The Effect of Splinting Methods on the Rearrangement of Periodontal Fibers after Tooth Movement in Adult Dogs (치아이동 후 고정방법이 성견 치주인대 섬유의 재배열에 미치는 영향)

  • The purpose of this study was to evaluate the effect of splinting methods on the rearrangement of periodontal fibers after experimental tooth movement. Orthodontic force was applied by placing closed coil spring between upper third incisor and canine in seven dogs, weighing 20 kg or more. After 3 weeks of force application, 0.0215 inch multistrand wire and polyethylene ribbon were bonded to each side, as a flexible and rigid splinting respectively in 6 experimental animals. The remaining one served as a control. Each two animals were sacrificed at 4, 8 and 12 weeks after splinting respectively and prepared histologically for hematoxylin-eosin and Masson's trichrome staining. The results of this study were obtained as follows: 1. After tooth movement, periodontal space was narrowed and periodontal titers were thick on pressure side while elongated fibers were observed on tension side. 2. After 4 weeks of retention, the rearrangement of periodontal fibers was observed in the flexible splinting group, but not in the rigid splinting group. 3. After 8 weeks of retention, the rearrangement of periodontal titers was observed in both groups, but the difference could not be detected between two groups. 4. During the retention period, the rearrangement of periodontal fibers was faster in tension side than in pressure side. These results show that the rearrangement of periodontal fibers is also obtained by rigid splinting after tooth movement. It is suggested that the rigid splinting by polyethylene ribbon can be used as a way of postorthodontic retention.

Effects on Osteoclast in Periodontal Ligament Space by Denerveation of Inferior Alveolar Nerve in Young and Adult Rats (하치조 신경 절단이 치주인대공간에서 파골세포에 미치는 영향)

  • Osteoclast action is necessary for alveolar bone remodeling in orthodontic tooth movement. The nervous system has also been reported to be associated with bone remodeling. This study was aimed to investigate the changes of osteoclasts in the periodontal ligament (PDL) space after surgical resection of the inferior alveolar nerve (IAN). Experimental rats were divided into young and adult groups. A surgical resection procedure of the IAN was carried out in the left side of the mandible and a sham operation in the right side of the mandible. The number of osteoclasts on the bundle bone surface and the resorption activity of the osteoclasts were histomorphometrically measured. The changes in distribution of substance P (SP) immunoreactive (IR) nerve fiber were evaluated in the PDL and pulp. SP-IR nerve fiber was depleted in both the PDL and pulp of the IAN resection side in both groups, which confirmed the resection of IAN to be successfully conducted. The number of osteoclasts in the IAN resection side was significantly reduced in both the young and adult groups (p<0.01 and p<0.05), whereas the resorption activity of osteoclasts did not show any significant difference between the IAN resection side and the sham operation side in both groups (p>0.05 and p<0.05). The adult group showed that the number of osteoclasts reduced significantly (p<0.01) and the resorption activity didn't change in comparison with the young group (p>0.05). These results suggest that surgical resection of the IAN and aging reduce the population of the recruited osteoclasts within the PDL, but don't affect on the osteoclastic resorption activity.

Histological Periodontal Tissue Reaction to Rapid Tooth Movement by periodontal Distraction in Dogs (치주인대 신장에 의한 치아의 급속 견인 시 성견 치주조직의 변화)

