Analysis of Mediatinal Lymph Nodes with Internal Low Density on Contrast Enhanced CT Scan (조영종강 전산화단층촬영상 내부 저밀도 음영을 보이는 종격동 림프절의 분석)

  • Background : To analyze the morphologic characteristics of low density lymph node in etiologic differentiation of lymphadenopathy, emphasizing the different features between tuberculosis and lung cancer, on contrast enhanced CT scan. Method : A total of 64 patients who showed low density lymph nodes on chest CT scan were analyzed. Primary causes were tuberculosis (n=28), lung cancer (n=27), malignant lymphoma (n=5) and metastasis from extrathoracic malignancies (n=4). CT scan was performed with 10mm slice thickness and 7 characteristic features were evaluated : location, size, presence or absence of the nonnecrotic lymph node, calcification, perinodal fat obliteration, thickness and evenness of the enhancing rim. Results : In patients with tuberculous lymphadenopathy, lymph nodes with uneven (68.0%) and thick (62.1%) enhancing rim were more common than lung cancer (p<0.05). Low density lymph nodes with less than 1 cm in size were found only in tuberculous lymphadenopathy(n=10). In 48.2% of patients with lung cancer, more than 1 nonnecrotic enlarged lymph node were coexisted, whereas 21.4% in patients with tuberculous lymphadenopathy(p=0.06). However, the size, location and calcification were not statistically significant between tuberculous lymphadenopathy and lung cancer. Conclusion : Tuberculous lymphadenopathy is strongly suggested when enhancing rim of enlarged lymph nodes is uneven and thick, when the coexisting nonnecrotic lymph nodes are few in number and when central low density is encountered in normal sized lymph nodes.

The Recognition of Students on Appropriateness of Clothing & Textiles as Educational Contents in Practical Arts or Technical Education.Home Economics in the 7th Curriculum of Korea (실과 및 기술.가정 교과 의생활 교육내용의 적정성에 대한 학생의 인식)

  • The purpose of this study was to evaluate appropriateness of clothing & textiles as educational contents in practical arts or technology home economics in the 7th curriculum. The factors of appropriateness were the amounts of learning, the degrees of understanding, interest and requirement on the educational content recognized by elementary, middle and high school students. The data collected from evaluation sheets were analyzed by frequency analysis, t-test, crossing analysis using SPSSWIN 12.0 program. Results were as follows; First. more than 50% of elementary, middle and high school students were recognized the amounts of tearing of clothing & textiles proper. But boy students of middle school recognized 'making clothes' in practice course too heavy. Second. It were from easy to difficult that the degrees of understanding on the educational contents of clothing & textiles recognized by elementary school students. Elementary school students understood both 'making cushion' and 'sewing machine theory and practice' difficult. Middle school students understood 'basic sewing for repairing clothes' and 'making clothes' difficult whereas high school students thought 'making pillow' and 'making bag' difficult. All students understood practice courses hard. Third, the degrees of interest showed from 'interest' level to 'not interest' level in case of elementary school students whereas 'interest' level in middle school students and 'common' in high school students. Fourth, the requirements of elementary, middle and high school students were in 'necessary' level to 'common' level. Editional contents such as 'learning sewing machine', 'making cushions'. 'making clothes', and 'making pillow-cover and bag' were low in the degrees of requirement. The correlationship between the amount of learning and the each level of understand, interest and requirement of students was indirect. On the other hand, Among the level of understand, interest and requirement were direct in case of high school students.

The Size, Form of Dental Arch and Occlusion in Dental Hygiene Students in Korea (한국 치위생과 학생의 치열궁 크기 및 형태와 교합)

  • The purpose of this study was to examine the size, form of dental arch and occlusion type in college students in our country and the relationship of the factors. The subjects in this study were 210 selected dental hygiene students. The collected data were analyzed by a statistical package PASW 18.0. When their size, form and occlusion of dental arch were analyzed, the inter-canine width of the maxillary was 34.38 mm, and the inter-first molar width was 52.05 mm. The canine depth was 8.60 mm, and the first molar depth was 28.69 mm. As for the mandibular, the inter-canine width was 26.42 mm, and the inter-first molar width was 44.83 mm. The canine depth was 5.54 mm, and the first molar depth was 24.38 mm. Concerning the form of dental arch, the percentage of normal dental arch in the maxillary stood at 29.0, and that of crowding stood at 60.5. The percentage of spacing stood at 10.5. In the case of the mandibular, the percentage of normal dental arch stood at 29.0; crowding, 55.7; and spacing, 15.2. In relation to occlusion, the percentage of normal occlusion stood at 16.7. As to malocclusion, class I that accounted for 55.7 was most common, and class II and class III respectively accounted for 20.5 and 7.1. When the size of dental arch was compared according to the form and occlusion of it, dental arch was largest (45.95 mm) in size when the form of dental arch in the inter-first molar width of the mandibular was spacing. The size of dental arch was 44.73 mm when its form in the same region was normal, and that was 44.58 mm when its form in the same region was crowding (p=0.032). Regarding the relationship between the form and occlusion of dental arch, crowding was most common when there were class I, II and III of malocclusion both in the maxillary and mandibular.

