Relationship between the Deposition of Tear Constituents and the Adherence of Candida albicans according to Soft Contact Lens Materials and Pigmentation (소프트콘택트렌즈 재질과 착색에 따른 눈물성분 침착과 칸디다균 흡착의 상관관계)

  • Purpose: The aim of this study was to figure out how the characteristics of soft contact lens materials and pigmentation affect the adherence of C. albicans on soft contact lenses pre-deposited with tear constituents. Methods: The adherent number of C. albicans on clear soft contact lenses (hereinafter clear lenses) and circle soft contact lenses (hereinafter circle lenses) made of etafilcon A, hilaiflcon B and nelfilcon A, respectively, was measured before and after the deposition of artificial tear. Also, bacteria adherence on lenses were observed by a scanning electron microscope. Results: Adherence of C. albicans was significantly different according to lens materials. The amount of adsorption was not different between clear lenses and circle lenses made of etafilcon A however, the number of bacteria absorption was bigger in hilafilcon B and nelfilcon A lenses. More absorption of C. albicans was found in the non-pigmented central area compared the pigmented area, and non-pigmented peripheral area has more bacterial absorption than non-pigmented central area. The number of C. albicans decreased in the case that tear protein was pre-deposited. The maintenance of antibacterial activity against C. albicans was different according to lens materials thus, etafilcon A has the longest period of its maintenance. Conclusions: It was revealed that the number of C. albicans was different according to the characteristics of lens materials, pigmentation or non-pigmentation, the pigmented area of soft contact lenses. Thus, it is suggested that the management method should be different according to the adsorption characteristics of C. albicans.

  • Purpose: The study was aimed to figure out the effect of materials and pigmentation of soft contact lens on the adherence of Staphylococcus aureus upon soft contact lenses deposited with tear components. Methods: The number of adherent S. aureus on clear and circle soft contact lenses made of etafilcon A, hilafilcon B, nelfilcon A was measured before and after incubation in artificial tear. Furthermore, the denaturalization level of tear protein with time after incubation in artificial tear was estimated by electrophoresis. Results: The adherence of S. aureus was significantly different according to the lens materials. The pattern of bacterial adherence on clear and circle contact lenses was different. That is, the adherent amount of S. aureus was somewhat larger on circle lens made of etafilcon A however, amount on circle lenses made of hilafilcon B and nelfilcon A was 89.3% and 71.3% of the number on clear lenses. When the tear protein was deposited on contact lenses, the number of adherent bacteria decreased and its degree was varied according to the lens material. The degree of decrease was the biggest in clear soft lens made of etafilcon A. Anti-bacterial effect of tear protein decreased with time after deposition of tear protein on soft contact lens and the amount of lysozyme also decreased. The reduction of anti-bacterial effect and quantity of lysozyme was different according to contact lens materials and pigmentation. Conclusions: It was revealed that the adherence of S. aureus depends on contact lens materials and pigmentation, and the specification of lens material affects more on adherence of S.aureus than pigmentation. It was further figured out the denaturalization level of anti-bacterial protein on soft contact lens varies according to lens materials and pigmentation, which produces an effect on the quantity of bacterial adherence.

A Comparison of the Movements of Circle Contact Lens and Soft Contact Lens with Identical Material on Cornea (써클 콘택트렌즈와 동일 재질 소프트 콘택트렌즈의 각막에서의 움직임 비교)

  • Purpose: The present study was conducted to compare any difference caused by pigmentation in the centrations and movement patterns on the cornea between circle and soft contact lens with identical material and parameters during lens wearing. Methods: Soft and circle contact lenses with identical material and parameters were applied to twenty eyes with normal tear volume for 3 hrs a day during a total of 5 days and then their lens centrations, the moving distances and rotations by repeat blinking were compared. Results: The lens centration beyond the fitting criteria was shown in initial wears of both soft contact lens and circle contact lens. However, the centration of soft contact lens was changed to the pupil center for being suitable to the fitting criteria with longer wearing time. On the other hand, the decentration of circle contact lens in the horizontal direction was still presented even with longer wearing time. The moving distances of soft contact lens and circle contact lens decreased with the expand of lens wearing but were not significantly different between total wearing period and daily wearing time. The rotation of circle contact lens by repeat blinking was significantly different from that of soft contact lens even when the wearing period was extended. Conclusions: We revealed that the lens centration and movement of circle contact lens on cornea were different from those of regular soft contact lens resulting in bigger difference with the expand of wearing period in the study.

