Clinical Characteristics of Korean Male Patients with Eating Disorders (남성 섭식장애 환자의 섭식장애 병리의 특징)

  • Objectives : The aims of present study was to investigate clinical characteristics of male patients with eating disorders. Methods : The present study included 32 male patients with eating disorders and 75 female patients with eating disorders, recruited from an eating disorders specialized clinic in a university hospital, Seoul, Korea. We compared clinical characteristics of eating disorders and comorbid conditions of depression and anxiety between men and women with eating disorders. Correlations between eating disorders psychopathology in men with eating disorders and their weight suppression(WS) were investigated. Results : There was no difference in age at presentation, age of onset, illness duration, and body mass index at presentation between genders. Male patients with eating disorders had higher rates of premorbid overweight or obesity than female patients with eating disorders had. WS in the male patients was associated with the severity of their eating disorders. Conclusions : The present study provides grounds for improved understanding for clinical features of eating disorders in males.

Analysis of Effects of Yoga and Music Therapies on the Care of Alcohol dependents (알코올 의존자에 대한 요가 및 음악 치료의 효과 분석)

  • Yoga meditation and music therapy are considered to be appropriate for the spiritual and mental treatment for the patients and employed in many clinics and rehabilitation centers as an alternative medicine. This study aims at the analysis of the effects of yoga and music on the treatment of alcoholism. For this, yoga and music treatments were executed for the patients hospitalized in an alcohol clinic hospital in Gimhae district. Only the patients who were judged as alcohol dependents by a psychiatrist with a diagnostic criteria of DSM-IV participated in the study. During five weeks, from November 10, 2009 to December 29, yoga program was practiced two times per week, totaling 10 times, and music therapy was practiced one time per week, totaling 5 times. Among the patients participated in the programs, 44patients were selected finally. They are 16 patients who performed all the yoga programs, and 28 patients who fulfilled all the music therapies. A survey was conducted on them to estimate the effects of yoga and music treatments for the three cases of 'angry behavior', 'anger expression', and 'psychological state at interview'. The results show that yoga and music treatments have positive effects on the patients, although the statistical significance is not so great. However, the more reliable and significant results could be obtained in further studies by prolongation of the practice period, and by introducing the clinical information such as brain mapping to compensate the emotional bias of the patients.

A Psychological Analysis of the Recurred TMD Patients by SCL-90-R (SCL-90-R을 이용한 측두하악장애 재발환자의 심리학적 분석)

  • Personality characteristics of recurred TMD patients were analyzed psychologically by using the SCL-90-R. 27 recurred TMD patients, 45 new TMD patients, control I were subjected at Orofacial pain clinic, Department Of Oral Medicine and Health Promotion Center, Pusan National University Hospital during the period from 2005 to 2006. 50 general dental patients, control II were subjected at a local dental office in Pusan during the same period. The obtained results were as follows. 1. Mean values of T-scores on 9 basic scales in all the groups were within normal range. 2. The T-scores of IS, Dep, Phob, Psy in general Dental patient group were significantly higher than those in recurred TMD patient group. 3. As compared with recurred TMD patient Group & control I group by sexual subclass, there was no significant difference of the scales. 4. As compared with acute and chronic groups, there was no significant difference of the scales in recurred TMD patient group. However, the T-scores of Som, Par, Phob, Psy in chronic new TMD patient group were significantly higher than those in acute new TMD patient group, control I group.

The Analysis on Clinical Characteristics of Out-Patients of Oriental Thyroid Clinic (한방병원 갑상선 클리닉 내원 환자들의 임상적 특성 분석)

  • Objective : Thyroid diseases are one of the most common endocrine disorders. However, the first chosen-treatments including anti-thyroidal and hormone-supplement therapy in western medicines, sometimes have failed to improve abnormal thyroid hormone secretions and clinical symptoms. Therefore, there has been a growing expectation for Oriental Medicine to take the role of alternative therapy. However, there has been little analysis of clinical characteristics. In this study, clinical characteristics of thyroidal disease patients who visited Kyung-Hee oriental hospital from January to July 2004 were analyzed. Methods : A total 86 patients diagnosed as hyperthyroidism, hypothyroidism or thyroid nodule were selected. Clinical history, including sex, age, symptoms, disease duration and methods of previous therapy and thyroidal test, such as thyroid function test (TFT), thyroid scan and other thyroid examinations. Results : In distribution of onset age and sex, the patients who have visited oriental hospital with thyroidal diseases showed similar characteristics with typical patients. In clinical symptom, the most frequent clinical sign was fatigue in both hyperthyroidism and hypothyroidism, which differs from the classical characteristics of those diseases. Most patients complained the unresolved symptoms, some of which are attributable to abnormal thyroid hormone secretions, in spite of western therapy with anti-thyroidal and hormone supplement agents. Conclusions : In oriental therapy, it will be necessary to consider the unique clinical features of thyroidal diseases different from classical patterns, and this study suggests that oriental therapy is a good alternative method for patients whose first treatment with western agents failed. Furthermore, more public information to these patients about oriental medicine would be helpful to increase cure rates, and give various choices to patients with thyroid diseases.