Changes of Stress Response and Physiological Metabolic Activity of Flounder, Paralichthys olivaceus Following to Food Deprivation and Slow Temperature Descending (먹이제한과 단기 수온하강 조건에서 넙치의 스트레스 반응과 생리학적 대사활성 변화)

  • To find the preliminary environmental conditions for a short-time transport of living olive flounder, Paralichthys olivaceus, the stress response and physiological metabolic activity of the cultured fish to feed deprivation and slow temperature descending ($15.8^{\circ}C{\rightarrow}13.3^{\circ}C$) were monitored for 8 days. The monitored variables were the plasma alanine aminotransferase (ALT), aspartate aminotransferase (AST), glucose (GLU), total protein (TP), electrolytes ($Na^+$, $K^+$, $Cl^-$) and thyroid hormones ($TT_4$, $TT_3$, $FT_4$ and $FT_3$). In food deprivation experiment for 8 days, we did not find any statistical change of level in AST, ALT and electrolytes ($Na^+$, $K^+$, $Cl^-$), but found a significant decrease in TP and GLU. In thyroid hormones, the levels of four hormones in plasma were all showing a tendency to decrease. Especially, $FT_4$ and $TT_3$ were significantly decreased, indicating a withering of physiologic activity. In the temperature test, although no any significant change in AST, TP and electrolytes ($Na^+$, $K^+$, $Cl^-$), we observed a significant decrease of ALT and GLU following to temperature descending from $15.8^{\circ}C$ to $13.3^{\circ}C$ (P<0.05). In the levels of thyroid hormones, any significant change was not observed for experimental period. We conclude that the stress response and physiological activity of olive flounder were more influenced by feed deprivation than slow temperature descending at a transport of living fish, and plasma GLU appears to be sensitive factor to physiological metabolic activity, indicating that it could be used as a monitering mark or index for a health inspection of the fish.

The Utilization Rate of Community Health Practitioner Post by Some Rural Residents and Its Associated Factors (일부 농촌지역 주민의 보건진료소 이용도와 관련요인)

  • To provide data for the improvement of primary health care through the study on the utilization rate of community health practitioner(CHP) post and its related factors toward some rural residents in Mooan County Chollanamdo, a questionnaire survey was made from 382 persons during August 1999. Comparison was made between persons at the seaside area under difficult transportation and persons at the railroad area under convenient transportation. The results are as follows: 1. For the last one year, 83.3% of seaside area residents and 67.0% of railroad area residents used CHP post. As the purpose of visit to CHP post at seaside area, 94.3% visited for medical care, 25.3% for chronic disease control and 22.2% for health counselling and 14.1% for chronic disease control. 2. By simple analysis, sex, age, marital status, educational level, residence area, distance from living village to CHP post, presence of chronic diseases, satisfaction with CHP and confidence on CHP were related significantly with the utilization of CHP post for the last one year. 3. By multiple logistic regression, statistically significant variables related with the utilization rate of CHP post for the last one year were analyzed as age, sex, residence area and distance from living village to CHP post.

Tensile bond strength of chairside reline resin to denture bases fabricated by subtractive and additive manufacturing (적층가공과 절삭가공으로 제작한 의치상과 직접 첨상용 레진 간의 인장결합강도 비교)