  • The objective of this study was to evaluate the changes that occurred over time in the distracted periodontal ligament space following the rapid retraction of a tooth by periodontal distraction after bone undermining surgery had been conducted in the dogs. The upper second premolars were extracted on the left and right side in 4 male beagle dogs. Immediately after extraction, the interseptal bone distal to the upper first premolar was thinned and undermined by grooving to decrease the bone resistance. Activating an individualized distraction appliance at the rate of 0.225mm twice a day, the upper first premolar was retracted rapidly toward the extraction socket. Periodontal distractions were performed for 5, 10, and 20 days, and 20-day-distraction cases were followed by maintenance periods of 0, 14, 28, and 56 days. After 20 days of rapid retraction, the average distal movement of the upper first premolar was 5.02mm, and the average mesial movement of the upper third premolars serving as an anchorage unit was 0.18 mm. On histological examination, the regeneration of bone occurred in a highly organized pattern. Distracted periodontal ligament space was filled with newly formed bone oriented in the direction of the distraction, and this was followed by extensive bone remodeling. This result was similar to those observed in other bones after distraction osteogenesis. In the periodontal ligament, the relationship between collagen fibers and cementum began to be restored 2 weeks after the distraction was completed, and showed almost normal features 8weeks after the completion of the periodontal distraction. However, on the alveolar side, the new bone formation was still in process and collagen fiber bundles and Sharpey's fibers were not present 8 weeks after the completion of the periodontal distraction. Reactions in the periodontal ligament of the anchorage tooth represented bone resorption on the compressed side and new bone deposition on the tension side as occurred in conventional orthodontic tooth movement. In conclusion, the results of this study showed that periodontal structures on the distracted side of the periodontal ligament were regenerated well histologically following rapid tooth movement.

Expression of UNC-50 DNA in periodontal tissue of rats after application of intermittent orthodontic force (간헐적 교정력 적용 후 백서 치주인대에서 UNC-50 유전자의 발현)

  • Objective: Periodontal ligament fibroblasts have an ectomesenchymal origin and are thought to play a crucial role for not only homeostasis of periodontal tissues but also bone remodeling, wound healing and regeneration of tissues. Recently, it has been reported that UNC-50 is not expressed in gingival fibroblasts but in PDL fibroblasts. The purpose of this study was to examine the expression of UNC-50 and osteocalcin in the periodontium after application of intermittent force. Methods: Twelve rats had 40 grams of mesially-directed force applied at the upper molar for 1 hour/day. Four rats were sacrificed at 1, 3 and 5 days. Immunohistochemical localization of UNC-50 and osteocalcin antibody was carried out. The results showed apposition of new cellular cementum and a slight increase in periodontal space at the tension side. Results: Strong UNC-50 expression was observed in the differentiating cementoblasts close to PDL fibroblasts in the tension side whereas it was barely expressed at the compression side. Expression was strong at day 3, and decreased at day 5. Osteocalcin immunoreactivity expression was strong in differentiating cementoblasts at the tension side. Conclusion: It can be suggested that UNC-50 is related to the differentiation of cementoblasts, and may be responsible for the molecular event in PDL cells under mechanical stress.

Roles of Sonic Hedgehog Signaling During Tooth Root and Periodontium Formation (치근 및 치주조직 형성과정 동안 Sonic Hedgehog signaling의 역할)

  • The aim of this study was to understand the roles of Sonic Hedgehog (SHH) signaling during tooth root and periodontium formation. In this study, we generated the dental mesenchyme-specific Smoothened (Smo) activated/inactivated mice with the activity of Cre recombinase under the control of osteocalcin promoter. In the Smo activated mutant molar sections at the postnatal 28 days, we found extremely thin root dentin and widened pulp chamber. Picrosirius red staining showed loosely arranged fibers in the periodontal space and decreased cellular cementum with some root resorption. Immunohistochemical staining showed less localization of matrix proteins such as Bsp, Dmp1, Pstn, and Ank in the cementum, periodontal ligament, and/or cementoblast. In the Smo inactivated mutant mouse, there was not any remarkable differences in the localization of these matrix proteins compared with the wild type. These findings suggest that adequate suppressing regulation of SHH signaling is required in the development of tooth root and periodontium.

