Effects of a Peroxidase-System-Containing Toothpaste on Whole Saliva in Vivo (Peroxida System을 함유한 세치제가 생체에서 전타액에 미치는 영향에 대한 연구)

  • Peroxidase system은 구강내 고유의 항균체계로서 항세균물질인 $HOSCN/OSCN^-$을 생산하여 다양한 그램 양성 그리고 그램 음성 세균에 대해 살균 및 정균작용을 나타낸다. 본 연구는 peroxidase system을 함유한 치약을 사용한 후 건강한 대상자의 자극성 전타액내 생성되는 $HOSCN/OSCN^-$의 양을 측정하여 최대 생성시간을 결정하였다. 그 후 건강한 대상자에게 임의로 실험치약과 대조치약을 할당하여 2주간 사용하도록 한 후 $HOSCN/OSCN^-$ 최대 생성 시간에 타액을 채취하여 치약 사용전과 2주후의 타액분비량, pH, 점조도 그리고 S. mutans와 lactobacilli에 대한 항균효과를 비교하였다. 구강건조증 환자를 대상으로 한 두 번째 연구에서도 peroxidase system을 함유한 세치제를 사용한 후 생성되는 $HOSCN/OSCN^-$의 최대생성시간을 측정하였으며, peroxidase system을 함유한 세치제가 S. mutans와 lactobacilli에 미치는 영향을 양치전과 양치후 $HOSCN/OSCN^-$의 최대생성시간에 타액을 채취하여 비교하였다. 이와같은 실험을 통해 다음과 같은 결론을 얻었다. 1. 건강한 대상자의 자극성 전타액내의 $HOSCN/OSCN^-$의 농도는 양치후 30분 후 대조군에 비해 peroxidase systems을 함유한 세치제를 사용한 군에서 유의성있게 높게 나타났다 (p < 0.05). 2. 건강한 대상자의 자극성 전타액의 타액분비량, pH, 타액점조도는 peroxidase system을 함유한 세치제를 2주간 사용한 후에도 유의한 변화를 나타내지 않았다. 3. 2주간 실험치약을 사용한 후 건강한 대상자의 자극성 전타액내 S. mutans와 lactobacilli의 양은 peroxidase system을 함유한 세치제를 사용한 군에서 상대적으로 낮게 나타났으나 통계학적 유의성은 없었다(p = 0.206 : lactobacilli, p = 0.944 : S. mutans). 4. 구강건조증 환자의 자극성 전타액내의 $HOSCN/OSCN^-$의 농도는 양치후 30분(p < 0.01)과 60분(p < 0.05)에서 대조군에 비해 peroxidase system을 함유한 세치제를 사용한 군에서 유의성있게 높았다. 5. 구강건조증 환자의 양치 후 30분에 채취한 자극성 전타액내의 S. mutans와 lactobacilli의 양은 peroxidase system을 함유하거나(p < 0.01) 함유하지 않은(p < 0.05) 치약을 사용한 군 모두에서 양치전에 비해 유의성있게 감소하였다. 6. 양치후 30분에 채취한 구강건조증 환자의 자극성 전타액내 S. mutans 양은 peroxida system을 함유한 세치제를 사용한 군에서 대조군에 비해 유의성있게 낮았다(p < 0.05). 7. 양치후 30분에 채취한 구강건조증 환자의 자극성 전타액내 lactobacilli양은 peroxidase system을 함유한 세치제를 사용한 군에서 대조군에 비해 상대적으로 낮게 나타났으나(p = 0.067) 통계학적 유의성은 없었다.

Detection of Helicobacter pylori in Saliva of Patient with Oral Lichen Planus (구강 편평태선 환자의 타액에서 Helicobacter pylori의 검출)

  • Lichen planus is a common, chronic inflammatory disease of the skin and mucous membrane for which no precise causes have been confirmed. But it is often connected with infections. Helicobacter pylori(H. pylori) among various bacteria has been associated with the cause of gastritis, peptic ulcer and gastric cancer. Considering the similarities of histological features between gastric ulcer and oral ulcers, it is resonable to assume that H. pylori might also be involved in the development oral mucosal ulceration. So we employed this study to investigate the possible involvement of H. pylori in the aetiology of erosive oral lichen planus. We analyzed detection rate of H. pylori in saliva of patients with erosive oral lichen planus by nested PCR. As a result, it revealed a significant difference statistically by showing positivity in 16 to 21(76.2%) saliva samples of patients group and in 11 of 44(25%) saliva samples of control group(P>0.001). We were able to suppose that H. pylori in saliva can be related to cause of erosive oral lichen planus.

