  • 노인장기요양보험 체계의 캐어매니지먼트 발달 배경과 제도적 문제점을 파악하여 이에 근거한 활성화 방안을 모색하였다. 기존의 케어매니지먼트는 선진국의 대인 서비스 기관에서 널리 보급되고 있는 사회복지실천의 분야이다. 특히 캐어매니지먼트 활성화를 위해 기존의 케어 대상자들이 양질의 서비스를 받을 수 있도록 케어매니저의 종합적인 지원체계와 교육훈련 및 업무에 관한 제도적 조항이 설정되어야 한다.

Case Analysis to Establish an Integrated Care System for Seniors (노인 통합케어 시스템 구축을 위한 사례분석)

  • This study conducts analysis on integrated care cases in Korea and other nations regarding health, medical, and welfare services that are segmented and fragmented in order to provide foundational data to establish an integrated care system appropriate for the situation of Korea. According to the result of the cases, integrated care provides various services in common through cooperation with organizations, collaborative participation of various professionals, provision of intervention and individualized protection, and experts for taking care of the cases. Based on those cases, this paper provides suggestions to establish an integrated care system of Korea. In fact, it is necessary to build integrated care general support centers, arrange service providing places in common, establish a care management system, develop resources and expand manpower, develop an integrated care case management system, and develop integrated care computer network.

Integrated Care Setting for the Elderly and the Roles of Social Workers (노인복지분야의 통합적 케어와 사회복지사의 역할)

  • The objective of this study was to explore the elements and effects of integrated care interventions for the elderly, and to describe the roles of social workers on the multidisciplinary teams. A rapid systematic review was conducted through a database-based search and a manual search. The main findings are as follows: First, Key elements of integrated care for the elderly were case management, individual multidisciplinary care plan, service characteristics, centrality of client needs, patient education, and self-management. Second, In terms of care experience and health of the elderly, and economic analysis, although the results were mixed, there were some positive effects on program participants compared to the control groups. Third, social workers have been found to serve as case managers, care managers, care coordinators, and direct service providers. Based on these results, educational and policy implications were suggested.

Policy Implications of the Long-term Care Insurance System Revitalization Process for Establishing Integrated Care System in Japan (일본의 지역포괄케어시스템 구축과 개호보험제도 개정과정의 정책적 시사점)

  • Recently, the Ministry of Health and Welfare has emphasized the necessity to build community care, and the interest of health care- long-term care-social welfare system continues. In order to examine the future vision of long-term care for elderly people, which can be a core system in establishing community models for elderly people in the future, we will implement aging in place as a national policy project. It is meaningful to analyze in depth the case of Japan's policy implementation through the amendment process of long-term care insurance system. The purpose of this study is to examine the concept definition, operating system, and major promotion process of the Integrated Care System in Japan and to suggest policy implications for the future Korea long-term care insurance system. As a result of reviewing the operation status and revision process of the long-term care insurance system for establishing the Integrated care system in Japan, it is necessary to clarify the basic principles and construction of community care system, diversification of residence type and upgrading work, establishment of linkage network of health care-long term care-social welfare, expansion of support for family and self-mind.

The Preliminary Study for Development of Occupational Therapy Model Focused on Improving Living Functions within the Community Care System (커뮤니티 케어 제도 내 생활기능 향상 중심의 작업치료 모델 개발을 위한 기초 연구)

  • Objective : This study conducted a preliminary study to develop a occupational therapy model focused on improving living functions within the community care system. Methods : From June to July, 2018, the literature on community care was researched, focusing on cases of Japan's Management Tool for Daily Life Performance (MTDLP), Sweden, United Kingdom, Germany and domestic S Elderly Care Centers and I Health Centers. Based on this information, a group of experts developed a occupational therapy model within the community care system. Results : Assessment tools such as occupation-based health promotional table, interest checklist, occupational goals for improving living functions, sheet for evaluation of living functions, survey of daily life time (weekday and weekend), and sheet for transition of living functions were developed to conduct evaluation for occupational therapy. The improving living functions program, analysis of activities based on ICF model, lifestyle redesign program, cognitive exercise therapy, the Lee Silverman Voice Treatment (LSVT), hospice, and home modification were also organized interventions already in place by occupational therapists. Conclusion : This study showed specific measures and models for the implementation of occupational therapy within community care systems. Occupational therapy is positioned as a specialized area that is essential to the client, and we look forward to the use of this model.
