of successful retrieval without complications. This study adds value to previous studies and trials showing that the Denali filter is a reliable and safe filter that can potentially improve retrieval rates, with increasing use of this device.

A Study on Improving the Estimation of Social Benefits Using the Economic Valuation of Ecosystem Services in Preliminary Feasibility Analyses for Ecological Restoration Projects - Focused on the Case of Janghang Wetland Restoration Project - (생태계서비스 가치평가를 활용한 예비타당성조사 편익분석 개선 방향 연구 - 장항습지복원사업 사례를 중심으로 -)

  • This study is to propose the ecosystem service valuation method as a complementary or alternative tool to overcome the limitations of the contingent valuation method(CVM), typically used to assess social benefits in preliminary feasibility studies. With an increasing interest in natural and environmental restoration projects, we assessed social benefits with theses CVM and ecosystem service valuation method from a case of Janghang wetland restoration project and compared the extent of the two social benefits. For quantitative evaluation of ecosystem services, the biophysical quantity for each ecosystem service indicator was calculated and then converted into currency (KRW) units to estimate the economic value of ecosystem services. The four ecosystem regulating service indicators were selected including greenhouse gas capture/storage, air pollution, water quantity and quality regulation. The amounts of CO2 sequestration and storage as a ecosystem's greenhouse gas regulating service in the study area were 73.04 tCO2/yr and 5,867.53 tCO2/yr respectively. The reduction of SO2, one of air pollutant gases by ecosystems was calculated to be 180.27 kg/yr, the reduction of NO2 to be 378.90 kg/yr, and the reduction of fine dust (PM10) to be 9,713.92 kg/yr. The amount of freshwater regulating service by the ecosystem was estimated to be 459,394,319ℓ/yr, and the amount of nitrogen in freshwater removed by the ecosystem was 78.00kg/yr. Study results show that the benefits derived from the CVM were KRW 227.8 billion over the 30-year analysis period and those from the ecosystem service valuation method were KRW 41.4 billion for regulatory services and KRW 148.8 billion for cultural services, totaling KRW 189.5 billion. With KRW 184.8 billion of the total costs, the benefit/cost ratio using the CVM was 1.23 and that with the ecosystem service valuation method was 1.03. This study implications include that the CVM and ecosystem service valuation method can be applied together to assess and compare social benefits for natural and environmental restoration projects.

Analysis of Domestic and Overseas Radioactive Waste Maritime Transportation and Dose Assessment for the Public by Sinking Accident (국내·외 방사성폐기물 해상운반 현황 및 침몰사고 시 일반인 선량평가 사례 분석)

  • Demand for RW transportation is expected to increase due to the continuous generation of RW from nuclear power plants and facilities, decommissioning of plants, and saturation of spent fuel temporary storage facilities. The locational aspect of plants and radiation protection optimization for the public have led to an increasing demand for maritime transportation, necessitating to apprehend the overseas and domestic current status. Given the potential long-term radiological impact on the public in the event of a sinking accident, a pre-transportation exposure assessment is necessary. The objective of this study is to investigate the overseas and domestic RW maritime transportation current status and overseas dose assessment cases for the public in sinking accident. Selected countries, including Japan, UK, Sweden, and Korea, were examined for transport cases, Japan and the U.S were chosen for dose assessment case in sinking accidents. As a result of the maritime transportation case analysis, it was performed between nuclear power plants and reprocessing facilities, from plants to disposal or intermediate storage facilities. HLW and MOX fuel were transported using INF 3 shipments, and all transports were performed low speed of 13 kn or less. As a result of the dose assessment for the public in sinking accident, japan conducted an assessment for the sinking of spent fuel and vitrified HLW, and the U.S conducted for the sinking of spent fuel. Both countries considered external exposure through swimming and working at seashore, and internal exposure through seafood ingestion as exposure pathway. Additionally, Japan considered external exposure through working on board and fishing, and the U.S considered internal exposure through spray inhalation and desalinized water and salt ingestion. Internal exposure through seafood ingestion had the largest dose contribution. The average public exposure dose was 20 years after the sinking, 0.04 mSv yr-1 for spent fuel and 5 years after the sinking, 0.03 mSv yr-1 for vitrified HLW in Japan. In the U.S, it was 1.81 mSv yr-1 5 years after the sinking of spent fuel. The results of this study will be used as fundamental data for maritime transportation of domestic RW in the future.

Analysis on Study Cases of Safety Assessment and Cases for Spent Nuclear Fuel Pool Accident (사용후핵연료 습식저장시설 사고 안전성 평가 연구 현황 및 사고 사례 분석)

  • Spent nuclear fuel corresponds to high-level radioactive waste that has high decay heat and radioactivity. Accordingly, Spent nuclear fuel withdrawn from the reactor core is primarily stored and managed in a spent nuclear fuel pool in the nuclear power plant to reduce decay heat and radioactivity. In Korea, most nuclear power plant store all spent nuclear fuel in a spent nuclear fuel pool. For wet storage, there are no defense in depth different with reactor core. The study related to spent nuclear fuel pool accident should be carried out to ensure safety. Therefore, it is necessary to analyze previous study cases related to safety of spent nuclear fuel pool and accident cases to build foundational knowledge. The Objective of this study is to analyze study cases of safety assessment and cases for spent nuclear fuel pool accident. For analyzing study cases of safety assessment, possible phenomena when spent nuclear fuel pool accident occurring identified, Subsequently, study cases for safety assessment about each phenomena were investigated, and materials & methods and results for each study are analyzed. For analyzing cases for spent nuclear fuel pool accident, we analyzed accident cases caused by loss of cooling and loss of coolant in spent nuclear fuel pool. Subsequently, causes and change of water level and temperature by each accident case are analyzed. As a result of the analysis on study cases of spent nuclear fuel pool accident, the results of the study conducted by each research institute were vary depending on the computer code, materials & methods of experiment and major assumptions used in the study. As a result of analyzing cases for spent nuclear fuel pool accident, it was found that accident cases for loss of cooling is more than cases for loss of coolant accident. Even though the types of accident in spent nuclear fuel pool were similar, the specific causes were different by each accident case. All the accident cases analyzed did not lead to severe accidents, such as nuclear fuel being exposed to the air. The result of this study will be used as fundamental data for study on spent nuclear fuel pool accident that will be conducted in the future.