The Effect of Circle Contact Lens on the Stability of Tear Film (써클콘택트렌즈가 눈물막 안정성에 미치는 영향)

  • Purpose: The effect of pigmentation in circle contact lens on lens wearers' tear film stability was investigated in this study. Methods: Non-invasive tear film break-up times of 54 normal eyes before and after lens wearing in twenties and the portion of first tear breakup were measured. The frontal and back surface roughness of circle contact lens was further observed with a scanning electron microscope. The clear lens which was made of hilafilcon B and circle contact lenses which were made of hilafilcon B, polymacon and copolymer of HEMA NVP were tested. Results: Non-invasive tear film breakup time was significantly reduced compared with it before lens wearing. In some case, statistically significant difference in tear film breakup time was also observed between circle contact lenses. The tear file break-up was usually early started in lower than upper portion, and in temporal than nasal portion although there was variation in the lens material. The tear film break-up of circle contact lenses made of hilafilcon B was more commonly observed in peripheral portion(pigmented area) compared with clear contact lens made of same material. Conclusions: The present study revealed that the stability of tear film was affected by wearing circle contact lens and the result seems to be used as the basis for the problem solving that can occur due to the pigmentation when wearing circle contact lens.

Changes of Lens Morphology and TBUT by Dehydration of Soft Contact Lens (소프트콘택트렌즈 건조로 인한 렌즈 형태 및 TBUT의 변화)

  • Purpose: This study was performed to evaluate the changes of lens morphology and tear stability during wearing soft contact lenses (SCLs) which were kept in drying condition like dry eye or became to be dried due to heedless care. Method: SCLs having different water content, thickness or material were rehydrated after being dehydrated artificially 2 or 4 times, and estimated their diameter and radius. Furthermore, the changes of tear film break-up time (TBUT) during SCL wearing were also measured. Result: Due to the dryness, the diameter of both 70% water content SCL and 59% water content SCL decreased, but the decrement was larger in 59% water content SCL. The more 59% water content SCL was dehydrated, the more its radius changed. However, the radius of 70% water content SCL did not change by 2 times dehydration and increased greatly by 4 times dehydration. The reduction of diameter of -1.00 D SCL was greater than that of -9.00 D SCL. Moreover, the radius of -1.00 D SCL increased depending on the frequency of dehydration but that of -9.00 D SCL did not changed. The diameter and radius changes of lotrafilcon B, silicone hydrogel lens, were less than those of hilafilcon B, copolymer of HEMA and N-vinyl pyrrolidone. TBUT during wearing SCLs decreased by wearing dehydrated SCLs. Conclusion: The diameter and radius of dehydrated SCLs as well as TBUT during wearing them were changed in spite of rehydration, which would be the important cause of uncomfortable feeling when people wore dehydrated SCL. The changes of SCL morphology and TBUT differed according to the water content, lens thickness and material.

The Effects of Ultrasonic Cleaner for Eye Glasses on Protein Deposits and Parameters in Soft Contact Lens (안경용 초음파세척기에 의한 소프트콘택트렌즈의 단백질 침전물 세척효과)

  • Purpose: The cleaning effect of protein deposit and the change of contact lens parameters by ultrasonic cleaner for eye glasses on the soft contact lenses were investigated. Methods: Etafilcon A contact lenses contaminated with protein, was ultrasonicated by ultrasonic cleaner for eye glasses and for the control group, spoiled contact lenses were cleaned by multi-purpose solution. The remaining protein deposits on the contact lenses were determined after extraction and the changes of overall diameter, base curve, center thickness power, and water contents on contact lenses were measured and surfaces of contact lenses were observed by scanning electron microscope. Results: The cleaning efficacies of multi-purpose solution on protein deposited etafilcon A contact lenses were 6.08%, and 23.73~33.92% in the group of ultrasonic cleaner for eye glasses with multi-purpose solution and 0~12.99% in the group of ultrasonic clear for contact lens with multipurpose solution depending on the treatment time. The changes of parameters and surface on contact lenses by ultrasonication were not observed. Conclusions: Ultrasonic cleaner for eye glasses can be used to eliminate protein deposits for the diagnostic soft contact lens in the office since it was effective to eliminate protein deposits and not caused change of parameters on soft contact lenses.

