Study of the fracture resistance of zirconia on posterior fixed partial dentures based on inter-abutment distance (지르코니아 고정성 국소의치의 지대치간 거리에 따른 파절저항성에 관한 연구)

  • Purpose: Zirconia fixed partial dentures with mandibular 2nd premolar and 2nd molar as abutments are fabricated and then the effects of inter-abutment distance on fracture resistance of zirconia fixed partial dentures is studied. Materials and Methods: The materials used in this study are Cameleon S zirconia block and S2 zirconia block, which are divided into CS Group and S2 Group applying different inter-abutment distance for each material, and the sintered zirconia fixed partial denture was luted to the epoxy resin die using a temporary luting cement, and then the fracture resistance was measured by placing a 6 mm diameter hardened steel ball on the occlusal surfaces of the pontics and applying pressure at a cross head speed of 1.0 mm/min on a universal testing machine with a load cell of 5.0 kN. Results: The fracture resistance of zirconia fixed partial dentures is not significantly affected by inter-abutment distance The fracture resistance of zirconia fixed partial dentures in CS Group was significantly higher in 15 mm of inter-abutment distance than in 13 mm and 17 mm of inter-abutment distance (P < 0.05). The fracture resistance of zirconia fixed partial dentures in S2 Group was not significantly different between the three groups (P > 0.05). Conclusion: The fracture resistance of zirconia fixed partial dentures with mandibular 2nd premolar and 2nd molar as abutments does not significantly affected by the inter-abutment distance.

Fracture Resistance of Incisal Tooth Fragment reattached with different Materials and Preparation (레진재료와 치아형성 방법에 따른 파절편 재부착치아의 파절저항성)

  • The purpose of this study is to analyze the fracture resistance of reattached tooth according to the resin materials and tooth preparation type under physiological conditions. Uncomplicated crown fracture in the oblique direction was reproduced on the extracted 64 anterior teeth. Depending on the composite resin material, reattachment was performed using a flowable resin and a packable resin. Depending on retentive forms, reattachment was performed using simple reattachment, 1.0 mm × 1.0 mm labial chamfer bevel, 1.0 mm × 1.0 mm lingual chamfer bevel and 1.0 mm × 1.0 mm circumferential bevel. A load was applied to the palatal surface of the tooth using a universal testing machine at an angle of 125 degree, which is the interincisal angle of normal children. Under the masticatory pressure condition, fracture resistance of lingual chamfer groups was 28.28 ± 7.41 MPa and 27.54 ± 4.45 MPa, which was significantly higher than those of simple reattachment groups, 17.21 ± 5.87 MPa and 20.10 ± 6.00 MPa, in both flowable and packable resin groups. When considering the lingual force similar to masticatory pressure, the fragment retention was significantly improved when the lingual chamfer was formed compared to the simple reattachment. Clinicians may consider the design of the lingual chamfer in order to improve fracture resistance to masticatory pressure during fragment reattachment.

Practicality and prospect of full-zirconia restoration (풀지르코니아 수복물의 실용성과 전망)

  • Zirconia polycrystalline (Y-TZP) showed better mechanical properties and superior resistance to fracture than other conventional dental ceramics. Zirconia-based ceramics have been successfully introduced into the clinic to fabricate fixed dental prostheses (FDPs), along with a dental computer-aided/computer-aided manufacturing (CAD/CAM) system. It has been clinically available as an alternative to the metal framework for fixed dental prostheses (FDPs). The most frequent clinical complication with zirconia-based FDPs was chipping of the veneering porcelain that was affected by many factors. Another option was full-contour zirconia FDPs using high translucent zirconia. Full-contour zirconia FDPs has many clinical advantages but it caused concern about the wear of antagonist enamel, because the hardness of Y-TZP was over double that of porcelain. However, many articles demonstrates that highly polished zirconia yielded lower antagonist wear compared with porcelains. In this article (1) advantages of full zirconia restorations, (2) clinical applications of zirconia restorations, (3) abutment preparation, (4) surface finish of zirconia restoration and antagonist enamel wear, (5) bond of zirconia with resin-based luting agents, (6) communication in clinical & lab.procedures for full zirconia restorations are reviewed.