Comparisons of the Effects of A-solution and 0.9% Normal Saline Oral Gargling on Xerostomia, Halitosis, and Salivary pH in Nursing Students (아로마용액과 0.9% N/S 구강함수가 간호대학생의 구강건조, 구취 및 타액 pH에 미치는 효과 비교)

  • Purpose: The purpose of this study was to compare the effects of gargling with A-Solution and 0.9% normal saline on xerostomia, halitosis, and salivary pH among nursing students. Methods: The study design was a randomized, controlled trial. The experimental group received oral gargling treatment for 15 seconds with A-solution while the control group received oral gargling treatment for the same time with 0.9% normal saline. Outcome variables were measured at baseline, 60 minutes, and 120 minutes. Results: The groups were homogeneous in general characteristics, and the outcome variables before the treatment between the experimental group and the control group. 1) There were no significant differences in xerostomia, halitosis, and salivary pH between the two groups. 2) There were no significant differences in xerostomia, halitosis, and salivary pH based on the time. 3) There were significant interactions between group and time in subjective halitosis (F=3.766, p=.034) only. Conclusion: These findings indicate that oral gargling with Aroma solution and 0.9% normal saline could have the same effect of reducing xerostomia and halitosis in nursing students.

Effects of Jeju Samdasu Forest Bathing on Saliva Cortisol and Vascular Conditions (제주 삼다수 숲 산림욕이 타액 코르티솔과 혈관 상태에 미친 효과)

  • Purposes: This study investigated how saliva cortisol, blood vessel health type, and blood vessel health index changed after forest bathing with subjects in the Samdasu Forest. Methods: After the forest bathing, we measured cortisol change in saliva and heart rate variability with subjects. Results: In the experimental group who took forest bathing for two hours in the Samdasu Forest, the stress cortisol content secreted in saliva decreased significantly compared to the control group (p<.05). Besides, the vascular age type significantly increased (p<.03), and the vascular age index significantly decreased (p<.032).There were no significant results in the control group. Conclusions: It is evaluated that forest bathing in Samdasu Forest in Jeju Island had the effect of reducing saliva cortisol and increasing blood vessel age.

The Relationship between Burning Mouth Syndrome and Helicobacter pylori in the Oral Cavity (구강작열감증후군과 구강 내 Helicobacter pylori의 상호관련성)

  • Helicobacter pylori (H. pylori) is bacterial infection, with more than half of the world population infected and relates to many oral disease such oral lichen planus, recurrent aphthous ulceration, periodontal disease and halitosis and so on. Burning mouth syndrome(BMS) is defined as a burning sensation of the oral mucosa, lips, and/or tongue, in the absence of specific oral lesions. The etiology of BMS is suggested local, systemic and psychological factors and researchs related BMS and to infection of H. pyloir in the oral cavity are few. The purpose of this study was to evaluate relationship between burning mouth syndrome and H. pylori in the oral cavity. We recruited 21 subjects with burning mouth syndrome and 21 subjects as control group. Samples in the oral cavity were taken area of buccal mucosa, dorsum of the tongue and saliva. We analysed samples by nested polymerase chain reaction(PCR). The results were as follows: 1. Among 21 patients with burning mouth sydrome and 21 subjects of control group, 6(29%) and 3(14%) were positive respectively(P>0.05). 2. In detection rate of H. pylori in area taken sample, 3(14%), 2(10%) and 4(19%) were positive in buccal mucosa, dorsum of the tongue and saliva of patient and 2(10%) and 1(5%) were positive in dorsum of the tongue and saliva of control group(P>0.05). Conclusively, we can guess that H. pylori in the oral cavity is not related with burning mouth syndrome.

A Study on the Relationship between Halitosis Developments and Oral Environmental (구취발생과 구강환경의 연관성에 관한 연구)