  • Purpose: The purpose of this study was to compare and evaluate the tensile bond strength of chairside reline resin to denture base resin fabricated by different methods (subtractive manufacturing, additive manufacturing, and conventional heat-curing). Materials and methods: Denture base specimens were fabricated as cuboid specimens with a width of 25 mm × length 25 mm × height 3 mm by subtractive manufacturing (VITA VIONIC BASE), additive manufacturing (NextDent Base) and conventional heat-curing (Lucitone 199). After storing the specimens in distilled water at 37℃ for 30 days and drying them, they were relined with polyethyl methacrylate (PEMA) chairside reline resin (REBASE II Normal). The subtractive and additive manufacturing groups were set as the experimental group, and the heat-curing group was set as the control group. Ten specimens were prepared for each group. After storing all bound specimens in distilled water at 37℃ for 24 hours, the tensile bond strength between denture bases and chairside reline resin was measured by a universal testing machine at a crosshead speed of 10 mm/min. The fracture pattern of each specimen was analyzed and classified into adhesive failure, cohesive failure, and mixed failure. Tensile bond strength, according to the fabrication method, was analyzed by 1-way ANOVA and Bonferroni's method (α=.05). Results: Mean tensile bond strength of the heat-curing group (2.45 ± 0.39 MPa) and subtractive manufacturing group (2.33 ± 0.39 MPa) had no significant difference (P>.999). The additive manufacturing group showed significantly lower tensile bond strength (1.23 ± 0.36 MPa) compared to the other groups (P<.001). Most specimens of heat-curing and subtractive manufacturing groups had mixed failure, but mixed failure and adhesive failure showed the same frequency in additive manufacturing group. Conclusion: The mean tensile bond strength of the subtractive manufacturing group was not significantly different from the heat-curing group. The additive manufacturing group showed significantly lower mean tensile bond strength than the other two groups.

Growth and Water Use Efficiency of Major Tree Species for Rehabilitation and the Impacts of Planting Trees on Microclimate Condition in Central Dry Zone of Myanmar (미얀마 건조지에서 주요 조림 수종의 생장과 수분이용효율 특성 및 조림이 건조지의 미세기상변화에 미치는 영향)

  • The Bagan, the central part of Myanmar, is dry zone where the mean annual precipitation is less than 600 mm for the last ten years. Forest in this region has been degraded due to biotic and abiotic disturbances. While there have been various efforts to rehabilitate the degraded area, the information on growth and physiological characteristics of planting species and the impacts of planting trees in the region still lacks. Therefore, this study was conducted to determine the growth and physiological water use efficiency characteristics of five species (Azadirachta indica A. Juss., Acacia catechu Willd., Eucalyptus camaldulensis Dehn., Acacia leucophloea (Roxb.) Willd. and Albizia lebbek (L.) Willd.) which are utilized as rehabilitation species in the dry zone and to identify the impacts of tree planting on microclimate change in dry zone. The growth and the foliar carbon isotope composition of seedlings and the above mentioned five species planted in 2005 were measured. And from February 2015 to January 2016, microclimatic factors air temperature and relative humidity at 60 cm and 2 m above soil, soil temperature, soil water contents and precipitation were measured at every 30-minute interval from the two weather stations installed in the plantation located in Ngalinpoke Mt. Range. One was established in the center of A. indica plantation, and the other was in the barren land fully exposed to the sunlight. Among the five species, A. indica and A. lebbek which showed higher water use efficiency could be recommended as rehabilitation species in dry zone. Planting trees in the dry area was shown to affect the change of microclimate with shading effects, declining temperature of the land surface and aridity of the air, and to contribute to conserving more water in soil by preventing direct evaporation and containing more water with fine roots of trees.

Ultrasound Measurement of Coracohumeral Distance in Patients with or without Subcoracoid Impingement (오구돌기하 충돌 증후군 유무에 따른 초음파를 이용한 상완오구돌기 계측)

  • Purpose: The purpose of this study was to evaluate coracohumeral distance (CHD) in patients with or without subcoracoid impingement with hypothesis that patients with subcoracoid impingement would have narrower CHD. Materials and Methods: One hundred twenty-four patients with subacromial impingement were evaluated. The subjects with subcoracoid impingement which was affirmed clinically and confirmed by ultrasound guided subcoracoid injection (n=28) was compared with patients with subacromial impingement only (n=96). Patients with stiffness and rotator cuff tear were excluded. Absolute CHD was measured on magnetic resonance imaging (MRI) axial images and on ultrasound with the humerus in neutral position and internal rotation. Also relative ratio of distance difference (RRDD) defined as the difference of CHD in neutral position and internal rotation compared with absolute CHD in neutral on ultrasound was also measured. Results: The distance measured in neutral position was similar between US imaging and MRI (p>0.05) and both measurements did not have significant difference between the two groups (p>0.05). On ultrasound, the difference in CHD in internal rotation between the two groups nearly met the level of significance (p=0.07). No significant difference of CHD difference in two humeral positions was seen between the two groups. However, RRDD value was significantly greater in subcoracoid impingement group (p<0.05). Conclusion: No significant difference of CHD was seen between the subcoracoid impingement group and the control group. RRDD value was greater in subcoracoid impingement group suggesting that individualized coracohumeral distance in internal rotation should be taken into account when assessing patients with subcoracoid impingement.

