Arthroscopic Treatment of Fungous Arthritis of Knee Joint by Candida pelliculosa - A Case Report - (Candida pelliculosa에 의한 진균성 슬관절염의 관절경적 치료 - 증례 보고 -)

  • Candida infection of knee joint is very rare. Diagnosis of Candida infection is difficult due to lack of obvious clinical symptoms. Candida albicans is the most common strain in Candida infection of knee joint, but Candida pelliculosa has been rarely reported. We report a case of Candida pelliculosa infection of knee after intra-articular steroid injection, which was treated successfully by repeated arthroscopic debridement and irrigations and arthroscopic implantation with antifungal agent-impregnated cement bead spacer.

특집_당뇨병환자의 감염증 - 당뇨병환자에게 흔한 '요로 감염증'

  • 요로 감염증은 당뇨병환자에게 흔히 발생하는 대표적인 감염질환 중 하나이다. 당뇨병환자에서 감염이 흔한 이유는 여러 가설들이 있으나, 다양한 기전에 의한 면역체계의 이상과 면역 저하 등 때문으로 생각하고 있다. 당뇨병환자에서 일반적인 감염이 좀 더 빈번하게 일어나지만 거의 당뇨병에서만 관찰되는 희귀 감염질환도 흔한데 그 중 요로의 기종성(공기주머니 형성) 염증은 특징적이다. 또한 일반 요로 감염에서는 흔하지 않은 칸디다와 같은 진균(곰팡이균)에 의한 요로 감염도 드물지 않게 관찰할 수 있다. 따라서 여기에서는 당뇨병에서 보이는 요로 감염의 특징과 대처방법에 대해서 살펴보기로 한다.

Preventive effects of shiitake mushroom extract on candida stomatitis (칸디다성 구내염에 대한 표고버섯 추출물의 예방효과)

  • Purpose: The purpose of this study was to investigate antifungal activity of shiitake mushroom yeast and hyphal type of Candida albicans. Materials and Methods: The extract from shiitake mushroom was collected by drying the supernatant after soaking shiitake mushrooms in water or ethanol. The antifungal activity of the extracts against yeast type of C. albicans was investigated by the susceptibility assay using microplate. C. albicans biofilm was formed on 12-well plate using Ham's F-12 medium in CO2 incubator and treated with the ethanol extract. Furthermore, C. albicans biofilm was formed on denture base resin disk and treated with or without the ethanol extract in the presence of denture cleanser. Live C. albicans in biofilm was counted by cultured colony forming unit value after inoculated on agar plate. Results: Ethanol extract from shiitake mushroom showed stronger antifungal activity against yeast type of C. albicans compared to its water extract. The ethanol extract significantly reduced count of C. albicans in hyphal biofilm (P < 0.05). Also, the ethanol extract showed synergistically antifungal effect with denture cleanser on candidal biofilm on denture base resin disk (P < 0.05). Conclusion: The ethanol extract of shiitake mushroom may be a candidate for preventing candidal stomatitis as well as denture-related stomatitis.

  • Candidiasis is one of the most common causes of late-onset infection among very-low-birth-weight infants (VLBW) in most neonatal intensive care units and is associated with significant morbidity and mortality. Standard therapy consists of the administration of amphotericin B, amphotericin B complex, and fluconazole. In many cases, candidiasis is not easily eradicated, despite the administration of these drugs. We report our experience of the addition of high-dose caspofungin to the conventional antifungal drugs in a VLBW infant with refractory candidemia.

Antifungal effect of electrolyzed hydrogen water on Candida albicans biofilm (Candid albicans 바이오필름에 대한 전기분해 수소수의 항진균 효과)

  • Purpose: Candida albicans can cause mucosal disease in many vulnerable patients. Also they are associated with denture-related stomatitis. Electrolyzed water is generated by electric current passed via water using various metal electrodes and has antimicrobial activity. The aim of this study was to investigate antifungal activity of electrolyzed water on C. albicans biofilm. Materials and Methods: C. albicans was cultured by sabouraud dextrose broth and F-12 nutrient medium in aerobic and 5% $CO_2$ condition to form blastoconidia (yeast) and hyphae type, respectively. For formation of C. albicans biofilm, C. albicans was cultivated on rough surface 6-well plate by using F-12 nutrient medium in $CO_2$ incubator for 48 hr. After electrolyzing tap water using various metal electrodes, the blastoconidia and hyphal type of C. albicans were treated with electrolyzed water. C. albicans formed blastoconidia and hyphae type when they were cultured by sabouraud dextrose broth and F-12 nutrient medium, respectively. Results: The electrolyzed water using palladium electrode (EWP) exhibited antifungal effect on blastoconidia of C. albicans. Also, the EWP significantly has antifungal activity against C. albicans biofilm and hyphae. In the electrolyzed water using various metal electrodes, only the EWP have antifungal activity. Conclusion: The EWP may use a gargle solution and a soaking solution for prevention of oral candidiasis and denture-related stomatitis due to antifungal activity.