  • The purpose of this study was to analysis know the important oral environmental factors which affect halitosis components of the adult in order to provide basic data for halitosis prevention and establish a device to eliminate halitosis efficiently. The 97 adults who visited at the Dental Clinic in Metropolis (M=68, F=30) participated in this study that performed from March in 2009 to in 2010. The obtained results through items as caries status, periodontal status, salivary flow, the viscosity, pH, Snyder test, plaque deposit, tongue plaque and halitosis check were as followings. The average shame of halitosis components appeared at hydrogen sulfide 36.71 ppb methyl mercaptan 31.46ppb dimethyl sulfide 54.33 ppb and Ammonia 22.60 ppm. The normality and the detection comparative result dimethyl sulfide above reverse appeared highly at 46.9%, ammonia appeared highly at 52%. According to the Hydrogen sulfide level was a high relationship among age, CPI, tongue coat status, DMFT index which were statistically significant (p<0.05). According to the quantity of hydrogen sulfide level there was relationship where tongue coat status Saliva flow rate considers statistically(p<0.05). The quantity of methyl mercaptan level there was relationship where Dimethyl sulfide level, tongue coat status, Saliva flow rate considers statistically(p<0.05). The quantity of Dimethyl sulfide level there was relationship where Hydrogen sulfide level, ammonia level, tongue coat status, Saliva pH and Saliva flow rate considers statistically(p<0.05). Ammonia level there was relationship where Methyl mercaptan level, CPI, and Saliva flow rate considers statistically(p<0.05).

Variations of Oral Cavity Environment according to Sodium Lauryl Sulfate Concentration of Toothpaste (세치제의 Sodium Lauryl Sulfate함유 정도에 따른 구강환경변화)

  • This study used 3 kinds of experimental toothpaste prepared with different contents of SLS, i.e. A (0%), B (1.1%), C (2.2%). These 150 subjects were subdivided again into three groups. After 4 weeks application of the three kinds of toothpaste, it was found that there were differences in dental plaque test (PHP) among the 3 groups; that is; a higher SLS content was associated with a lower PHP index. In addition, it was found that all 3 groups showed a reduction in simplified oral hygiene index (OHI-S). After 4 weeks application of the three groups of toothpaste, it was found that a higher SLS content was associated with a lower salivary flow, but there was no significant variation in salivary mucosity and pH. Further, it was found that SLS was negatively correlated with salivary flow, which supports the theory that SLS may induce xerostomia.

Risk factors for the development of oral bacteria in workers according to oral environment (근로자의 구강환경요인에 따른 구강세균 발생의 위험요인)

  • This research examined the oral environmental factors to identify the risk factors for oral bacteria detection. This study comprised of 60 office workers aged between 20 and 65 years, and was performed from January 15 to February 28, 2015. The study variables measured were the stimulated and unstimulated salivary flow rates, salivary buffering, saliva pH, dry mouth at the dorsum of the tongue and the sublingual region, halitosis, and the degree of tongue-coating as oral environmental factors. To identify the presence of oral bacteria, pathogens were detected by extracting the gDNA of the resting salivary flow rate. The risk of S.mutans detection was 15 times higher with smokers, 1.3~1.6 times higher when the resting or stimulated salivary flow rate was reduced by 1 mm. The risk of P.intermedia detection was 13 times higher in smokers, 4.3 times higher as the severity of oral dryness was lowered, and 4 times higher for adults with a tongue coating than those without. In addition, the risk of detecting TM7 was 5.5 times higher as sublingual dryness was decreased by 1mm. The oral bacterial count will be reduced considerably by smoking cessation education and habits that facilitate a salivary flow rate. Furthermore, adults with good and well-managed dental hygiene are anticipated to have less oral bacteria and fewer dental diseases.

The Effect of Subjective Xerostomia and Salivary pH in Salivary Glands Stimulated by Laughter Therapy in Frail Elderly Women (웃음치료를 통한 침샘자극이 허약노인의 구강건조와 타액 pH에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of subjective xerostomia and salivary pH in salivary glands stimulated by laughter therapy in frail elderly women. Methods: The research used a one-group pretest-posttest design. Data were collected from July 2, 2015 to September 30, 2015. A sample of 41 frail female elderly patients was recruited at A Nursing Home in K City, Korea. We measured xerostomia and salivary pH using a questionnaire and BCP test paper (pH 5.6~7.2). Laughter therapy was given once a week for four weeks (3 items). Data were analyzed through descriptive statistics, independent t-test, ANOVA, paired t-test and Cronbach's using the SPSS 18.0 program. Results: There were significant differences in salivary pH according to oral health (t=-2.06, p<.05). There were significant differences in xerostomia (t=4.41, p<.001) and salivary pH (t=-7.94, p<.001) after salivary glands stimulated by laughter therapy. Conclusion: Salivary glands stimulated by laughter therapy improved xerostomia and salivary pH of the frail elderly. Therefore, salivary glands stimulated by laughter therapy may be useful in promoting and maintaining oral health among the frail elderly in the rapidly increasing population of old people.