치과용 Ni-Ti 파일의 피로파절특성에 미치는 표면개질의 영향

  • Ni-Ti 파일은 근관치료 시 근관형성에 사용되며 근관계 내 조직 잔사와 세균 을 포함한 모든 내용물을 제거하고 미세누출이 생기지 않도록 성공적으로 근관충전을 할 수 있는 근관의 형태를 만드는데 사용된다. Ni-Ti 파일은 수동형 파일처럼 날이 풀어지거나 예각으로 꺾이는 등 시각적으로 나타나는 파일의 피로도나 손상정도를 인지하기 어려워 파일이 근관 내에서 부러지는 것을 막기가 어렵다. Ni-Ti 파일은 육안으로 관찰 할 수 있는 구부러짐이나 풀림 등의 소성변형 없이 기구의 탄성한계 내에서 갑작스럽게 파절되는 경우가 있는데, 이는 만곡 근관 내에서 기구가 회전하는 동안 만곡의 안쪽에는 압축응력이, 만곡의 바깥쪽에는 인장응력이 반복적으로 가해짐으로써 파절의 표면에 미세 파절과 균열이 발생하고 전파되어 결국 피로파절(fatigue fracture)을 야기하게 된다. 또한 Ni-Ti파일이 반복응력을 받으면 균열이 형성되면서 파절이 야기되며 연성파절(ductile fracture) 양상을 나타낸다고 보고하였다. Ni-Ti 파일을 이용한 피로파절에 대한 이전의 연구에서는 파일의 직경이나 경사도 (taper), 단면 형태 및 회전속도, 표면결함 등이 파절에 영향을 미친다고 보고되고 있다. 사용하지 않은 Ni-Ti파일을 구부려 응력을 가 한 상태에서 주사전자현미경으로 관찰한 결과 기계 가공 과정에서 발생한 균열, 미세 결함, 긁힌 자국 및 불균질성 등이 원인으로 알려져 있다. 따라서 본 연구발표에서는 표면결함을 최소화하고 기구의 회전응력 하에서 피로파절저항성을 향상시키기 위한 방법에 대하여 알아보고자 한다.

Stress Analysis and Fatigue Failure of Prefabricated and Customized Abutments of Dental Implants (치과 임플란트에서 기성 지대주와 맞춤형 지대주의 응력분석 및 피로파절에 관한 연구)

  • This study was to evaluate the stress distributions of prefabricated, customized abutments and fixtures according to their material and shape by three-dimensional finite element analysis. And to investigate the fatigue life and fracture characteristics. Mandibular models were fabricated by reconstruction of the CT scan of patients with normal occlusion. A total of six finite element models were designed, a load of 100 N was applied on the buccal cusps vertically, and 30 degree obliquely. 10 specimens each were fabricated for the more clinically widely used 4 type abutments and were loaded according to ISO 14801. Differences in stress distribution patterns were not found according to the materials of the abutments and fixtures. But a slight difference in the stress level was detected. Customized abutment groups showed lower crown stress levels. One-piece zirconia implant showed the lowest bone stress levels. In the fatigue test, highest values were measured in group 7. Prefabricated abutments showed less variation of fatigue life (P<0.05). Use of customized abutments can improve the fracture resistance of restorations. Especially, use of customized zirconia abutments reinforced by titanium screw connecting parts is recommended.

FRACTURE RESISTANCE OF CROWN-ROOT FRACTURED TEETH REPAIRED WITH DUAL-CURED COMPOSITE RESIN AND HORIZONTAL POSTS (수평 포스트와 이중중합 복합레진으로 수복된 치관-치근 복합파절 치아의 파절 저항성에 관한 연구)

  • The purpose of this study was to investigate the fracture resistance of crown-root fractured teeth repaired with dual-cured composite resin and horizontal posts. 48 extracted human premolars were assigned to control group and three experimental groups. Complete crown-root fractures were experimentally induced in all control and experimental teeth. In the control group. the teeth (n=12) were bonded with resin cement and endodontically treated. Thereafter, the access cavities were sealed with dual-cured composite resin. In composite resin core-post group (n=12), the teeth were endodontically treated and access cavities were sealed with dual-cured composite resin. In addition, the fractured segments in this group were fixed using horizontal posts. In composite resin core group (n=12), the teeth were endodontically treated and the access cavities were filled with dual-cured composite resin without horizontal posts. In bonded amalgam group (n = 12), the teeth were endodontically treated and the access cavities were sealed with bonded amalgam. Experimental complete crown-root fractures were induced again on repaired control and experimental teeth. The ratio of fracture resistance to original fracture resistance was analyzed with Kruskal-Wallis test. The results showed that teeth in control and composite resin core-post group showed significantly higher resistance to re-fracture than those in amalgam core group (p < 0.05). The resistance to refracture was high in the order of composite resin - post group, control group, composite resin group and bonded amalgam group. Within the scope of this study, the use of horizontal post could be beneficial in increasing the fracture resistance of previously fractured teeth.