Periodontal Regeneration Using the Mixture of Human Tooth-ash and Plaster of Paris in Dogs (성견에서 치아회분말과 연석고를 이용한 치주조직재생술)

  • 흡수성 차페막을 이용한 조직 유도 재생술시 차폐막의 견고성으로 미루어 보아 재생을 위한 공간의 유지가 어려울 수 있다. 조직 유도 재생술과 함께 골이식술을 시행함으로써 공간 확보와 함께 적절한 혈병의 유지를 도모할 수 있고 이식된 골은 선생골 형성을 위한 핵으로 작용할 수도 있다. 최근에 사람의 치아회분말과 연석고를 혼합한 골이식재가 여러 연구를 통해 좋은 골이식재로 평가되었다. 본 연구에서는 성견 하악 소구치 2급 치근이개부위에 외과적으로 형성하여 흡수성 차폐막과 치아회분말-연석고 혼합 이식재를 이용한 조직유도재생 술을 시행하여 치주 조직 재생의 양상을 조직학적으로 관찰하고자 한다. 생후 12개월에서 16개윌 된 체중 15 Kg 내외의 성견 4마리를 이용하였다. 실험 재료로 생체흡수성 차폐막 (Biogide(R), Swiss) 를 사용하였고, 골이식재로 치아회분말-연석고를 혼합매식 하였다. 양측 상악 소구치 부위에 변연 치조골하방에 4 mm ${\times}$ 4 mm ${\times}$ 4 mm, (깊이 ${\times}$ 근원심 ${\times}$ 협설폭경) 깊이로 골내낭을 형성하였다. 형성된 골내낭의 기저부위 치근 표면에 1/4 round bur로 notch를 형성하여 참고점으로 하였다. 무작위로 선택된 한 쪽의 결손부를 대조군으로 오직 생체 흡수성 차폐막을 사용하였고, 실험군으로 치아회분말-연석고와 생체 흡수성 차폐막을 결손부로부터 2 mm 이상 덮을 수 있도록 다듬어 결손부 위에 위치시킨 후 협측 판막을 덮고 봉합하였다. 4주 후 2마리 ,8주 후 2마리를 희생시키고 통상의 방법으로 고정, 탈회, 포매의 과정을 거쳐 광학 현미경으로 검경하였다. 그 결과, 1. 4주 대조군에서 Bio-gide(R)는 완전한 흡수를 보였고, 치근이개부내에는 큰 공간이 존재하였다. 2. 4주 실험군에서 역시 Bio-gide(R)는 완전한 흡수를 보였고, 골 결손부내에 더 많은 신생골 관찰되었다. 그러나 아직까진 기존골과 신생골간에 명확한 차이가 있어서 쉽게 구분할 수 있었다. 또한 골이식재 주변으로 파골세포가 다수 관찰되며 이로 미루어 보아 활발한 골흡수가 일어남을 알 수 있었다. 3. 8주 대조군에서 결손부내에서는 기존골에 인접하여 신생골 형성이 부분적으로 일어났으나 연조직 침입이 관찰되었다. 4. 8주 실험군은 신생골이 기존골과 매우 유사한 형태로 관찰되었고, 신생골 형성 부위에 신생 혈관 증식이 관찰되었다. 또한 골내낭 기저부위에서는 백악질과, 치주인대가 재생됨이 관찰되었다. 이상의 결과에서 치아회분말-연석고 혼합매식은 골재생을 위한 골전도성이 있는 재료로 사료되며, 이를 이용히여 치주조직재생술시 흡수성 차폐막과 병행하여 사용한다면 더 많은 골재생이 있을 것으로 기대된다.

AN EXPERIMENTAL STUDY ON THE STRESS DISTRIBUTION IN THE PERIODONTAL LIGAMENT (치주인대의 응력 분포 양상에 관한 실험 연구)

  • In order to achieve a desirable tooth movement, it is of great importance to control the M/F ratio and to know the location of the center of resistance. The purpose of this study was to locate the center of resistance and the axis of rotation, and to estimate the stress distribution in the periodontal ligament with experimental model. After preparing a model of an upper canine with a simulated periodontal ligament and alveolar bone, the force and moment were applied. The tooth movement was traced using measuring device with LVDTs(Linear variable differential transformers) that can measure three dimensional tooth movement in real time. The results were as follows. 1. The location of center of resistance by transverse force was $29\%$ of root length measured from alveolar crest to apex regardless of force magnitude. The position of the center of resistance is more coronal than that of two-dimensional model($42\%$). 2. The center of resistance and the axis of rotation coincide when couple moment was applied. 3. As the magnitude of moment increases, tooth tends to extrude irrespective of the direction of the moment. 4. The relationship between location of force and axis of rotation (a x b = $49.6\;mm^2$) was obtained. A tooth movement can be predicted through this formula. 5. The centers of rotation by transverse force were plotted linearly.