  • Candida vertebral osteomyelitis (CVO) is a rare disease that is a complication of intravenous drug use, but recently it has been recognized as mostly an opportunistic infection. Because CVO appears to mimic pyogenic spondylodiscitis in terms of the clinical and radiologic presentations, it is often neglected in a usual clinical setting. The clinical, radiological, and biological characteristics of CVO are often used to make a differential diagnosis with vertebral osteomyelitis from other etiologies. Once an initial proper diagnosis was performed, the treatment relies on the prompt initiation of appropriate pharmacotherapy and serial monitoring of the clinical progress. This paper report late-onset CVO in two young patients who underwent a heart transplant surgery and had postoperative systemic candidiasis. These two cases are a good reminder of the potential of CVO in immunosuppressive patients treated with anti-fungal agents. This paper presents these two cases with a review of the relevant literature.

Biomorphometric Studies on Oral Candidiasis (구강 칸디다증의 생물형태계측학적 연구)

  • This study was undertaken in order to get biomorphometric significance of oral candidiasis as a diagnostic method. Specimen was collected from 23 oral candidiasis outpatients(10 male and 13 female) of dental infirmary of U.C.L.A. Health Center from Sept. 1,1981 to Aug. 1,1982. The results were as follows : 1. The male-female ratio on oral candidiasis was 5/6.5. 2. The mean age of male was 42.8 and the mean age of female was 60.1. 3. The nucleocytoplasmic ratio of outmost epithelial cell layer was 0.60 ane (the nuclecytoplasmic ratio of)basal cell layer was 0.87. The difference was significant.

A case of laryngeal candidiasis confined to vocal cord in an immunocompetent patient (정상 면역 기능을 가진 환자에서 성대에 국한되어 발생한 후두 칸디다증 1예)

  • Primary laryngeal candidiasis is rare in immunocompetent patients and is prone to confusion with early glottic carcinoma or leukemia. We experienced a case of 74-year-old man who has 3- month history of hoarseness. The pathologic diagnosis was laryngeal candidiasis. He was treated with antifungal agents for 4 weeks after vocal cord stripping under general anesthesia. After treatment, the patient had no candidiasis or discomfort with his voice. We report this case with a review of literature.

Candidiasis in layer chickens in Korea (산란계에서의 칸디다증 국내 발생 사례)

  • Candidiasis is a mycosis caused by the mycelial yeast of the Candida genus which is opportunistic pathogen of humans, animals, and birds. Under some conditions such as prolonged antibiotic therapy, overcrowding, and immunosuppression, the opportunistic Candida can cause disease. Chicken candidiasis is sporadically occurred and characterized by unsatisfactory growth, listlessness, roughness of feathers, and death. A case of 23 weeks old layer with history of increased mortality and anemia was submitted to our Lab. At necropsy, the characteristic lesions were observed in the crop and proventriculus. The whitish pseudomembrane, that are peeled easily, was found in the crop. Proventriculus was swollen and the mucosa was covered with hemorrhagic exudate. The histological changes of the affected crop are epithelial hyperplasia, hydropic degeneration, and mycelia formation. Smears made from the necrotic mucosal surfaces of the crop revealed the presence of large number of yeast cells and mycelia. Pure cultures of yeast colonies were obtained from the potato dextrose agar. The yeast cells were identified as Candida albicans by gene sequencing. To our knowledge, this is the first report of candidiasis in chickens with anemia in Korea.

Isolation of Myxobacteria Carrying Soraphen Biosynthetic Gene Clusters (Soraphen 생합성 유전자군을 갖는 점액세균의 분리)

  • Polymerase chain reactin (PCR) could be a simple way to screen new microbial strains producing useful secondary metabolites if their biosynthetic genes are known and candidate strains to be screened are available. In this study, we have screened two myxobacterial strains, KYC3047 and KYC3076, carrying genes appeared to be biosynthetic genes of soraphen A, a potent antifungal substance, out of 50 cellulose degrading myxobacteria using PCR. The two strains were identified as Sorangium cellulosum based on morphological, physiological, and molecular biological characteristics. Both of the strains produced substances having strong antifungal activities as expected against Candida albicans, a causative agent of candidiasis, and Colletotrichum acutatum, a causative agent of anthracnose on pepper.
