  • The purpose of this case report is to introduce a case of immediate results with TMJ balancing therapy (TBT) in the treatment of chronic patellofemoral pain (PFP). A 17-year-old female patient with 4-month-old chronic knee pain that was unresponsive to conventional treatment was treated with TBT for a total of 5 sessions. The primary outcomes were evaluated using the Numeric Rating Scale (NRS) and the Korean version of the Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC). Following the first treatment, the NRS decreased rapidly, and the KWOMAC improved after a total of five treatments. These findings suggest that TBT can be an effective pain management option for chronic PFP patients with temporomandibular joint malposition. However, as this study is a report of a single case, further research is necessary.

  • Objectives : The purpose of this study was to examine the relationship of stress symptoms, oral habits and temporomanbibular joint symptom among 20-30 ages adults. And it's also meant to investigate the direct and indirect influence of these factors by using a path model to determine their causal relationship. Methods : The subjects in this study were 287 selected 20-30 ages adults, on whom a survey was conducted from June 15 to July 10, 2014. The data were analyzed using SPSS 18.0(SPSS 18.0 K for window, SPSS Inc USA) and IBM SPSS Amos 18.0(SPSS Inc, Chicago, IL, USA). Results : There were significant differences in oral habits, TMJ symptoms and stress according to gender. There were significant differences in oral habits, TMJ symptoms, physical and psycho-emotional symptoms according to Systemic disease. And stress, physical, psycho-emotional symptoms, oral habits and TMJ symptoms were correlated to one another. Stress exerted a direct influence on physical, psycho-emotional symptoms, and psycho-emotional symptoms had a direct impact on physical symptoms, oral habits and TMJ symptoms. Physical symptoms exercised a direct influence on oral habits and TMJ symptoms, and oral habits affected TMJ symptoms in a direct way. physical, psycho-emotional symptoms and oral habits served as parameters that produced partial mediation effects, and the two factors had an indirect impact on TMJ symptoms. Conclusions : It's found that stress exerted direct and indirect influence on oral parafunction and TMJ symptoms. Like other diseases, oral habits and TMJ symptom that stem from stress is likely to lead to chronic diseases if the two are not noticed at the right time. Therefore individual people should try to get rid of stress in a manner to be appropriate for their own characteristics in order to maintain their oral health.

디지틀적외선 체열촬영은 DTI-16-DOREX21(Dorex Inc., U.S.A) 기기를 사용하였다.

  • There are many studies on the indications and efficacy of splint therapy commonly used in patients with temporomandibular disorders (TMD). However, there have been no studies on the splint weaning in terms of the splint use tapering period in relation to symptom improvement of TMD. This retrospective study aims to analyze a proper splint weaning method in patients with TMD based on symptom improvement. Materials and Methods: The authors examined 130 TMD patients with TMJ disorders, masticatory muscle disorders, and clenching/bruxism who had received splint therapy (occlusion stabilization splint, anterior positioning splint) of patients who visited the Department of Orofacial Pain and Oral Medicine at Yonsei University Dental Hospital from 2015 to 2020. They were evaluated according to the method to wean splints. Results: The mean splint therapy period was 29.0 months, during which patients wore splints 7 days a week for 8.4 months, 3 - 4 days a week for 9.5 months, and finally 1 - 2 days a week for 11.1 months (a total of 29.0 months, about 2.5 years). Conclusion: It seems that TMD symptoms can be alleviated and side effects such as occlusal change can be minimized if patients wear a splint 7 days a week for the first 6 months, followed by 3 - 4 days a week for the next 6 to 18 months, and finally 1 - 2 days a week after 18 months.

  • Objectives: This study aimed to assess the characteristics of temporomandibular disorder (TMD) symptoms and to determine the correspondence between TMD symptoms and clinical examination findings. Material and methods: A total of 218 patients (143 females and 75 males; age=$31.3{\pm}14.0$) were enrolled in this study who completed a questionnaire and underwent a clinical examination and radiographic assessment. Patients were asked about all the symptoms and complaints, including onset or duration, and locations of the symptoms. Clinical examination included amounts of mouth opening, palpable temporomandibular joint (TMJ) sounds, and tenderness to palpation of the TMJ and all masticatory muscles. Tenderness scores obtained from palpation of the masticatory system were summated to define the variables for further analysis. Results: Pain was the most frequently reported symptom (78.9%), followed by joint sounds (45.4%), and limitation in mouth opening (17.0%). Jaw pain comprised 91.9% of pain complaints. The subjective intensity of jaw pain was low to medium in most patients (93.7%), but it was poorly correlated with the sum of tenderness scores of the TMJ and masticatory muscles (Kendall tau = 0.084). In contrast, the side in which pain was reported by patients was well associated with the clinical examination results (pain of the right side, p < 0.001, and left side, p < 0.001). There was moderate agreement in TMJ sounds between the side identified by patients as symptomatic and clinical examination findings (kappa = 0.482). Finally, patients who complained of restricted mouth opening showed about a 10 mm less opening in all three measurements, compared to other patients (p < 0.001). Conclusion: The most frequent symptoms reported by TMD patients were jaw pain, TMJ sounds, and mouth opening limitation. The side of jaw pain, the side of TMJ sounds, and the presence of opening limitation were highly concordant between symptom reports and examination findings.