Coronary Artery Bypass Grafting in Elderly Patients Older Than 75 Years (75세 이상 고령환자의 관상동맥우회로술)

  • Background: The number of elderly patients undergoing coronary artery bypass grafting (CABG) is increasing. Elderly patients are at increased risk for a variety of perioperative complications and mortality. We identified determinants of operative complications and mortality in elderly patients undergoing CABG. Material and Method: Between January 1995 and July 2003, 91 patients older than 75 years underwent isolated CABG at Asan Medical Center. There were 67 men and 24 women with mean age of $77.0\pm2.4$ years. Thirty clinical or hemodynamic variables hypothesized as predictors of operative mortality were evaluated. Result: CABG was performed under emergency conditions in 5 patients. The internal thoracic artery was used in 85 patients and 10 patients received both internal thoracic arteries. The mean number of distal anastomosis was 3.7 per patient. Operative mortality was $3.3\%$. Twenty-two patients had at least one major postoperative complication. Low cardiac output syndrome was the most common complication, followed by reoperation for bleeding, pulmonary dysfunction, perioperative myocardial infarction, stroke, acute renal failure, ventricular arrhythmia, upper gastrointestinal bleeding, infection, and delayed sternal closure. None were the predictors of mortality. Renal failure, peripheral vascular disease, emergency operation, recent myocardial infarction, congestive heart failure, New York Heart Association (HYHA) class III or IV, Canadian Cardiovascular Society (CCS) angina scale III or IV, and low left ventricle ejection fraction below $40\%$ were univariate predictors of overall complications. Actuarial probability of survival was $94.9\%,\;89.8\%,\;and\;83.5\%$ at postoperative 1, 3 and 5 years respectively. During the follow-up period $93.3\%$ of patients were in NYHA class I, or II and $91.1\%$ were free from angina. Conclusion: Although operative complication is increased, CABG can be performed with an acceptable operative mortality and excellent late results in patients older than 75 years.

Relationship between XRCC1 Polymorphism and Acute Complication of Chemoradiation Therapy in the Patients with Colorectal Cancer (대장, 직장암 환자에서 화학방사선치료의 급성 부작용과 XRCC1 유전자 다형성과의 상관관계)

  • Purpose: It is well known from clinical experience that acute complications of chemoradiation therapy vary from patients to patients. However, there are no known factors to predict these acute complications before treatment starts. The human XRCC1 gene is known as a DNA base excision repair gene. We investigated the possibilities of XRCC1 gene polymorphisms as a predictor for the acute complications of chemoradiation therapy in colorectal cancer patients. Materials and Methods: From July 1997 to June 2003, 86 colorectal cancer patients (71 rectal cancer, 13 sigmoid colon cancer and 2 colon cancer patients) were treated with chemoradiation therapy at the Department of Radiation Oncology, Inha University Hospital. Twenty-two patients were in stage B, 50 were in stage C, 8 were in stage D and 6 patients were unresectable cases. External radiation therapy was delivered with 10MV X-ray at a 1.8 Gy fraction per day for a total dose of radiation of $30.6{\sim}59.4 Gy$ (median: 54 Gy). All the patients received 5-FU based chemotherapy regimen. We analyzed the acute complications of upper and lower gastrointestinal tract based on the RTOG complication scale. The initial and lowest WBC and platelet count were recorded during both the RT period and the whole treatment period. Allelic variants of the XRCC1 gene at codons 194, 280 and 399 were analyzed in the lymphocyte DNA by performing PCR-RFLP. Statistical analyses were carried out with the SAS (version 6.12) statistical package. Results: When all the variables were assessed on the multivariate analysis, recurrent disease revealed the factors that significantly correlated with upper gastrointestinal acute complications. Arg399Gln polymorph isms of the XRCC1 gene, the radiation dose and the frequencies of chemotherapy during radiation therapy were significantly correlated with lower gastrointestinal complications. Arg399Gln polymorph isms also affected the decrease of the WBC and platelet count during radiation therapy. Conclusion: Although the present sample size was too small for fully evaluating this hypothesis, this study suggests that Arg399Gln polymorph isms of the XRCC1 genes may be used as one of the predictors for acute complications of chemoradiation therapy in colorectal cancer patients.