Autotransplantation of an impacted maxillary canine using Rapid Prototyping : A case report (Rapid Prototyping을 이용한 상악 매복 견치의 자가이식 치험례)

  • Management options for impacted maxillary canines can include (1) continued observation, (2) extraction of the primary canine to aid spontaneous eruption, (3) uncovering and bonding of the impacted tooth and its eruption using orthodontic traction, (4) autotransplantation, and (5) extraction followed by prosthetic replacement. Autotransplantation should be considered when the degree of malposition is too severe to correct by orthodontic alignment. The present report describes the management of an ectopic eruption of the left maxillary canine in an 10-year-old girl. The treatment included the extraction of primary maxillary left canine and the autotransplantation using a Rapid Prototyping model. By using RP model to contour the recipient bone and check for fitting in the prepared socket, the extra-oral time can reduce. The autotransplanted canine showed mobility within normal limit, negative response to percussion and positive to electric pulp test after 6 months.

  • Maxillary canine impaction is a frequently encountered clinical problem, and it may cause the resorption of adjacent tooth or cystic change. Treatment plan for maxillary canine impaction should be decided among extraction, orthodontic traction and autotransplantation according to several factors such as direction and position of unerupted tooth, degree of developing root apex, eruption space, exsitance of supernumerary tooth, odontoma, or cyst. Autotransplantation is a valuable alternative to extraction of impacted teeth, where surgical exposure and subsequent orthodontic realignment are difficult or impossible due to unfavorable impaction position. And its prognosis is dependent on a number of factors such as preservation of periodontal ligament, degree of root development, surgical technique, patient's age, endodontic treatment, time and type of splinting and storage medium, etc. The patients in these cases visited our dental clinic in the late permanent dentition with the chief complaint of unerupted maxillary canines. And it was thought that the spontaneous eruption guidance or orthodontic traction and alignment were difficult because of its unfavorable impacted position. Therefore, autotransplantaion and endodontic treatment were done and have been checked periodically until now.

MULTIPLE ANKYLOSIS ON MAXILLARY AND MANDIBULAR PRIMARY MOLARS WITHOUT PERMANENT SUCCESSOR (계승치의 결손을 동반한 상, 하악 유구치의 다발성 유착에 대한 증례보고)

  • Ankylosis is defined as a fusion of cementum or dentine with alveolar bone. Due to the loss of the periodontal ligament on the ankylotic area, the tooth is incapable of continued eruption and hence is unable to follow the normal vertical development of the neighboring teeth and alveolar process. A 6-year-old female was referred to the Dept. of Pediatric Dentistry for ankylosis of primary molars and congenital missing of permanent premolars on both jaws. She had neither specific past medical history nor trauma and infection history in oral and maxillofacial region. Radiographic finding is that the maxillary primary molars were the early onset of ankylosis and had fast root resorption rate. However the mandibular primary molars were ankylosed later and being resorbed slower than maxillary primary molars. The object of treating this case is to maintain the proper alveolar bone growth and retention of deciduous molars. The point of managing this case is as follows: Proper treatment (observation, restoration, or extraction) should be established after thorough consideration of the time of onset, the root resorption rate, progression of infraocclusion and the development of alveolar bone support. We should consider the timing of extraction of the ankylosed teeth without problem of neighbouring alveolar bone growth and tilting of adjacent teeth in the view of growth spurt. Early diagnosis is important to avoid many of the complications with infraoccluded primary molars.