Analysis of Questions and Answers Posted on the Internet Blogs about Prenatal Genetic Diagnosis and Screening (블로그를 통해 본 산전 기형아 검사와 양수검사에 대한 질문과 댓글 분석)

  • The purpose of this study was to identify pregnant women's needs for information on prenatal genetic diagnosis and screening. This study is consisted of two phases. In the first phase in December 2011, six blogs featuring questions and answers on prenatal genetic diagnosis and screening were selected from four major search engines in Korea by using the keywords "prenatal genetic diagnosis," "prenatal genetic screening", and "amniocentesis." An analyzing framework was constructed on the basis of 389 posts on six blogs between November 2006 and October 2011. In the second phase, the contents of the "MomsHolicbaby" blog posted from November 2010 to October 2011 were reviewed. Then, pregnant women's questions on prenatal genetic diagnosis and screening (100 questions) and amniocentesis (200 questions with 1,665 answers) were analyzed using descriptive statistics. Among posters who had ever been recommended to undergo amniocentesis, 56.5% described feelings of anxiety, 25.5% did not know the purpose of the test, and 33.9% refused to undergo the test. Among 295 posters answering questions about amniocentesis, 61.4% disagreed with undergoing the test. The results show that there is a need for healthcare professionals to provide more educational and emotional support to pregnant women considering prenatal genetic diagnosis and screening. Providing online health information can be integrated into prenatal genetic education for pregnant women as well as nurses. In addition, prenatal women's preferences about undergoing amniocentesis should be reflected in the current legal discussion on criteria for termination of pregnancy.

A Shared Electronic Medical Record for Lung Cancer Clinic (폐암 클리닉을 위한 공유 전자의무기록)

  • Since the year 2000, lung cancer has become the leading cause of cancer death in South Korea as in many other parts of the world. The current multidisciplinary approach for lung cancer includes a wide range of modalities, not only surgery, radiotherapy, medical drug therapy but also pain control, as well as social and psychological support. Therefore, thoracic surgeons, radiologists, nuclear medicine specialists, anesthetists, psychologist, nurses and social workers as well as medical doctors care for lung cancer patients. Sharing a common treatment protocol and optimal communication are vital aspects of shared care both from a medical and cost-effectiveness point of view. We developed a shared electronic medical record (SEMR) for treating patients with lung cancer in a university hospital to facilitate the sharing protocols and communications between doctors involved in a lung cancer clinic. A SEMR system was developed within a order communication system(OCS) for a lung cancer clinic. The records of radiological, laboratory and pathological studies as well as the records of surgery, chemotherapy, and radiotherapy were stored and presented to all doctors who treat the same patient. Every doctor was allowed to change his/her own records. They could review other doctor s records but could not alter them. With the SEMR, it was expected that the time to complete the medical records for one patient could be reduced because it was easy to review all the data from the other doctors who share the same patient. In addition, the confidence of the doctors who share a common treatment protocol would be higher. Therefore, a shared electronic medical record is expected to improve the quality of patient care.

The Esthetic Impact of Extraction and Nonextraction Treatments on Korean People (발치, 비발치를 동반한 교정치료 전후의 안모의 변화에 관한 인지도)

  • The concept of extraction in orthodontic treatment has been changed many times. Even today, criteria of extraction or nonextraction is still changing. In this study, changes depending on the evaluator's perception of treatment outcomes were compared in both extraction and nonextraction cases. In this study, premolar extracted 59 patients and nonextracted 60 patients, totally 119 patients who finished orthodontic treatment in Dankook University Dental Hospital orthodontic clinic were enrolled. Evaluation sections made up of specialists and laypersons assessed soft tissue traced from lateral cephalometric radiographs with visual analogue scale before and after the treatment. And the results were statistically analyzed. Thus, the conclusions drawn are as follows: 1. Average score is 5.76 in extraction, which is larger than 5.28 of nonextraction case. Improvement of facial profile was more favorably accepted in extraction case. 2. 5.875 in the group of specialists were higher evaluation than 5.165 in the group of layperson. 3. Specialists gave significantly higher ratings in the extraction than nonextraction. 4. A higher rating in extraction case of the layperson group has no significant difference with nonextraction case. 5. Nonextraction patients were given higher ratings from specialist group. 6. A higher rating of specialist group in extraction case has no significant difference with layperson group.