  • Human corneal epithelial cells and mouse fibroblast L929 cells were grown to 60-70% confluency in 96 well plates. 24 hours after the well plates are inoculated, the medium on the test plates is aspirated and replaced with an extract supplemented medium prepared from the materials to be tested. The contact lenses manufactured by 7 companies were collected from optical shops and used for this study. The exactracts having been prepared by autoclaving 8 lenses in 2.5ml saline. The cell monolayer is then cultured for a further 48 hour period. MTT and SRB assys were performed for cytotoxic effect on cultured cells An inhibition of 30% is considered clear indication of cytotoxic potential in the test material. All the materials were not cytotoxic, but 3 storage solutions of them inhibited growing L929 cells.

The Effect of Circle Lens and Soft Contact Lens with Identical Material in Clinical Application on the Eyes (써클 및 동일 재질 콘택트렌즈의 임상 적용시 눈에 미치는 영향)

  • Purpose: The present study was conducted to investigate the actual condition of circle lens wear and the differences caused by the pigmentation in blinking rate, non-invasive tear break-up time (NIBUT), and lens surface between circle and soft contact lens wear during clinical application. Methods: Eighty subjects were surveyed to know the actual condition of wearing circle lens. Blink rate, NIBUT of 20 subjects in twenties were measured after separately wearing soft contact lens and circle lens for 5 days. Their lens surfaces were also observed by scanning electron microscope. Results: Fifty percent of circle lens wearers answered that major reason for changing circle lens was discomfort and 67% of answerers wore circle lens more than 6 hours a day. The tendency of increased blink rates in both wearers of circle and soft contact lens at 30 minutes later when tear film stabilized and 3 hour after lens wearing was shown on the 1st, 3rd and 5th day with expanding wearing time. Especially at 3 hours after lens wearing on the 3rd and 5th day, the difference of blink rates was statistically different in circle lens wear and soft contact lens wear. The NIBUTs of soft contact lens wearers and circle lens wearers were 6.0 and 3.7 secs, respectively, at 30 min later on 1st day and were significantly different. NIBUTs of soft contact lens wearers and circle lens wearers were also statistically different 3 hrs after lens wearing on the 1st day and the similar pattern of NIBUTs was shown on the 3rd and 5th day. The roughness of both surfaces in soft contact lens and front surface in circle lens was same. However, pigmented front surface of circle lens was rougher and uneven. Conclusions: As the results, the circle lens wearers might feel discomfort in clinical application since the difference in lens surface of circle lens would change lens wettability during wearing. The difference by tinting contact lens was shown from the result, which could help better wearing circle lens.

Comparison of Corneal Edema Induced by Material of Contact Lens (콘택트렌즈의 재질별 각막부종의 비교)

  • Purpose: This study has been compared the degree of corneal edema arising from the material of the lens after wearing the recommended wearing time for the contact lens. Methods: For the study, 43 adults of 77 eyes who did not use drugs and have not any disease participated. Three types of lenses including narafilcon A, HEMA, and silicone hydrogel material regular replaceable lenses were used. Central corneal thickness was measured before and after 8 hours from wearing lenses using Pentacam. The degree of corneal edema was determined by the difference before and after wearing. Results: The average corneal edema of narafilcon A, HEMA material lens, and silicone hydrogel material regular replaceable lens were $2.36{\pm}6.15{\mu}m(Mean{\pm}SD)$, $23.61{\pm}10.71{\mu}m$, $18.25{\pm}8.64{\mu}m$, respectively. The central corneal thickness after wearing narafilcon A was not statistically significant with the central corneal thickness before wearing the lens. Central corneal thickness before and after wearing HEMA material lens and silicone hydrogel material regular replaceable lens were statistically significant. Conclusions: The amount of corneal edema induced by silicone hydrogel material regular replaceable lens was no statistically significant difference to the amount of corneal edema induced by HEMA material lens, but there was significant difference to narafilcon A. Therefore, it is judged to be careful not to exceed the recommended wearing time depending on the composition of the contact lens material.

  • The purpose of this study is synthesizing silicone polymer which is used the material of contact lens and solving the problems of water content and light transmittance for gas permeable contact lens. We used NVP, MMA, HEMA monomer for polymerization and EGDMA as cross linking regent. Also, we polymerized with a several formulation arrangement for the best condition as contact lens. After that. we measured water content and light transmittance by each sample which was polymerized. We polymerized the silicone polymer which is simultaneously pursued by the transparent and water content of the material and measured their physical nature of each sample on this study.