A Study on the Correlation of Hormonal Status with Depression - Anxiety Traits in Menopausal Women (폐경기 여성에서 호르몬 상태와 우울 및 불안 성향간의 상관관계 연구)

  • Objects : There has been the controversy that menopause may or may not influence the psychological distresses. The purpose of this study was to investigate the correlation between the hormonal status and traits of depression & anxiety in menopausal women. Method : Among the women attending menopausal clinic. menopausal women, defined as who having a amenorrhea for more than 12 months, were selected as a study group(n=83). The control group(n=73), who visited to screen the cervix cancer with regular menstruation, hod no history of hormone replacement therapy. Individual data were collected by self-reporting questionnaires. Depression state was evaluated by Beck Depression Inventory(BDI) and anxiety state by Stale Trail Anxiety Inventory(STAI), and the female hormones such as E2, FSH, and LH were obtained by blood sampling at visting clinic. Statistically the data were processed by t-test and pearson correlation analysis(p<0.05). Results : 1) The mean age of menopausal was 45.3 years. 2) In menopausal women the scores of BDI & STAI were significantly higher than in control group(p<0.05). 3) There were significant differences between menopausal and control group by E2 & FSH. 4) There was no correlation between female hormonal status and the STAI & BDI scores in both groups. Conclusion : Although menopausal women had more traits of depression & anxiety than control group, it was difficult to insist that the lack of estrogen was the only etiological factor for the traits of depression & anxiety in menopausal period. In further studies, we must consider another factors, including biological, sociocultural, psychological factor, as the cause of psychological symptoms during menopausal period.

The Differences of Psychological Symptoms According to the Level of Parenting Stress for Mothers of Infants With Autism Spectrum Disorder (자폐스펙트럼 장애 영유아 어머니의 양육 스트레스 수준에 따른 심리적 증상의 차이)

  • Objective : The purpose of this study was to examine differences of psychological symptoms for mothers of infants with autism spectrum disorder(ASD) according to the level of parenting stress. Methods : The subjects were seventy-two mothers of infants with ASD(aged 22 months to 71 months) completed Korean Parenting Stress Index (K-PSI) and Minnesota Multiphasic Personality Inventory(MMPI-2). The data was analyzed using SPSS 23.0 program. Results : First, total and parent domain of parenting stress were positively correlated with several clinical scales of MMPI-2. However, there was no significant association between the child domain of parenting stress and clinical scale MMPI-2. Second, as a result of verified the difference of psychological symptoms according to level of total and parent domain of parenting stress, high-risks group was significantly higher scores in several clinical scales of MMPI-2 relative to those in normal range group. However, regarding the association between the child domain of parenting stress and psychological symptoms, there was no significant difference in psychological symptoms between high-risks group and normal range group. Conclusions : The results of this study have implications for verified the difference of psychological symptoms according to level of parenting stress among mothers of infants with ASD.

Differences in Social Maturity and Behavioral Problems According to the Level of Sleep Problems in Infants With Autism Spectrum Disorder (자폐스펙트럼장애 영유아의 수면문제 경계선 및 임상 수준 집단과 정상 수준 집단 간 사회성숙도와 문제행동의 차이)

  • Objective : The purpose of this study was to examine differences in social maturity and behavioral problems according to the level of sleep problems in children with ASD. Methods : The participants were 102 mothers of infants with ASD aged 1-5 years. The Social Maturity Scales (SMS) and Child Behavior Checklist 1.5-5 were used as the measuring tools. Results : The level of sleep problems in infants with ASD was 56.58T on the sleep problems scale. The participants were grouped based on scores on the sleep problems scale: those with scores <65 (good sleepers) or scores ≥65 (poor sleepers). Sleep problems significantly correlated with all the scales on the CBCL. However, no association was found between sleep problems and social quotients. Poor sleepers achieved significantly lower scores on the social quotient scale of the SMS than good sleepers. Poor sleepers achieved significantly higher scores in internalizing problems, externalizing problems, and DSM-oriented scales on the CBCL compared to those in the good sleepers. Conclusion : The significance of this study is that it has verified the severity of sleep problems in infants with ASD and has examined the differences in social maturity and behavioral problems between poor sleepers and good sleepers.
